Experiencia fantástica: Hello I was at Sage Rehabilitation Hospital from July 10, 2025 until August 1, 2025 entire staff was so nice and kind to me even my roommate was so nice until my last day when I found out that two of their workers stole my bag of medicine and I had two rings in that bag as well that was the only thing that went bad and the Administration worker didn't believe me that hurted me so much. But other than that all was good food, transportation drivers, rehab workers ,and cleaning crew who kept my room clean. So I do recommend them minus the stealing part.

I appreciate all the nursing staff also, as well as the activity director and the transport driver. If you need Physical Therapy and Occupational Therapy, I highly recommend Sage!

They were not only knowledgeable and answered my questions aboutmy wound care in a way they were easy to understand. Their bed side manners made me feel like a friend, they always had a smile on their faces even when having a hard day all, which was comforting and important to me as i was in their care for a good couple of months. The surgical procedure I had at a facility in Covington was radical, and I thank the team at Sage for continuing the level of care when I was transferred to Sage in my hometown. Kudos to these professionals.

Experiencia positiva: The best CNA Is Anthony by far. He is always professional. Answers when you need anything and doesn't hide or ignore patients like some of the other staff does. He is awesome!!! The 5 stars are for the therapy staff (my 2 favorites are Michelle and Stephanie) they explain every thing that they ask of me and are very patient.tney are the sweetest people with the cheeriest attitudes that I have ever met. Love them..., food service (I'm have been executive chef for 28 years of universities, restaurants,and hotels all over the country and the food that I have had here was excellent everyday. and Anthony. I enjoyed my stay here except for weekends when there is nothing to do and trying to find a nurse or CNA to help you after hitting the button for hours straight. Thank you Eric B

Experiencia negativa: The staff insulted my husband's diesel mechanic career during their speech, physical, and occupational assessment last week. The staff refused to provide us with actual charges for the therapy all three therapists' recommended when asked. The staff is also holding his medical records hostage from his short term disability company although they have three signed HIPAA documents. They also have no coordinator on staff to help coordinate simultaneous therapy sessions to one day needed for stroke victims (speech, physical, and occupational) and refuse to admit him into inpatient therapy although they claim he needs therapy in all three 6 days a week multiple times a day. I work and cannot just bring him back and forth 6 days a week. Rudest, most unaccommodating, and very poorly staffed facility I've encountered and I am a claims director for a very large entity. Never send your loved ones here.

Experiencia negativa: If I could give Sage Rehabilitation Hospital zero stars, I would. My mom was admitted here after being diagnosed with a seizure disorder, and the level of care was absolutely appalling. On the night my mom fell, I called Sage 11 times before I could finally get someone on the phone. The first person I spoke to wasn’t even her nurse and didn’t seem to care at all, saying she would “have the nurse call me back.” It took repeated calls before the nurse finally answered. I explained that my mom sounded confused, which is a clear sign that she was about to have a seizure. I begged them to check on her, to make sure she was in bed, and that the bedrails, which the doctor had approved, were up. I was assured she was "fine." That couldn’t have been further from the truth. Around 11 PM, I received a call saying my mom had fallen and was taken to Our Lady of the Lake. When I rushed there—within 15 minutes—I was told they had no patient by her name. Frantic, we drove to Baton Rouge General, thinking maybe there was a mix-up, but again, no one had any record of her. We returned to Our Lady of the Lake, and still, they said she wasn’t there. Finally, my friend went back to Sage to get some answers (mind you, it was almost midnight and their doors weren’t even locked). Sage confirmed she was at Our Lady of the Lake. Only after a new staff member checked did we find out she was listed as a trauma patient, and her name couldn’t be listed. When we finally saw her, we were devastated. The doctor told us my mom had a severe fall, was on a ventilator, and her eye was swollen shut. All of this could have been avoided if they had simply listened to me. I knew something was wrong with my mom, but the nurse on duty that night made it clear she didn’t care and acted like she hated her job. Earlier that day, we witnessed three other emergencies where patients had to be taken to the hospital. This facility is a disaster waiting to happen. Sage Rehabilitation Hospital is the absolute worst. I wouldn’t trust them to care for anyone. Please, for the sake of your loved ones, avoid this place at all costs—don’t even send your enemies here.

