Experiencia fantástica: Thank you Everyone, For Taking SUCH GOOD care of my Mom, Bonnie B. She was extremely pleased with everything. & that's not easy to do. She LOVED the cuisine! & made a joke recently about seeing if she could make a reservation, for Valentines dinner. I thank you, because you eased a worried daughter's mind, trying to maintain my Mom, from Michigan. Dr. Vasques, & staff, went above & beyond. THANK YOU, from the bottom of my Heart. Experiencia fantástica: My grandmother was recovering at this hospital for a couple of months and whenever I’d visit there was this care taker she always kept talking to me about. How great and friendly she was and how she looked forward to getting to spend some time with her and the conversations they’d have, Elisabeth is the one I want to thank she brought the menu options for the day. The Dietary department; always providing great service and food, bringing a smile to the people I love, thank you.

Experiencia fantástica: Yessenia, where do I start. Thank you for everything you did for my father. I know at times he could be persistent and demands attention immediately but you never put him aside you put your work aside to see what Rudy needed. I’ve never had someone who wasn’t family to give him that attention. You always knew what to do to calm him down and make him feel happy till my mother arrived. Simple things like going to his room to get him for bingo, arts an crafts etc. putting money out of your pocket to make sure ALL patients enjoy their activity time (not many people do that) and holidays. Even had singing time because my dad loved to sing. I will forever be grateful to you and a lot of employees that work there. Keep being you because it doesn’t go unnoticed.

Experiencia negativa: I wish I could give this place more stars since some of the staff at least try to help. Unfortunately there are too many people here that don’t know what they’re doing. 30 minutes stopping people in the hall to tell them my grandma needs to go to the restroom. Different stories from different people. She is going downhill quickly because she can’t eat the dry, bland food. She came here for recovery for a broken foot and will be leaving with more issues than she came with. I’m still here so I will see how the rest of the day goes Update. She came to this facility with very low sodium. For some reason these geniuses put her on a no sodium diet, now after a week of being at that place she is In the icu at the hospital because she now has extremely low sodium. Changed my star from 2 to 1. If you love your family members DO NOT SEND THEM HERE!!!! 2nd update and response to the owner. There is no need to contact you. If you cared,your people would be trained better.40 minutes to get someone to help a patient to the bathroom is unacceptable. We had to ask 5 different people. You know what kind of facility you have there, don’t try to act like you care just cause you get bad reviews

Experiencia negativa: 10/10 do NOT recommend. My dad was there for less than 24 hrs and we pulled him out. He just had a trach put in but it got infected. Took hours to get him his pain meds for his severe pain. He had roaches in his room. His trach was supposed to be cleaned as close to every 2 hours as possible due to the infection and it was not done once out of the 16 hours he was there. He was suppose to be on antibiotics but never got them so he missed 2 doses of it.

Experiencia fantástica: I've been coming gor 6 1/2 years off and on with diabetic ulcers they've treated them every time and given me physical therapy as well. The staff always has been caring and kind. My needs have been met and now that I'm an amputee their care is the best I've had. I love country oaks. They make me feel like family.

