Negative experience: I find patients who need help either getting back up because they took a tumble and no one was out there to help them or they need help getting back to the facility. This doesn’t happen all too often, but it shouldn’t be happening at all when you have staff that actually care for their patients instead of half-assing it.
Negative experience: DO NOT SEND YOUR LOVED ONE HERE!!! my grandmother is 84, has an ongoing infection on her leg that isn’t healing fast due to her diabetes. Had her at John c lincoln then transported here, from the beginning this place is filthy, SEVERELY understaffed and their nurses are so poorly trained. They have literal signs REMINDING them to glove up and to keep good hygiene. She had a call button which was utterly useless it takes hours for anyone to respond to her needs. They’ve had her completely drugged up and hallucinating her entire stay and due to lack of staffing she was laying on the floor for god knows how long- after falling out of her bed. there are no guard rails here.they leave their patients sitting on the toilet while they go assist others or talk in the hallways. None of these people have adequate training or any hints of remorse to check on a patient that is way older than most, more frequently than 4 hours. The filth is unmatched, dust/mold on window seals, ghetto ac unit held against the window with duct tape. They even managed to lose her 2000$ set of dentures!! waste of money, time, and hope. please look elsewhere and if you do choose to send someone here they’re going to need 24/7 watch from family.

Negative experience: My mother was sent here following just having open heart surgery. The staff here is very inattentive. She calls for them to help her to the restroom and it takes them well over an hour to even respond. They don’t respond to the call light. She asked for pain meds in the early evening, didn’t get anything until midnight. Then had to wait until they talked to the Dr to get it put into her chart to get it more frequently even though that was the first time she had even asked for it, though it was her third day there and her surgeon from the hospital had it in her discharge instructions. The food is horrible, to say the least. The nurse put my mom in a wheelchair (which is hard for her to sit in that position to long), so they could add an air mattress due to a pressure sore. They then left her sitting in the chair for a few hours while they took their sweet time. I called and talked to the case manager about my concerns because my mother was begging for us to get her out of there, was told they were going to go talk to her and the staff to rectify the situation, clearly that has not happened. Ironically, after I complained, they decided she should be discharged in a few days…she just had major surgery, I don’t feel she’s ready to be home yet, especially because she’s still having trouble breathing and just yesterday she coughed up a blood clot. But clearly she needs to be somewhere else. She’s only had one day of physical therapy to help her get back on her feet. Am I mad as heck, you betcha!!!! I would give this place zero stars if that was an option!!!!! I would never let another family member go here again! Shame on you Horizons!!!!!!!!!!!
Fantastic experience: Great place if you need their services.
Negative experience: If this facility isn't under heavy investigation then they really should be and ASAP. My mother arrived late evening after 2 major surgery's on her spine. They allowed us in after hours very briefly just to be told we had to leave by 3 or 4 different employees as we made our way down the hall. Pretty upsetting, especially bcuz my mother just arrived & was feeling overwhelmed and alone. Who wouldnt in that dump. Anyhow, there was a child about 8 yrs old playing near the nurse station when we got there, after hours. As we left we saw the same child tucked neatly into a couch. Cool for kids to apparently accompany the parents while they are getting paid to tend to folks trying to recover. Not ok horizon. Also, what times dinner? Nearly 11pm? Bcuz I saw 3 residents in the dining room nearly 11 pm. Definitely unaware of their surroundings sadly, it really isn't right. They had obv been sitting since dinner and it didn't appear these folks would be moved to a more comfortable plc to sleep any time soon. So sad. Big time neglect going on here. My mother is completely aware of EVERYTHING and says she called them around 2 or 3 am to help bcuz she didn't make the potty and was wet with p. Nobody came till they delivered breakfast. This is very very wrong. Again today mother had same issue and waited over 2 hours while sitting in p. Are there laws in plc so these things don't happen to our loved ones? Yes. Yes definitely. And horizon is breaking many of them. This plc needs to pull it together or lower the staff to patient ratio. This is heart breaking. My mother's mind is still fully with her. Can you imagine how they tend to patients that may have some dementia or similar? Folks that may not be aware of what's going on!! That is something to consider when placing your loved ones here. Today I plan to speak with mom about a better facility, she deserves the best and this is not it. Shame on the dire ctor and shame on all the staff pretending all is well, it's far from well here. I understand nursing can be hard and everyone is over worked, this is basics though. Using the bathroom and getting cleaned up for bed. Basics. Come on!

