Our highly skilled and friendly staff provide excellent rehabilitation and therapy services in luxurious, resort-like environments.
Belleair Health Care Center is dedicated to continuously improving and providing secure, high-quality care. It has been awarded a 5 Star rating by the Centers for Medicare and Medicaid Services and has been awarded the Governor's Gold Seal. This program recognizes and rewards skilled nursing facilities that demonstrate excellence over a long period of time, promotes the stability of the industry and supports the physical, mental, and emotional health of skilled nursing facility patients. Our facility has been approved by the JCAHO Joint Commission on Accreditation for Health Care Organizations.
Improving quality of life is more than just medical treatments, rehabilitation services, and technology. It is about being conscious of and responding to the individual needs of each patient, as if they were our own. We take into account the desires of the patient, involving their family in the formation of the care plan. This way, we can provide a personalized approach that meets the physical and emotional needs of the patient, and gives their loved ones a feeling of safety.
The success of our inpatient rehabilitation and outpatient therapy services is reflected in the positive feedback we receive from the community. Our inpatient program is designed to ensure a safe and successful return home, while our outpatient services offer a range of care options to help patients progress towards their individual therapy goals.
Physical therapy can help patients become more independent and give them the ability to move around again after suffering from an orthopedic or neurological issue or injury. Advanced techniques are used to maximize function and reduce pain. Increasing range of motion, strength, coordination, and muscle control will help you reach your goals.
Occupational therapy works to help people become more independent in their everyday activities such as bathing, getting dressed, and eating after they have been ill or injured. Our therapists use rehabilitation techniques and teach patients how to use specialized tools to maximize their function and movement.
Speech-language therapy helps people improve their communication skills, such as speaking, thinking, reading, writing, and expressing themselves verbally. In addition, therapists can assess and treat various swallowing problems.
We are incredibly thankful for the amazing care our Uncle received at Bellaire Rehab. The physical therapy he received allowed him to go back home and be independent. We are deeply appreciative of the staff, especially David King, RN, for all their hard work.
I would like to express my sincere appreciation to the staff, nurses, CNA's, and therapists at Belleair for their hard work with my mother. This facility is truly outstanding, and the case manager was incredibly knowledgeable and provided our family with the necessary assistance. Thanks to their efforts, my mother was able to walk again in July 2020. Thank you all so much!
The staff are excellent and caring. The building is welcoming and aesthetically pleasing. This is the best rehabilitation center I have ever been to. The Physical Therapy is excellent. I highly recommend this place.
We are incredibly thankful to the Belleair staff for all their hard work in aiding our mother to get her energy back. We could never have imagined her being able to live such a satisfying life after being confined to the house for so long. We will always keep your generosity in our minds.
Petra's husband was very ill and going in and out of the hospital. She feared she was going to lose him, but Belleair provided him with excellent care and therapy. The nursing staff was outstanding and treated him as if he was part of their family. Petra is very thankful for their assistance.
I made a journey from the east coast to the west coast to visit my daughter. Unfortunately, I had a very bad fall and broke my shoulder. As time passed, I had other accidents and was taken to Belleair each time. Initially, I was feeling lonely and depressed due to being away from my family, but eventually I found a new family in the staff and residents at Belleair. They were kind, understanding, and cheerful. I now realize that this is my true home and I am very content with the people I live with.
I am thankful that my mom was able to leave this facility on Friday. She had suffered a stroke and we were very concerned that she would not be able to leave the rehabilitation center. We had been at a different one prior and it was awful. She was so miserable there. Although it is not the same as being home, this place is much better due to the care and attention from Katie, Dale, and the nurses on the 100 floor.
My mom had a great experience with the therapy department at Belleair. Her doctor sent her there after her surgery and Karen, the driver who took her to her appointments, was very kind and caring.
Our mission is to provide our patients with the highest quality of care and services. We strive to provide a safe and comfortable environment for our patients and their families. We are committed to providing our patients with the best possible care and services. We strive to create a positive and supportive atmosphere for our patients and their families.
We are committed to ensuring that excellence and compassion are part of our daily lives in our facility. We firmly believe that caring for the elderly and disabled is a calling, and we are committed to answering that call.

Belleair Health Care Center: Reviews
Fantastic experience: Mom was sent to this facility to recover her physical strength after a heart attack. The staff has been very helpful and supportive. The food does not look great, but what institutional food looks good? She looks very comfortable in her room. She’s in with a permanent resident who does crafts and knitting so mom has a friend with similar interests now. It seems nice and they say that there are otters in the pond out front.
