Experiencia positiva: I spent approx. 4 wks. at this rehab, I was released to go home and have been home a week. I was fortunate to have a good experience at this rehab. Where I was located the staff gave me good care. Mentioning: the day and night RN's were great, the LVN's were attentive (Jacinta) the CNA's day Monique and night Maria also very helpful, the Physical Therapy I had every day. The staff of RT's (they know who they are ), all of them day and night taking care of my Traech and seeing how I was. I am so grateful for all their help. Thank you

Experiencia negativa: Everyone has thier own experience. I will just say I would never bring my mother back here. At first facility appeared ok but nights and weekends that facility really drops the ball.

Experiencia fantástica: This is not a 5-star facility. I give it 5 stars nonetheless for sheer effort. Like all of these "skilled nursing facilities" Upland Rehabilitation is understaffed and underfunded. But what we found, during the entire 3 weeks that my mother was a resident due to a stroke, was a staff full of people who genuinely cared about the wellbeing of the residents. Every single physical therapist, CNA, LVN, housekeeper, and support staff we encountered was kind, and quick to offer assistance when needed. The food was also better than expected. The rehabilitation was effective. And the real testament to how well cared-for my mother felt was manifested when, after 2 weeks, I offered to take her home and care for her. She felt she wasn't quite ready, and chose to stay another week. Considering that when the 2 other times she has had to be in other care facilities she couldn't get out fast enough, and begged us to "break her out" after 1 day, that is really saying something! Kudos to the entire staff at Upland Rehabilitation. You really do an amazing job and we are so very appreciative of the kindness you showed our mother.

Experiencia positiva: Staff has been great and always professional; nursing staff seems overworked at times so things have fallen through the cracks (ie, changing my bandages, answering call lights). Social and case workers also very nice but sketchy in answering questions (ie, how much is home care going to cost). Physical and occupational therapy staff VERY nice and VERY capable people. Food always good. Great experience so far.

Experiencia negativa: My father was transferred to this facility after a fall. They never bothered to xray his hip and proceeded to force him to exercise it even though it was broken. They sent him to us with Covid and infected us. He lost 65 lbs and the list of abuse and neglect is too long to post. He died thanks to their awful care of him.

Experiencia negativa: Denise is so disrespectful. I work at a senior living and am assisting a family move their mom from this facility to our community. I have called Upland countless times because the family has asked me to since they are busy moving and working and Denise is no help. We sent our nurse over then she had an emergency so hasn't been back to work and Denise is so rude about it. No helpful, has a voice that mocks you and is difficult to connect with. She called us dysfunctional and difficult. She is a terrible person and employee. And from the amount of 1 stars, the community itself is clearly not very good. I had my grandmother AND father there for sometime and we ALL HATED it.

Experiencia fantástica: The people at Upland Rehabilitation Center were really friendly and helpful. My husband, Tower, was transferred here after his surgery and hospital stay. When he first arrived he was helpless with a bad temper, but the people here took good care of him anyway. After the care team's professional care, he can walk with his walker now and he was discharged yesterday. He wanted to special thank Dr. Diana Tran and her team for their professional treatment so that he could go home smoothly. Finally, he wanted to thank Richard Lomel, Cathy Mendoza and Diane Sanchez for their smiles and hard work. You guys are angels.

Experiencia positiva: My husband is currently in this facility and I can't say enough about the care he has received so far. The staff is caring and professional. My husband has not complained about the food so it must be good. But our best experience is with Shirley from PT. She is beyond awesome. She has done more with him in 3 days than the hospital was able to do in a month. She is a tiny powerhouse and I would put my loved ones in her care any day. They are also taking good care of my husband's wound and it is already beginning to heal. Thank you for all the good work.

Experiencia negativa: My mom was there for 2 years. They did not take care of her. She died there by herself. They were only after the money. PLEASE I'M BEGGING YOU ALL, DO NOT PUT YOUR LOVED ONES THERE!!!! THAT PLACE NEEDS TO CLOSE!

