We are extremely grateful for your support and prayers! Your contribution to stopping the spread of COVID-19 is invaluable and has enabled our healthcare workers to stay healthy, as well as our patients. We would also like to thank the Texas Department of Health & Human Services, Dallas County Department of Health, and our state and federal governments for their help in supporting skilled nursing and long-term care facilities. We are so appreciative of everyone who is doing their part to fight this pandemic.

We are incredibly thankful for the support and prayers we have received. We cannot express our gratitude enough. We are confident that our prayers are being answered as we continue to make progress. We are thrilled to report that more and more tests are coming back negative, and we are filled with joy each time someone is declared "recovered." Here is the latest news:

Since the beginning of this journey, we have administered more than 550 tests. Currently, we have seven individuals in our care who are still fighting the virus. Five of them have mild or no symptoms and are stable, while two are in a guarded condition due to having moderate symptoms and multiple underlying conditions. Fourteen of our patients have been declared as recovered and have been moved to our step-down area. Our team has done an incredible job in supporting our patients and encouraging them to keep fighting, even when it is difficult. We believe that the outcome for some of our recovered patients would have been different if it weren't for the prayers and the team's efforts. Two of our employees are still fighting the virus at home, but they are doing alright and we anticipate that they will soon be recovered.

We have achieved some successes in the fight against COVID-19, but the struggle is far from finished. Our region is still one of the most heavily impacted in Dallas County, and we recently stopped a potential new worker from bringing the virus into our workplace. This individual tested positive for the virus but had no symptoms and was astonished to find out about the positive outcome. This demonstrates that the virus is still very much present in the DFW area, and it is essential that we continue testing. We must stay alert and keep up the battle. Please keep us in your thoughts and prayers.

So far, 12 of our employees have tested positive for the virus and have been required to stay at home. One of them has now tested negative twice consecutively and is considered to have recovered. We are hoping that more people (both patients and staff) will be in the same situation this week. We have been attempting to test everyone every seven days to try and identify any positive cases and stop the virus from spreading. Nevertheless, some people have tested positive up to three weeks after their last known exposure and most have not displayed any symptoms. This makes it difficult to track where the virus is and where it may be spreading.

We have finished testing all patients and staff. We have retested those patients who had initially tested negative, and we are also retesting employees. Due to the presence of COVID-19 in our building, we have been able to get more testing kits and are making the most of this resource. After the retesting, we have identified more positive cases. 13 patients and 8 employees have been confirmed to have the virus (this includes previously reported cases). 12 of the patients are showing mild to no symptoms, while one has been sent to the hospital with moderate symptoms. The employees who tested positive are in quarantine at home, and they are having mild to no symptoms. We have gotten over 150 negative results from the retests. We will keep retesting as kits become available to us.

Many of you have asked what donations we can accept. With the number of cases rising and other businesses reopening, the need for PPE and other supplies has increased even more. We are thankful for any PPE donations, as well as Isopropyl Alcohol (70% or greater). These items can be brought to the quarantine bins at the front of the building. We are so grateful for the help and support we have received. Every contribution is greatly appreciated! We especially want to thank Chasing Wishes Boutique for their donation of KN95 masks and Heather Molchadski for organizing this effort and providing refreshments for our team. We are truly thankful.

We are very thankful for those who have provided us with assistance. We are making the most of the situation, timeframe, and resources we have. We are proud of what we have achieved and of our team. We are grateful for your continued prayers.

On Monday 4/27, as previously mentioned, we had a patient who tested positive for COVID-19 and was placed in isolation until the test results were received. We received the positive result at 4:30am, and the Administrative Disaster Response Team arrived at the facility shortly afterwards. They immediately put our Emergency Protocol and Disaster Plan into action. They evaluated the risk of the residents and staff that were already in the building and prevented anyone else from entering until they could be properly screened. The CDC guidelines were followed, and contact tracing, Risk Stratification guidance, and COVID-19 tests were administered. As a result, many employees were sent home until we could assess them further. They were asked to stay in their vehicles while we made these decisions.

We would like to express our deepest appreciation to our AMAZING and LOYAL team who have risen to the challenge. They are working diligently to improve the lives of our patients and community. We look forward to sharing more about them in the future.

At present (4/30), the results of tests conducted on Tuesday and Wednesday have not been received yet. Therefore, we have taken additional precautions. We have reduced our medical personnel and placed one of our units in quarantine, while keeping the other units under surveillance and separated from each other.