Fantastic experience: Case manager Holly has been absolutely amazing. Her efficiency, empathy and overall command of her job has made my dads experience so much better then all his other rehab facilities and hospitals he’s been to. Highly recommend working with her for any needs for the elderly or disabled. She’s the best
Negative experience: This place is unorganized and unprofessional. I asked to be moved to a different room due to a disruptive patient. Once the patient was discharged she was replaced with someone who had been there several times. This patient was having her family and friends sneak in drugs for her. She stayed constantly with drugs and medication given to her at the facility. She stayed up all hours of the night. She had constant questionable vistors. The staff did not do anything about the situation other than tell you she could not go outside and her vistors would be screen coming in. They never checked the room for additional narcotics I asked to to be placed in another room several times but was ignored by Admistration. The only time I recieved feedback was after I left and they needed to confirm my address to send me a bill. Avoid this place your better off going somewhere else.
Fantastic experience: I love this facility! The staff go above and beyond to help anyway that they can. The leadership team are so friendly and welcoming. The therapy department are top notch! The nursing department from the CNA to RN really know how to provide Excellent customer service! I would recommend this facility to anyone who is needing any type of care from long term care needs to shorter more acute needs. 10/10

Fantastic experience: My mom stayed here for several months. She received such great care. The facility was nice and clean. The staff was very friendly and always cheered my mom up. She would laugh and joke with them on a daily basis. She also stated "the food was good." I would recommend them to care for your love ones.
Negative experience: This place is scary. Under staffed. Help comes too infrequently. Food is terrible (who feeds people who really need help and support cold sandwiches for dinner and tater tots for lunch?). Staff is hard to track down and when you finally do find someone they are ready with an ‘I have no authority,’ or, ‘that’s a different nurses wing.’ Please tell your doctor to send you somewhere else if you need inpatient rehab.

Negative experience: No staff around. Nurse "call" button is useless. Took 6 hours to get a pain pill. Patients screaming for help ignored. One star is too high.Hubby is here for PT to get him walking. ONE time in 2 days they had him stand up by his bed. He got bad bed sores from improper care
Negative experience: My daughters in there they had in a cramped in a tiny room way in the back with no room for her wheelchair or anybody to visit then they temporarily moved into a bigger room towards the front with room for her and visitors so of course they moved her back there because it's to much trouble to take a hoyer lift up front to get her in an out of her chair when she was much happier in the the larger room (instead of where she was much happier in in a lot better mood) seems to they're less concerned with what residents prefer than their own convenience.
Fantastic experience: Horizon post acute so far from what i’ve seen is they care a lot about their patients and staffing. They make sure their patients are comfortable and also having fun with activities. Staff is great! They are very polite and have patience with everyone. I would definitely send one of my family members here !
Fantastic experience: Horizon as I have seen and experienced really does care about all their patients. They do everything to answer call lights in a timely fashion and help anyone who needs it. I’ve seen patience and compassion when sometimes not the easiest situation. all the workers from kitchen to admission, nurses and CNA’s have passion for their work and knows it matters to their patients. They make it as easily as possible for everyone and do their best to make time for all there patients even when their is a lot per section. I would send any loved one, friend or even acquaintance here because I trust the staff fully. As I’ve seen a lot of workers having their family members getting help here already
Negative experience: Much like any care facility, there are wonderful nurses and CNA's that give great care, and the others that are horrible. This facility is no different, and I have witnessed both. The turnover is terrible and some nurses will turn a blind eye to patients illegal activity. The sad thing is, the good staff members suffer with those who choose to do nothing, and the good ultimately get blamed. I'm surprised they are still open. State really needs to keep an eye on this place.

