Fantastic experience: This spot has the most incredible therapy team. The knowledge and skill is second to none, not to mention the love they have for the residents. HG is full of committed nursing and administration who are working diligently to ensure top quality care for their residents.

Negative experience: My dad, 87 years old been here for couple days. He fully disability with blind,bed 24/7 and also with light dementia. The hospital transfer him here for therapy, hope he can stand up after failing. I beggly request assistance for feeding with meals but he is always the last person was feeded and sometimes not. As now, at this moment, 9.00am but no one feed him breakfast. Myself and my mom too worry and have to visit him any time we can. Staffs seem to be very nice talk but NOT ACT after several times discussion. They are lack of nurses, especially during the night shift. I am in the hospital early every day before going to work to check on my dad and see him lay down there with full dirty stuffs diaper. I press the red button, waiting for an hour but the magic never happen, no one show up. I clean him by myself daily. I keep praying God give people more sympathy, patient and understanding when they are working in the medical field, especially with all old and weak patient need help, that is the reason why they are at the rehab. My advice is research carefully before you send your love one to this rehab. 1. Room is dirty and always smell of urine 2. Nurses are lazy or tired with their schedule 3. Lack of providing medicine for patients 4. Night shift nurses/tech never change diapers (was confirmed by staffs there) 5. You cannot change this organization, I feel so sympathy for any victims still here without relatives visit.

The doctors are top notch and all are bilingual, accessible 24/7 and easy to reach. This place is always fully staffed and all the team works so well together.. The kitchen services are also great, the food is awesome and residents families and even employees. I highly recommend this place. They are Angels who work miracles.

Fantastic experience: We are really thank to Loren - a marketing agent. She is the one who help my mom to have transfer from another facility. She was very professional and polite. I was guessing other facility didn't want to let my mom go,(loosing a patien) But Loren came there and handle them like a professional.Ended up they let her go. My mom needs dialysis program, now she having dialysis in the house. Our family is so happy for that. Thank you so much LOREN and all the crew working in there.

Negative experience: If you’re looking for a place to give your love one extra care/or to gain their strength back, then Heritage Gardens Rehabilitation & Healthcare is the place to go. The staff there goes above and beyond to take care of their patients. They treat them as their own family. One person that stands out to me is Mrs. Tia Heard. She will make sure your loved one is given the proper care and attention they need while in the facility. What ever problems or questions you may have, she is the person to call and ask for. I would recommend this Facility to anyone, who was looking for assistance.

Fantastic experience: Haritage is An amazing facility, and has some of the most caring staff I’ve seen in a long time. Everyone has made my auntie feel like family. They take therapy/rehabilitation seriously, and potentially saved my aunties life. I will say this don’t let some of the newer , fancy facilities fool you when it come to care they are lacking. My family member was at one of those places prior to us moving her here, and it’s night and day difference . I would definitely suggest this facility to anyone looking for a place for a loved ones rehabilitation , long or short term care . Because good care is hard to find , but this place has it !!

Negative experience: The state of Texas and Medicare has this facility rated as barley 3 stars. So don't be fooled by the fake post you see on here because we were. My wife and I brought my dad there from the hospital and therapy was awesome. But the nurses care and hygiene for him was terrible and very inconsistent. They moved my dad to a hall they call long term and it only got worse. It broke my heart to see my dad so dirty and nasty when my wife and I went to see him. We removed my dad and got him to a different nursing facility. Please be careful leaving your love ones here I would not recommend it.

Positive experience: This is a well cared for facility although much older than the newer fancier places. If you desire care that is from staff that really want what is best for your loved one then you would enjoy this facility. I must say the food is above average for a rehab center that again demonstrates that people try to please at Heritage Gardens. If you address a concern the staff (from experience) put forth an effort to solve any problem mentioned.

Negative experience: This place doesn’t care about patient care. They’re all about the bottom line $. They terminate good employees because they make too much money. Really this industry is infested with people who run places like this but there are some good ones out there that don’t mind paying employees what they deserve. Do your research, make your own decision.

Negative experience: Not happy with heritage when I visit the facility. They over staffed during the day and hardly any staff after 4 pm. They take to long to tend to the patients. My mother dislikes the meals they seem to be out of a can or frozen food she has lost about fifteen to 20 pounds in less than a month.my mom often pees on herself because nobody tends to her at night.we are trying to move her to another facility but we haven't had any luck

Fantastic experience: Heritage Gardens has one of the most amazing rehab teams! They truly care about their patients and work hard to help them. All of the therapists show an astounding amount of love and care for their patients to make the place really feel like home. The newly remodeled therapy gym is warm and welcoming . Every department at Heritage Gardens work their hardest and make the facility truly one big happy family!

Negative experience: Horrible! Do not place your loved one here unless you want them to develop a stage 4 bed sore! Absolutely no care given to my mother and she is suffering a 6 1/2 inch stage 3/4 bed sore. How can anyone treat a human being with such little regard?

Fantastic experience: Heritage Gardens is one of the best nursing home facilities and Rehabilitation. All because of the care of our residents here, and the team work of staff starting with the rAdministrative she will go up and beyond for the residents and staff. I Love the interaction that is provided to the residents even though out the COVID-19

Negative experience: Dining room has 15 tables but only 5 chairs to sit in. I suggest you shop around with competitors. Transportation van has been inoperable for 2 months. Their website says never a dull moment here. They advertise via website that they take you shopping. Not so!be careful with your concern to website promise

Fantastic experience: The main thing that stood out to me while being at heritage gardens was the generosity of the staff. They were all so helpful and sweet to me and to the residents. Patient care is the top priority at this facility and that staff will do anything they can to assure that the residents/patients are taken care of.

