Experiencia fantástica: My mother broke her back and had surgery. She came to Trellis after her surgery and was there 3 months. I have been to other rehabs. This one looks like a nice hotel instead of.a dreary nursing home. They have private rooms and baths. It has a good atmosphere. The staff is helpful and courteous. The rehab was great. Always remember to be with your loved one every day and the staff pays more attention if you are there and on top of things, just like in the hospital!

Experiencia fantástica: My relative was admitted to Trellis in May 2024 and discharged in early Oct. unfortunately, a prior facility failed to provide appropriate nursing skills and she was verybdeconditioned on arrival. Great team work and careful slow work, she was successful in gaining back strength to continue therapy at home. Kudos to Emily from Speech Therapy; Roya, Robert, Raymond, Lindsay and Eunice from the therapy department. She is back to walking using a walker and soon be walking independently! Love licensed staff RN’s and LVN’s watched over planning and progress- Jodi, Lila, Ben, Gio, Arranza, Liz, Tracey, Adrianna, Tracey, Eddie and Jose were excellent. Nurse aides Mayra, Daisha, Merlin, Maiacela, Jonathan, Guillermo, Anna were so caring and encouraging. You each made a difference! The Administrative team listened to concerns, worked to make adjustments and changes to correct problems. The facility is clean, well maintained and the EVS staff engaged. Food service is a work in progress. Diets are bland, dry and need some work! Thank you all so very much! I recommend Trellis to all who need help and can work hard! Case manager Yuri was key to communication and a clone! All our best!

Experiencia fantástica: After going to another facility for 48 hours I was so thankful and love the staff at Trellis. I think it would be hard to find such a nice, helpful , caring place as this. If I could give it 6 stars I would...

Experiencia negativa: If I could give 0 stars I would, I’m typing this as my loved one is being carried out of here back to the hospital in an ambulance. The fireman himself said “we’re down here all the time this place is horrible you must advocate for your loved one” Genuine patient care is non existent these days! Stay healthy because you don’t want to get ill in this day and age things just aren’t the same. Truly sad. DONT BRING YOUR LOVED ONE HERE TO RECOVER!

Experiencia negativa: My father was transfer here in august 2023 from kindred in Ontario. He has Pancreatic cancer and osteoporosis and malnutrition. So he needed extra physical therapy to gain strength and gain weight to give a better chance with the cancer. Nurses here are amazing and are so kind. Wound care also is absolutely amazing! They know how to provide professional care. The one star is for Administrative and Physical Therapy, my father was admitted and I would go everyday and ask the physical therapist how is he doing. They would always tell me hes doing good and getting there, my father went into there walking and came out completely bedridden. I ask to speak to whoever in charge to explain to me what happened and no one can give answers. Administration/social worker is absolutely horrible! Lady never follows up and I was not notified of his discharge till the day before! Someone called me stating they needed my approval to send him to a long term care because his physical therapy still needed help. I brought him home and thanks to this facility his care was completely gone backwards and no hope of him walking again. DO NOT SEND YOUR FAMILY MEMBERS HERE if they need physical therapy.

Experiencia negativa: Do not send your loved one here. They killed my mother after being there for 5 days. They don’t answer the call buttons resulting in your loved ones trying to do things on there own.

Experiencia positiva: I would like to give this place higher grade, but cannot due to several issues. Pros: very clean, individual rooms, friendly staff and front desk, good physical therapy Cons: food is not good, they are understaffed, so patient calls and they come 45 minutes later if not longer. Staff changes all the time

Experiencia negativa: This place is very clean and you do have your own private room and bathroon and shower and good PT dept and the nurses are nice but overworked.. Have only seen a Dr once only after being here 2days. Food is not Diabetic or Chonic Disease Friendly unless you are very vocal about it small portion unless you are vocal about this also. Medication has only been right 1 day. Take long acting insulin in the morning the nurse tried to give me 2 more units after dinner because it was 187 before she checked my chart. Suppose to check my sugar before breakfast lunch and dinner. On Satruday 2/19/2022 had to ask for my meds and to have my sugar checked twice. Didnt get it done till after dinner nurse said she was late because they are short staffed. What do they do about a patient you is older and can't speak up for themselves. Very concerned about patients who are sicker than me with no family or advocate to speak for themselves Rooms are private very clean but the medication and meals and lack of nurses are a big concern. Arrived 2/11/2022 still having problem after over a week here. UPDATE finaly got an appt for an MRI the ball was dropped no one made the appt for me. Been here almost 2 wks and no one followed thru with this appt. UPDATE #2 went for MRI order was for wrong knee how can one place make mistake after another!!!!! UPDATE #3 After 2 wks still problem asked to be released one prescription filled by them sent it to wrong pharmacy and same brand of insulin but wrong type

Experiencia negativa: Do not go to this facility. My brother is currently there he is diabetic and just had his leg amputated. He has been there 3 days so far to continue his physical therapy and wound care, but hasn't been moved yet. He has asked numerous times about his therapy and they all ignore him.. We will be bringing him home soon to care for him.