Thank you so much, Bruce, for your wonderful review! We’re glad to hear that your mom is finding comfort here and has found a friend with shared interests. Our team is dedicated to providing a supportive environment, and knowing that your mom is happy means a lot to us. We hope you and your family have a Happy Holiday season. We'll strive to work on our food presentation. :)
Fantastic experience: This is my first experience with a skilled nursing facility and I can say I am 100% satisfied and very impressed with the staff and the facility. They treat my mom very well. I’m very happy that she is there and the care is top-notch. Definitely five stars.
Thank you so much for the wonderful review, Jesse! We're delighted to hear that you are impressed with our staff and facility and that your mom is receiving top-notch care. It’s an honor to be trusted with her care, and we truly appreciate your kind words.
Negative experience: My husband was lack of care kept having uti from not cleaning him properly an kept trying to discharging him because of his confusion from a stroke refused to give him the therapy he need an took out his appointment for his pacemaker replacement
Negative experience: After a fall, my 72 year old wife, who also has cognitive issues was brought here from a nearby hospital at 5pm. The only interaction we recieved was a polite but uninformed visiting nurse who took our information but couldn't answer any specific questions about the facility. The attendant on duty was also very polite but had limited knowledge about specifics. We were just dropped off and left to fend for ourselves. No orientation, nothing. We requested oxygen as my wife also suffers from COPD and uses it at home. It was never provided. She was not given her proper nighttime meds. By 10 am the next day, we had still not heard from anyone in administration so we decided to leave.
Negative experience: I hardly write reviews but I do not recommend this facility. Very delayed and poor communication/responses and execution of my dad’s needs. Seems like they’re in it for the money versus what is best for the patient…
Fantastic experience: I am very impressed with Bellair health Care Center my mother is there and they’re working hard to get her transferred to another skilled nursing facility to be with her husband. I’m very impressed with their quick response
Fantastic experience: My wife and I loved everything about her stay. Therapy got her walking within two weeks. She had 1 concern about the telephone but maintained replaced it immediately. If my wife or if I have to have a stay for therapy we will be returned. Management seemed to really care about their residents and fixed things immediately. 5 stars!!!
Fantastic experience: Management was extremely quick at responding to all of my very insignificant concerns. They made sure I was comfortable. They made my appointments for me and arrange transportation and I was able to see my doctor and ARNP weekly/as needed. I would return if I needed to.
Negative experience: It was one of the worst nights of my life. It rates no stars. Nightmare
Negative experience: Why are the dietary staff working 7 days In a row shifts regularly? This is very wrong and needs addressed immediately.
Fantastic experience: Very clean facility and the staff is caring & professional. Thank you
Fantastic experience: My husband has been at Belleair Health Care for three months. It is by far the best rehabilitation facility in the county. I have toured many of them. Initially he went to two others that were neglectful & were reported. Everyone there treats the residents like family & care how they improve. The food is delicious with many meal choice alternatives. Never has an odor problem like some facilities. High praises to the nurses, CNAs, kitchen staff, housekeeping, physical therapists, receptionist David, case manager Craig & office staff. Special thanks to nurse Donna and CNAs Sophie, Jackie & Ann for going the extra mile.
Negative experience: The most incompetent nurses and employees ever. My boyfriend and fiance of 16 years had sepsis and none of the nurses "supposably" didn't notice. I kept asking what the dark red spots on his skin were and they just said they were normal. I went to visit him after work and he was laying on the floor with eight staff members trying to wake him up. One nurse was slapping him in the face yelling wake up . I wanted to slap her in the face. When the ambulance showed up one of the paramedics told me he had Sepsis. He died 3 days later in the hospital. Yes the place was clean but Katie and the the so-called staff were just looking for another check from Medicare. I could go on and on and the sad thing is, he was only there for 3 days. Staff laying in the empty bed next to him, no personal hygiene care, no food brought to him until I would go ask why. Why they are still operating in business is beyond me. Please avoid this place. Understaffed and gross negligence.
Fantastic experience: Excellent group of skilled therapist with big hearts
Fantastic experience: My dad has been in and out of rehabs the past 4 months. I looked and experienced different facilities with him. From the moment I walked through these front doors my heart had peace. The nurses are amazing and have treated my father with grace and dignity. The food is great. I’m not having to buy 3 meals a day or wait hours for someone to answer a call light. They are there immediately. It’s the first time I have slept peacefully because I know he is in wonderfully hands. I spend a few hours a day and watch him with therapy and the therapist are amazing. They all have great sense of humor . It’s an older facility but is clean and decorated with beautiful colors. They are meeting all his needs exceptionally and he is happy that he is in a nice, caring place
Fantastic experience: We were on vacation in the area when my father-in-law fell and broke three ribs. We needed help and Belleair came through for us and then went out of our way to help us transfer back to a facility near our home. The staff was super friendly and helpful.
