Legend Oaks Healthcare and Rehabilitation - Garland

Negative experience: I was recently a resident at Legend Oaks. The rehab dept is fabulous. That is the only reason the place gets even 1 star. Big change over in staff lately and who they have gotten in are horrible. Can't or won't do their jobs. Weekly staff is good. CC, who works halls 3&4 as an aide is Great!!! However, after 2pm and weekends, it is a 'Shit Show'. The linens never smell clean and the food is the absolute worst that I have ever experienced.

Fantastic experience: I have been contracted at Legend Oaks for 6 months now providing transportion services. I am here Monday through Friday interacting with clients and staff The experience for me has been great. Staff is very polite and the clients are their number 1 Priority. Clients have nothing but great things to say about staff. Therapist are very hands on and very respectful when it comes to the clients. If you have a love one that’s needing proper care I highly recommend Legend Oaks.

Negative experience: Worked a prn day in July. Never got paid. To this day, I have been trying to get my check. Spoke to the HR lady. Stated they sent my check in mail. Never received it. Spoke to her again in September. She told me that once my check arrives, she would call me and I can come pick up my check. Spoke to someone AGAIN today, she said my check was sent in the mail by HR on Tuesday, after we already discussed that I will pick up the check once it arrives. Very shady business. Be careful of working at this place. Finally received the check 10/20/24 and it bounced!!

Fantastic experience: My name is Victor, I have been a CNA for more than (4) four years here in legend oaks of garland and now I was promoted to the position of staffing coordinator. I enjoyed wording here in legend oaks of garland because is a place I will always recommend for our seniors. Its serene surroundings and the staff here are awesome and the residents are great. I feel happy and relaxed working in legend because it is a home to live in.

Negative experience: Do NOT I repeat do NOT send your loved ones to this hell hole! With a few exceptions, the staff is rude, unprofessional and uncaring. To name only a few incidents, my loved one was not given pain meds on time, not bathed according to the schedule, room was not cleaned daily, pan of urine left on the bedside floor, wound was neglected and got worse during the stay. Ben the meds nurse should be disbarred and never allowed to work again anywhere! He is rude and unprofessional! Stay far, far away from this place!

Fantastic experience: Great facility and welcoming staff!

Negative experience: There are few good nursing homes in the area and this isn't one of them. Since being there my husband has had diarrhea. They probably aren't clean enough. I saw a male nurse or therapist or tech sneeze right on the water and tea pitchers yesterday. I wish we had money to get decent care. Update: he's been waiting hours to get changed. I know this place makes money hand over fist so I don't know why they don't either pay people more or hire more people. Update again: nothing has changed. Still neglecting patients. Got morning meds at 1:00 PM. Someone will get sick and die here and I'm sure it will get covered over.

Fantastic experience: This facility is awesome, the staff is great, very caring and helpful. I recommend this facility to anyone.

Negative experience: I would give less stars. This facility failed on every level possible for my family. From something as simple as not having drinking water available in the cafeteria daily all the way to NOT being honest about medicines administered to my loved one. I regret having had my father here as they were neglectful in their duties even with me present ever day to be his advocate.

The building is so clean and the residents are free to move around. I will not hesitate to bring my family in this facility.

Negative experience: Place is terrible. Care is not any better.My mom has dementia and they kept changing her meds. She was on hospice for extra care but facility Dr and DON decided to take her off day meds which was when she had her sun downing. Almost killed her with over dose of expired medication. State sited them for chemical restraining her. She was out 3 days. They kicked her out day before Thanksgiving saying she was a threat to a resident Would not give 30 day notice. Called State and Omsbudsman as usual they sided with facility. Don’t ever put your loved one there.I had to call Garland Police Department twice for welfare check as the phones were not answered by nurses. Too much turnover. DON is a pompous jerk. It’s a hell hole.

Negative experience: RUNNNNN!!!!! DO NOT PUT YOUR LOVED ONES HERE… my mom called me crying saying she’s being mistreated someone twisted her arm , she was left in a pee soaked bed for over 4 hrs and was told she was not allowed to have her phone for 2 days. I’m thinking okay maybe she’s over exaggerating until I show up and this place is a madhouse and not bc there’s so many people but because there’s nobody here, I was walking around for 15 min and couldn’t find anyone after finally finding my mom and getting to her room we were getting her out , a lady across the hall pulled me into her room to tell me she’s been mistreated as well and her loved ones are coming to get her too , a door alarm was going off for 45 min until someone finally came to silence it a lady was in the back hallway screaming for help over and over again and nobody came and when the guy did come he told her to be quiet and walked off. NEVER will any of my family EVER be back at this hellish place. Will be posting videos all over social media as google won’t let me leave the video here

Positive experience: Under new management--making progress, rapidly, in an effort to live up to the pics and promises. Great staff--just not enough of 'em. Keep your eyes open and glasses on, especially nights & weekends! nts.and weekends. hrs.

Negative experience: Called looking for a place for my father. Receptionist answered with a mouthful of food. Kept talking and eating. Barely understood her. And absolutely no help at all.

