Experiencia fantástica: I checked in for rehabilitation after my back surgery. The nurses, CNA's and all the staff were very attentive to my needs. Joe, my PT, helped me transitioning to a new way to do things. I also received help from the occupational therapist. Thanks to everyone.

Experiencia positiva: Was very disappointed in the lack of professionalism of one particular nurse who specifically wrote on the prescription "MOOD DISORDER Yelling at staff." Husband had TIAs and was told by a doctor that this was a common issue as well as Aphasia. The remaining staff was great, helpful and professional. To them I give 5 stars.

Experiencia fantástica: Freedom Plaza rehabilitation Care Center was a God send! Honestly, the staff went above and beyond to accommodate our loved one. It made her feel safe and comfortable as she began her journey to work back from a recent hospitalization due to some health set backs. The food was delicious. Specifically nurses Favi and Isabel from the 200 UNIT were exceptional! LPN Mary was great as well with a good bedside manner. The CNAs Henri, Bernard, Angela, and Jorge were attentive and quick with responding to her needs. Not to mention the Receptionist Sara is stellar and professional such an awesome first impression and smile coming in the door. Thank Freedom Plaza

Experiencia fantástica: I would like to praise all the people who help my pain and did so with compassion and untiring effort. Very impressed with all who helped me. In therapy Annie and chance were so outstanding. Help me a lot and taught me a lot. Also, I would like to bring special attention to, nurse, Isabella and my little buddy Molly. Don’t want to be graphic, but having a lady give me my showers and have the ability in the skills to make me feel as much at home as I possibly could. I thank her after a little while felt like a mannequin I was at Ease. An excellent staff in all areas. Thank you gang.

Experiencia fantástica: My mother was in skilled nursing at the FP Care Center for about 3 weeks. She received excellent care. The PT director spoke with us to give updates and was helpful and informative. The nurse on duty that we dealt with the most, Sandra, was helpful, an excellent communicator, and a caring provider. Thanks to all the others whose names I don't have, but that worked with my mom directly. She really appreciated all of you. She was transferred directly from skilled nursing to assisted living for a temporary stay. The process was simple and smooth thanks to the coordination between the two. Thanks for making a challenging time much easier.

Experiencia negativa: My mother was admitted to Freedom Plaza for rehab for 3 weeks. The day she got there, I asked for rails on her bed as she is a fall patient and she didn't receive rails for several days. My daughter stayed at the facility to assure my mom wouldn't fall. After several days, my mom began to decline. Me and my dad asked the nurses and CNAs several times to speak to a doctor to no avail. We never seen the doctor. I spoke with the social worker about releasing my mom so we can get her to a hospital and we kept getting excuses that she fails to thrive and they are doing the best they can. PEOPLE! Your best would have been to listen to her family instead of milking insurance and ignoring us because my mom was finally raced to the hospital due to her vitals dropping and guess what people!!! My mom died two days later on June 23!! She had a brain bleed and died from Covid due your facilities piss poor handling of your covid outbreak in June. I even dealt with a CNA who left my mom sitting on the toliet alone although she was wheel chair bound. UNacceptable!! It's clear that this facility isn't use to families advocating for their loved ones and I wanted to give a review once I processed all of this. I pray no no one elses loves ones suffer like my mom did in this awful place! Also save the excuses and apologies as I know your jobs is to act like this never happened, lie in charts and deflect. Me and my dad was there everyday so I witnessed the crap and spoke with everyone about my concerns!!

Experiencia fantástica: I want to explain my gratitude for OT and PT. This was an unexpected pleasure, I learned so incredibly much from OT and PT, never realized how important this was. It’s given me more confidence and in my ability to survive outside of freedom Plaza rehab. Anyway I’m grateful for you people and what you have given to me, thank you thank you.

