Experiencia positiva: I was here to Rehab after I broke my femur. I learned so much and how to get around and care for myself. Andraya and Kayla were awesome therapists. Sara and Renee were so supportive and caring. Catrina snd Carolyn was awesome nurses. They were the best nurses. Melissa was a wonderful CNA. Judy also was a wonderful CNAMy case worker Natasha was so supportive and helped with my needs. Very good facility. Very happy with them.

Short staffed, limited rehab, very poor if any communication, problems with oxygen system / generators causing oxygen not to be delivered to patients who need it, phones not answered /not functioning, denials from leadership all the way around (nothing wrong here), poor excuse for health care / rehab facility all the way around. Wow - employees giving this place 5 stars. Guess they need to do something to bring reviews up, reeks of desperation.

Experiencia negativa: This rehab facility is only concerned about themselves and the money. They don’t care whether the patient gets the rehab that they’re there therefore. If you need upper management, they are always unavailable to resolve concerns. The only person there that genuinely was concerned about the patient was Mike G , LPN. Way to go, Mike. I’m giving it a two star because of Mike.

Experiencia positiva: The place is very friendly and clean. Gives you a sense of being cozy. There are a lot of spaces for families to visit. They encourage family members to visit by adjusting the night time hours according to your situation. I would have put five stars if the rehabilitation part with physical therapy and occupational therapy was a little better. I think they should be doing more with my husband for his rehabilitation. But for the most part it's a very nice comfortable place to go in and recover from a hospital stay.

Experiencia fantástica: The staff and facility are excellent! My father transferred to The Center for end of life hospice, and the team was excellent. The nurses and CNAs were attentive, but also respectful to our family wishes. The business staff was professional, and walked our family through costs and procedures. The facility itself is well appointed, and well cared for; we were able to enter easily as visitors, and the patient accommodations are high quality. Nothing about the scenario within which our family found itself was desired with my father’s conditions, but The Center allowed us to focus on our family connections in a comfortable, supportive setting.

Experiencia negativa: They don’t have adequate staff. I only saw staff for meds and vitals. Meds were always late. Got maybe an hour a day of rehab just walking around the hall. Waste of time could have went home. Don’t bring your family here

Experiencia negativa: My MIL was there had to remove her for medical negligence. No drs on call during the weekend. Then they prescribed pain meds for a fluid build up problem.

Experiencia positiva: The staff at the facility are exceptional from the nursing staff to the rehabilitation staff. They were all wonderful with me treated me and respect and to help with anything I needed. I would definitely recommend miss facility for any One that has the need for professional nursing care my care plan was given to me, and I understood what I needed to do and our goals were and and help me to get my life in a better direction again, but kudos facility for professional and friendly staff and medical expertise

Experiencia negativa: My grandma had stayed here for two weeks and they didn’t help her do anything. She sat in the room alone pretty much the whole time. And by that I mean they didn’t do any type of rehab with her(the reason she was In there) She also only got a shower one time while being there! And I walked in on a worker being rude to her because my grandma couldn’t hear her and said excuse me I can’t hear you. She seen my little family walk in and got embarrassed and turned very red because she didn’t notice us until after she was rude. She was having my grandma sign papers which she shouldn’t have been because my grandma is forgetful and had no idea what she was signing. I would not recommend this place to anyone! We had multiple family members visit on a daily and was told she’d get a shower when we left and she never got one except the one time. I had put her clothes to hang on the shower and they stayed there the entire time. They don’t wash the clothes you have to take them home and wash them no big deal but that’s how we all knew she wasn’t getting showers daily or even every other day. Also she had an iPad and it disappeared. I called to ask for it and the nurse that answered said I don’t know that’s not my problem that’s not my job. They lost her iPad and say it’s not their problem! They also didn’t read the paper work because she wasn’t supposed to eat certain things and I had to tell them to not feed her. She has a hole in her stomach so certain foods can’t be digested properly. Don’t trust them to watch your loved ones!