Negative experience: I was very disappointed with my father’s stay in this facility. The physical therapy was minimal. The food was horrible and not for a restricted diet. I was told that there were diet options available but they were never given to us. The information booklet says there will be a morning snack and an evening snack. Those do not exist. They preferred to keep him in a diaper rather than assist him to the bathroom. They never got his insulin dosage corrected even after I brought it to their attention. When he was discharged, I had to wheel him out myself and load him into the car with no offer of assistance from any of the staff. I would NOT recommend this facility.

Negative experience: BELIEVE THE NEGATIVE REVIEWS! DO NOT DISREGARD THEM! If your family never is ill and cannot look after themselves without help.. DO NOT SEND THEM HERE. My father spent the last 3 days of his life suffering in this place. Instead of being on oxygen he was on a concentrator when he clearly expressed it was not enough. This place DOES NOT have a resident doctor. Their doctor only comes in certain days.The excuse what that since the doctor was not in they could not administer or change anything. He laid on his feces for hours. Food was undercooked and was on the incorrect food diet. He was left without checking on him for long periods of times as he would call me every time he needed something so i can call the front and have someone check. Don’t waste your money and help your family have a good quality of life if they are ill. DO NOT SEND YOUR LOVE ONE TO LEGEND OAKS…YOU WILL REGRET IT!!

Negative experience: I beg with people to choose another rehab center for their loving family member. This place looks very nice at first look. Clean and great smelling. They are different sections for different needs. If you can mostly do things for you and just need rehab then you are in luck. if you are unable to help yourself and doctor deems you just still need rehab with help… you are not in luck. This place does not have a doctor 24/7. The doctor comes in when he is schedule too only. So your love one is left with the help they have but again, that is if that tech or nurse is actually caring and not fed up for having that job. Pay attention to all the reviews that show why this place earned and deserved zero starts.

Negative experience: ***updated 10/08/2021: Stay away from this rehab!! My father is now at Garnett Hill and they are terrific! Legend Oaks is horrible. Updated: the administrator listened to our concerns seriously and his people started to provide better care today. I leave it at 3 stars for now and will update accordingly after my father is discharged.

Negative experience: I just moved here for a week to finish wound rehab. At this point it is the most unprofessional place I've ever been. I was supposed to get a dressing changed since they didn't have a machine I needed. It's going on 3 hours and still haven't gotten it. They dumped me in my room, no one wears name tags. I hope that the foam in my wound doesn't become covered in dried blood. Waiting to see how long I have to wait for the medical attention I need. I was told by (I believe the head nurse) that I will meet everyone who will handle my care in the morning. My son stopped me from going somewhere due to covid. It might have been a better place to go. Pretty much every meal is cold, then you have to wait for it to be heated. I had to change my own bed linen & ask to get that bed linen. I've still never met the admissions person. The Don talked to me & stated she had 133 people to deal with. Then I was told there 90. Don stated it was the hospital messed up on paper work being sent over. I'm begining to believe it was her people. I understand that there are people who are more in need, but give me a break.

Fantastic experience: I love how much the staff cares about their residents, and go through great lengths to make sure they are happy and well cared for. They listen, work with and support families. The community is beautiful and clean. Keep up the good work!

Negative experience: Awful place!!!

Negative experience: Completely unimpressed. Slow service. Poor food. Waiting for hours to get much needed medication. Supposed to be rehab but doesn't even have ice packs.

Negative experience: They are the worst nursing home there is. My mom has fallen 5 times since being there. She was released today. They are money Hungry. Under staffed. I feel sorry the residents that have to stay there.

Negative experience: They allowed a urinary tract infection to get so bad that my mom was listing (leaning) to one side and wouldn't eat. They were so intent to transfer her rather than caring for her medically (because they aren't properly set up by their own admission to handle patients with even moderate dementia). Anyone with two eyes could see she was very sick and very confused (UTI and confusion go together). They dumped her off at another facility who believed her inability to walk normally by that time was normal for her. She fell within the first 2 hours and damaged her neck, bruised her elbows and knees. She spent a week in the hospital and an additional 4 weeks in Daily PT just to get her chin off her chest. She went from being able to do most activities to none overnight. The Drs at the hospital confirmed she had the UTI for some time for it to progress to that level. This means Legend knew or should have known something was very wrong. They are not equipped for this. All, this is neglect. While there on an evening visit before all this, I watched as the ambulance came for another patient while one of the staff berated another (both in broken English) for messing up a medication. Guys, don't let new buildings fool you. All nursing homes were all once new. All the ones you hear about on the news and in attorney ads, were all new too at one time (many still are). What does new investment for financial returns have to do with care? Buyer beware. And make no mistake, this is BIG BUSINESS. This is not 501C3 charity work here. Please don't confuse these guys for Catholic Charities or Samaritan's Purse. They are unaccountable when it comes to care, using sly maneuvering to get around the law. Be careful.