Experiencia fantástica: My sister, Donna, was admitted to Freedom Plaza Rehabilitation Care Center on 6/12/2024 and discharged on 7/2/2024, 105B. She wanted me to post the following: "To each of the people, in each of the departments at the Freedom Plaza Care Center- Thank you for your OUTSTANDING HELP with EVERYTHING!" Most respectfully & gratefully. Donna I will add my own remarks. I was very impressed with the administrator, Ian Lewis, and the ladies, Josie and Alia, in the admission office who were extremely helpful with the issues regarding insurance and getting Donna admitted. I would also like to give a shout out to Joe Barnett, PTA who was excellent with my sister, encouraging her to do her best. Susanna, CNA, was extremely helpful to Donna. Lyrrae was so helpful in discharging and arranging transportation back to Donna’s home. Last, by not least, Sarah, who greeted me so warmly every time I came to see my sister, set the whole tone for the rehabilitation center. She is the first impression and always gives a warm welcome. I give Freedom Plaza Rehabilitation Center 5 stars! I would recommend them to anyone! Ginny Carlson

Experiencia fantástica: I was In Rehab at Freedom Plaza Care Center from 5-17-2024 thru 5-30-24. My experience with the entire staff, from the office, nurses, cna's, & therapist was perfect. Every one made sure all my needs were met in a timely matter. Call buttons were always answered promptly, and I was always treated with respect. Experiencia negativa: Horrible, my mothers correct medications were not given, Docotors orders nor Patient plan followed, Medical department's did not work together, rooms were filthy after multiple requests to clean. Shout out to Molly a CNA who went above and beyond her job duties. I would not recommend this place nor have my mother return, we pulled her out before her discharge date.

Experiencia positiva: Nursing staff was very courteous and caring.Very Johnny on the spot. However I was appalled that they did not fill an order for Trilogy when the currrent prescription was empty. I went 2 days without my medicine.

Experiencia fantástica: My mother-in-law, Veda was in Freedom Care Center for 20 days and absolutely loved it! And she is a very picky person but don’t tell her I said that! She felt she had the very best of care and loved all the people who helped her to heal and be ready to go home. The front desk gal was extremely friendly and my father-in-law, husband and I appreciated being greeted by her every morning! We would highly recommend Freedom Plaza Care Center!

Experiencia positiva: My wife was at Freedom Plaza Care Center for rehab for 12 days following a hip replacement. Our experience with PT and OT were great. We would rate them 5+. The nurses and CNAs on the floor were very good people, with a few negative exceptions. But they work under such intense conditions that their response time was commonly 20-30 minutes. It was difficult for management to provide an up-to-date list of the meds they were giving her, or for them to notify us about changes they made to meds. Food was usually tasteless and cold, with a few positive exceptions. More than once, when my wife ordered hot tea, she received a cup of hot water with no tea bag, or ordered bread and butter, but received butter with no bread. Several medical professionals told us that all rehab facilities are highly regulated, so there's not much difference from one to another.

Experiencia fantástica: Unlike other rehab facilities, the CNAs and nurses on each hall have 10 or less patients to care for on each shift. Other facilities I have had experience with had as many as 20 patients per shift. I received efficient, fast, caring help at Freedom Plaza far above any other facility. The fact that the furniture wasn't luxurious paled the first day compared to the compassionate care and love from the entire staff - CNAs, nurses, dining room staff (they actually remembered my name and how I needed my food presented after the first meal!!). I want to particularly commend CNA Henri - what a dear lady she is - thank you, Henri for all your love and concern and CARE. You get 10 stars **********!!

Experiencia negativa: I would NEVER recommend this care center. My mom was in this facility for 3 weeks. Her bathroom was always unkept and a strong smell of urine throughout the halls every day we visited. The nurses are so very rude. She suffered a fall, was on the ground and the nurses weren't aware of this until 2 hours later. One of the nurses attending to my mother, told me that one of her patients could wait for his pain meds "because he was mean." My mom was sent home with pneumonia and a horrible rash on her bottom. She had to be taken to the hospital 3 hours later because of this. Her case worker we met One time, and that was only to sign her release papers. Our experience with this facility was the WORST. It's very evident that the nurses and office staff need more training. If you really care for your loved one, Do NOT have them placed in this facility, please place them somewhere else.

