Experiencia fantástica: My Mom has been a resident of Life Care for nearly five year's, not only is "she" happy there, but I can sleep at night knowing she's in good hands! From her nurse's to the CNA's, she is loved, well taken care of and communication between Life Care and myself is excellent.

Experiencia fantástica: My mom needed to find a home and this is the place I found for her. She loves this place she is up and down sometimes back in the hospital and the life care center of pueblo she likes. she likes that she Is offered something to do every day. This place keeps her busy and they treat her so well. She enjoys the food and the people working and living here. I would definitely recommend if you need a place for mom or dad.

Experiencia positiva: Sarah, Roberts nurse, is so understanding and I love her bedside manners. Meeting today went will with staff for his family plan. All in attendance were very understanding. I addressed his needs and I’m hoping it can be handled within a reasonable amount of time.

Experiencia fantástica: Today Life Care Center of Pueblo had an outside event with TrikeTreks. Residents enjoyed fall sunshine, warm weather, rides, food, music, a raffle, and a “snowball” toss. Lots of enjoyment. It was great to.share while we visited.

Experiencia fantástica: I appreciate how attentive staff is in helping my Grandma’s move in to this facility. She moved in Mar. 31, 2023 and receive immediate assistance from the front desk, maintenance and nurses.

Experiencia fantástica: I did my nursing clinical rotation here for over a month. This facility is one of the cleanest long term centers I have ever been it. It does not smell at all like a "typical" nursing home. I even told my family members that I would want to go and live at this facility when the time comes. Everyone was very helpful and caring to the residence. Thanks for the opportunity.

Experiencia fantástica: Life Care Center is like night and day compared to The Center at Parkwest. From the time we arrived which was late in the evening the warm welcoming staff was wonderful and the communication is a huge ongoing part of my loved ones stay in the rehabilitation wing. It is so comforting having staff that is familiar on a daily basis and knows the needs of their patients. PT and OT is very positive and they are very willing to work with patient on patients good days and bad days. All staff is more than happy to help with whatever is wanted or needed. Would highly recommend to family friends, neighbors and even strangers if asked.

My sister is in there because she is on hospice and can’t take care of herself physically and I have no place for her to be with me at the moment, trying to find a place to live myself! They keep telling her she should be able to do things herself and pull herself up and she physically can Not do these things; plus the nurses get upset with her for calling me and me calling them to help her! I shouldn’t have to call them every day.

Experiencia negativa: The family member I was visiting waited over half an hour for the bathroom and mentioned she often has accidents because of waiting so long to be helped after pressing the call button and there was a lady sitting in the hall just hollering for help be moved and nurses just standing around

Experiencia fantástica: The staff at Lifecare is attentive, professional and very caring. The nurses are always available to help out with the tenant’s needs. Sam and Nicole ( physical therapists) went above and beyond with my mother’s rehabilitation program. I highly recommend this great facility.

Experiencia negativa: Mother is in last days, during the last care meeting Life Care Center employees were laughing, mother disrespected, neglected, abused. Supposed care staff rude insensitive, mean. Mother experienced two suspicious falls. Mother experienced ID Theft after being admitted to this facility. We need more accountability from nursing homes like this. I will be an advocate for that, legally, socially, and using any media available. Richard Marks, Author

Experiencia negativa: Horrible place. Almost all of the residents have severe & even multiple pressure sores from not being cared for properly. Life Care Center of Pueblo also had the highest death count during their covid outbreak and they were sited for it twice in 2019 and FIVE times in 2020. I've witnessed CNAs throw used diapers & wipes on the floor while changing residents then picking it up with out gloves to put it in the trash can. I've also witnessed CNAs and a couple nurses go into covid positive and rsv positive residents rooms with out wearing the proper PPE. Life Care Center of Pueblo also owed $101,419 in fines due to their many violates. Extremely unsanitary facility, would not recommend this nursing home to anyone.

Experiencia negativa: I’m very disappointed with this facility especially during this pandemic. Activities assistant ERIKA never has my mother video chat ready when we are scheduled. When I do finally get to speak to my mother she says staff isn’t asking if she would like to join groups such as exercise and birthday parties. I hear the lack of care in her voice!! This company seriously needs to reevaluate their staff!!

Experiencia negativa: My dad was at this center for six months in both the rehab and the long term sections. I was there every day to protect him! During those six months my dad had multiple medical issues for which the nursing response was slow. These incidents resulted in multiple transfers to the E.R., two hospitalizations, one resulting in an emergency surgery. The surgery was due to an unexplained problem with a catheter resulting in massive hemorhaging. The other hospitalization was due to pneumonia which developed from undiagnosed h1n1 flu. The DON and the Medical Director were less than professional and honestly I was embarrassed for their response and lack of action when I approached them on these issues. Once the COVID lockdown occurred my dad only survived two weeks.