Our team has come together to ensure that all patient needs are met and satisfaction is achieved. Once we receive the results from the tests we have conducted, we will contact the families of those who were tested. Furthermore, we will be testing more patients and staff, and will continue to do so in the future as we receive more test kits. Given that many of our positive cases are asymptomatic, we have decided to test all those who are eligible for the test kits.

We are incredibly thankful for your support and prayers! We are doing everything we can to fight this battle and have had to reduce the number of our staff and make sure that our PPE is being used to its full potential. We are planning our actions day by day, based on the events of the previous day, and need to recruit a hardworking and compassionate team. We are looking for nurses, medication aides, and nurse aides to join our team or be on standby, as well as additional help in other areas. We also need more medical isolation gowns and N95 masks. We have taken all necessary steps to contain the virus, including testing all staff and residents, quarantining any infected individuals, and increasing sanitation and hygiene protocols. We are also providing additional support for our staff, including mental health services and financial assistance.

At around 4:30am on April 27th, we discovered that one of our patients had tested positive for COVID-19. This individual had chosen a palliative care plan and wanted to stay in our facility rather than going to a hospital. We are doing our best to respect the wishes of our resident while also protecting the other residents and staff members. The patient has been put into isolation and we are assigning staff to provide 1:1 care. We are following the regulations set by local and state health departments to ensure that the latest protocols are being implemented. Unfortunately, we cannot accept any new patients at this time. We are doing all we can to stop the virus from spreading, but the long incubation period is a challenge. We are trying to identify anyone who may have been exposed to the virus in the weeks leading up to the diagnosis and are testing them and other patients in the building. If you are worried about your loved one being in our facility, we understand and sympathize. We strongly suggest that anyone who is able to take their family member home does so. We will hold the room for as long as needed and will help with the transition in any way we can, as long as it is in line with Dallas County orders.

We would like to take the time to recognize everyone who is part of our team. Despite the difficulties, our team has remained strong and is determined to keep going. Even in the face of a pandemic, our goal remains the same: to provide the best care and support to our residents and patients. While some have chosen to step away, others are still showing their courage and bravery by continuing to give care despite their fear and the unprecedented times. We are truly humbled by this incredible group, and we give thanks to God for them.

We would like to express our appreciation to all those who have contacted us to ask how they can assist. We take infection control very seriously, particularly during the flu season and now with the novel coronavirus. We were already concerned before the restrictions were imposed, and now that the virus has spread across the country, the situation is even more worrying. We have heard about the tragedy that occurred in a nursing home in Washington, and now the virus has spread to almost 150 other facilities across the country (as of 3/23). Dallas County Judge Clay Jenkins recently held a press conference to announce that there are now multiple nursing homes in Dallas with outbreaks. We have not been informed that any of our staff are connected to any of these facilities, but we understand that this virus can spread quickly and are taking every precaution to protect our community. How can you help?

We need your prayers and positive thoughts. We are not accepting any care packages or letters at this moment, as we are taking extra precautions to clean and disinfect anything that comes into our facility from outside. We are very thankful for the prayers we have already received and we are thankful that we have not had any confirmed cases of COVID-19. We are praying that we can all work together to prevent it.

We have the necessary supplies to take care of our residents; however, if you know someone who has access to masks, gloves, and other medical equipment and you think we could benefit from them, please contact us. Although we have enough for now, we are uncertain of what the future holds in terms of the supply chain. As usual, we are being proactive and preparing in advance. We understand this is a challenging time for many healthcare workers.

It is essential for everyone to take responsibility for protecting those who are vulnerable to this virus. We are doing our part by screening, taking temperatures, and requiring masks to be worn in our facility, but it is up to the community to do their part as well. This means more than just banning visitors and disinfecting items that come into the building. Our patients have to go out of the facility for necessary treatments and specialists, and new patients come to us via ambulance. There are also medically necessary providers that come into the building, and these people must remain healthy in order to protect our vulnerable. We urge everyone to act in the best interest of each other and stay home whenever possible.