Experiencia fantástica: Freedom plaza care center is an amazing care center!! The nurses are very caring. I felt very comfortable leaving my mom "Dianne" in the care of these wonderful caring people. Amazing employees, front desk always greeted me with a smile, and my mom said she felt safe knowing the wonderful nurse's were there taking care of her. Physical therapist were able to help my mom build her strength and confidence to leave the wheelchair behind and moving forward on a walker. Thank you

Experiencia negativa: My grandmother went there after her hip surgery in June. From the very start she was unable to do any rehab. It hurt her too badly to bear any weight on her left side. Which should have stood out as abnormal since people usually are up and walking with help on the same day as surgery. Sadly no. They instead trested her as a pain pill junkie. Which is sad. I personally had to speak with the nurse about her receiving a specific pain medication that she is normally on at home. The Dr. finally approved this for her. However they still would not even give the medication she was normally taking at home for a disease the entails a permanent and growing pocket of fluid on her cervical spine. Located in such a place that even neurologist will not touch for fear of paralysis. So here we have a woman who has just had hip surgery ad they are not giving her anything for that plus she was not receiving her normal dosages that she is used to at home. So I spoke with the for her on one of my visits and reasoned with this nurse about how difficult it is for people to heal well and do physical therapy when in so much pain. That shouldn't she at least be getting what she was taking at home. I feel additional for the surgery she just had . The nurse looking over her medications discovered she was not being givien her baclofen for muscle spams. Said she would speak to the doctor and get back to me. When she did she assured me that she had been taking the baclofen but that it must not be working any more so they put her on something similar. (Right) she wasn't being given it period. When will these companies hold some kind of accountability? So you made a mistake great fix it and lets move on. It is when you deny your faults that make people want to sue. Adding to that During her stay my grandfather passed away. Before he passed we attempted to take her to the hospital on 2 separate occasions. She was in too much pain she could not do any kind of transfer. Finally they were going to transport her but it would have cost us a fortune for transportation. Which is ridiculous. Plus they would only give her 4 hours. 4 hours for a family emergency. Finally a 3rd attempt because on that day grandfather was going to pass away, my uncles came also and helped her into the vehicle. She is still in an unusual amount of pain, and still only allowed 4 hours for him to die. The remainder of her time there was ridiculous And equally negligent. Upon her return to home i helped her get into her recliner. At which point I could see something was terribly wrong. I called her hip surgeon and 3 hours after being home went back to the hospital via ambulance. Where the ED discovered she had a broken pelvis that looked several weeks old. Causing her to be placed into another Rehabilitation facility. And i dont know why but the hospital asked freedom plaza they refused to take her. WOW! I mean really how low life can you go. Apparently all the way. Had this facility taken the time to listen to her complaints, provide her with the correct medication, and try and see if there was a reason she could not bear weight on her left side they would have discovered her broken pelvis. Instead she stayed shut away in her room, ignored her calls for assistance, and very judgmentally placed her as a pain pill junkie. Then could rid themselves of her fast enough. And her discharge papers HA. Your lucky she is too old to sue you. Aint that what itss about anymore? Dont waste the resources on someone so old and no contributions to society any more? Unless you want your loved one treated worse than a slave I recommend going elsewhere. Places like these should be put out of business. What has happened to the healthcare professional's and patient advocates? Smh I made a complaint heard nothing. Dont give me the no record or its your fault not ours or ask me to contact you about this. Already did. Makes you look more incompetent

Experiencia fantástica: SHOUT OUT to physical therapists & occupational therapists JOE, KATIE, LISA, (student-Brendon) and “Doctor”Jamie for not giving up on me. The inspirational personalities in that room are what this world needs today. Thank you SHOUT OUT to the rest of staff. You guys rock!! You know who you are; Namely; ARSENIO, GUITARAS, Henry, LISA, GEORGE, Courtney, Evelyn, Christine, Favi, Maura, Anna, ….. just to name a few. (It’s been over six weeks now.) And this staff is successfully run by a management team, led by DOCTOR MANOJ MAKHIJA, MD, Medical Director. Thank you sir. I came in to Freedom Plaza Care from Banner Thunderbird, ER with a major groin pull coupled with a full left side sciatica unable to sit or stand; And left three weeks later, able to walk with a cane at home and drive a week later. Strict adherence to policies and procedures are what I believe was, and still is a major cornerstone of their success; Especially with their individual professionalism, patience, AND Sincere friendliness. This made my stay very comfortable and less stressful knowing this “TEAM” literally had my back. After 40 successful years of retail and restaurant management and ownership, I believe I know what I’m talking about. I am very grateful. THANK YOU DR. From the time you walk in the door to see your family member to when you leave for the day your are treated with the greatest kindness & feeling of family From directors to maintenance . His nursing staff were so awesome & friendly & helpful They all became family to us. Therapy was great & challenging and the food served was so good tasty. Experiencia fantástica: If I could give this place 10 stars, I would. I selected this facility after my mother had been at Banner Estrella Hospital for a month due to a heart attack and stroke. I was given a list of rehab facilities, and after doing my research I chose Freedom Plaza Care Center for their great reviews and how nice their facilities were. I am extremely happy with the care and rehab therapy they did with my mother. They were very polite, professional, friendly and caring. My mother received amazing care. I would like to thank everyone for the great work that you do. I am truly grateful and appreciative of Freedom Plaza Care Center. You guys truly do amazing work. Thank you!

