State-of-the-art rehabilitation and therapy centers with highly trained and courteous staff, providing a luxurious vacation-like experience.

The East Bay Rehabilitation Center is committed to continuous improvement and providing secure, high-quality care. It has been awarded a 5-star rating from the Centers for Medicare and Medicaid Services and has been given the Governor's Gold Seal. This program recognizes and rewards nursing homes that have achieved excellence over a long period of time, which helps to stabilize the industry and ensure the physical, social, and emotional wellbeing of nursing home patients. Our facility has also been approved by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO).

Improving quality of life is more than just medical treatments, rehabilitation plans, and technology. It is about being aware of and responding to the individual needs of each patient, and treating them as if they were our own. We take into account the wishes of each patient, and work together with their families to create a plan of care. This approach allows us to provide personalized care that meets both the physical and emotional needs of the patient, and gives their loved ones a great sense of security.

The quality of our services is demonstrated by the tangible results and positive impacts we have had on the community, making us the preferred provider for both inpatient rehabilitation and outpatient therapy. Our inpatient rehabilitation is designed to ensure a safe return home, and our outpatient services provide a range of treatments to help the patient progress and reach their therapy goals.

Physical therapy can help patients become more independent and regain their ability to move around after experiencing orthopedic or neurological issues or injuries. Advanced techniques are used to maximize function and minimize pain. Increasing flexibility, strength, coordination, and muscle control will help you achieve your goals.

Occupational therapy is designed to help people with illnesses or injuries to become more independent in the tasks of everyday life, such as showering, getting dressed, and eating. Our therapists use rehabilitation methods and teach residents how to use special equipment to enhance their ability to move and function.

Speech-language therapy is beneficial for those who need to improve their communication abilities, such as speaking, reasoning, reading, writing, and verbalizing. Additionally, therapists provide assessments and treatments for various types of swallowing difficulties.

My mom had an amazing time here. The staff were very fond of her and she returned their affection. I was reassured knowing she was being taken care of properly.

I would highly recommend this rehabilitation center to anyone looking for excellent service. I must give a special acknowledgement to Haley and Marvin. Haley was incredibly helpful in getting my mother admitted, and she was always available to answer my questions even after working hours. Marvin is also amazing; he is kind, caring, and very efficient. I give this rehab a perfect 10 rating. All the praise goes to the incredible staff and how they treat our family like their own.

I am incredibly grateful for the care my father received during his two-month stay. Everyone we interacted with was courteous, competent, and understanding. The nurses, physical therapists, aides, and social services personnel were all incredibly compassionate and made sure that my father was content, even making him a sandwich when he could not eat the dinner one night. The nursing staff was always available by phone to answer any queries I had or to provide me with updates on my dad when I was unable to visit him. If your loved one needs skilled care, this is a great option.

My father has been living in this place for the past five years. The staff, nurses, and therapists are very kind and caring. The current Administrator, Nate, is very devoted and visits at least twice or three times a day to check on him. The Director of Nursing, Steve, is an incredibly caring person who always keeps the family informed of any changes in care. It is a truly wonderful place.

Our aim is to provide compassionate care and specialized rehabilitative services to those in need, and East Bay Rehabilitation Center is committed to being the leading provider of health care and rehabilitative services in the region.

We are committed to ensuring that excellence and compassion are part of our daily lives in our facility. We strongly believe that caring for the elderly and those with disabilities is a calling, and we are dedicated to responding to it.

Fantastic experience: I am so happy that we were able to find a bed for my mother and move her to this location as the place that she was in for over 4 years was just getting worse and worse, worse care, dirtier etc.and it was in evacuation zone which was absolutely horrible to have her in all those residents move twice for the two hurricanes this year.

