Fantastic experience: The new changes to management have been very positive.You are greeted by staff who welcomes you in. My mother is 91 with dementia and with the normal setbacks. is doing remarkably well. She is encouraged to be with others and tells me she is happy and staff helps her. It gives me a peaceful feeling knowing when I can't be there for her, she is being cared for. Thank you for a great job!

Positive experience: My friend recently moved into the facility in Carrollton, TX. I know all the staff work hard because there are so many patients that need help, but I'd like to give a shout out to a few of them that have made my friend's adjustment easier. Corey - the head floor nurse has been very kind and helpful with overseeing some scheduling wishes - we appreciate his kind spirit and always being willing to listen and oversee the need. Kiki, Jane and Jumi are nurses on my friend's wing and they were so kind, cheerful and understanding about my friend's needs. It is really great to have staff like these individuals who help make an adjustment easier for a new person who just moved there.

Negative experience: They let people stay here with drugs they let people run their room mate out of rooms because they don’t want to share a room with anyone but they don’t make them pay for a private room the staff are always on their phones nothing is ever done about that either

Negative experience: I think there security of the residents is good but the overpowering smell of urine I hope was just a fluke I have not been in a skilled facility that smelled that bad since the 80s like I said I hope it was just a fluke because if that’s normal I feel for all the family and residents

Negative experience: My father lived there for many years and staff members stole all my fathers nice clothes and large music cd collection upon finding out he died at a nearby hospital . They are shameful and shameless.

Negative experience: I drive for Doordash as a side hustle and I was just trying to drop off food for what I later found out was a patient. I was literally treated like a criminal and asked so many questions. If this is how you treat random people I can only imagine how you treat your patients.

Fantastic experience: I am currently a resident at Brookhaven Skilled Nursing and I have been in several skilled nursing units since losing my right leg above the knee. They have given me a place of shelter in a safe, warm and caring environment. They listen when you have an issue and they actually rectify any wrongdoings. The nurses and staff are top notch and the trainers in PT and OT are amazing as well!! My experience here has been for the most part awesome!

Fantastic experience: Mom is currently under care and I have no complaints (dementia). Facility is clean and staff is fantastic. Hoped it would never come to this but Mom is comfortable

Negative experience: We sent my grandmother here and although the staff was friendly, they couldn't even administer her medication correctly. They had my mom giving her meds because they were incapable. My grandmother went hours without being changed! The staff couldn't all get in the same page and we ended up having to take care out discharge, transportation and medication ourselves. So they weren't really necessary! Don't send loved ones here!

Negative experience: I would give Dr. Tompkins a double zero if it was available. She use to be a good doctor but nowadays she become a horrible person and doctor. My husband and I were patient’s of Dr. Tompkins. Known her for years, most recently she took care of my husband while in Brookhaven rehab. My husband had Parkinson’s Disease with mild amnesia. He had a tracheotomy and received his nutrition by feeding tube. My husband was telling me about Dr. Tompkins changing his trach size but he couldn’t remember what exactly she said. I called Brookhaven 3-4 times requesting to speak to Dr. Tompkins. The 4 time she finally returned my call and explained that she wanted to change my husband’s trach size. I then said, “ My husband told me that’s what you wanted to do, but he couldn’t remember why. “ At that time Dr. Tompkins said,” She was glad she spoke to me about my husband not remembering what I said to him. I now won’t be changing his trach size since he can’t remember why, I’m wanting to do it.” Dr. Tompkins is one heartless sole and in my opinion shouldn’t be allowed to take care of anyone. I wouldn’t treat a dog like that. Especially, not a human being. She is one cruel and unkind physician. I have personally had medical power of attorney of my husband for years because of his Parkinson’s and dementia related illnesses. If, Fr. Tompkins had any compassion for her patient(s), she would have explained it to my husband again with me present and giving consent for the necessary procedure. This would have been the right thing to do and to avoid a potential lawsuit. If that is indeed what Dr. Tompkins was afraid of. It very scary that Dr. Tompkins reasoning and compassion for her patient(s) have dwindled over the years. The number one goal of hers, should be showing caring, compassion and doing what’s is in the best interest of all her patient(s). Dr. Tompkins has turned into a careless, unprofessional, in-compassionate and heartless physician. Very sad and sorry!!

Fantastic experience: My elderly mom of 87 yrs was in Rehab at Brookhaven for a few months. A broken shoulder. The staff there took good care of her and had good attitudes. My mom has a good mind and can tell you this for her self. If she had any complaints, they will be made known. I can also tell you that it has been really hard on healthcare workers these days!! They are doing the best they can with a shorter staff(people not wanting to work) and with Covid restrictions. The staff works hard and should be praised. I’ve seen it for myself. I am very familiar with nursing homes and rehab. I can tell you that they are some of the best. Loved the hair stylist that took care of my mom. The rehab therapist were so good to my mom and were tough when they needed to be. They had to be…or my mom would not be recovered. Loved the male aids that loved on my mom and took great care of her. Thank you all!!!! God will bless you!!!