Experiencia negativa: It's been a while and I guess things could have changed but my experience with this place was VERY bad. To start with when I came to this place I was very delusional and had no understanding of what was going on around me. I was asked to sign my initial paperwork and I told them I was confused and was not comfortable with signing anything until I could talk with family. The next day my mom explained to them I was not at a point to understand what I was signing and that my husband would be in later to take care of any paperwork but they insisted I sign them. They did an assessment and said I was coherent enough even though I answered every question wrong (I only know this because my mom told me about it). After that I would say things that were clearly delusional and instead of helping me see reality the staff would feed into my delusions. At one point my teen kids came to visit me and when they came into my room I was sleeping. So my kids were going to go grab something to eat and come back later but on their way out a CNA said hang on I will get her up and came into my room and plugged my nose till I woke up gasping for breath. My friend, who brought my kids, immediately told the nurse and the CNA was ultimately fired. However, as I was becoming less delusional another CNA came to me and told me that I got the other CNA fired. I remember telling her no that if my kids said it happened then it did. Another time I remember is falling out of my bed because the staff would not answer my call light and I tried to get out on my own and fell. They deny it ever happened but I know it did. The final straw is when I wanted a pass for the Thanksgiving holiday and they told me they would only give me 4 hours (I lived an hour from the facility). At that point I asked to be discharged and they stated they didn't think I was ready even though I was able to do what I was sent there for (I think they wanted my insurance money). I told them I wanted a second opinion and they said they wouldn't do that. By this point I was no longer delusional and replied with something to the effect of let me talk to a lawyer about that because I knew I had the right to a second opinion. I was discharged shortly after that. Later when I tried to get my medical records I was denied access to them. Most times I would not get a return call but one time I was told they didn't have any records. Eventually I gave up finding things because I had no way to prove all that happened. Now I just am writing my experience so that I can hopefully stop someone else from going through the same thing.

Experiencia fantástica: Best ever. When Discharged from Boswell I really didn’t know which care center to choose. A friend told me his family members had good experiences here so I choose Freedom Plaza. I am so thankful. They truly demonstrate patient care. When I pushed the nurses button I had an immediate response. They were friendly caring and educated. Discharge was when I decided. PT and OT great didn’t let me get lazy, progress so much more than I expected. Absolutely the best possible. My stay was in July of 2023.

Experiencia fantástica: OK if you have to be have the experience of being in a rehab center I must say that freedom plaza has been a wonderful place as much as could be a wonderful experience it has been from day one I knew I was in the right place everybody's been wonderful the care it's been just great and I have nothing bad to say about it so I recommend this place to anybody who is in need of rehab center as you don't want to be here but it's been wonderful experience and if I had to come back again I would be glad to come back again and I recommend this to anybody who has a good need for a wonderful rehab center thank you

Experiencia fantástica: My husband was treated here from 12-15 to 12-31 2022 the work they did was great in getting him stronger in just 2 weeks. The staff was amazing from nurses, CNAS, PT & OT. Keeping me informed answering any questions I had. Also arranging for home health after his release. I have no complaints with this facility. I give it 5 stars

Experiencia fantástica: I chose Freedom Plaza because of the proximity to my home and the excellent reviews it had on the internet. I certainly made the right choice! Not only did I get convenience for my family and friends to visit, but soon after I was admitted, it was so apparent how caring and dedicated the staff is to its "residents!". I have had excellent care, superb therapy, (OT and PT), excellent food, and capable, friendly, people that I truly enjoy, surrounding me. Thank you everyone, at Freedom Plaza!