Fantastic experience: I just brought my 86 year old mom home today after eight days at Belleair Health Center and we are so grateful for her incredible care. Everyone on the staff from the front desk, the nurses, the aids, the doctor, her case manager, the physical and occupational therapists, and the activities workers treated us all with kindness, respect, and empathy. I wish I could name them all, a few names that come to mind are Jamonica, James, Ashley, Maria, Creig, Nick, Karen, Craig, Emaline, David and the list goes on. During such trying times, it was so touching to see people acting with compassion and enthusiasm for their extremely difficult jobs. I cannot thank you all enough for your help. My mom is now stronger than she has been in years and our entire family is truly grateful.
Fantastic experience: These folks are spot on. My wife passed away yesterday but her best was with Bellair. Thank you for Everthing Katie.
We are so sorry for your loss. Ty for your kind words. We will pass it along to our staff.
Negative experience: If you care about the quality of treatment your loved one is going to get in their recovery ....do NOT choose this facility ,!!
Negative experience: I have been a resident at this facility for 7 months. In that time I have had food taken from the refrigerator snacks stolen from my nightstand and finally 16.00 disappeared from my nightstand. I would not recommend this facility to anyone who has a loved one that needs proper care. The people that are supposed to care for you can't be trusted. This place is woefully understaffed. Only the rehab department is to be applauded. I do not take any pleasure in writing this. But the truth has to come out.
Negative experience: The best thing I noticed was that it was clean. A few staff were good at their job the rest should be ashamed.I would never allow a friend or family member to be admitted there. This is nothing more than a warehouse for the infirmed. Skilled nursing is rare. Indifference best describes it.
Fantastic experience: My 87-year-old mother broke her hip in Late July of 2022. After getting out of the hospital she spent a couple of weeks at the Belleair Health Care Center. All of the medical and administrative staff went out of their way to make my mother comfortable while they helped her along on her path to recovery. After a couple of weeks mom was ready to leave and she's back home now doing well. I'm hoping that my mom never needs rehabilitative care again, but if she does, the Belleair Health Care Center will be our first choice.
Fantastic experience: Caring professionals for short term rehab and long term residency
Positive experience: It was a ok experience sone of the staff is friendly don’t remember names I apologize I’ve come across so many new people in there last few months of back surgery and this was a suggested rehabilitation from my dr whom did my back surgery dr carrie shulman other then i didnt want to be there it was a ok Experience
Fantastic experience: Best experience we could have ever imagined! Thank you!
Fantastic experience: My husband was a patient there after a hospital stay, he needed rehab for balance issues. I was very happy with the all of the staff, from Management to Nurses, Aides, Rehab staff and everyone that works there, all are very dedicated to the patients. The rooms were very large and very clean. My husband was well taken care. Love this place.
Fantastic experience: My mother transferred here from Largo hospital and I finally feel she is getting the care and rehab she needs. The therapists treat my mom really good and she is happy. This is our first time dealing with this situation with covid which has been stressful but she says she is okay and everyone treats her well.
Negative experience: My mom had to call me so I could call them to come to her room. They really don’t check on them much. They let her have soda and chocolate knowing she was a diabetic number one. When I called to check on my mom they didn’t have a clue to what was going on with her. The manager Mike is the rudest person I have ever meant!! When I went to say bye to my mom and try to give her a hug he banned me from coming back for 14 days when they had a patient outside with no mask on!! It’s going to be Mother’s Day wow how sad to treat people that way. Huge mistake to send your loved one there!! Also she has been there a month and has got no where with physical therapy!!
Negative experience: Whatever you do, do not send a loved one here especially if they suffer from Dementia. They are not equipped or trained to deal with this horrific disease. They will accept your loved one and tell you everything can be handled. Yet as soon as the typical behaviors common for Dementia patients surface they will Baker Act them. Then you are left dealing with a commitment to Largo Medical Behavioral Unit till your Insurance no longer pays. After that you are alone in trying to find a Facility that will take someone with a Baker Act on their record.
Fantastic experience: Belleair Staff, Nurses,Cna's,Therapists, Thank You for all you have done for my mom!. Overall Outstanding place. Excellent knowledgeable Case manager helped our family get through. She came out walking this july 2020.