Experiencia negativa: My husband’s stepfather was given very poor care here. Minimal movement, he developed bed sores. Released him when his legs were severely swollen and he was unable to walk or care for himself. The doctor did not want him released but due to insurance reasons they forced him out. Family never met with a caseworker or any staff in general the entire time he was there, almost 2 weeks. Within 12 hours of being released he was taken by ambulance and put back in the hospital due to the swollen legs and pneumonia.

Experiencia negativa: 2 stars only because there are a few decent care team members that actually care. Otherwise , I advise others to do your research on other facilities and avoid Park West, if at all possible.

Experiencia negativa: I have a family member in here for therapy, the weekend CNA attending to my mother's care is very lacking in compassion and caring. The only reason for 2 stars is the weekday staff. The CNA was great. I would recommend taking your family members to Colorado Springs if at all possible

Experiencia positiva: My 91 year old mother in law broke her hip, had surgery and needed to be in rehab for a couple of weeks so we chose The Center based on the description and pictures. There are good and not so good things about this facility. The staff is awesome but so short staffed and seem overwhelmed at times. Not their fault. Everyone was kind, caring and helpful. We expected more in physical therapy but it could be an under staffing problem again. What therapy she did have was good and the therapists were very kind. The food is very good and everything is very clean. The common areas are bright and clean. There is currently no staff for the beauty parlor so that was disappointing. Overall her stay was very good given the short staffing situation. I would recommend this facility and hope they get more help soon.

Experiencia negativa: The Most Horrible Two Months of My Life. Sent there for Recovery/Rehab after horrific car accident.. "I thought I Died And Went to Hell!" Begged for.shower.. After over One month! Got a shower!Food was awful.. No One Answe my Light for Over An Hour! I was Bedridden! Got a Bedsore! Nurse asked me "do you take the small pill the big one!?I HAD TO GET OUT OF THERE!

Experiencia fantástica: The staff at The Center at Park West was awesome, personal, and very caring! My grandma went in for rehab , but took a turn for the worst. They did everything in their power to make her and us comfortable. I will be recommending this facility to everyone who will listen. Thanks again to the staff you are forever in our hearts!

Experiencia fantástica: This place is amazing. All my needs were taken care of quickly and correctly. Physical therapy was wonderful, helping me get back on my feet after TKR. Food was pretty good too! I would recommend The Center to anyone.

Experiencia fantástica: I have been a patient for two months, one week, today. This is such a great place for recovery! I have really been blessed to have stayed here. All the care givers have, without exception, been wonderful (Thank you!). The meals have been exceptional and delicious. All services - transportation, custodial have been great and much appreciated. Experiencia negativa: I was totally ignored by the overnight staff! I got there at 2 pm and the staff who were there at that time were great! I had a great dinner at 5pm and after that I was totally ignored by the late night staff a male nurses assistant and the female nurse. I was a fall risk and I went ahead and put myself to bed. It was now 12 hrs, since my last pain meds. (which I received from the hospital before I got transferred to this rehab center) and the nurse seemed totally unconcerned that I was now in some pain. So I called my friend to come get me and bring me home. I have never been treated this way before from someone who is in this line of business to help and treat people with medical conditions.

Experiencia negativa: My Mom was a high fall risk, needed rehab from surgery. Advocated for a room close to the nurse station & took turns being there to help her as she could not press the call button. Staff seemed friendly and when there, were helpful, but LOW to NO response time. Inadequate staffing, equals Inadequate administration! (Staff do not hire staff.) Mom needed the restroom, pushed the call button twice within 20 minutes...Literally had to find someone & plead. They dont have any safety on beds for fall risks. (ie: bed rails, bed alarms). Her fall happened in between our visits (2 hour window) broke her hip and now in Hospital. If you are 100% functional this may be the place but would you need to be there? Innovage only covers 2 places in town, why is that?$?$? Park West is not Parkview but Dr's from Parkview own Park West. My husband was there a year ago and also had a bad fall was left alone after admission for 2 hours and needed the restroom but inadequate staff and had to try himself. I thought it was against the medical laws to have inadequate staff?