Negative experience: THIS FACILITY IS AWFUL, DO NOT PUT YOUR RELATIVES IN THIS PLACE , UNLESS YOU WANT THEM TO DIE QUICKER. MY MOTHER WAS JUST ADMITTED INTO LARGO MED FOR NEGELECT. LOW BLOOD PRESSURE FROM BEING OVERMEDICATED, UTI, OFF THE CHART SUGER LEVELS, DEHYDRATION, BED SORES, COMPROMISED OXYGEN LEVELS.. THEY TELL YOU ONE THING, BUT THERE ACTUALLY NOT DOING IT. FOOD IS DISGUSTING

Negative experience: My mom was placed here for rehab after breaking her hip. The therapy was great but I can say the rest not so much. The food is terrible. They offer alternatives but you are lucky if they actually bring it. The place itself is so outdated. Rooms are small and 2 people in them with wheelchairs is very difficult to navigate. The staff, although maybe nice, is slow. My mom asked for a pain med at 2:30pm and got it at 10 PM one night. Meanwhile I see nurses by the nurses station all talking together. In the 3 weeks she was there they changed her sheets once and that is because I asked. I am the one who actually changed them after they brought them to the room. She asked for her showers to be earlier like by 6pm and never happened. She got one sponge bath in 3 weeks. My mom is very sweet and never complains or bothers anyone. I of course had to make any requests. I was a CNA in NY and personal care was done daily on any resident. I guess not here in FL. Her sugar was over 300 and I had to call there to ask why they were not giving her insulin. The nurse supervisor told me he was not sure why they discontinued it and made sure to fix that. She has slipper socks she came with from the hospital since she was not wearing shoes. I asked them once to possibly get her a pair. That was it 1 pair, same pair all day everyday never washed! I went and asked for another and was told to go to the laundry. I asked me? Fine where? Then the girl said she would get them but guess what? Nope never happened. The overnight nurse my mom says was awesome. Anytime she rang the button he was there and brought pain meds immediately. Not sure how he could take care of things fast yet day staff couldn't. All in all if anyone in my family needs a rehab again we will stay far away from this one. I advise you to do the same.

Fantastic experience: Thank you East Bay Rehab! My friend was in a tough spot. Long hospital stay and no clue what to do next. He really just wanted to get home. He suffered a hip fracture and needed rehab they said. Lots of choices were given at the hospital for rehabs after. We toured the facility for our friend and was instantly impressed on the cleanliness and friendliness of the staff as we walked through. Needless to say we chose East Bay. Excellent rehab and all around care. He has a dog at home that we were looking after and they welcomed those visits for Bug! This really encouraged him to get home. So many thanks! 3 weeks later he is home and playing with Bug!

East Bay Rehabilitation Center

Thank you for your kind words, Stacy! We're so glad to hear that our clean facility and friendly staff made a positive impact on your friend's recovery and that Bug's visits encouraged him. It's wonderful to know he's back at home and enjoying time with Bug again!

Negative experience: I have a love one here and the nursing don't care. They half bath him. Don't give him his food and the rudeness is insane please don't send your love ones here. Try home care or another avenue of care.

Negative experience: Staff here doesn't care about patient care when they are over medicating the patients and not listening to family to not over medicate my partner. He had2 falls within 10 hours of each fall and staffing making excuses instead reprimanded the nursing staff. The room having bugs and roaches where no one want to clean properly. When collecting his belongings, money missing, clothes missing and medication missing. Guess this facility is ok with stealing from the residents. BEWARE, YOUR LOVE ONES WILL BE AT RISK FOR THEIF, MISTREATMENT AND LIVE IN FILTH. Staff should be fired and this facility should be shut down.

Fantastic experience: My mother was in rehab for about 4 weeks. There she received good care and the staff were attentive. From the time my daughter and I first checked out the facility, we liked it very much. Very clean and pleasant and the air smelled nice..Katie Crawford (transitional care manager) was very knowledgeable while we toured the facility. I cannot speak highly enough especially of the Physical Therapy department. James and Lisa really motivated my mom in doing the therapy exercises even though my mom was not happy about it! Keep up the good work. Angie Barbaro

Negative experience: My mother stayed at Eastbay for 6 weeks. The day after she got home I had to take her to the emergency room where she was diagnosed with COVID and passed 2 days later. Not only did Eastbay fail to protect her from COVID now they have a lien on her estate for $1628.63