Experiencia negativa: Very upsetting experience. Not sure if the receptionist was having a bad day but when I went in, myself and a friend were delivering a shopping order to a resident. The resident's daughter had called in the order via Instacart and specifically requested we deliver to the resident in her room. No problem on my behalf so when I show up to deliver it was as if I was asking the receptionist to complete an impossible task. The receptionist wanted us to leave the order with her but we explained the order request ,being completely respectful. However the receptionist said she couldn't take us because she was busy (understandable) yet made no effort to help us find the room or provide a different person to escort us. Not that it was needed but the level of attitude we got from her was not very inviting. I expect the "face" of the facility to be alot more inviting and helpful which she was not. Once we were finding the room we asked a worker for assistance to find the room and all she did was tell us which hall the room should be down. I would have liked to explain our Instacart operation and offer the services to everyone in the facility. All the residents (or family of residents) have to do is use the instacart app to order from select stores (Safeway, Albertsons, etc.) And it will be shopped for and delivered by our workers straight to the resident. I plan to return to try and give another chance to inform the facility of this app that will benefit their residents but this first experience was very unflattering. If this is how they treat random visitors I can only imagine the neglect the residents receive.

Experiencia negativa: Lemme start by saying I wish I didn't have to leave one star. My mother was a previous resident of Life Care Center Pueblo. During her stay at this awful facility she was abused and neglected and treated horribly by the terrible staff. My mother then passed away after this mistreatment. My family took legal action against Life Care Center and in 2015 WON the case against them. Life Care Center was found liable for the neglect and abusive treatment brought on my mother. I'm asking you to reconsider if your thinking of bringing a loved one to this wretched place. The reason for posting this now is because the case is finally settled now in 2018.

Experiencia negativa: Physical therapy was good. Nurses especially at night gets meds when she feels like it. Asked for something to help sleep, doctor refused. Several CNA's were great, most were terrible. Most ignore patients, was yelled at for calling on phone to nurses station when call button unreachable. Was not released to move with out aide in room with me. One CNA came in multiple times to say to get to meal and walked out. Nurses lied, so once my hip doctor released me, went home. If you care dont go here. All complaints did is get labeled trouble maker. One CNA even told me that where i was a troublesome patient, what did i want.

Experiencia negativa: My mother in law went here for therapy after a major surgery. She was not only mistreated, but neglected. I was so angry that they put me in touch with their facility director. Long story short I had to look into filing a law suit before it got their attention. I was otherwise ignored. We (her family) ended up removing her from the facility. If you have a loved one who needs care DO NOT send them to Life Care Center in Pueblo! Awful, horrible facility!

Experiencia negativa: The physical therapy with Romeo and his co- workets in PT was the absolute best my husband could receive. They were truly caring and had the residents best interests in mind. The medical care was mediocre. There were some nurses who truly cared about the residents, but others were just earning there paychecks. One nurse told a patient she was not allowed to touch patients. My husband's hygiene was a huge concern and depended on who was assigned to him. He had dried feces on him and his bed. When these concerns were brought to the director's attention, they seemed to be ignored. The good nurses and CNA's were overworked and the lazy ones were rewarded for not doing there jobs. Also, when my husband became very ill, the facility doctor did not make the time to check him and he had to go to the emergency room and was admitted into MICU. I recently found out that an employee broke the HIPA rules by sharing medical and therapy information to family members. To those nurses and CNA's, the Social Worker and Romeo and the physical therapists, I am grateful. I would not recommend this facility for a loved one.

Experiencia negativa: Well my mom went into the facility after knee replacement, and left within 24 hours she came home, due to the staffing not giving her pain meds on time waiting untill the next day, when should have had them every 4 hours or so, would not let her shower cause she was not on a list, waited a few hours for water to drink. The therphy lady was rude and demanding telling her that she was going to do what she says. The day she went in i was there over 3 hours, and the poor lady across the hall screamed for somebody to go help her, not one person went into that room nor did i see a nurse on that hall except the therphy lady and she did nothing at all. I finally walked down the hall to get a nurse and while walking down to the nursing station i seen one person laying on the floor in their room half naked, i tell the nurse about the lady screaming for help she rolled her eyes and walked away from me but finally went to check on her about 15 min later. Its a beautiful place but the health care people freaking suck. Rude and unfriendly looks like they might be understaffed and they just dont care. Im not saying this to be an ass but i dont want other peoples family member to be in a place like that. Then to boot one staff member tells my mom i dont know why they sent u here our physical theraphy sucks here. There might be good care in other parts of the facility but where my mom was it was horrible. Just thought i would share our expireance there.