Fantastic experience: Holly was truly amazing. She went above and beyond to make our transition smooth and was incredibly attentive to our needs. Throughout our stay, she checked in with us daily to ensure that everything was taken care of. She was always readily available whenever we needed her. The place was consistently clean and our entire family, including my toddler, felt welcomed and comfortable. One aspect that I particularly appreciated was that even our furry family members were allowed to visit as long as their vaccines were up to date. I highly recommend this location, as it is situated in a peaceful and safe neighborhood.
Fantastic experience: Horizon really makes people feel so worthy of living. The staff cared so much for me during my stay. They got me better and I'm now getting to go home and that's what matters.
Negative experience: I wouldn't give this place any stars cos my boyfriend father went there, and we had nothing but problems due to his hearing aid being gone and no one seems to know where it was or where it went!! Then we brought him another hearing aide and I realized that his personal walker had been taking to another person's room and when I asked for it at the nurses station Noone seem to know where it went!! Apparently the other resident was trying to leave so they just took my boyfriends father walker , the nurses looked at me like I was off my rocker , but they have plenty of walkers and wheelchairs not to be taken someone else property!!
Positive experience: During my stay at Horizon I received exceptional care from the nurses, CNAs and other personnel. They were attentive, courteous, friendly, careful and all around wonderful persons. The room was kept clean, there was plenty of space for storage of personal belongings, the bed was comfortable and changed daily. The food was appetizing, varied and options were offered to suit one's tastes. The only thing I didn't like was the lack of sufficient light for reading. Also the food, though plentiful was not hot when it arrived. Although that was no fault of personnel or preparation - I believe it was due to the fact that everyone's meals had to be delivered due to ongoing construction. Minor faults in an overall excellent stay. Thank you.
Fantastic experience: If I could give it 6 stars I'd give it 7! Excellent staff. My dad was here for 1 month during his recovery and he still talks about how the staff helped him daily. Thanks again for helping him on his journey

Negative experience: -5 Stars -- My mother has been at Harizone for years and now I'm discovering that the financials that they keep are not in line. Not only finances, my mother has safe 4 cancer. she is 3 weeks on her medication for chemo 1 week off. I called the facility as I showed that her prescription needed to be refilled and I was informed she still has 27 pills left for her Cancer medication. If you do the math she was not given her chemo for over a month. If you try and get ahold of that director Josh, he will not call you back for a couple of weeks. If you're lucky, nothing gets resolved. I will be searching for a new facility for my mother. I have contacted the Attorney General.

Negative experience: This is the worst place I' ve ever experienced. It smells and no one cares about the patients. My mom was there for 3 weeks before I just put her in a wheelchair and wheeled her out. I had ,had enough. Stay away...& put her

Negative experience: This place is horrible! I just took my mom home against medical advice, because of the lack of care here. No one to help her go to the bathroom in time, so she was forced to go on herself or get up by herself, WHICH SHE COULD BARELY DO. Then when I went to the case manager, Jennifer, to let her know that I wanted my mom discharged with home health, she acted rude and acted as if I, or my mom, was the problem. She clearly doesn't like her job. I didn't feel my concerns were a concern to her. Maria the nurse was more compassionate. Jacqueline the CNA was as helpful as she could be when my mom was unable to get her pain meds this morning, but she can't pass meds out. The nurse never came until I called to the front. I will also be reporting this to the C.M.S. . Do not send your loved ones here!!!
Negative experience: My wife had 3 strokes they let her sit there in a dirty diaper and they neglect her and have already abused her so I called Adult protective services and next time it's going to be the POLICE that I call
Negative experience: My mom just recently went to this place after breaking her knee and is miserable there!! My mom is forced to urinate and defaecate in a diaper, just for them to make her sit it in and not even fully change the bed after. The therapy is a joke as they don’t even have a walker for her to be able to get up. I’ll be calling other places on Monday for her to go. This place is no different than when it was Glendale Care Center!!

Negative experience: This place never answers the phone it just rings , you can never get ahold of anyone . And when they finally answer after 20 try’s they put you on hold for 10 mins .The staff here is rude if you don’t like your job find another job you shouldn’t be working with people’s love one if you have a attitude your in the wrong job.They should be surrounded by kind understanding people , not rudeness. I only gave this place a 1 star because I had to comment. You know it’s really sad that you have to place your love one in a place to get therapy and your not able to check on them .
Fantastic experience: I'm sixty eight and have had many problems over the last few years. Horizon Gave me the best rehab possible to get me better. And on my way to enjoy the rest of my life. Both physical and occupational therapy. Staff nurses, LPN's R n's. Everyone careveryone cared, thank you.