Fantastic experience: I’ve had the pleasure of referring many patients to Brookhaven for respite and for LTC services. Great facility and the staff is very knowledgeable and loving.

Fantastic experience: I am a staff member of Brookhaven Nursing and Rehab, I just want to put that out there first and foremost. I have worked at 4 other nursing facilities in the Metroplex with the majority of them being in this area. I have to say that Brookhaven is the best I've worked at. They really allow the residents to be themselves and truly let them make Brookhaven "their home."

Negative experience: Staff is rude and abusive ! All the vital machines are broken providing inaccurate readings! Aides are trained not to talk with patients for too long . Nurses are racist treating whites badly. I was told multiple times whites stab you in the back. The home is unclean with laundry bins overflowing with dirty linen, stained linen being reused on resident rooms ! Aides told to do 2 person people alone ! I don’t suggest ANYBODY TO WORK HERE OR PUT ANYONE YOU LOVE HERE! Very abusive and unsafe ! Multiple OPEN state reports !

Negative experience: This is the worst place to take your love ones, terrible staff, they all have a attitude, if you ask for help they ignore you. Nursing homes should have people that care and are willing to help the patients on their recovery, not to insult or made fun of the situation they are. Terrible service, terrible staff, terrible place. The star is only because google doesn’t let you post without a star. I give negative star to this place.

Negative experience: I rate this facility with one star. I give it one star because the ceiling wasn’t leaking and climate control within the facility worked. My brother needs special attention due to terminal cancer and barely lasted 24 hours in this facility. The staff is clearly biased torward people with similar ethnic backgrounds. With that said, my brother was not looked after and was forced to lay in his urine because the staff were too busy to answer his call or even bother to check in on him. When they did make an appearance, they were rude, short and anything but helpful. This is a shame because these people are paid to provide a service for those who need a little extra attention. This place should have an internal audit, from top to bottom and change the rotten culture that is rampant amongst its staff and leadership. If you have the means to go somewhere else, do yourself or your loved one a favor and not use this facility.

Fantastic experience: My wife has been a resident of Brookhaven for the last five weeks because of a broken arm, but will be staying there as a long term resident due to her Dementia. I visit and talk with the staff several times a week and have found all to be helpful, caring and sensitive. The place is very clean and they have been most responsive to my wife's needs. They have daily activities to keep my wife occupied and though she constantly complains about her food and physical rehab I see her gaining strength and mobility each week. I have had several relatives over the years in these types of long term care facilities...and Brookhaven is the best that I have experienced. Thanks Brookhaven for taking such great care of your patients/residents!

Negative experience: Living conditions are dispicible. Most of the staff has no compassion. You walk up to the nurses station and everyone ignores you. The head of nursing and wound care nurse were great. Everyone else needs to find a new career. Let's start having some respect for our elders.

Negative experience: I went to see a friend and a nurse came in and said if I have time I’ll massage you. If I have time Then went I was walking out she was sitting and texting on her phone. Even though most of these people are in their 80s or 90s their still someone’s mother, sister, grandma. So come on management do something!!!!!!!!!!!

Fantastic experience: Great staff. All of them are like a family there! It's sad to see these people's negative comments. They must be trying to cope with the fact they put there loved one in a nursing home. A lot of people feel guilt when doing so and then they lash out negatively towards the facility they choose. If the facility is that bad you should move your loved one somewhere you think is better. There therapy team is actually a different company and from what I have seen they are fantastic! As far as laundry goes it... it is like that in any facility you go to. Especially when there are over 100 residents there. That is a lot of laundry to do. You would be behind too. My grandmother was in a few different nursing homes and lost laundry happened a lot. When we are unhappy instead of talking so harshly about a place we would just put in our complaints and if nothing was done we move her. It's that simple. I say this home is 100% a great home to take a loved one to for long term or rehab. They are actually very known for rehab and there success rate! GREAT PLACE!!!

Negative experience: Hey u can tell all ur cronies at Brookhaven UNSKILLED Nursing and Rehab that my Mom passed away on Memorial Day. We buried her yesterday. So tell me why Director of Physical Therapy u never had a staff member spend 45 minutes - WHICH WAS PAID FOR BY MEDICARE - and none of u came to her room to give her the individualized therapy she needed to progress?? At Sonoma House - Senior Select Home Health Care came to her room and provided 45 min to one hour of individualized physics therapy to my Mom? She performed exercises on her and had her walking 30 yards in a walker U didn't do ANYTHING!! So I hope u can sleep at night knowing u had my mothers muscles atrophy so bad and none of ur staff were equipped or knowledgeable to get her back to the day she walked in ur facility. I HOPE U ENJOY DRIVING UR MERCEDES WHILE DOING NOTHING TO HELP THE PATIENTS U ARE IN CHARGE OF. BTW IM contacting Medicare for all the charges u billed. WHAT A JOKE!!!!!!