Experiencia negativa: They have excellent therapist and most nurses are good you have a few who say cruel things to the patients and go without being reprimanded for their actions the case managers do not work with one another as each one tells you something different they also do not get their facts straight before telling you how the patient is progressing they told me my husband had to be helped totally after I had just left facility with rehab team and saw for myself his progression like I said they all need to communicate better

Experiencia fantástica: The therapy staff at Sage Rehabilitation hospital in Baton Rouge is excellent and I would recommend them to everyone. I came in not being able to walk, talk or swallow food without choking. Now I am able to do all of these things with minimal assistance. Shoutout to Michelle, Stephanie, Luke, Katie, Helen, Allen, Lisa, Dawn, Daniel, Kristen and Jonathan in therapy! Thank you to my wonderful CNAs D’Ana, Natalie, and Crystal.

Experiencia negativa: I would not recommend this rehab! Bad bed side manners, takes forever to get wound care done, and to get my parent changed. It’s ridiculous! Also it is ridiculous during the night you can’t get a CNA to come in the a timely manner to changer the patients. Rehab is supposed to help the person get Better, not to put them in a diaper instead Of coming in to assist My Mother to the restroom like they did in the hospital.

Experiencia negativa: Nursing needs a major pickup. It's a crapshoot if you end up with a good/tentative nurse I've been here since May, 2024 it's been a rocky stay. If given a choice request Angelica, or Makayla as day nurse and lakesha for nights. Food served needs to improve meals lack fiber which leads to constipation. Does not feel good. Therapy team was the best. Christen gets you right along with Alan, Courtney teaches leaving patients well informed, Michele brightened my mornings, Katie made for a great instructor. I will miss you guys xoxxxo

Experiencia negativa: My friend was a patient there. The nurses and aids were great! Food was terrible and always cold. Problem at Sage is the case manager, Melinda was unresponsive and did not file reports with insurance on time. Had different story everytime as to what was going on. Refused to give the patient a copy of medical records, first said she would have to sign paperwork, then said another person had to email the paperwork, then flat out said she could not get the records. Patient has been attempting with the aid of her doctor to get into a different rehab, Melinda will not send the records to them either. Patient finally spoke to Melinda who for a week failed to return calls, Melinda denied the other rehab had ask for the records and after two weeks they still do not have records. The director was notified by email, he did not respond or call. Do not let anyone you love go to Sage

The therapy team helped me make tremendous progress and as of today I am able to sit up on my own and continue to make gains. Thank you to Kristen, Kourtney and Allen of the PT/OT therapy team as well as Katie and Michelle with Speech therapy. Also, thanks to Larrion with dietary. She made sure I had things I liked to eat and was super accommodating.

Experiencia fantástica: I had a loved one at this facility on the skilled nursing unit and EVERYONE was great! They had really good nurses who always responded when I called or asked questions. My loved one came there and he was not walking or eating by mouth. The therapy team worked with him and when he discharged in less than 20 days he was walking and able to eat by mouth. The case manager communicated with me and helped tremendously on discharge get everything I needed. The techs were always there and on top of their jobs. The unit secretary was even happy and helpful. The administrator was there to wish us luck on discharge and the DON helped her staff load him in the car! All of the reviews can't be accurate, because ALL of the staff was great, even when I did pop-ins, my loved one was clean and up. I would recommend this facility 100%.

Experiencia negativa: My uncle was a patient at Sage overall it was not a good experience. I wouldn’t recommend this facility to anyone. After 3 weeks we were told he had to be discharged which he will need long term care. I spoke with the social worker on several occasions. I asked her to find him a facility in the area so our family would be able to visit. I was told the day of discharged that he was going to a facility that’s an hour and a half away! Thats the type of treatment my poor uncle received.