Fantastic experience: My S.O. broke her hip, and after hip replacement and Hospital stay, we were given a choice of rehab facilities. We chose Horizon. So glad we did it was clean and well maintained, the staff was fantastic including but not limited to Alex,Scott,Caden,Giselle,Lanqucia and many others. Unlike several other facilities I've had experience with Horzon's staff were friendly, helpful, and very accommodating to special needs of my S.O. Thank you Horizon !
Negative experience: My brother was taken to the ER last night his doctor called my sister and his wife saying her tried calling the facility to have him rushed to the hospital and the doctor was unable to reach anyone at all the doctor tried for 3 days 3 hole days three days and someone's care in life is very critical my family arrived there he was having a hard time breathing he was unresponsive there were food plates left in his room and he hadn't touched anything had to call 911 he's now critical on life support saying that he's malnourished and that he had an infection that has been brewing for a very long time that's also went septic I would avoid this place by all costs I I know the workforce right now is now suffering but when a physician and oncologist tries to contact the facility for 3 days because of the medical emergency and cannot reach anybody that is not a good place to be I pray that my brother makes it it is very unprofessional they put his life in jeopardy to know that he's malnourished and to also know that he has such a bad infection that he is on life support is heartbreaking and heart-wrenching knowing that this could have been avoided

This is my Dad's stay for 2 wks and I am a particular person when it comes to my dad I couldn't have asked for a better stay for him and to get the therapy he needed so he could walk again and be on his own for his age of being 79 I couldn't have asked for better care and success for him thank you for all you do horizon team ❤️

Fantastic experience: I was in unit 1 and the care was very good, Staff in unit 1 and O/T also very good.
Negative experience: I had arrived at the facility on February 16th 2023. I was hoping for the best but that the worst. It started out with them not giving my medication on time and also trying to withhold my psych meds.. the occupational therapist tried to discount my pain the first four days were just a nightmare.. I tried to speak with the case manager about the issues I was having care seemed to get a bit better but I wouldn't recommend it.. The last straw I ended up with food poisoning..

Negative experience: Brought my mother in law here after a knee surgery. It was such a bad experience. We pulled her out the next day. She was treated poorly and the environment was gross. Don’t send your loved ones here
Fantastic experience: I have been having issues with my IT Band for years and after a couple of weeks of therapy at Horizon, I am running the way I used to. Life changing therapy and kind staff!

Negative experience: I still have nightmares about this place after almost 4 years. They gave me MRSA. I had pneumonia and they refused to give me my antibiotics. The staff were bullying me constantly. I was known as the fatty in 117.
Negative experience: This place is FILTHY. It's LOUD it's got people hanging all out of beds barely clothed.It smells it's got a very large receptionist working the front door that's sitting there eating when she should be CLEANING!!!It's horrifying!!I wouldn't send any member of my family there even my pets! This place DOES NOT LOOK LIKE THE PICTURES THEY HAVE POSTED.
Negative experience: My family was placed in severe danger within hours of arriving because of the medical staff refusal to administer emergency medication to meet medical need or even respond to expressed concerns. When they finally did it was with hostility and disregard. I was a nurse and understand both sides, but blatantly ignoring life threatening conditions goes against every reason the facility exists. After this experience their website claims is clearly a facade.
Negative experience: This place is terrible. People walk around like zombies. Never greeting anyone. The phone rings unceasingly and no one answers it. Nurses ( if you can call them that) don't go check on the patients. Diabetics are given PANCAKES for breakfast. Do NOT take your patient there. They will die
Negative experience: My brother been at this facility for three months after having a stroke He has had ONE rehabilitation session He been there three months and has not got any more movement in his leg and arm then the day he got there. Not very profesional
Negative experience: My mother has been in several rehabilitation centers over the last year and we thought we would give Horizon Post Acute a try because of location. Do NOT send your love one here! Starting with the bright spots, there were several good care givers and the physical therapy department was good. Besides that nothing else. Communication in particular was the worst I have ever experienced. I left multiple messages for several days in a row to the Director of Nursing as well as the case worker for my mother. I never received a call back. I was just looking for some information on my mother's course of treatment and never heard from anyone. The nursing station was no better, as the phone would ring for ever with no pick up. Finally, my mother had a wound that was being treated. The night before we took her out of the center (by our own choice), her leg started bleeding to the point she needed cauterization. When I asked why this happened, no one knew a thing and there were no notes as to what happened. When I went to inquire about getting my mother out of the center, the case worker was extremely rude and dismissive.. This experience borders on negligence. It was a horrible experience.