I would like to personally Thank Katie, Cindy, Maysom, Amanda, and Robert. I truly believe they should get a 20.00 per hour raise for the excellent service they provided. I Appreciate and Thank all my nurses and CNA's they were always kind and concerned about my well being Arsenio, George, Debbie, Ana, Angela and Lina. Experiencia fantástica: I have stage 4 emphysema and had been in the hospital twice in 30 days. Left me terribly weak. The staff at Freedom Plaza helped me in every way to regain enough strength to return home. From the Dr., nursing staff, physical therapy and numerous other staff members, ready to help and listen. The facilities are very roomy and cheerful. The food was wonderful.

Experiencia fantástica: My name is Linda Inglis I was in room 118 for 2 months. It became laughable between the staff and I as I was there so long. The care I received both by the staff, nurses and CNA's was caring, kind and professional. Amada and Jasime also Cindy, Robert, Ina so many more I can't remember. I want to thank you for all you did for me. It was a hard 2 months but the care I received I was able to go home, I recommend Freedom Plaza to anyone looking to place a loved one for excellent care. Jasmine and Robert were the best they really showed that they cared and did everything they could to get him up and moving and ready to move. Some of his nurses A+ we loved Favi, Joy, Anna, Cho, Norma, Molly and George. I am sure there were others but these are the ones that stood out me. Experiencia fantástica: My husband was transported here after being in the hospital for 10 days. He did so well with his rehab sessions that he was released to return home on day 6. He was treated very well by nurses and staff.

Experiencia fantástica: FREEDOM PLAZA CARE is a 5 star facility. This is a very well run organization. My mother was admitted for 15 days to receive rehabilitation after a fall. The rooms are spacious, the care was very good, housekeeping is top notch. The facility smelled clean and the decor was very attractive and inviting. There are several covered patios to sit outside. The Dining room was elegant and the food was very good. The facility also has a beauty and barber shop. The rehabilation staff provided sufficient hours of physical, occupational and speech therapy. Nursing staff were all kind, caring and knowledgable. The Doctors provided great care. I would highly recommend this facility without any hesitation. Freedom plaza has a contract with Medicare so the transition from the hospital to the facility occurred over the weekend without any problem. Update on Freedom Plaza. My mom was referred to Freedom Plaza from the hospital because she needed rehabilitation to strengthen her body and rehabilitate due to low blood pressure. Once again they gave her a tune up for 14 days and she is better than ever. She has regained her appetite and is stronger than before. This is definitely our go to place whenever she requires rehabilitation. I can't say enough good things about this facility. I believe, they are assisting us in keeping her living in her own home. I Thank every staff member there. We are grateful.

Encouraging and well organized, they and the nursing staff have assisted me to become stronger everyday. Facility is very clean, no negative smells. My room is spacious, with character. Food is good and is served very younger staff who do a great job. It's been a most positive experience rehabbing following Open Heart Surgery.l

Experiencia fantástica: Shirley was a resident at Freedom Plaza from Oct 20 to Nov 5 , 2021. In spite of the fact that Shirl was there because of a severe “brain bleed stroke” and subsequent paralysis, we can not say enough positive about Freedom. Her room was great and the support from the Nurses , CNAs and Aides was outstanding. Shirl’s condition made their jobs difficult but they did it in a professional and CARING manner. The OT, PT and Speech Therapists and Aides are to be commended for all their work in guiding Shirl down the path of recovery. Freedom Plaza has our deepest thanks and gratitude for a job well done

Experiencia fantástica: The therapists at Freedom Plaza Care Center were wonderful when a family member needed therapy after a joint replacement. They were caring, friendly, knowledgeable and fun. They provided therapy each day, even on weekends. I highly recommend this great group of people.

Experiencia fantástica: My mother's stay was short at the Freedom Plaza Care center and that is no doubt a testament in itself of the great work the nursing staff and therapists do there! Although at times they were short-staffed, the nursing assistants remained calm, kind, and helpful - always apologizing for the wait. She gets emotional thinking about her own progress and how quick they got her up and about, and shudders at how in the hospital she just lay in bed becoming more and more immobile and sore. From a visitors standpoint, the visiting hours are plentiful, the communication is open, and we always felt welcome. We definitely recommend this rehabilitation center! Thank you, Freedom Plaza!