Fantastic experience: Two and a half years ago, our family faced a challenging journey when my dad had a stroke. After heavy duty amounts of research of nursing and care facilities in the Tampa, Florida area, I found East Bay. East Bay came with high praises, and they exceeded them. The caring staff stepped in with compassion, guiding us through this tough time with their dedication. Their concern for my dad's well-being was clear in every interaction. They greeted him warmly and made sure he got his regular time at the gym and social circles (which he may have hesitated to admit, but he loved). Therapists, nurses, and staff worked seamlessly, creating a personalized plan for my dad's needs—both physical and mental. Their patience and encouragement brought comfort to him. For top-notch care with genuine compassion, I wholeheartedly recommend East Bay Rehabilitation. The facility itself is well-located with a pretty outdoor courtyard that my dad loved. Thank you for being the beacon of hope and healing our family needed. Sincerely, Pat Healy and Family ❤️

East Bay Rehabilitation Center

Thank you so much, Matthew, for your heartfelt review! We're thrilled to hear that our compassionate staff and personalized care plan made such a positive impact on your dad's recovery. It means the world to us to know we could be the beacon of hope your family needed.

Fantastic experience: Wonderful outpatient pulmonary rehab program. When I started, I was in a wheelchair. I can now walk 30 minutes or more. It's not just physical therapy but includes pulmonary education. I've learned so much that will help my breathing through out the rest of my life. This is the only rehab center, I'm aware of, that has a certified pulmonary therapist on staff. I can't say enough good things about JoAnne and everyone I've come in contact with there.

Fantastic experience: My husband has been in Eastbay for more than 20 months. The staff the administration the physical therapists have all gone out of their way to make him as comfortable as possible. They are always responsive to the families requests and have made this trying time in our lives so much easier. The activities staff is phenomenal, there’s always something going on for the patients to do. They even had a live Elvis impersonator entertain the residence. I just want to say that after some of the horror stories I’ve heard about rehabilitation centers I feel blessed that my family found Eastbay.

East Bay Rehabilitation Center

Thank you, Patricia! We're so glad to hear that our staff and activities have made this time easier for you and your husband. We strive to provide the best care and it's heartening to know that we've met your expectations.

Negative experience: Jennifer from admissions called me to get information on my mom. My mother is in the hospital seeking rehabilitation. She told me she would be accepted there. But after 21 days she would have a co-pay. I called my mom and she was very happy to go there. Though her medical plan pays 100% of her bills. This morning I was told she was denied of care. My mom is very devastated now, so am I. I advise any Afiican American individuals, or people of color. Do not try to get into this place. They discriminate based on race and wealth.

Fantastic experience: My experience with east bay rehabilitation has been excellent. The staff is very professional. They go out of their way to make my 97 year old mom comfortable. Fantastic experience: My Mom stayed at East Bay about 8 years ago and my Dad stayed there from October 2021 until he recently passed. Some of the same people who cared for my Mom also cared for my Dad. During his long life my Dad always took care of himself and he was resistant at first to the level of care he needed. The staff was very caring, patient, attentive and kind and became our extended family during the last year of his life. We developed a great relationship with everyone who worked there. My family is so grateful for the way they cared for our Dad.

East Bay Rehabilitation Center

Thank you so much for your kind words, Cyndi. We're honored to have provided care for both your Mom and Dad, and we're glad to hear that our staff could be there for your family during such important times. Your appreciation means the world to us.

Negative experience: My 89 year old mom was in this facility after time spent in the hospital for Covid-19. The first week she was at East Bay Rehab she seemed upbeat and very motivated. The 2nd week she seemed more anxious and was somewhat agitated. By the beginning of her 3rd week she really wanted to go home. My mom came home on a Monday and died the next Saturday. The staff was nice enough, but communication, organization, and training was very poor. I think in retrospect my mom may have had a stroke while in rehab. Please find a different place for your loved one.

