Experiencia negativa: While the staff are friendly I'm not sure anyone communicates. Between all the staff working with my mom no one recognized a problem and it almost killed her. I would never let any family member go here again.

Experiencia positiva: Many friendly, caring, and kind staff personnel. Much improvement over the year I have stayed here.

Experiencia fantástica: There is something to be said when you walk into a community and see smiles and hear laughter. This is what I experience every time I come to Madison Health and Rehabilitation Center.

Experiencia fantástica: I stayed here in September 2023 and did not have a great experience. I stayed again from the beginning of September to today, and it has drastically improved. The staff were pleasant and very helpful. The food was nice and much better than last time, even better than where I live. I'm actually going to really miss this place and the people here. I even asked if I could stay longer. Some of my clothes were lost in laundry but the facility made so much effort to find and replace it, the stuff they couldn't replace, they purchased for me and sent it to my home. Leaving here is going to hurt my heart, I feel like I'm leaving my best friends. I came to really care about the staff and love them all. The social worker, Marissa, is especially wonderful. She remembered me from my stay last year. She's bright and cheerful, very friendly, and helped with all my questions and made sure my items were looked for and replaced. I'm going to miss everyone dearly.

Experiencia fantástica: I was admitted to this facility for physical therapy and the experience was amazing. Prior to my stay there I was wheelchair -bound and could not walk without some form of assistance either from staff or with medical devices. However, through treatment and care I received I was able to walk without assistance or medical devices. The therapy department took the time to explain how and why my mobility was so limited and what could be done to improve my condition. Janet and William really helped me to build confidence in myself and helped to achieve my goals of walking again freely. I would recommend this facility for anyone in need of quality therapy with medical professionals who add a caring, personal touch to all that they do for their patients. I'd like to thank everyone there who always made sure that I was cared for and that my personal, physical goals were reachable.

Experiencia fantástica: The staff are amazing very caring and passionate. I am very pleased with the placement of my loved one. She complains about the food to me but that is normal she complained at the hospital as well. Unless she is making the food herself she will not be happy about any food. But she is taken care of. I feel safe when I leave after visiting that she will be taken care of like I would do at home.

Experiencia negativa: I was transfered to Madison rehab on a Friday put in a room and left there. No heat in the room. Cold food no condiments. Employees treated me horrible. No PT. I feel this place should be shut down. Have requested to be transferred. I have now been told I have to leave. Because they won't take my insurance. They are denying a disabled veterans widow treatment. My husband gave his life for them. The social worker is the most arrogant person I have every met. They refused to give me the corporate phone number or the name. Please don't send your love ones here.

Experiencia fantástica: This nursing home is humble in appearance but they are absolute rock stars in patient care. The staff is loving, kind and helpful. The CNAs are exceptional. My father was happy and comfortable during his stay and we grew to really love and appreciate the staff. An undiscovered gem in my book.

Experiencia fantástica: My dad was a recent resident before he received his angel wings due to his failing health. Our family was always greeted with a smile. I do recommend this facility to others. I did speak with a few residents and they seemed happy as well. They were lonely but that falls on family or resident that chooses not to participate in activities.

Experiencia negativa: You can NEVER get ahold of them over the phone. Whenever you call the number listed, it is a busy tone. Very unprofessional and a little shady.

Experiencia negativa: If you are there for more than a year, you’ll die there. A nurse told me that It is a TOXIC environment! If you love someone don’t send them there!

Experiencia negativa: A close friend of mine worked at this facility and what I’ve heard about the faculty makes me sick. A nurse didn’t want to take the time to feed a dementia patient and when my friend questioned the nurse they said the patient “wouldn’t remember anyways”. The dementia patients get it the hardest considering this is a lot of the nurses attitudes. This is just one of the many examples of how poor the mindset of the staff is. If you care for your loved ones don’t send them here!!!

Experiencia positiva: I'm the POA of a formerly homeless elderly man who has been a resident at Belmont for several years. I've found Belmont nursing staff to be caring of him and dedicated to providing him the best care possible. The facility is not a vacation designation but the love and concern of all the staff is comforting.

Experiencia negativa: As a medically complex patient family member was doubled up in a room without access to anything. Had almost no needs met - medications weren't verified, they didn't even have life-saving epipens. Response times to room were 45 minutes plus for emergency calls, and had to call 911 multiple times on a barely charged phone, and got yelled at by staff for saving their own life. Extremely low care ratios. Place smells, most residents are cognitively incapacitated or drugged so cannot complain. They put on a nice show at daytime when visitors are allowed and management is there, but when you're left there to fend for yourself it's a death trap, with prison high carb food, no support for special diets, and zero support. They accept anyone, even those beyond their capacity and hospitals are happy to send patients there. Also have seen by neighbors to be one of the most highly dispatched for EMS. If you care about a loved one do not send them here.

Experiencia positiva: This place used to be terrible about 10 years ago when my father stayed here. It was with hesitation that I went in to visit a family member 10 years later. I was quite pleasantly surprised. Most of the carpet and living rooms are new. It's well lit and well staffed. The food was decent nothing fancy nothing bad. There are still some pretty strong smells in the air that they don't a dress but most importantly it's really well staffed when we have been there. The one area that would be well improved as if they had some nice outdoor seating for more than a few people.

Experiencia negativa: Just spoke with someone on the phone, I assume a receptionist. Very bad attitude. I do not recall her name but she was very rude. I would not recommend this place to anyone. Especially if they talk to people in that manner over the phone and in person.