Experiencia negativa: My Mother in law was admitted to Trellis Chino for rehabilitation following surgery. She was not tended to with any regularity during her stay, frequently left for hours in a wet adult diaper and bedding, not given her pain meds, and her colostomy bag was not changed during her entire seven day stay. We finally pulled her out early to bring her home to give her proper care at the resistance of the Trellis staff.

Experiencia fantástica: Trellis Chino is a God Send and Life Saver! They truly Care about their patients. I was very ill and depressed when I first arrived and they quickly turned my world around. They're superseding my expectations. They're doing excellent in helping me heal. Everyday I feel better and better. Experiencia fantástica: Trellis is the most beautiful rehab facility. The quiet, private rooms provide the patients the best environment to rest & recuperate so that they may participate in therapy to the fullest. This place is one of a kind!

Experiencia fantástica: I have a friend who is presently there and, to all appearance, is being well cared for! I daresay even "spoiled!" Get well my friend.

Experiencia positiva: Current patient. Wound care 2nd to NONE. I have been seen by an Optometrist since my arrival, 5 days ago. Glasses have been ordered. I"ve been to 6 SNF's since 08/2018. Clean modern well staffed facility. I will keep you posted throughout my stay until the end of July.

Experiencia negativa: I nave been at Trellis for almost 10 weeks during that time four (4) separate medical appointments were missed due to a variety or reasons that include transportation failure to notify patient not to eat food prior to appointment to name a few. I have also returned from medical appointments with doctors orders that were and still are not being fulfilled. This is my life we are talking about.... My recovery so that I am prepared for a major surgery DEPENDS on these doctors orders be followed to the letter and in a timely manner. NIETHER of these are being done. When bringing my concerns to staff by me personally I receive NO clarity. I don't have family or friends that can challenge this facility to do better....at times I'm TERRIFIED

Experiencia negativa: 0/5 stars. Not diabetic friendly, the patient ended up back into the ICU due to lack of insulin and ketoacidosis and forced her to eat yogurt and sweetened foods if she wanted her insulin. She requested many times to come back to the hospital their care was so bad.

Experiencia fantástica: My mom was a patient of this facility and received wonderful care above and beyond what would be expected. Her physical therapy made her stronger and more confident. The staff are superior and quality of care outstanding. My sister and I had viewed some recommended facilities for her rehab and was horrified at the condition of these facilities. My sister and I was then given the name of Trellis and we thought ok we'll give it a look. We were absolutely amazed at the cleanliness of the facility and the welcoming staff. I would highly recommend this facility to anyone.

Experiencia negativa: Do not ever use this facility. I would rate them a minus 10. They do not care about the patients well being at all. The patient was no where being able to come home. They kicked him out saying their transportation would help bring him in the house. They put him in his wheelchair and left. This patient is 6'8" and over 400lbs. My son and his friend had to manage to get him into the house. This facility lied. This patient cannot get up or walk. Do not take anyone you care about here. I will be filing a complaint with every agency I can.

Experiencia negativa: This is the worst place on trying to get feedback on your relative. I have called and 10 times they have told me that the nurse is busy having to wait all day into the next day worried about your family member because they are not awake and cannot talk trying to get information out of these people is horrendous. And trying to get the administrator to call you back is even worse I have left messages and have not gotten any call back. I will be trying to get my sister moved from that facility ASAP.

Experiencia negativa: Went to see my niece and clearly understanding that due to COVID-19 many protocols and procedures are either have been changed or implemented, but it was the worse experience trying to have a visit. We were told no visit during meal hours, shift change. That leaves you very limited. I understand call ahead for them to prepare your loved one for a visit, but then the screens are so filthy you can’t see through them and you think they could make a small pathway through the landscape that you step on to get to the window. Our visit consisted of seeing our loved one from the knees down cause they didn’t bother to move her to the window as we were told they were preparing to do during our wait time for our visit. Shame on Trellis for trying to make families feel as though they are not welcome to come see their loved ones .