Fantastic experience: I had came in to tour this facility, and being honest I looking around and seeing that they were roots here. HOWEVER I have seen the nurses and how they take such great care of the resident. The call light system in unbelievable with a quick response. The administrator is always in the building you you need her, she DO NOT mind helping you at all.The food I have tasted for myself and it was great. The staff is so nice everyone haves a big smile on their faces. The building DO NOT STINK AT ALL, it smells so good and this was surprising to me. i go to other nursing home and the smell hits you at the door. You love one is save here trust me. My aunt was here had no complaints. PT had help her every bit of the way and now she walks better then ever. THANK YOU GUYS SO MUCH FOR ALL THAT YOU HAD DID FOR MY LOVED ONE.

Fantastic experience: I toured the facility and I was very impressed. You can tell that the team especially the nursing staff genuinely cares about the patient. Patient care is their number one focus. According to Medicare.gov they are a 2 star facility, however the nursing care and therapy far exceeds 2 stars. I would rate this facility 5 stars. A new Administrator has been in place for a little while, which has made a huge difference. He has made a drastic impact, in a very positive way. This facility is part of a locally owned company that prides themselves on care rather than the mighty dollar. I encourage you to tour and meet the team so you can experience outstanding customer service along with top of the line patient care. Don’t get caught up on the looks of the building, care for a love one is priority. When you tour ask for Brooke or Mila.

Fantastic experience: My name is Mr. Thomas and came in for some short term therapy from Baylor Carrollton. I wasn't able to walk steadily and now I feel I can get back into the gym! I can't express enough how awesome the team has been from the rehab department to Administration. I came in with some complexities and the entire team came together to come up with a plan. This team truly cares for the outcome of the patients. One thing I thought was incredible was everytime I had a concern it was resolved immediately. Now that's what I call GREAT patient care... I want to give a special thanks to the nurses and all the directors for helping me reach my goals and allowing my transition back home to be smooth.

Negative experience: Sister almost died walked in found her unresponsive no one had suctioned her or checked in on her had to call 911 to pull 200 milli of fluid from her was drowning in her own fluids laserdisc nurse and no r/t on staff as promised. First three almost died tragedy nobody does their job.

Negative experience: I wish there were negative stars. WARNING stay away from this place. I placed my Father in here Friday night at 6:00. He was delivered by ambulance. I had made prior arrangements for his records and history to be delivered prior to his arrival so that they would be prepared for his condition, he is an ALS patient. The staff had no idea he was coming nor his condition. The attendant did not even know what ALS is. To make a long horror story short let me just list a few of the conditions of our experience. 1. There was no one on the evening staff who spoke english that I was able to understand 2. The room was filthy and stunk 3. They had attached kite string to the pull chain to turn the light on/off and clothes-pinned it to his pillow 4. They had rigged the call button to a kite string attached to his pillow 5. The A/C did not work 6. The TV did not work 7. They couldn't get a mattres pad because the supervisor was gone and locked the linen closet 8. It took over 5 minutes for anyone to respond to a call 9. They did NO admissions assessment 10. They were ill equipped to handle my Father when moving him. They actually were hurting him while changing his shorts 11. By this time I informed them I was taking him home and they said I couldnt til they called the doctor. They would not bring a wheelchair so I had to get it myself. At no time did ANYONE offer to help me get him in the wheelchair or into my car. When I asked for his meds they made excuses and wanted me to sign a release to waive my rights to file a claim against them ( which I refused to sign) As of today no one from the facility has tried to reach out to me or my parents, not that they could say anything to justify such horrid treatment or conditions in this horrible place Simply put...stay away!!

Fantastic experience: When Baylor Carrollton gave my grandmother the opportunity to choose between Heritage Gardens and another location around Brookhaven, the first thing I did was read the reviews. They were awful and scary to read. Since this place is about 5 blocks from my house, I decided to give it a try. I cannot begin to catalogue the many selfless acts of service we witnessed during our admission. I have many examples of their staff that did extraordinary things, which are illustrative of their commitment of the entire team: HG is a place where I found sympathy from the minute I started talking to them. HG saw the need I had to spend every single night with my grandma and placed an extra bed for me next to hers. HG has clinical techs that ran to get a blanket because my grandma was cold. HG has a nurse who during her midnight meds, was praying for her while giving every single pill (he thought I was asleep). HG has kitchen staff that noticed I was not eating because I was feeding my grandma and they brought a hot meal plate for me. HG has housekeeping who appreciates when you give them a smile and leaves your bathrooms smelling like flowers. HG has a maintenance guy who traded my grandma’s mattress for a more comfortable one within minutes. HG has staff who treat their residents and patients with DIGNITY and respect. Not mentioning the jokes, the compliments, the coffee, and the hugs. The only thing I did not like, was the Social Work, he never had the decency to stop by our room and do his job. However, only one bad employee would not change the way I was treated by the rest of the staff. I found not a rehab center, I found a home. Negative experience: Hated it and was only threre for a little more than a week before I asked to be transferred to Corinth Rehabilitation Suites which was heaven compared to this place. I don't know how they keep a license to serve through the state! Don't send anyone to this place!!! They need to be reported and shut down and as soon as I can contact the proper authorities I will file a comprehensive report hoping to get them shut down or at the very least thoroughly investigated.