Fantastic experience: I just got home after staying at Horizons post acute and rehabilitation centerwhile the place is clean and they take great care of you, what made my stay great was the actual people so I would like to shout out some of them..so here we go Sarah, Jennifer,lemla, Emily, Alicia,Audrey, Jeremy, Keith ,Brandon, Cindy, Michael, Areceli, Jess,scott, and of course Ms Lucy (I still want my clothes () I will miss these cast of characters couldn't have gotten thru this without them and I can't forget Vanessa. who I probably got on her nerves lmao thanks for everything...hope I didn't forget anyone of I did please forgive me and I love you too

Fantastic experience: I can't say enough good things about Horizon!!!! We placed my dad here after we couldn't find anybody to fill his needs at home. He was there only a few short months but they made a huge difference in his life. He was attending social activities and making friends. He was getting physical therapy on both arms (his left arm was immobile due to MS) and he was thriving. The nurses were wonderful to him and attentive. They were always quick to notice when he wasn't feeling right and made sure the doctor saw him. The Director Michael knew who he was and made sure we were aware that he was thriving. Michael sat down and talked with me assuring he was doing well and wasn't staying in his room all the time. If you're looking for somewhere for short term, respite, rehab or long-term I would highly recommend Horizon!!!
Fantastic experience: I’m a longtime resident here at the Glendale location. And I’d like to praise a cna who hasn’t been here too long. But in the time she’s been here, she’s literally made my time here so much better! Her upbeat mood and personality, her ability to actually really care for us and definitely go the extra mile. All of that and much more makes all the difference for the better in our lives. I look forward to each day she’s here. I wish they all could be more like her. I know it’s hard when staff is short and what not. But she’s a keeper! She works hard here for us, then goes home and takes care of her own family and whatever else pops up. She is amazing. Fantastic experience: Horizon Post Acute is an outstanding facility! We had over 3 weeks of experience with almost all the staff members including director, nurses, doctors, physical therapists, dietitian, housekeepers, case managers, etc. ALL the staff members were compassionate and provided excellent care. It was a very clean and well-managed facility. They were very welcoming to family members.
Fantastic experience: My stay here has been great. The staff treated me good and the service for my care helped me get back on my feet. I have no complaints as to how things were up here. Fantastic experience: I have really enjoyed my experience with Horizon. I want to thank the therapy team, Jonathan and especially Lucy on social services she is the #1 she is very attentive and always keep her word she help me alot. Thank you!
Fantastic experience: I live of state and they’re always amazing at communicating any concerns about my aunt and keeping constant communication with me. I never worry about the care she’s receiving there! Thank you Horizon.
Negative experience: What part of HEALTH, THERAPY and REHAB involves depriving patients of water and food, and leaving them in their soaked and soiled undergarments FOR 10 HOURS? They only changed her bedding and undergarments when I was there. And there is the psychological mistreatment of the patients. My wife's right arm is paralyzed (still is). They'd leave unopened food containers for her. IT TAKES TWO WORKING HANDS to open them. There are two beds per room in the wing where they kept my wife. This presents a dire and dangerous situation as the CNA that brought the medications had mixed them up between my wife and her roommate. Luckily I overheard her mention the medication that she was giving the dialysis patient as my wife's epilepsy medicine and stopped her. AND their staff needs to learn good manners and common courtesy. I can't find any licensure on this place, either. Their priorities: 1st: Money. 2nd: Your tone of voice. 3rd: Foul language from frightened, medicated, paralyzed patients in pain cussing at them. (They told me what my wife said to the help when they hurt her. It was so extreme that it's funny. Reminds me of my adolescent years.) 4th: Appearances. They need to LOOK LIKE they are helping people. 9th: I don't know how low a priority they consider the comfort, health and safety of the patients.
Negative experience: Granted I was there in August and September of 2020, but that was the worst experience of my life. There were a couple of bright lights working there, but some of the staff were horrible. I would never go there again.
Negative experience: I've been here 2yrs and I just want to say that, unless you have the money to have 3 meals a day brought in for your loved one, you might want to think twice about bringing them here. It's not bad enough that they can't even get the grilled cheese right (and that's off of the 3 item alternate menu) oh no, not this place... the only other option you've got is between a meat sandwich (1 slice of processed turkey or ham and if you're lucky a slice of cheese) and a peanut butter and jelly. I had been eating the p,b& j's until they've become as bad as they have
Negative experience: My brother in law was placed in this torture chamber after he was discharged from the hospital around two weeks ago. My mother in law visits him at least 4 time a week and during her visits she has noticed the staff doesn't perform even basic duties. for example. Feeding he has numerous times hasn't been fed she has to remind the staff that her son wasn't fed. That is when she is there lord knows how many meals he has missed. He is often left in a soiled diaper which is very unsanitary. His linens are left soiled and not to often changed and bathing is virtually non existed. My wife has tried to call numerous times to complain but no one has ever called back. Of course the answer back from you will be the same cookie cutter answer you give blah blah blah. Just updating my review same garbage as before they need to shut this place down if they don't care about their clients.
Negative experience: Not a good place to recover after hospital release some days are like you in mental hospital and not vary assisting whit basic services like pain medication that tells you never back again too this place
Negative experience: My mother is currently in there, the place is so understaffed that she and probably the other patients aren't getting the care they need. My mother was left with a soiled diaper for up to three hours and now she is severely ill, likely a UTI among something else, we tried getting a hold of her or the nurse, but we just get the runaround.
Fantastic experience: Honest caring staff. My mother stayed for 2 weeks and the stay was just what she needed.
Fantastic experience: My cousin was receiving therapy there for a few months and everytime I called to check on him, they were very informative and helpful. He's doing great now, thanks to the staff there!
Negative experience: I want to start off by telling everyone how horrible this place is!!! My father has been in here for almost 3 weeks I believe and every single day he has called me to complain about the staff! Well one specific nurse and I believe he had said her name was latisha to be specific!! I first thought my father was just being a pain until he had me on the phone with out her knowing and I had heard the way she was speaking to him! It was unbelievable and uncalled for! Like why is this women even a nurse if she can’t handle this type of work!! She was unbelievably rude and now I know why my father has called me everyday to tell me how she is treating him! He has said for all the other workers are great except for this one nurse!! She should be fired and honestly who ever gets her as there nurse I wish you luck! She’s not a pleasant women and I thank god that my dad huh b huh leaves on Sunday!
Negative experience: Just Don't. Most of the 5 star reviews are fake if you look at them. They are severely understaffed. Your loved one will die here!!!!. If I could give them negative stars I would.
Positive experience: Our daughter was there for 2 1/2 months and the staff there was wonderful! They took the time to answer all of our questions and concerns. Lucy Rodriguez, her Social Worker went above and beyond helping us along with Jesse Hintz the Executive Director.