Experiencia fantástica: What a great place! Rehab team was so good to my family, trained everyone on how to care for our loved one when it was time to go home. Their cnas are always smiling and so caring. Clean rooms and good tasting food. Nothing but great things to say about this place!

Experiencia positiva: I had GBS and was completely paralyzed. They took great care of me! Most of the staff is caring and attentive there were a few who have absolutely no clue but for the most part the overall facility, protocol and patient care is good. Whats important and I know that made a difference not only at this facility but at any facilty (I was in another facility previous to being transferred to Upland Rehab) is the family involvement my wife stayed on them and insured i received the care that I needed. I would most definitely recommend this facility but just as I stated before you have to make them aware of your needs and sometimes they are understaffed and will forget but if you or your family are on it this place will work for you.

Experiencia fantástica: What an amazing place! My grandmother enjoyed her stay! She said the physical therapist were fantastic and the nursing staff were so kind. The food was great and she said the staff went above and beyond to meet her needs. Highly recommended!! 5 stars!

Experiencia positiva: The staff are really nice however their food is horrible. They gave my mother rotten milk three times and when asked to speak to the leadership to work on a resolution , the manager was unwilling to talk. Manager on the phone directed staff to tell me to call the dietician on Monday. After 3 servings, obviously there are no internal controls in place to safeguard the basic nutritional health of its patients .

Experiencia fantástica: My mom came to this facility after having major back surgery when she lost feeling in her legs and after learning that she had cancer. I would frequently call to make sure she was ok and she would say “this is a good place & these are good people”. I want to give a special shout out to the Kaiser team that watched over her: Rhonda, Karen, Rachel, and the physical therapists. And thank you Claudia at the front desk for always being kind when I called.

Experiencia fantástica: I’m very thankful to all the staff here. Very professional and the whole therapy staff are great. When I got here I couldn’t move do to two strokes Thad I had. But here they help me so much that I’m able to move and walk again. Thank you so much.

Experiencia fantástica: They are caring for my mom that has a list of issues. They are organized and have great cna, lvn, and food. They have a very focused team on rehab and my mom is making progress. My mom really needs the skilled nursing assistance and they are doing great with that. There is a priority on safety, cleanliness and comfort. Their is a slow down when the cna goes to lunch but they cover that ok. They cover a lot of details for moms care.

Experiencia positiva: Upland Rehabilitation And Care Center definitely does their job and does it well. I personally have observed the facility and staff with respect to patient privacy. The facility is very well maintained and presentable. It definitely has a welcoming and comfortable vibe for the patients. I must include the fact that every time I have entered this facility it as always had such a beautiful scent at the main entrance. That is very well done. However I feel that I must conclude this review by stating that a small group of nurses have a lot of room for improvement. Not all, but some. I have observed specific staff members who don't really see their charged patients as a priority enough. The other staff members do an excellent job. Upland Rehabilitation And Care Center does have my approval and recommendation.

Experiencia fantástica: When I went in there I couldn't walk... Completely bed ridden. I was there for about two months and walked out. I go back a year later to visit all the time. I can't say enough good about uplands.

Experiencia positiva: Would love to give a 5 but my Fathers Potassium intake was never address. They order blood test and used my name to get it done. No information was passed on from Emergency room Doctor to the transportation to the care facility. My family thought I was lying about the danger of his potassium intake.Dietitian never confered with his doctor.Everyday his numbers keep increasing. The staff were pleasent. Most of them. Some were rude and very mean to there patients. Everyday the menu had something my dad should not have.pineapple 966 mg of potassium. He was givin 2 bowls.lack of bathing was all over the facility.On the most part my father was treated pretty well.Just keep the one nurse away from him. Shes ery mean and rude to all the patients. Why is she thesh?

Experiencia fantástica: I love working here! The staff truly care about the residents and are always trying to improve. The medical staff is kind, attentive, and thorough. Feel free to come in for a tour! We would love to meet you and answer any questions you have regarding skilled nursing.