Fantastic experience: So blessed our parents are in this nursing home facility! The nurses, staff, doctors, therapists, CNA’s and all the employees employed in Legend Oaks Garland treated my parents the way our family treats them and that is with unconditional love, unconditional care and tenderness and lots of unconditional patience! We personally would like to thank Kristy, Shameka, Jason, Rex, Maria, Marie, Mary, Cici, Lana, Otis, Eddie, Tina, Peggy, Doris, Jeri, Cassandra, Indu, the ever fun, beautiful and ever helpful front desk Cynthia, our awesome comrade Charlie and to all the amazing staff, nurses, therapists, doctors and admins of Legend Oaks, with grateful hearts we thank you a million times for taking care of our parents!! Additionally, would like also to emphasize of what a wonderful nursing facility Legend Oaks in Garland is, it’s very clean, well maintain and such a beautiful facility and best of all yummy fresh tasting food!! Fantastic experience: Our entire family had the best experience at Legends! A great name for a legendary experience. My dear MIL broke her arm at the tender age of 89. After post acute medical care, these folks came in and finished the job. Do estecits matter? Sort of...like the smell? Having visited many a facility during her many maladies, I can tell you that smell does matter. If you want to run, not walk, out of a facility, run for the door. At legends, they have addressed the minor and major accidents with flooring that can be sanitized properly, that isn't slippery. When the grandbabies hold thier nose, it's not a good sign. Although Legens is a new build, I don't see thier efforts declining any time soon. My MIL was recovering from a spiral fracture, but her hall mates ranged from Alzheimers patients to cancer survivors. It seemed to me that no matter the circumstances, they were ready for any issue that arose. The food was good and plentiful. I had lunch and dinner there and was pleasantly surprised at their offerings. At $5 per visitor, it was a healthy dining option compared to an unhealthy trip to fast food before a visit. Our mother loved the daily activities and the sneaky way they made her work out so fun! There was a great exercise facility and games that made staying in shape more like play time. There were so many times she missed a movie because she didn't want to cut our visit short. They have an extensive library of media and literature available to everyone. She fell in love with so many of her caretakers and she felt that they cared for her too. Although she was ready to go home after a long 3 months of rehab, she misses them, the interaction, and bonding she had with the other residents. From a caretakers point of view, I couldn't have asked for more. If I were there 24/7, she wouldn't have had as much fun as she did there with so many people at her beckon call. She did love the white glove service, and I feel like she's a little bit spoiled now! Legends of Garland is an upscale facility, filled with dedicated and loving prefessionals. The care and comfort of my MIL was at the forefront of everything they did for her, obviously. The doctors, techs, and Jason, maintenance man( new room phone issues) were so kind and helpful. Christine was a savior and helped us through the ugly issues of billing, yada yada yada. She has a thankless job, but I wanted to make sure to thank her here for her care and consideration. I have her cell phone number! Who does that? My advice to anyone reading this hoping for a light in a black hole of where now for mom and dad is to do your research. Visit a few facilities. Listen to the folks in the dining room and the hallways that are dependent on their professional care. Look, listen, and feel them out. If the staff is on their cell phones, get going. If you are unhappy with where they are now, you can always transfer them to Legends, in a jiffy! We did! I know how hard it is to make all the 'right' decisions all the time. Legends is a solid choice for your loved one. Take it from someone that has been through 6 places over the last 5 years, including a major hospital run rehab in Dallas!

Positive experience: I have a son with Multiple Sclerosis and he is a resident at Legend Oaks. I want to say that for a new place, the staff is really good and the Rehabilitation staff are awesome. The therapists have been really good with Jonny, always checking in on him. I'm sure there have been those employees that have come and gone (mostly because I don't see them anymore). That tells me that if employees don't meet the criteria at Legend Oaks, they don't stay. Legend also has a great kitchen and chefs. Their food is like good home cooking and breakfasts are great! Everyone I've met at Legend Oaks has been friendly, compassionate and loving. The Director of Nurses there are willing to listen to anything I need to say about any subject and they strive to get it right. Have I had issues? Yes. But with good communication those issues have been resolved. Will I have issues in the future? Probably, but I know the administrator cares and he will attend to Jonny's needs. Do I want Jonny to be at Legend Oaks as a resident? No. I want him to get better and someday be independent again. Legend Oaks is helping me to see my dreams come true.

Fantastic experience: Legend Oak have been exceeded my expectations. From the caring nursing staffs to housekeeper, management, therapist, and the cafeteria crews. They are very caring and involved with my family. They always greet us with a smile and a how are you. I am very happy to have my grandfather here. Thank you!

Negative experience: My father has recently been there a few months and since his arrival the facility has lacked to give him proper hygiene care. There has been several times that we have asked to please shower based on their process (3 times a week) but the lower and higher staff seem to have the lack of care of experience to effectively do this small task. There has been other task where they have stated that they did shower my father but after speaking with my dad and taking pictures of his clothes it was very clear that they did not shower and forged the task. If they are being unethical about this, what are the other task they are not being forthcoming? Will write more soon!