Negative experience: Absolutely horrible place to send anyone I wouldn’t send my worst enemy here anytime you bring them stuff for your loved ones they take it they stole a stimulus check from my uncle and after he had passed they did not offer to give us anything back and we had spent hundreds of dollars on stuff for him that he would never receive do not send your loved ones here as for they do not care for them they do the absolute bare minimum
Negative experience: I agree with the other comments that staff is not helpful. They discharged my mother without her prescription eyeglasses, and when I called attempting to get the eyeglasses the person who answered the phone was rude and dismissive.
Fantastic experience: Horizon Post Acute and Rehabilitation Center has been my home for the past 6 years after a surgery left me paralyzed and in need of long term care. The administration staff has always been available to talk to and they have always treated me like a real person and not fake just telling me what they think I want to hear. Lucy Rodriguez the Social Services Director is the NICEST and most CARING person I have ever met. She has helped me out with so many things and has always had a minute or two to listen to me weather it pertains to my medical coverage or a staff situation and even if it's a personal issue. She always is running around the facility helping the residents, patients or family members who have questions about their loved ones stay here. I don't know what I would do if she wasn't here and I hope I don't have to. When it comes to the nursing staff and the cna's I have seen a lot come and go through out the years. Most of them have been reliable, respectful, caring and attentive to my needs and limitations. Now dealing with the covid19 pandemic thing's became so scary and uncertain, but the administration staff, nursing staff, and the cna's have been working really hard to keep us healthy by cooperating with all the CDC guidelines as they change. I am thankful that they have taken the measures that are helping to keep us safe and healthy. Even if it means getting the invasive covid19 cotton swab test once a week. J/K they don't go all the way back anymore. I

Fantastic experience: The marketing director is really helpful around the facility and is always a team player. He always steps up and helps out others when needed. He is a great asset to the company and the facility. Kudos to Jennifer, the receptionist who is always calm with callers when expressing grievances. Jeanette is also helpful with admissions and makes sure she is thorough with information. Mahogany is also a fantastic CNA, she has a calm demeanor with staff and residents

Fantastic experience: I have been an employee here at Horizon PARC for over 6 years now and I have had the pleasure of watching our team grow. We have some amazing nurses, Lucy in social services is always willing to help and our rehab team will go above and beyond for our residents. I would recommend Horizon Post Acute and Rehab to my friends and family!