Negative experience: Absolute worst nursing facility . My grandmother passed away here and the staff was not comforting during her stay . The only good thing about the place is the DON Steve batten if he is still there . He is kind an compassionate . The nurses were horrible !!!

Negative experience: Father was at East Bay for 5 weeks. He got decent care. Some of the staff was friendly. They keep the building clean. Communication could have been a little better.

Fantastic experience: My father just spent 2 months there and I cannot say enough wonderful things about the care he received. Everyone we came in contact with was courteous, professional and COMPASSIONATE, from the nurses, physical therapists, aides, and social services were all very kind, and went above and beyond to make sure my father had whatever he needed to be comfortable, including making him a sandwich when he was unable to eat what was served for dinner one night. The nursing staff was always available by phone to answer questions or updste me on my Dad when I could get in to see him If your loved one needs skilled care, this is a great option.

Positive experience: My wife has been a patient at East Bay for almost 2 months now and about the only one thing that is constantly good is the rehsb department the nursing and nurses aids are sub par at best and the kitchen should hire someone to run it that actually knows what they are doing. I feel really bad for the permanent patient's that cant speak for themselves .i was lied to by the admission girl about number of therapy hours a day and was told they would do their best to keep my wife in a private room they have 3 or 4 other rooms with empty beds but choose to keep putting them in my wifes room.so just remember what they tell you is not what you are going to get.

Negative experience: "0" stars! and I mean it. I had to put one for this web site to allow me to post. Many of the staff have very aloof attitudes and don't seem to care regarding the patients. When my wife was first admitted, I talked with the nurse regarding her medication and he went off about his orders and said I would have to speak with the head nurse. For the rest of the week that nurse never acknowledged me even when we passed directly in the hallway. My wife rang for help to go to the bathroom and it took 45 minutes for and aide to respond. This time was verified by my wife's roommate. Needless to say, she messed herself! She was served BBQ chicken and when I started to cut it up for her, there was still red blood inside. I complained to the person who brought the food and she came and took it away. My wife ordered a second meal and when that was brought in it was just thrown down and the lady walked out in a huff. There was no silverware so I went out and asked politely for some. The lady didn't acknowledge me and just turned and walked away. The nurse with an attitude was right there and said nothing. I followed the woman and she just grabbed some silverware off of another tray and shoved it at me with a bad attitude, saying nothing. That night my wife rang for help to go to the bathroom and waited 1 1/2 hours. Yes, 1 1/2 hours. This morning no one would come and she messed her pants. Finally someone came to change her. She rang again for help to go to the bathroom and no one came and she messed herself again. The aide came in after that and said, "I just changed you. You will have to wait". Making my wife sit in her excrement for punishment? What kind of person can do that? Not the kind that I will tolerate in caring for my sick wife.

Negative experience: I have been in 4 facilities since contracting this disease. This is by far the worst one staff and aid wise. Not all of them, but a great nunber of them seem less than motivated to care for patients. Call lights seem to mean very little to most. I have messed myself twice waiting for bed pan and then to get cleaned up. Their rehab and therapists are top notch and i would highly recommend that dept.

Negative experience: this the worst place I did not like it when my dad was here but thought it was because he was very old they practically ignored him my husband had knee replacement-- they treated it like NO BIG DEAL they asked me if he had fallen at home I don't think they read the admittance sheet there were 2 nurses on the week-end that were GREAT unfortunately that was only 2 days

Fantastic experience: My aunt and uncle have recently moved into East Bay Rehab due to their health issues. I'm writing this on behalf of my parents. My mom has been very pleased with this center and its staff and wanted to make sure her review was posted. Michelle greets everyone with a smile and a boost of sunshine. Faith has a bubbly spirit and you can't help leaving her presence (and Michelle's) without feeling better about the world. There are so many others who make visitors and residents feel welcomed upon arrival. Renee makes certain the residents' belongings are safe with her diligent eye for detail. The care is top notch and Shirley is a noteworthy night nurse, without taking anything away from all the other great health care providers at this facility. You can rest assured knowing your family members being well taken care of.