Experiencia negativa: Where do I start! My father needed a nursing facility and he needed to be monitored for several deferent things. If i wasn’t there always my father would have gotten sepsis from and infection. So many things went unnoticed. At times i would push the call button for my Dad and no one came till about 40 minutes later. Some of the staff was rude and did not know what they were doing or what to look for in order for their patience’s to stay safe. The management lacking to say the least. Horrible place!!!
Experiencia fantástica: A case manager suggested I go for a tour of this facility yesterday. Though hesitant to send my loved one to any skilled nursing facility, I saw that Country Oaks Care Center did have a high Medicare star rating. Upon arriving I was pleasantly surprised by helpful, welcoming staff. Their admissions person gave me a tour and answered all the questions I had in addition to offering some additional help with questions I had regarding discharge from the hospital. I now understand that my loved one can benefit from the additional hours of physical therapy here. I also learned that I can visit whenever I like to ensure my family feels comfortable. Thankfully I also had a chance to speak to their head nurse who made me feel very comfortable. I am now feeling more confident we can get the help we need.
Experiencia negativa: If you want a caring, nurturing & responsible place for your loved one(s), PLEASE LOOK SOMEWHERE ELSE! This establishment is NOT recommended to anyone by me, or my family. My grandmother has been here for nearly 2 months and she has far more problems now, than she did when they first placed her in here from Pomona Valley Hospital. They're highly secretive with the medications they're giving to her which makes me wonder what they're REALLY giving her since she is only getting worse by each passing day. She had foot problems for the last few years and she has been taking great care of it herself but since she's been in this place, they don't clean it, dress it or anything to help the healing process. They seem highly neglectful, especially with things like this. 2 weeks ago (prior to finding someone from hospice), her legs and feet were extremely swollen due to water retention and based on how much worse she got, I can safely assume that they were not giving her any lasix although, it was recommended that she did. *If it wasn't for the worker from hospice getting on their ass about it, who knows how much worse it would've gotten.* The staff seems nice, at first glance but in reality, they've shown that most of them don't care for those that stay here. If they did, our loved ones would be getting better, NOT worse. And to add insult to injury, everything we brought from home (bed comforter, pillow cases, cell phone, her favorite head band, and other miscellaneous things), have came up missing or, with other patients. So, in a nutshell: •They're neglectful of skin & general care •They do not give the proper medications needed to help with certain diseases/illnesses •They're secretive with the medications they're giving •Some of the staff members have really bad attitudes when we ask them questions regarding their patients •They steal from patients It's best to look for another facility that will care for your loved one(s) than to have them stay here. This is an awful place. *To those that have lost someone due to this place, my condolences to you and your family.*
Experiencia negativa: This is my dad's email my dad has Pseudomonas which Pomona Valley Hospital put him on intervenous anabiotic's from there he went to Cedar Sinai they kept him on the intervenous anabiotic's he went to country Oaks here center in Pomona and they changed his medication to pills which anybody that knows about Pseudomonas , knows the very dangerous disease anybody that's a transplant or has a zero Meun system that's on anti-rejection pills will catch it no problem it doesn't attack people that have an immune system you could look it up it's the new hospital superbug nobody wanted at the facility yet they gave my dad poor treatment it started to come , which my mom and sister tried to talk to them and they're both RNs my mom worked at Pomona Valley Hospital Charlene O Banuelos, 26 years labor and delivery. My sister Lisa Perez works at pediatric for Pomona Valley Hospital 17 years they both tried to talk to these people and ask them what are they doing why did they change the medication they started giving us the runaround in the meantime my dad infection came back because of the change of the anabiotic's I had to get pictures off the Internet what pseudomonas does to people and downloaded how you catch it and if somebody's got it how people can catch it to get them to get my dad treatment because the next day we went to visit him and we were going there from seven in the morning till night time sometimes with leave at 11 o'clock we find out they send him back to Pomona Valley the very next day he was quarantined in ICU they put them back on intervenous anabiotic's we didn't want him to go back there but he did they changed his medication again back to pill form a couple weeks after that my mom in one of the aids was changing my dad shirt and we noticed bruises on his chest and shoulders I tried to ask him what happened nobody wanted to talk about it then I said look you change them every day why weren't we notified the very next day you seen these bruises they blamed it on the paramedic people that transport him to dialysis they said it was from the straps . Which the straps don't go over his shoulder because we had the paramedics when we finally took him from that facility transport him from my moms house to dialysis and the straps across his chest . When they were doing the discharge we brought all that up they play the part like they said that they were here to help us they weren't. The case manager said they'd have the paramedics drop my dad off from dialysis to the house which my mom had to contact them because the case manager never did that I wouldn't take my dog here because these people are liars I have pictures and recorded tapes of conversations with them about the care of my dad's getting after the bruises to prove what I'm saying is true if they we're doing what they were supposed to be doing their job taking care of my dad . Wouldn't be in the shape you he is now there's some days he don't even know who I am then he talks to people ain't even there. We also put a pic line in his arm which this place never change the dressings or flushed it it's a good thing you didn't have a clot because it would've killed . When he was released they gave us somebody else's dressings for the pick line which we got the name on the package we took pictures and we still got it the nurse that came to check my dad today seen the bruises and the name for the dressing for the pick line wasnt my dads , like the person that left a review a year ago we were had to bring my dad food. My mom brought one of their sandwiches home my dog wouldn't even need it. I took pictures of that too so anybody that wants the call me a liar will be able to back it up back up back up what I'm saying, couple other things but this is getting long they will make contact with this I'll be sending all this to the newspaper to show A my dad was assaulted Date 8/3/2017, two days after his release my dad died at dialysis 8/5/2017 i'm going to the paper with the pictures and the tapes
Experiencia negativa: I was very upset with the security and maintenance of this facility. My sister witnessed the ceiling pouring down rain on my mother's bed. They were going to move her to another room where the floor was flooded. She ended up in the hallway - this is not acceptable in any situation. The staff was uncompassionate and did not seem to want to help the patients with any of their needs. My mom was left on the toilet over 30 minutes without any supervision and she has a shattered pelvis. Her roommate was also treated poorly when they dragged her across the room to the bathroom. My mom never had a shower or was bathed. If it was not for my sister being there to give her a sponge bath she would not have been clean. The meals that are served have no nutritional value and my sister took all of my mom's meals that she prepared herself. I do not feel that any patient is safe in this place. Housekeeping never did a thorough cleaning on a daily basis and indicated that was only completed once a month. After four days at home my mom has improved 100% with only my sister's care. I gave the one star because there was a night male CNA that took the time to show my mom compassion and made her feel that she was not living a nightmare.