Fantastic experience: No one wants to see their parent have to go into a long term facility, but my mother has been there for about 5 months now and I really could not be more pleased with the facility and the care she has received. The nursing staff and aids have been really good with my mom. Overall everyone there has been very friendly and very attentive to my mom's needs. She has had very good physical therapist that have worked with her and they have a done a really good job with her. I would also love to give a shout out to the activities director and all the staff that help arrange activities for the residents. It's very important for these residents to have something to do and keep them busy. These love one's lives have changed so much after entering a long term facility and having daily activities to look forward to and keep them busy makes such a big difference. They just put on a prom party a few weeks ago that was a big success. The residents got to dress up and enjoy a live band and refreshments. It was really moving to see the smiles on their faces! The staff really put in a lot of hard work and time to pull off such a nice party. It meant a lot to all these residents and their families and I want to say thank you!! We really appreciate all the extra work you guys do to take care of and our love ones. I must also point out all the hard work the kitchen and dining employees do daily to keep everyone fed. They really do a good job keeping this many people fed 3 times a day 7 days a week. I am there a lot and they are always friendly and don't hesitate to change a dish if my mom's does not like it or want's something different. So in closing, I really am pretty happy and satisfied with where mom is, even if i would rather have here home with me. It is very important to feel comfortable and feel that my mom is at a place where I know she is safe, happy and being taken care of. One last thing! The place is very clean. They are always cleaning and mopping the hall ways. I have never walked in and noticed a bad smell. That is a big deal with me!!

Fantastic experience: Avante Rehabilitation Center is a wonderful place for your loved one in need of rehabilitation care. I have placed my elderly mother in the facility several times, and she has always received the best of care. The admission process answered all my questions and arranged for transfers from the hospital to the facility. The team leaders and staff were very helpful and kept me informed and up to date on her progress. The doctors and highly skilled nursing staff and physical therapist are dedicated, competent and thoughtful and are fully committed to the patient's needs and care. I am confident that my mother returned to her home because of the quality care she received from Avante Rehabilitation Center.

Fantastic experience: Avante is a great facility for rehabilitation for your family member! My mother had rehab and the staff was very supportive and always kept me informed on my Mom’s condition. She was able to be discharged and I requested that she be placed on their waiting list for long term care because of her gradual decline in her health. After a year and four months she had another medical condition that required her to go to Avante for more rehab. I am blessed that she was on the waiting list because she is now a long term care resident and I am at peace with the excellent care, attention, and professionalism of the ENTIRE STAFF. I recommend Avante 100% for you or your family member!

Fantastic experience: The facility is very clean, very efficient and employees are attentive in their duties. We always had fresh ice water and the foley was checked frequently. the food was good and sometimes the good southern cooking was better! The night nurse Marlene was exceptionally attentive and innovative. My wife became fast friends with her and her bedside manners was solicitous and empathetic. I am glad she was recognized as an outstanding employee and moved to management but sad to see her go. Adrian was the ice man. He made sure we had fresh ice when he came on duty and also before he left. He made Connie feel comfortable when attending to her most personal needs and he was always there when we needed him. He too was efficient and professional when attending to Connie's most personal needs. Kurstin always made sure she had her bed bath should she want one. She straightened her bed when she was not in it and insured the covers and sheets were clean. Ken was an efficient day nurse, and he answered all my questions and if he didn't know the answer he would find out. He insured Connie took her meds. The physical therapy team Jessie and Alex encouraged Connie to participate in the required therapy and had her walking by the third week. She walked up to a hundred steps .That was good considering she could not hardly turn over in bed when we arrived. Dr. Seethamraju was and is a good Doctor. I wish she could be our primary care doctor. All the rest of the staff was professional in their duties. There are too many to mention and I could not write all their names down. I would be amiss if I did not mention Telma, the lady that cleaned our room. I always felt comfortable when she cleaned because she was conscientious and thorough in her duties. Fantastic experience: My husband has been in Avante off and on all summer. We have had great care. The staff is very good and knowledgeable in their specific area of expertise. Our nurse has been Cherryl and she is great. Very compassionate and cares about her patients. Kurstin was my husband's CNA. She is very caring and takes good care of her patients.

Fantastic experience: As of this writing (19 Apr 2013), my 92-year-old mother has been in Avanté for nearly a month. Mom and our entire family have been extraordinarily impressed at the quality of care she has been receiving. Every single person on the staff we have encountered has been extremely competent, professional, caring, warm, and responsive. The few problems we have encountered have been addressed promptly and thoroughly. The facility has been recently renovated from top to bottom and is bright, cheerful, and safe. The food is good and carefully tailored to patients' needs. In Mom's words, this place is "Awesome!"

Fantastic experience: My father was there in his rehabilitation phase. And we feel very comfortable in this place. The staff was nice. I liked CNA Lianys Vilahomat because, in addition to basic care, she also gave him massages during the bath to improve circulation. Not even in the hospital did they do it. I think this place has very well prepared girls, and that's very good. I recommend this place.

