Fantastic experience: My wide had an excellent stay at this facility. The staff took very good care of her and welcomed me. Shannon went above and beyond to help us with her every need and always answered our calls regardless of what hour it was. Edwin always was there to help. The staff over all went above and beyond to care for my wife and always included me. Thank you all for everything

Positive experience: When I first got there I met (Shannon) Social Worker and developed a relationship with her on communication I wasn't introduced around the nursing home my sister clothes didn't return but on a note I left feeling some type of way but I appreciate Shannon getting me through step by step and the RN LPN CNA and PT took care of sister well also thank you all

Fantastic experience: The director and assistant director of nursing, as well as the staff are compassionate and accommodating. My mom went there for respite care only. She took a turn for the worse and she could not go back home. They allowed me to stay overnight with her until the end. The staff brought me a cot and snacks. They checked on me and made sure our environment was comfortable. She was not a resident, but they treated her like one of their own. Thank you for a beautifully bittersweet experience.

Negative experience: Please please do NOT leave your loved one here! This facility is awful. My mom was a resident here for 6 months. The staff is uncaring. The food is barely edible. We had to bring her dinner at least twice a week. They would ignore her when she hit her button for help. She would be left sitting in her own excrement for 2 plus hours. She had money, jewelry and clothes stolen regularly. We had issues with the staff communicating. When it was brought to their attention, they started calling us at 10pm at night to let us know things like my mom had a stomach ache, or had needed a tylenol. Things that didn't warrant a call at 10pm in the evening. For important issues, we had no communication at all. My mom passed away at this facility recently. Her time of death was at 5:02am. We didn't get a call about anything until 5:26am. What happened during that 20 or so minutes? What happened leading up to her death? We never got told anything. Was she not feeling well? Not a single word. Just a phone call that she had passed with zero details. I cannot express it enough, stay clear of this place! No one deserves that kind of treatment. UPDATE: THEY HAVE CURRENT EMPLOYEES LEAVING POSITIVE REVIEWS TO BOOST THEIR SCORE. THE MOST RECENT REVIEW COMES FROM A DIRECTOR OF DIVERSICARE!!!! All you have to do is Google the name. This is so dishonest! This person doesn't even have a loved one at the facility!!!!

Fantastic experience: The management and staff at pleasant valley are very friendly and are quick at answering any questions and finding solutions. I’ve noticed they really care about the residents experience and are always looking for ways to improve.

Fantastic experience: This facility is the cleanest I’ve seen, the residents seem genuinely happy there! I’ve been to other facilities that just feel dreary but pleasant valley has a great energy from staff who care. I went to an event for family and friends of residents and it was a wonderful time and I’m very thankful for management who cares about each resident.

Fantastic experience: One of the best facilities that I have ever been in. The management Is engaged and making sure your loved one is cared for. Clinical wise and therapy, we have had zero issues. If your loved one ever needs a place for rehab or long-term care, this seems to be the place that can do it better than anyone else.

Fantastic experience: When I arrived here I couldnt walk, was in severe pain from my lymphedema foot wound along with various other ailments but I was blessed to have a therapist that inspired me to work through the pain named Amanda, a nurse who patiently worked with my legs named Drusilla,vand a wound nurse who worked diligently to heal my foot wound and keep me laughing! The food service is still a work in progress but Christina is trying to make it better! If you just have to go in rehab I strongly suggest you try this one!

Fantastic experience: I have been to this facility several times. It is very clean, up to date and the staff is amazing. I highly recommend this place for any family member. The activity department is wonderful and they have plenty of activities for the residents. I especially liked that they did the strawberry shortcake day. If you have to choose a place for your love one this is the one.

Fantastic experience: I and my family want to share our gratitude to the staff of Pleasant Valley for the care, kindness, and love they gave my brother "Conrad Wayne Gahagan", who recently passed away from cancer on April 27th. Shannon even came to see Wayne when he was in the hospital. I was blessed to be at my brother's side for the last 11 days of his life here on earth. I had to go to my house and was going to make it back in time to help my brother eat his lunch. When I walked into Wayne's room, one of the aids (Dimaine) was sitting next to Wayne and was feeding him. That blessed me to see that, for I worried about Wayne getting the care he needed. On Wayne's birthday, the staff came into his room and sang happy birthday to him. Again, thank you, Pleasant Valley, for being there for me and my family.

Fantastic experience: My experience was a a great one when I arrived at pleasant valley I came in the back of a ambulance And I couldn’t move my legs within a months time they had me up and walking they are very dedicated to there patients needs and do every thing to reach the patient goals.They are very dedicated to the patient.I enjoyed every day in therapy Hope to go back and visit very soon!

Fantastic experience: We are grateful for a very good experience with Pleasant Valley.. My mom stay was a little over a month there and she regained her vitality back and her strength was improved... She made friends with other residents.. She was especially admirable of ALL the staff and administration, even visiting her in the hospital prior to her discharge.. and she reported the food was very good. The facility atmosphere is warm and friendly and the residents are easily approachable and sociable.