Positive experience: nathan takes pride in his work as the cook there. my mom says it like 4 star dining. she loved the hush puppies, the salad with creamy italian dressing is actually worth writing about. plates are clean. he always gets the order right and makes a good cup of coffee. its shocking how good the food is there.
Fantastic experience: Our Uncle was recently a patient at Bellaire Rehab and our family is grateful for the excellent care and physical therapy he received. He is home now and independent because of the staff at Bellaire. DAvid King, RN
Fantastic experience: I came to visit my daughter who lives on west coast I came in from the east coast. I had a very bad fall and broke my shoulder as time went on I had many other accidents I was brought to belleair every time. At first I was lonely and sad, I missed my family when I decided to stay long term but then I found a second family in the staff and residents at belleair. They are kind, considerate, and joyful. I know now this is truly my home and I am very content with the people I share my home with.
Fantastic experience: Excellent, caring staff. Building is friendly and attractive. The best rehab facility I have been at. Physical Therapy top notch. Highly recommend.
Fantastic experience: I want to say that I have had nothing but great experiences with this facility. I cannot day enough about their care and compassion that they show their residents. Their therapy was excellent and very attentive to the personal goals of their residents. I see how hard working their nurses are and give them so much credit for what they do everyday. Thank you to all at Belleair staff from dietary to housekeeping all the way to the administrator and director of nurses.
Positive experience: Overall I would give This facility a grade of B+. It would be A. Except for the fact That 2nd shift sometimes has a lack of attention to detail..... On a personal note one of the workers(BRIANNA) You are an absolute rock star at your job it has been an absolute pleasure getting to know you a little and most of your coworkers who do a thankless job very very well quick update...2nd shift is approaching being totally useless except for a couple of people they REFUSE to put my dad on the toilet and leave room in total disarray gotta love people who are just there collecting paycheck
Negative experience: Treated my loved one horribly and over-medicated.
Negative experience: Horrible place that talks about patients after they have gone
Negative experience: Don't trust these people.
Negative experience: Horrible place
Negative experience: This place is trash and needs to be shut down. If you care about your family member DO NOT leave them here. The staff and administration are deplorable and don’t deserve to have their jobs. This place needs to be investigated and fined for their POOR treatment of their patients. Shame on them!
Negative experience: Please do not send your loved ones here ! I am warning you this is a terrible place trying to save you pain and suffering . Terrible staff terrible facility and the way we were treated afterwards it’s just beyond words !
Negative experience: If I could I would give this place a negative star! The way they treat their patients and guest is disgusting!
Negative experience: This was by far the worst experience that my wife had to endure. If it were in my power, I would put a lock on the door so others wouldn't have to go through what we did. Rude, uneducated staff, many HIPA violations, drugs dispensed and left without supervision. Staff sleeping while on shift, antiquated beds, patients call buttons and much more. Food at times was inedible, staff didn't even know what they were serving. Personal property missing from my wife's end table. Hospitality, housekeeping sub par. Feces on toilets, trash on floors, need I go on. Those with dementia who couldn't advocate for their selves were a sad site. This place really needs to be investigated by an outside source
Negative experience: I was in Bel air from June to the end of August in 2018. They shut me up with insulin which I did not need gave me many other drugs which I never had before and did not need and do not continue since I left there over a year ago. The staff particularly the night staff used to sleep in my room on a chair and the body's indentation was there I reported it and I was told I was paranoid. I drink tea in the morning and for almost two months I was never served tea because I was told that it was too hard to boil the water they would have to go back to the kitchen for that then I found out that they have hot water on the cart while they are serving the meals. The night staff wood go into the bureau and take what they needed usually just sugar-free candy creams gum and drink my water. I know this for a fact the night staff also used to drug me apps that I would sleep through the night that is how people the staff would know when it would be safe to go into my room to sleep and not be caught by the other staff members because they knew when I was taking the drugs. There was a nurse tech named Talia she tried to serve me in an old cup with old tea bag and old sugar I explained to her that I don't use any sugar fake or real in my tea. She fought with me to just drink it and I refused. She didn't called one of my brothers in New York City to tell him that I was be I was agitated and my brother replied why not just give her the tea in the morning it was a horrible experience everything! I shared a room with a woman who is about 30 years older than me and screamed every night for help. I would press the emergency button and eventually maybe somebody would come and I would explain it's for the lady next door to me or actually right next to me screaming for help. This happened pretty much first month. I could not get any sleep of course because the poor woman was screaming for help and for someone to save her baby. The physical therapist wood take you from your room