Negative experience: One week ago. Very bad place to put your loved one. They don’t take of their patients. Had to wait 45 minutes to go to rest with help. Food not fit to eat. Didn’t give the medications. Cleaning solutions so strong your eyes burn and give you a bad headache. Would Not recommend.

Fantastic experience: My sister was placed there for rehab after a hospital stay. I was very impressed with their attentiveness and the fact that they scheduled care meetings to keep you informed of the progress of the patient. My sister loved the staff and really made improvements while there. I would highly recommend this center.

Fantastic experience: The entire staff is efficient and know how to do their jobs. More importantly they care about the patients. They took care of my dad from the beginning from the admissions team to the nurses and doctors. I would highly recommend this facility for a loved one. My only wish would be that they had more availability for long term care.

Fantastic experience: I love this place for my dad. They sure set the bar high for any comparison to another facility. My dad is here short term for rehabilitation after a severe stroke. Unfortunately, now that we need long term care for him there is no room. This facility is Always clean. I’ve been there different times of the week and it’s always clean, and bright and the staff is amazing. I recommend this facility hands down for sure.

Fantastic experience: After living independently mom transferred to Avante from a lengthy hospital stay. She arrived on a rainy late Friday afternoon a little over 2 years ago. She was warmly welcomed by Phillip and his staff. They were so attentive in getting her settled in. The care shown for my mother has continued to be exceptional! It was an adjustment for mom from living in her own place but, she herself will tell you, “if I can’t be on my own this is where I would want to be”. She has thrived in an environment where she is well cared for and receives a lot of love and affirmation. She has become involved in the activities offered and has made friends with the residents and staff. Avante was an answered prayer and continues to be such a blessing in our lives. I would highly recommend them to anyone. It is a place where mom calls “home”.

Positive experience: Some of the staff are lazy, you ask them a question and half the time they don't know and if they do know it's not their job to do... pass the buck is what they do here... When you need something they look at you like it's coming out of there check and guess what the people that are staying there is what pays them so they need to remember that. If you don't like your job then you need to find something else to do....08/02/2019 We are no longer at this rehab but wanted people to know that what I commented on this does not refer to everyone but I understand that there are alot of personnel that have been there awhile an are family and they did make every effort to make things better and we appreciate that.... Fantastic experience: My mother lived at Avante for 6 years. It was awesome. She made friends, attended bible study, played games, ate good cooking, went to the State Fair of Texas and saw lots of movies that she had forgotten. Avante was gracious enough to put up with our family on a daily basis...they even gave us Avante shirts and allowed us to participate in all the activities. My mother moved to her permanent Heavenly home on Good Friday of this year, but we are still part of the Avante family. Mother told us not to lose our Avante family just because she moved on. I can’t say enough good things about the love and care we all have gotten.Thank you Phillip and Ericka and your wonderful group for continuing to be there for us. Nothing is ever perfect but we keep striving for that perfection. Thank you for letting me add my 2 cents

Fantastic experience: My Grandmother was at Avante for about 6 years and she couldn't have been at a better place. The owner, Phillip Prince, and his staff are the most amazing people. They are caring and loving to all their residents. I highly recommend Avante to anyone that has a loved one that needs to be taken care of.

Fantastic experience: Avante Rehabilitation Center is over the top with their customer service, the care they provide their residents, the quality of the home and the quality of their overall care for EVERY client and their families. I am EXTREMELY impressed with being called by the staff to review my dad's healthcare plan and management and they continue to meet with the families providing them updates on the plan. Ellie, Brittany, Phillip, Marlene, the nursing, kitchen, facility, technical, therapy staff are overwhelmingly AMAZING! These folks work hard to produce the highest QUALITY of service. Negative experience: Beware. I got a recommendation for Avante from a previous patient. I went to visit and get a tour. Everyone was very nice but the owner admitted up front that they did not like working with supplemental insurance plans, it made more work for them. He actually asked me if we would be willing to dis-enroll my father from his Humana plan so he would be on just Medicare. I didn't know anything about the procedure or the ramifications so I said I would look into it. At the end of the visit they said they would most likely take my father. Today they turned us down. My gut feeling is it was because of his supplemental insurance. Just be prepared for this.

Negative experience: This place might be a 5 star facility but far as staff it's not run right n they don't care about you they will work you like a dog and won't say thank u and they only pay 12 dollars an hour which is not enough to survive on when you only work double weekends my suggestion to you before selecting this facility is keep your love one at home in fact they have even meds to the wrong lady and yet the worker still works there as a cna smh.....