Experiencia fantástica: My Mom had excellent care at this center. She was there for rehab, but ended up on hospice. She told us she liked it there,and the food was good too! The staff was wonderful,caring, and professional. Shout out to nurses Matt, and Darrell! Her case manager Noelle was so kind and even took care of us. The hospice nurse, Darlene, was a big help and a comfort to us. We have no complaints for this center. After our sweet Mother passed into heaven the care gave her meant so much. They cleaned her, fixed her hair, put the clothes we chose on her, and put her stuffed animal next to her, and she smelled of lovely lotion. Thank you for going above and beyond.

Experiencia fantástica: If I could give a 10 star review I would. My mom was there for rehab. Then went on to stay for hospice. I can't say enough about the care they gave my mom. We received compassion, thoughtfulness and prayers from the entire staff. The care manager Noelle was fantastic. She checked daily (sometimes several times per day) to see if we needed anything. She often sat with mom and visited. The nursing staff is phenomenal. They told us often how much my mom meant to them. The CNAs were awesome. Always available and willing to help. I can't say enough about The Center at Park West. Keep up the excellent work, you are appreciated.

Experiencia negativa: My husband needed to be sent back to a hospital rather than stay at this facility. I questioned this and was told they could do everything he needed there. The Dr in charge refused to let him see a hematologist as recommended by his cardiologist. I was told by an employee that it was all about money. I advocated for him and was met with resistance. The temporary nurses were horrible. As far as I’m concerned, I would never send anyone here except for the simplest PT. They are not skilled nursing on any level. My husband died. There was no capability for a code treatment there.

Experiencia fantástica: I came here not being able to move my right arm but a few inches, my neck pulling my head down and no ability to sit up on my own, let alone walk. Through the help of the staff here and some hard work, I'm using my right arm to type this review and I walked out on my discharge days on my own. The staff is awesome, and the food is great. Of all the choices between Pueblo and Colorado Springs, I'm glad I chose the center at park west for my recovery. Couldn't have done it without them. Very grateful!

Experiencia negativa: Disappointed to say the least. I’m only giving 2 stars because a few staff members are actually providing good care while most others don’t seem to care. If my mother needs more rehab it will not be here. The lack of communication between the staff is probably this place biggest problem. The bad definitely out weights the good. Mistakes made are simply covered up. Once my mother received a discharge date the care got even worse. I witnessed some really negative and childish treatment to a few of the patients, done right in front of me which proves they simply do not care, didn’t even try and hide the poor treatment! These individuals who can’t seem to control their temper need to find a new career! Do not send your loved one here……

Experiencia negativa: No goals were given as to progress for rehab. If you are grouchy because you’ve been terribly sick and don’t have any strength, after 2 weeks they will kick you out. Being quarantined while in there doesn’t help with rehab. No plans after discharge, you’re on your own to get in home help. Had to call the fire department to get into the house. The food is horrible. Forget The Center, go someplace else! The pretty outside doesn’t match the inside.

Experiencia negativa: This place is a joke, I would not send anyone i care about here. Thier staff is extremely rude and not help. They dont keep the families informed on what is going with your family. They treat the patient rude and leave them on thier own for hours at a time. No care from the nurses and cna. All they care about is the money, not about the patient or getting them better. I have been placed on hold for 30 minutes trying to get answers on my family member. No drs to be found but for maybe 2-3 times a week (forget it on the weekends- patient have to wait till the weekend is over to get anything done). Doctors dont contact family to keep informed. Dont answer calls back. Dont call with results. Sad that this place still has a license to run, wont recommend ever! I am very disappointed with the care from this facility.

Experiencia negativa: My grandpa went in for rehab for a hip replacement, contacted covid from one of the employees (family was unable to visit him) and instead of sending him to the hospital to get treatment, they let him get worse to the point he was on a high level of oxygen with it not helping his breathing. My mother went at the beginning of Thanksgiving week asking if they would send him to the hospital to get treated, the director told her no that they would not do so unless it was dire need.. They finally sent him to the hospital for his breathing after having covid for 2 weeks with no treatment and the facility put in his records that he has dementia (never once has had dementia, all his confusion is from the covid and not receiving the care he needed). Not only did they not help him with that but they let him get bed sores and his phone went missing and family was not able to talk to him for a few days. I do not recommend this facility.