Physical therapy was every day except Sunday and commendations to Jaime for getting me into wheelchair for 1st time after surgery, Pt therapists Cindy & Joseph, Ot therapists Lisa, Maria & Robert, scheduler & tranports Paula & Ariana. Therapists were kind, caring & understanding as my therapy was unique due to non weight bearing restriction. Exceptional care by nurses with special thanks to Norma, Sandra, Kristine, Favi and Arsenio. Also cnas Anna, Nylma, Lauren (sp), Cho, Joseph & Jorge (only had him one day but understand he is great). Didn't get to know the night staff very well but a big thank you to them also. They all were very prompt in answering call buttons and to my special needs during my stay. Experiencia fantástica: My dad was a patient at Freedom Plaza Care Center and the care that he received there was outstanding. He enjoyed his stay and spoke very highly of everyone that helped him become independent again. I agree with that as I visited daily and they far exceeded my expectations. The physical therapy and occupational therapy teams were fantastic. The nursing staff and case manager and administrative staff were caring and kind and thorough. My dad asked me to give a shout out to Molly who was amazing to him making him feel like family, as well as Joy and Carmen. They were all encouraging and got him back home to my mom. I highly recommend this place.

Experiencia fantástica: I, Robin Silverman, enjoyed my stay at Freedom Plaza. I found everybody exceptional, caring and very professional. This includes nurses, rehab staff and building administration. I would recommend this facility to anyone who needs gentle, caring rehabilitation and other services that are wonderful. I thoroughly enjoyed and made great progress with Maria, Robert, Amanda, Joe, Katie, Cindy and Becky in the rehab department. Ian and Jasmine took also took time out of their busy schedule to connect with my family and myself.

Experiencia negativa: My mom goes to dialysis and was happy at first....but once she got COVID, they all worked like chickens without heads. The staff did not know where to send her for dialysis and would leave for the day without informing us that she did not go to dialysis. My mom had to tell us that she did not go! When Freedom finally got it together, they would send my mom to get dialysis and not pick her up for HOURS later! Freedom sent my mom to Banner emergency to get dialysis after not taking her to dialysis for four days. My mom called me after several hours of being at the hospital. I called and spoke with the emergency nurse she said, " I have only had one person in my time working here to ever need emergency dialysis". They sent my mom to the hospital at 11:40 am and did not pick her up from the hospital until 8:00 pm. She had not eaten all the time that she was in the emergency room. Since my mom had COVID she had to go to another facility to receive dialysis. On the first day, the dialysis center called myself and my siblings to see if we had another number for Freedom Care Center because they had not picked up my mom. Imagine how that must feel to receive a call and not be able to do anything yourself. There was no personal answering the phone after 4:00 pm, I know, I tried, several times. I found that most of the staff left for the day before relaying information to the incoming staff and when questions were asked, no one had answers. The Case Manager Kim often times did not know any more than the CNAs. Terry the Unit secretary never made contact with transportation or the dialysis centers to make sure times were consistent. Freedom may have high ratings for "therapy", but if you are looking for care beyond therapy, THIS IS NOT THE PLACE TO STAY. Notice that the high stars are only for therapy. During time of COVID, Freedom had two years to get it together, I can't believe they are still working like it is 2019. Didn't know I could EDIT to comment on owner's response....pre-recorded calls about COVID updates on staff and patients DOES NOT COUNT as "direct communication" in my book.

We went to two other nursing facilities prior to coming to Freedom and they don't come close to comparing to how exceptional Freedom Plaza is. The facility is clean and tidy and the food is delicious.

The occupational and physical therapists are above reproach in their method of getting the patient back to how they were before they came to the center. The therapists are mindful of the diversity of their patients and try to address each individual in their special needs. The therapists demonstrate outstanding patience with everyone. The patient's diversity does not preclude the therapists the attention required for a positive outcome. P.S. The food is great. I'll be dieting when I go home ;)

Experiencia fantástica: These services for my husband were recommended by a physical therapist in the hospital. She really promoted the fact that the facility has superior physical and occupational therapy services. I would have to agree. Having been a speech therapist and a hospital administrator in my past, I recognize superior services in this area. The therapists are very knowledgeable and extremely patient with their clients. Since most of the clients are elderly and may need extra attention to hear and understand, this is extremely important. There are enough therapists for them to help each other when they need 2-person lifts for patients. I would highly recommend these services.