Experiencia negativa: They parked my dad in an oversized supply closet with 2 other patients, lost his dentures and did not let him have any of the activity books I brought for him to do when I was not there. He was one of many elderly patients sent here last year bc this facility accepted patients with covid. My 73 year old dad was wheelchair bound and also had dementia and he was sent here from olol for skilled nursing and occupational therapy to recover from covid but instead he sat in his bed, developed a bed sore, and was mostly ignored. He was there a week when he developed respiratory complications and was sent back to OLOL and put on a vent. He passed one week later. I understand things were tough during covid but the care or lack thereof my dad received here is heart-wrenchingly sad.

Experiencia positiva: My dad was in their inpatient rehabilitation side of the facility for three weeks after a hospitalization. We would definitely use their services again. They were very patient and understanding. I visited all throughout the day and found him clean, fed, and content. I think they must have stepped up their game, because our experience does not parallel these reviews at all.

Experiencia positiva: I was in Sage Rehabililitation Hospital in Baton Rouge for three weeks in Feb., 2023. I guess you get what you are expecting because I found the staff to be overwhelmingly professional, compassionate, joyful and ready to be of aid. It was a positive experience for me, but I am high functioning with a broken ankle, very present and verbal. I could get myself in and out of bed and to the restroom. I did notice that those who were more helpless received slower response, which was disheartening. The rehab staff is top notch. They are always diligent and work hard to see that patients make progress. They were always cheerful and upbeat. I can't speak of them highly enough. I think the problem with the hospital lies in the lack of supervisory/corporate guidance on weekends when the rehab staff is off. It seems that everybody with any say-so is off on weekends. I had one hateful, nonresponsive nurse one Sunday, and the negative experience with her prompted me to contact the Director of Nursing. I was told that the nurse would not cross paths with me again--she should have been fired. But for that one incompetent nurse, there were 10 others who were delightful and willing to provide the best of care. I want to give my highest cudos to Nurse Esther and CNA Clarissa. I will always remember them as bright spots in my three-week stay. When considering Sage, it is best to consider the level of care needed. If the person is highly impaired, family will need to drop in at varying times to make sure the person is being taken care of. I personally will always be thankful for the wonderful place to heal, and for the delightful, professional people I met there. Margie Tubbs

Experiencia negativa: Horribly hospital, attention they never take care my partner, for 1 month now he has a infection and they told us is a old infection, this just happened because they never give him the attention, it’s the most horrible rehab. People so rude. They NEVER NEVER make therapy to him. Only 2 nurses are good on this place. Everybody else is bad. Don’t bring you family here. this place has to be reported to be closed.

Experiencia negativa: My brother was a patient in this horrible facility from late November until January before being transferred to a skilled nursing facility. I would not put a dog in this place. A CNA would complain and belittle my brother in front of him and us. She did it on two separate days! They also lie about their PT. My brother had suffered a major brain bleed stroke and he was completely dependent on them for his care. His care was neglected! If you want to see your loved one...good luck on even finding a parking space! They let visitors in knowing Covid was in the building, according to two patients who were outside smoking. They did close it up to visitors but not after letting people visit without them being informed of the presence of Covid in the facility, one of them was me...my brother was diagnosed with Covid later that day. They promised to give my brother's wife and daughter updates on his condition/progress and did not, not from facility personnel or doctors. They were kept in the dark. No phone calls from them when they were on lockdown and no phone calls from providers, either. If you want to throw someone away, this is the place to do it!

Experiencia negativa: The reviews for this facility are exact. The patients are not treated as needed. Noone seems to know anything. My mom cried to leave, every visit was met with complaints about how she was treated. Some days the patients have to remind the staff about feeding them. I had to speak with a nurse over the phone about dinner for my mom, just disgusting! I heard other patients complain about needing help as aids and others walked by saying "I'm busy." My best advice to anyone considering this facility, FIND SOME PLACE BETTER. As another member wrote, if I could give no stars I would.