not take any vitals not care if you had any breakfast or if you took pills and put you in a nautilus room and you took it for 20 minutes to half an hour waiting for piece of Nautilus equipment and all the Nautilus equipment was lineup extremely close to one another as if you were in Manhattan! When I fell on one of them Nautilus machines they put me in a wheelchair end left me in front of the nurses station for about 2 hours I needed water I was so thirsty after fainting they gave me a tiny cup of water but my thirst was so great. I still don't know why. I called my family in New York and told them what had happened and they call the nurses station that was the only way I can get attention and said I need to see a doctor because I have a large lump on my head falling on the Nautilus machine. They never took me to see a doctor. They did however take me to see a psychiatrist that same day that I fell on the Nautilus and the psychiatrist started the conversation by asking how long I was paranoid! I replied I'm not paranoid I never have been paranoid. That was my experience at Bel air rehabilitation center the screaming the smells of urine and feces people over drugged screaming for help the staff laughing in the Halls it was horrible and I was 60 and was told over and over again that I was too young to be there I knew that but I had just had two strokes and one brain surgery. I had to learn to walk again talk again think again etc. To stay at this place for almost 3 months was letting me believe that death is a better alternative them being at belleair rehabilitation center. Also I forgot they said they had no doctor or nurse OnStaff! So when it says their doctors available no not medical doctors and no not nursing that could actually help when somebody smashes their face on a nautilus machine. Everybody covers for everybody else and the patient suffers it is a scene from twilight zone.
Negative experience: The after hours care is almost non existent. The facility is so understaffed at night that patients are often left to try to take themselves to the bathrooms and risk injury/fall or they are hooked up to a catheter so that the nurses aren't bothered. Patients are left in the hallway in wheelchairs and barely attended to. Do not be fooled by the name. Just because it is in Belleair does not mean it is a safe or reputable place for your loved ones who are in need of rehabilitation or long term care.
Negative experience: Do not bring your loved one here. I had to move my mother after three trips to the hospital from this place. The last time, she went into septic shock sitting less than 2 feet from numerous healthcare people. I could tell right away something bad was wrong. There is little to no communication. They overdosed my mom on seizure medications. Knowing the rx was for 250mg b.i.d., they gave her 500 b.i.d. , because "that is what everyone else gets"... Are you kidding me??? If you complain they treat you like the enemy. They have thieves there that take things from your family members. We lost undergarments, shoes, pillow, robe and 3 outfits. I would love to give it 0 stars but you can't. There are two great CNA's there in the evenings, Evelyn and Virginia. The rest of the staff is pretty useless.
Fantastic experience: Very friendly night shift nurses! I would be exhausted but these girls just keep on going!!
Positive experience: My wife had a stroke in early December 2017. She spent two months at Belleair. It was the best place I could have selected in Pinellas County. The rehab was fantastic and the staff was caring and professional. Before selecting a facility, I suggest you visit Belleair.
Fantastic experience: I have worked in many skilled nursing facilities (SNF) in Pinellas County as a per diem, PRN, or contractor. I have held various employment titles as upper management to staff care provider during my two decades as a medical professional. There are many places out there that have doors to a place - unknown. However, BELLEAIR HEALTHCARE meets the requirements and expectations that I have come to suggest - should be the standard for other SNF. I have seen staff / teammembers provide good customer service, patience, and quality care. Patients have on-site access to various doctors and medical support providers which come here to support patient care. Nursing team provides the type of care that loved ones need and families can appreciate. The rehab team is a solid, interactive, progressive and giving group of professionals. Patients could use many types of machines to aid in their rehab process. They are expecting an approval for an expansion to the already large rehab gym this year. The management team guides all of us to be a more informed, safer, patient focused team. I do understand that not every single person, accomadations, or conviences can be meet by even BHCC, but they will make every attempt to provide good customer service. I thank the management team for their approachable style, support of good patient care and it's staff members. Because of this, their annual state surveys have seen successfully high ratings. I have compared other SNF to here. We are clean, fresh and safe. The women in activities department are very busy. They have a bus to help transport patients on outings and help outpatient rehab customers to their appointments. The large shaded patio deck facing the pond is a plus. This gives patients options to "just get some fresh air or catch some sun" or a spot for working out outside throughout the year. These are some the reasons that I stopped travelling around to other SNF for work and came here for fulltime employment.
Fantastic experience: Caring and lovely staff. My mother had a new "family" at Belleair East.
Negative experience: Instead of them assisting dementia patients, they very quickly get rid of anyone who shows any emotion and in my personal experience they ruined the life of my husband and his family by abandoning him to be Baker acted.