Experiencia fantástica: The employees are very friendly, competent and accommodating. The rooms are spacious. It looks more like a upscale hotel than it does a rehab center. I'm always greeted with a smile and a hello from the staff, including maintenance, housekeeping, receptionist, nursing, physical, occupational, speech, social workers and even the administrator! Working in a facility like this is very demanding. Several of the staff have personally asked me what they can do better to meet the needs of their patients. They always welcome suggestions for improvement!

They care about the patients. The therapy staff are awesome! I will be havingjoit replacement surgery in the future and this is where I will come for my therapy.

I just think the first person you see should be a ftiendly greetor. But that's just me. Every one else was extremely friendly. Very nice facility.

Keith and Jenna are available for questions and support. The gym is enormous and well laid out. The meals are delicious. Thank you!!

Experiencia positiva: I have only a very small complaint and that is with the kitchen. While the food is very good the small mistakes are aggravating. For instance, ordering soup with a meal and receiving the soup but no spoon in which to eat it or having a chicken sandwich with absolutely no condiments furnished (pretty dry sandwich). There are more instances but so much for the minor complaint. The staff is caring and efficient and do their jobs well. The rooms and facility overall is very clean and well kept, the medications arrive at the proper time and the therapy sessions are challenging. Overall I would not hesitate to recommend the Center at Park West, but bring your own spoon.

Experiencia positiva: When first arriving the facility seemed very clean and kept up well. I feel the whole process was rushed. They put my dad in the room and the CNA came in. She was very nice and gave him the run down of what he could and could not do. Although nice it was all rushed. I was waiting for the nurse to get a run down of the place and learn what the facility had or how everything is handled with appointments, etc. Nobody ever came and I had to leave. I still have not had anyone meet or greet me with that information. My dad has struggled eating and taking the new meds prescribed. I spoke to the nurses but it seems nothing has changed. They were very nice and listened to my concerns, but I'm not sure they are being taken care of. The one thing I did not like was the person who came in to assess my father. She was not friendly, spoke in medical terms that he would not understand, and seemed like she was irritated from the get go. Even when I asked questions she was short with her answers and made me feel like I did not want my father around her. Am not happy with her at all.

Experiencia positiva: The facility is fairly new and updated. The food is generally a high carb diet. The employees are polite and accommodating, however they are a skeleton staff but it meets the minimum standards. The departments could work better if they communicated and worked as a team. The patients need an advocate to ensure their needs are met every day. Elderly patients need more supervision regarding skin care, decubitus prevention, hydration. Volunteers would be of great benefit for those more debilitated patients that cannot or do not leave their rooms. Nurses were professional. Providers not sufficiently available.

Experiencia negativa: My mother went to Park West hoping the care would be great, new facility, associated with Parkview. NO WAY! PLEASE do not take your loved ones here for any reason. My mom had to have a second surgery because noone would acknowledge her screams for help and pain. Her pain meds were held back and the nurse called her a druggie, a nurse! My mother cried like a baby and pleaded to NOT send her back there "it's like being in hell" she screamed. Trying to get any call backs from anyone at first was next to impossible! Many acquaintances and friends all said the same thing, "you should have asked around, several complaints ". Please do your research and don't trust your loved ones to a facility and staff that don't care!!!

Experiencia negativa: If you love yourself or the person you are thinking of admitting for rehab, DO NOT use this facility! Don't listen to the promises they make...they are lies.. my mother came out worse than when she was admitted. So many things wrong!

Experiencia negativa: If I could honestly rate this place I would rate it zero stars! The staff here are lazy! They don't come check on their patients right away. It takes them about 15-20 minutes just to get to a patients needs. If a patient has bed sores, the staff would rather be lazy and use the wheel chair cushion to prevent more sores instead of rotating them every two hours or moving them every two hours. CNA's on the second floor are always seen on their phone as well. Some CNA's, Nurses and Staff are nice however thats the only positive thing that can be said.