Experiencia negativa: Omg, I don’t know where to start or if I have enough area to post my complaints. I was at Sage from 6-24-22 to July 5th and it was the most horrible experience of my life. let me start off that the first 3 days was ok, but then that 4th day is when it all began. I was left in urine soaked sheets for 2 days straight after I called repeatedly and asked to be changed. The male nurse came repeatedly and asked me what I needed after I told him repeatedly that I needed to be changed he advised that he would tell my tech, well that didn’t happen!!! Also this male nurse when he was doing my accuchecks was very rough and was digging the needle into my finger causing my fingers to be bruised. When the tech finally came to change me, she had to change my whole bed and both days I was lying in urine for 6 hrs, both days. Then one nurse that I had from 6 pm to 6 am had an attitude when I asked for my headache medicine, after I specifically told the tech that I needed medication for a headache, first she told me that I didn’t have any medication for that, she came bring me something for pain and then got an attitude when I said it was the wrong medication. Then 2 days later was when that same tech that left me in urine came in to change me and she caught an attitude when I asked her to properly clean me up. Not only that when she did clean me in my private area she manhandled me and then didn’t apologize for what she had done. Her name is Brunetta, please y’all be aware of this lady, she will run that place into the ground. She is not fit to be an aide. Two of the nurses there are not empathetic and need to go back to nursing school to learn how to deal with patients. Now 2 people that I will compliment on is Ms Kori, for one she was a very good tech, she tended to my needs with no complaints whatsoeve, and Ms Towanda, omg she was my light at the end of that dark tunnel while I was there, if it wasn’t for her I wouldn’t of made it as long as I did. She was my light, she was awesome, like I said she is a valuable asset to Sage, she was there for me all the times I have gone to Sage this was my third and final time going there. I will never ever ever go there again and I encourage anyone who is thinking of going there please don’t, now the rehab part of the facility was encouraging but my experience there was just awful

Experiencia negativa: This place is horrendous! My sister’s visit here was extremely short lived. All of the reviews are true. One of my colleagues told me that Baton Rouge General had a skilled nursing floor dedicated to rehab. We immediately moved her there. That was the best decision we could have made. I wish we would’ve brought her there first and avoided the nightmare of Sage Rehabilitation.

Experiencia fantástica: I am extremely happy with the services received from Amanda Dover and her professionalism with the handling of my mothers transfer and appointments back to the facility. She is very prompt, has shown that this is more than just her career, I can tell that her heart is really in her job. This reiterates and makes me have comfort to know that my mother is great hands at Sage.

Experiencia negativa: OPEN AN INVESTIGATION! If you can find your love ones or patients a different facility with real reviews please do. From a patient in September 2021. My bed is broken and dr don’t seem to care, nurses and techs can’t do anything. I have to set my own turn alarm schedule or I would be turned one day HOURS over my turn time. I was never giving a wedge, which resulted in some terrible pillow foldings. Clearly admins/owners are pocketing the money because nothing is clean or has been up dated in decades, nurses lack the supplies they need, the supplies they do have is cheaper than cheap, and a majority of the staff don’t even like working here. Very few staff members take the advice you have about your health and care. To the ones that do, thank you or I would have cried myself to sleep every night. OPEN AN INVESTIGATION!

Experiencia negativa: I wish I would have never found this place to put my dad in after his open heart and bypass surgery. The staff is so unprofessional and rude. I know my dad is a hard person to deal with but he is afraid for is life with the way they treat him. The staff dropped him 2ft trying move him from his wheelchair to his bed luckily he landed on his bed but it could have been the floor. When they dropped him he landed on is sore shoulder from surgery and refused to give him a pain pill for 3hrs.