Fantastic experience: Pleasant Valley has a great therapy team with lots of success in transitioning patients home and getting them back to their prior level of functioning. The staff are very well trained and the management team goes above and beyond to meet the needs of patients.

Fantastic experience: Pleasant Valley Healthcare has been a blessing to my family. Our grandmother needed rehab services after she had a hip replacement. I felt the love in the building. We didn’t know what to do and where to start. The hospital sent my family a list of places to tour. I scheduled an appointment with Chana and she walked me through the entire process. I was pleased to know that the my grandmother was treated very well. She loved the food. She raved about a tomato soup that was served. Overall I’m so grateful for the entire time. My grandmother has recommend Pleasant Valley to everyone.

Negative experience: If I could give this place a negative star I would. The staff is so un-experienced and so unprofessional. My mother was referred here after back surgery, never did I imagine these places to look or smell like this and to have ever have anyone in the medical field scream at me like this place did. My mother was pressing the button to call nurse all night and they did not ho help her. Little to say my sister took her home the next day. So my mother was only there 1 night. I can not imagine what other people who do not have any other option go through. Please do not have your loved once go through hell. Do your research on these places. We did not and we truly regret it.

Fantastic experience: Excellent and clean facility. Staff are outstanding. If there was a problem it was quickly and completely solved by Grace and Shannon. Shannon is amazing. She always brightens my day. Cherry is very great everyday. Thank you for amazing service during a difficult time.

Positive experience: My niece has been a resident there for over a year. She moved there from an assisted living facility where she was very unhappy and inactive most of the time. She is much happier now and enjoys many activities and a caring and attentive staff. No facility is perfect, but I feel they do a good job at Valley View.

Negative experience: It is a place that they don't mind interrupting the patient's treatment. The social worker never answers the phone nor does she care to work on the needs of the patients. Mark Woods head of therapists does not answer the mailbox is full. Do not take anyone from your family to this place. Authorities need to remove this place.

I want to give a special praise to Mark Woods, Rehabilitation Director, for his willingness to go above and beyond to answer all our questions, and to Tresea Kennedy, Business Manager, for her compassion and enthusiasm in helping us with my friend's financial needs. She is very knowledgeable, caring and never in a rush to help us find solutions.

Fantastic experience: Phenomenal Experience Director, and a Superb Staff on the cutting edge of rehabilitation. If you’re looking for a Rehab Team who are the experts in what they do, results driven, and has a genuine love and care for their clients this is the place for your family and friends! Your will see, significant improvements, regarding mobility, for you family member!

Positive experience: My dad is getting the rehab care he needs here and they keep his room clean and update on his care. You should always be involved in your loved ones care. They need our voice to help them with the best outcomes. This place has saved my dads life.

Negative experience: I don’t know where to start. This Rehab/Nursing facility needs to be closed down! Staff are rude, unhelpful, understaffed unfriendly,unprofessional, uncaring people! The Social Worker as well! They’re just there for a paycheck not for the patients! Two people were nice. The facility is dirty and the food is disgusting and they don’t bathe some of the patients etc., Pleasant Valley Healthcare needs to hire a new staff “compassionate” people! Health care has gone downhill! ‍⚕️

Fantastic experience: This facility continues to serve the community with a focus on improving the geriatric community. Since new management have taken over they have improved in dietary, housekeeping, nursing and therapy. My uncle was here and made a lot of improvement after his stroke! They truly made him feel at home.

Fantastic experience: So happy that our new flooring install is complete. It looks great and it so easy for the resident’s walkers and wheelchairs to navigate across. Proud of the our current outpatient caseload too. They report that they are making returns to deer hunting and swing dancing with much thanks to the hard word that they’ve put in with our therapy team.

Fantastic experience: My friend had been here for a year now! The staff is consistent and friendly and helpful.They go out of there way to make sure she feels safe and comfortable! Since new management,they have increased activities,which means a lot,very grateful.

Negative experience: New owners July 1st so my old review is gone. Laundry service has become nonexistent and my mothers clothes seem to be lost! Also communication is terrible (no response or unprofessional response)! And I’m very upset my mother has been placed in quarantine even tho she hasn’t tested positive! Just because a employee was positive doesn’t mean everyone is positive. The prior owners never did this. Will update if improvements are made.

Negative experience: My mom was sent here from the hospital for therapy on 7/24 and then back to the hospital on 7/25 and while she was gone for a week back to the hospital all her snacks that were bought for her were taken. Her shoes and some personal items were taken from her room and no one seem to know what happened to them. She did go back from the hospital once again and that's when we noticed all things gone. They replaced some of the snacks but the other things were not ever mentioned. Eventually mom had a relapse and was once again sent to hospital but she did not return to this facility. Most of the staff members there were rude and did not care about answering your concerns. The food was not so great. The place smelled of urine when walking down the halls. Just not a great place to be. Now take it there were 2 aides (Whitney and Wilson) that we did come to appreciate so with them there when mom was there they were a blessing to her. I hope this place gets some much needed attention because we as family members leave out loved ones in your hands. Please be kind and loving!!!!!