Experiencia fantástica: I had a stay of 2 weeks at this facility after my back surgery. The staff was amazing, very caring and tuned into all my needs. PT/OT was geared to my needs, so I can function at home after my discharge. I can highly recommend this facility to anyone who has a need for Rehabilitation after an injury or surgery. You are treated like a guest, not a patient.

Experiencia fantástica: My 82 year old mom was there for 5 weeks to recover after shoulder surgery, we were very blessed that she had such amazing staff to take care of her, the CNA's that she had went above and beyond, they treated her with dignity! A very special thanks to both Debbies for taking care of mom like she was their own mother! The facility is new and clean, the food is very nutritious and selections were great, they even cut her food up because she could not use her arm. Mom was visited twice by the director and the very few challenges we had were addressed immediately! I never had to worry once about her care, I can't say thank you enough for their care and personal touch to making mom feel at home!

Experiencia fantástica: As a retired minister calling on folks in assisted living for over 30 years, the Center at Park West can not be beat. It is clean, they do what they say they will do and the staff is outstanding. My wife has been here the last 7 days and if I ever needed assisted living, this is where I’d want to be. The Center at Park West Pueblo Colorado.

Experiencia fantástica: The CNA's and Nurses on the third floor where my my Mom was staying for 60 days were wonderful. Kudo's to Rose, Frankie, Chanel, Donna, Wendy and Angie! All these people are the kindest and sweetest human beings on this planet. I only wish everyone who needs rehabbing could do it with them. Thank you all again!!!!

Experiencia negativa: This place doesn't even deserve one star. My grandma was there after having back surgery. Her care was very minimal and she was often ignored until someone from the family was there to speak up for her. She was constantly left lying in her bed complaining of sores and pain. She was even left sitting in her wheelchair for hours by her therapist who would tell her they'd be right back but never came back. The first day she was there it was 7 hours before pain medication was offered and then it took the nurse another 20-30 minutes to figure out what the medication was. My grandma was often left to sit in her urine and feces and I was constantly complaining to the staff yet it continued. She was left without fresh water, and the food was a joke. She made a lot of backwards progress during her entire stay. They could have killed my grandma due to their horrible negligence. Because of their horrible care and ignorance, my grandma ended up in the hospital with not one but two blood clots, horrible sores on her backside, and a nasty UTI that the staff chose to ignore. The hospital said it was immediately clear when she was admitted that she had the infection as she had a catheter that hadn't been cleaned and her urine was clearly not right and it made no sense that staff caring for her didn't "notice" and take action. My grandma was beyond sick and they didn't give a damn. We brought issues to their attention and they basically treated us and her situation like it was no big deal. If you're looking for a safe and decent place for your loved ones, please look elsewhere and don't let the newness of the building fool you.

Experiencia negativa: No star if possible. My aunt was in this facility and nobody ever checked on her to help her to restroom. Always see staff on computers sitting around and never see any one checking in on patients. Lack of professional help/assistance. It looks gorgeous however don't be fooled the service is bad.

Experiencia negativa: My mother was there for 4 long days. This facility is severely understaffed. She waited no less than 30 minutes for assistance to go to the restroom. She had just had a total hip replacement. The day she was admitted, they had several hour to prepare for her arrival. When she arrived she was in excruciating pain. It took them almost 4 hours to get her pain medication. We transferred her to another facility , and they failed to send her medications with her. So once again after a transfer in a vehicle she was in excruciating pain. However, this time the new facility went and picked them up, and she had medicine available in 40 minutes. While I have no complaints against a specific employee, this facility is very poorly run. We also had to take food in for mom because the meals they provided were atrocious.

Experiencia negativa: I would rate this zero stars if possible. The care your family will get here is horrible. My grandmother wasn’t even here a week. She had to wait between 20-30 minutes for someone to come help her to the restroom by that time she had wet herself. Not only is it horribly unprofessional for this place to be open because they are so severely understaffed. My grandma had a hip replacement and it should not be exposed to anything like urine but at this facility it was.