I have nothing but praise for the staff and facilities. The care center staff are excellent: knowledgeable, kind and great communicating with me about all aspects of her care. Experiencia fantástica: Outstanding care all around! My mother spent a bit less than 2 weeks here for nursing care and therapy. It was an exceptional stay that I would recommend to anyone or their loved one. Room was spacious and private. They could use some updating of furniture and décor however. It was all functional but dated. Nursing was caring, accommodating and encouraged my mom to get moving and spend more time out of bed. Encouragement to attend all meals in the dining room even gave her more socialization, making her enjoy her stay more than expected. The therapy department was great. She had a few different therapists during her stay, with scheduled therapy times and sessions 5 to 6 times per week. The gym was friendly, inviting and even had a sitting area for family members. They focused on functional tasks so mom could get home sooner than expected. Case management arranged a seamless discharge, setting up home health for continued therapy at home. I recommend Freedom Plaza Care Center to anyone!

I hope that our paths may cross again as I feel a close bond to them, Kristine, Julie, Ann and all the other nurses as well as the CNA's. I am so glad that I came here rather than the other options I had. And hats off to the chef and kitchen staff. Excellent food and variety. Experiencia fantástica: I wish to share comments about the superb treatment received by my wife from the staff of the Freedom Plaza Care Center for the period January 1-January 16 2020. She transferred to FPCC from a hospital stay of seven days for surgery and recovery of a hip broken in three places from a fall. She arrived in great pain and complete loss of use of her right leg. Through progressive pain management, effective physical therapy, and a professional and committed group of care giving individuals, we are truly grateful that Sue has progressed physically, mentally and emotionally to return to our home. I stayed here the entire 16 days and came to understand and appreciate the caring service provided and feel compelled to pass my observations to management. Nurses: Julie and Kristine are true professionals. They go beyond expectations in proactively managing the dispensing of medications so that the patients can trust that their needs are met. They are responsive to the many diverse personal medical needs that occur on a daily basis. Most of our interactions were on the first two shifts, though we did call on Susan at night and she was equally responsive to our needs. They are always kind and helpful and it was comforting to have them available at all times. CNA’s: There were many caregivers who skillfully and with genuine care meet the basic needs of Sue and the other patients who have shared similar comments. The majority of our time was with Mia, Susana and Mabel, and they were each excellent, and we are thankful. The sincere personal attention to our care contributed significantly to Sue’s comfort and recovery. They seem to be the front-line warriors, the unsung heroes who make a real and positive impact. We were blessed with the relationships that were developed. PA: Patty is a great communicator who listens to patients concerns and who makes the common person comfortable when she explains their medical situations. Dining room: The food always tasted fine, the menu items are upscale quality, and the servings are sufficient and well presented. The white table cloths is a nice touch. The staff was polite, efficient and showed care for the welfare of the patients. The dining environment promotes fellowship with others and healthy opportunities to meet and encourage fellow patients. Physical/Occupational Therapy: Very professional group who take a personal interest in each patient. They know their names and create a personal relationship that comforts and make the PT/OT a pleasant experience. I spoke with many patients who were pleased with results and experience. Frendiha: She worked tirelessly with our hospital, insurance company and the transport company on New Year’s Day to make this transfer to FPCC possible. We feel like we are leaving many friends from this facility. We will highly recommend FPCC as a fantastic care center to anyone who might have such a need. God’s Blessings to all of you,

I just had a first hand experience for 10 days to have OT and PT due to my hip replacement. I was thrilled to be in their care! The staff was above and beyond friendly and it was obvious they knew their professional skills to work me through all therapy to provide healing. The admission people, case management and social services should all be ashamed of themselves for how they treat their patients and conduct their business. I could not get my dad out of their fast enough. His medications were sometimes administered hours late only to be told by the head nurse when asked multiple times for meds, I’m busy” . Some of the nursing staff had less than desirable hygiene practices. The prices you pay are ridiculous for the care provide. You can never get ahold of the same person and no one there ever has a clue what is going on with their patients. The state of Arizona federal disabilities department should do a check on this place. My dad who is in a wheelchair, was given a room that was not wheelchair accessible without bumping into walls. The room was More like a closet and certainly not to any handicap codes. I have been dealing with my dad’s care for a year and NEVER have I dealt with such rude and unprofessional people!! If you care about your loved one LOOK SOMEPLACE ELSE!!!