Nursing director is great , very nice man and very compentent , however the nurses are very rude and act like they hate being there so their mad at the world !!! Ive been here 3 times now and requested it again this time because the therapist are wonderful at their jobs and so nice .

He went in with a small tear on his behind (bedsore from being on his back so long, he is paralyzed from a broken neck) he left with a hole you can put your fist into. This place is absolutely disgusting and should not be in anyway considered a healthcare establishment. Please don't let your loved ones be sent there. And talking to administration does absolutely no good. He was at there demean springs location

Experiencia fantástica: My mom stayed here and they were absolutely amazing!! Renny and Shelby changed her life! The nursing staff was good as well but she was mainly there for therapy and they were amazing. We have our second session at their GORGEOUS outpatient facility this week thank you all!!

Experiencia positiva: My Dad is a patient at Sage undergoing inpatient physical therapy rehab for two weeks. He has been there one week now and we are pleased with the facility and the treatment his is receiving there. Staff is kind, cheerful, and efficient. Facility is nice for what it is. Ask for what you want. Don't suffer in silence and get mad. The staff wants to help.

Experiencia negativa: My Mom was at this facility for two days and fell a total of three times which caused her multiple injuries. The staff kept assuring me that she was okay. After the third fall that bruised her face and had her eye swollen shut I demanded she be sent to the ER. I really should have believed the bad reviews, but what business does not have a few bad reviews? The staff to patient ratio is a joke. The medical staff does not have the appropriate training. The nursing staff is still using paper documents and not Laptops or computers. Consider anywhere else for your family or love one. This place is simply trash.

Experiencia negativa: From the moment my mother was brought in the door to Sage Rehab she was carelessly and poorly treated leading her to self discharge in less than 36 hours into her 7 day (min) prescribed rehab. The facility lacked cleanliness, working technology, knowledgeable(or questionably even qualified) staff and a bedside manner that was one of the worst I’ve seen including threats if she was to leave on her own will. Please do yourself a favor and seek out another facility if your love one is in need of rehabilitation services.

Experiencia positiva: My mother has been recovering at Sage and making progress. Rehab is a business, and the staff do their jobs accordingly. My mother has her needs addressed, they treat her with recovery in mind, they address post-rehab efficiently. You get out what you put in. When I've sought out staff, they've been helpful.

Experiencia negativa: More of a bandaid station than anything else. CNA's don't want to do their jobs and expect family and caregivers to do it for them. The therapist are actually good and kind to patients and family. A few of the nurses are really good with patients...Cornell and Florence are very good! The hospital is infested with German cockroaches!

Experiencia negativa: Worst Facility I've ever been to. I was with a patient at this poor excuse of a rehab. The staff was very rude and one specific aide was the worst of the worst. I wrote a review back in February and I see it has been since taken down. I will not go into everything I wrote before because it just to much. What I will say is, if you have a Loved one that needs Rehabilitation Sage is not the place to send them to.

Experiencia negativa: This facility was horrible!!!!!!! We were told that my mom needed to go to a Rehabilitation Center and we needed to choose one. I asked the Social Worker at OLOL if the choice of places were graded on some type of scale and she said no, I also asked if it were her mother where would she send her and she told me that she couldn't recommend a facility nor were they able to visit the facilities. She did tell me that I could google and read the reviews, but she said "there will probably be bad reviews, because most people write a review when they are unhappy". Well I write a review whether it's good, bad or indifferent, so needless to say, here is my review: If your loved one is suffering with Dementia or Alzheimer's this is not the facility for your loved one. I found the staff to be lazy, not knowledgeable to the patient's diagnosis, uncaring, and at times just plain rude. My mother was there for 6 days and was rushed back to the hospital in worse shape than when she arrived. Hospital seems to think she wasn't receiving her medication, but of course will never go on record as saying that. My mother still needs rehab care to regain her strength but she of course will not be returning to this facility! They are on the DO NOT USE LIST!!