Experiencia fantástica: Trellis is the most cleanist upscale rehab and nursing home. I was in the hospital for 37 days and got transferred to this rehabilitation center for wound care. I have a wound in my bottom so keeping it clean everyday is a must. Having your clean personal shower and bathroom is A+. I have my own private room and big screen tv, I connected my PS4 to the tv so time goes by quick while healing. I feel like im at a marriot or 5 star hotel with room service, and doctors and nurses. The food is great, custom order the way I want. I asked for any food, and they go make it. Even the awesome kitchen manager Jessica comes and ask me about my food and ask what kind of food I want. I feel very spoiled being here, but being here is helping my wound. The CNA and Nurse are great, they all have great attitude and personality. The nurses come with one push of the button. Always available 24/7. Trellis should be the standard in Rehabilitation and nursing centers across the country. I seen places that makes you run out the door with hand covering your nose because of the smell. At Trellis this place smells good and is modern. Im giving Trellis Chino 5 stars and A+ seal of a approval

Experiencia fantástica: More like a hotel than a SNF. The administrator Gary Little is amazing. 90% of the staff amazing. Clean, beautiful, space to meet with family. Head and shoulders above any SNF we visited.

Experiencia negativa: I would not recommend this place to anyone. The person who answers calls are very nice but the case manager Julie never calls back no matter how many times you leave messages. I’ve tried asking the receptionist for Gary’s number to complain but nope, they refused. Until now 3 different people are trying to get in touch with Julie and left messages but no return calls. We are waiting for an update on the person under their care and she’s been there for 3 weeks and we have no idea what’s her health status at all. The facility is very nice BUT what good is a nice facility if the workers doesn’t do their jobs? I rather put my patient to an old place but the workers are doing their jobs.

Experiencia negativa: The social worker decides on the patients phone calls and visitors when its none of her business she just cuts into family and friends conversations. Not right. The patient is able and stable enough to decide who he wants to see. Right This social worker has caused alot of problems between family and friends and we will take it higher up. Shes starting problems that were never there . Not a professional at.. She needs to check herself.Before someone else does from Her stirring things up.

Experiencia fantástica: I’ve been at Trellis for a week for my therapy treatment and I’m very happy I was sent here, very nice place, new, confortable, rooms are beautifully and modern decorated, food is great, we have a daily menu to choose from but the best at Trellis is the exceptional service by everyone specially by every single nurse, they go out of their way to make you feel better and always with a smile and positive attitude, they seem to be very happy working at Trellis. I’ve been in past years at several rehab centers in the area but nothing compared to Trellis. I wouldn’t go in the future to another rehab center, only to Trellis .

Experiencia fantástica: Imagine having your own private room, plus a huge bathroom with a shower. I have been a happy patient on the two times I was there, learning how to walk again with a walker. The physical therapists are not only professional, but they work with you on your goals to better health. The nurses and aids give their very best,, along with their personal touch leading to healing. I have never come across a person at Trellis with an "attitude." I sing praise for this wonderful rehabilitation center. If in doubt, come and visit. After my family's tour, they had one word, - Amazing!

Experiencia fantástica: What a fabulous rehab facility!!! Not only is it beautiful, but the staff keeps family members updated on progress of loved ones. Attention to detail by all; nursing, physical therapists, culinary, social services, housekeeping. My only slightly negative comment is that marketing needs to be a little more educated on insurance plans. The only place I would send my mom in the future!!!

Experiencia negativa: My Sister who is BLIND has been there for 3 weeks ? I've left 4 days of messages for Denca the case mgt ?no reply Some of the operators who answer the phone are RUDE? My Sister an i contacted the supervisor Nurse because a cna tryed to tell my Sister that she was pulling her tubes that's in her back out due to surgery for blocked kidneys? The supervisor Nurse spoken to the cna who made that statement?then the cna confronted my Blind Sister while she was at there karaoke in front of OTHERS in the room? telling my Sister she's a lier? Then recently my Sister said a nurse came in her room an tell her that she's leaving the hospital with the tubes still in her back the following day ? Then my Sister said she was confronted by a gay nurse trying to turn my Sister gay ? Now what's all this demonic demons is working there I've told my Sister to Report them to there Supervisor an PRAYERS for there Soul? But when my Sister ask someone for the person in charge they tell her ?there busy or they don't no who that is just some excuse?that's how the BLIND is treated there but I'm planning on contacting the insurance company an filling a Report on this place ??Peace be with you

Experiencia negativa: I would not recommend this place to anyone. My grandmother’s health declined tremendously just being there one month. I honestly would like to know how these nurses and respiratory therapists still have jobs there. The food does not look appetizing at all. They say that the patients can put in special requests for something else to eat but nothing gets done. The lack of communication between the nurses is mind boggling. We were told my grandmother would be put on a list to get physical therapy, and it never happened. My grandmother needed to see the podiatrist, we were told that she would be put on the list to see one, and again it never happened. The respiratory therapist would put my grandmother’s oxygen cannula on wrong, and multiple times it wasn’t even being administered. It is a nice and clean facility but it is not staffed with smart people.

Experiencia fantástica: My mother stayed at Trellis for a little over two weeks. The facility and care was amazing. I've visited many SNF in the area and Trellis was the best. The staff are very friendly and helpful and the physical therapy helped my mother get back on her feet. Can't say enough good things about the facility.