Experiencia fantástica: My mother was placed at Freedom Plaza after suffering a stroke and continues to live there. The staff take excellent care of my mother and meet her needs and expectations on a daily basis. Being 2,000 miles away, it was important for me to have constant and open communication to be able to manage her needs. Freedom has done that. Having regular communication with her Social Worker Helen, I am comfortable that mom is being taken care of and better yet, someone is always looking out for her. Lola, Connie and Carmen, some of her clinical team, show constant care, compassion and humor. Ian, one of the leaders has become a sounding board fro my mom as she deals with her new life. Ian is supportive and also acts as an advocate for mom. thank you all! Most recently, they started aromatherapy, at night , which my mother loves. Great job Debbie.

The food was outstanding, every meal I could eat with her in the dinning room. The doctors on staff kept me informed on what to expect next. The physical therapy group also work very well with each patient. Thank you for everything you did to make my wifives stay as pleasant as possible.

Experiencia fantástica: I worked as an aide at Freedom Plaza for four years and was very impressed by what I saw. The staff all truly care about their residents and want what’s best for them. They go out of their way to make sure that residents and family members alike feel at ease and at home, and when there is a problem or concern they all work together to find the solution that is best and most reasonable for all involved. I would recommend Freedom Plaza for employees and residents alike.

Experiencia negativa: My Father was at this facility and some of the staff were extremely rude with him. They yelled at him if he had trouble with anything. He needed to take his pills one at a time and they didnt like that. He dropped one once and they got mad about that. Just about anything. My sister asked for some orange juice for him and they told her he said he didnt want any. She asked if we could just try again and the nurse threw her hands up and walked off. She did bring the orange juice but not without making my Sister feel bad. He did drink the orange juice by the way. Overall there were more than one medical professionals there that were totally nasty with him. As healthcare professionals they should know that a person with diabetes can get grouchy at times if their sugar is off. I know this because my Father is an extremely mild mannered person when his BG levels are where they are supposed to be. Throwing pills at him and giving him a shot every day isnt worth anything if you are not aware of his BG levels or at least what unstable levels can do to a person. The experience was less than pleasant and if you have a choice go elsewhere.

The admissions personnel is terrible; they do not return phone calls, and Frandiha seems uncaring and definitely unprofessional in her duties! The facility looks like it has gone down in last couple of years, so ask a lot of questions. Do your research and ask family you see in a facility what they think about the care... Beware: 5 star rating is not a good indicator, Freedom is not a 5 star, but in this case, it is accurate. Good luck and be selective. Putting your loved one in short term or long term is a BIG DECISION. Choose wisely for the one you LOVE. Experiencia negativa: Last September (2017) my mother was cared for by Freedom Plaza after a right hip surgery due to a break. Sadly, 12 days later she fell one early morning that resulted with a femur break above the knee on the same right side. Freedom Plaza contacted me 3 hours later. Staff and administration shared their concerns with a meeting that was held, they reassured me that if given a chance she would be cared for in the best possible way after the femur break surgery. Their reassurance convinced us to allow my mother to return to Freedom Plaza for rehab. Approximately six weeks later, she was discharged with what we were told was a blister on the right calf that was bandaged, and could be cared for very easily. The following day, at home, my mother was assessed by a home health care nurse - the "blister" was a stage 3 wound that needed to be treated by a wound specialist. This was our experience. Though a number of the staff were pleasant during my mom's stay - her care, particularly with the stage 3 wound that was described as a blister was suspect, if not just negligent.

Experiencia negativa: Review # 2. I gave raving reviews 2 weeks ago...However something went drastically wrong !! They speak a good story...however our fi al experience was horrible. They released my Uncle on 1-25..due to Insurance Medicare and not making progress. He could not.he was unable to walk and he was in so much pain. He was moved anyway to a Group Home that day ..and died on Friday January 26....shame on you FreedomPlaza for releasing my Uncle in such appaling condition. I left a message with the social worker on 1-25 the day he was released...NO ONE ever called back.. PLEASE beware and do not allow this to happen to your loved one.. Moneen Durkin
