Experiencia fantástica: Place is decent ,,what is above par is the therapy ,,catrina is the best therapist I she helped me when I was in pain and slowly worked me ba k to shape , got me walking after being in a wheel chair and trying to better my situation I am so grateful to the therapy she provided,,I would recommend this place to any one,,thank stamford health center and Barbara great nurse made sure I got every thing I needed thank you guys
Experiencia negativa: Honesty is the best policy. Please do not bring your loved one to this facility. I only selected 1 star because google doesn't allow a post without a star. This place gets no stars from me! My advice**************************************************** IF YOU CAN-- keep your loved one home, for rehab. (most insurances will have cover rehab @ home). Don't listen to the hospital. If your loved one can do rehab at home, it's your choice. The hospital will recommend a rehab, but you have the last say with the care plan. This place has no time for compassionate care. There are different LPN's most days. No one takes time to learn the patients' requirements/needs, and only what they see on the chart for them to do - distributing meds and that's it!! And how can they, with a constant rotation of different LPNs. *** My dad was brought in and needed extra care, notes posted on the wall to alert them of certain things. Such as, notes on the wall to suction him constantly, which means to me, check the patient constantly because he can aspirate. No one bothers to pay attention to those requirements - I had to constantly tell the LPNs to suction my dad. I noticed that they are extremely short staff. Staff calling out constantly, which led to the management staff getting LPNs from agencies. 1 LPN on all shifts. Stolen blanket! This facility has no blankets, like the hospitals does, and they only have sheets for the beds. I get this - no compliant there. But, when your loved one comes in if they want to stay warm, they have to get their family to bring them a blanket. This is what I noticed throughout the floor - personal blankets brought in from home by the family. *******Final straw was on my father's 1.5 week there someone stole his blanket. You got it! My father was transition from the hospital to Stamford Care (Cassena Care) and brought in with limited abilities with no eyesight! Someone took (stole) his blanket that we brought for his bed to keep him warm. The facility knew from his care plan that he suffered from poor circulation, so the blanket was a necessity. ****Week 2, my dear father passed away. The blanket was never found and there was no reason given to me why, or who removed it. All the family know is that our sick, blind father, in less than 2 weeks, blanket was removed from his bed, and he was left with only the sheet. When I came in and found this situation, I engaged the LPN/CNA on duty and the next day, I called management - gave them a few more days ****they couldn't find the blanket. I put through a grievance and haven't heard back from the facility. IF YOU CAN'T --If your family members MUST go to the rehab, has special needs like my father did, please be on rotation with family members to watch your loved one and to watch the rehab. And even if they don't have special needs, please be present! While my family is mourning the loss of our father. We are not disgruntled but I felt the need to share my experience at this . That is the best advice I can give based on my experience. Everyone's experience is different. This is what happen to my father at this facility. Also, I asked, Is the place being monitored? I was told NO! I hope this information was wrong and this facility is truly being monitored. Experiences in life will only make you stronger!
Experiencia negativa: Terrible facility unprofessional social work and office staff. Incompetent nursing staff lack basic g tube training. If you care about your loved ones do NOT bring them here. Will be seeking legal action very soon
Experiencia negativa: we had a guest stay at this location for 2 months and she came out of there worst then when she went in, she is traumatized over her experience and does not suggest to send ANYONE you love here, she compared it to prison, from the RUDE and neglectful staff to the DIRTYNESS, they would not even give her a cup of water when she asked.

Experiencia negativa: Do not bring any of your loved ones to this place! Absolutely the most ghetto, unprofessional, SNF I have ever witnessed. The one star is for Kathy at PT she is absolutely amazing and was very helpful to my grandmother.’ The nursing staff would literally laugh at elderly people in pain that need assistance, I attempted to call the facility 14 times because my grandmother was very sick and needed help. She had to call 911 from her room because no staff would help her

Experiencia negativa: A family member was admitted to Cassena Rehab and it was by far a nightmare of an experience. Her care there was sub-par and after attempting to make contact with the management team (who seems to work only on Friday but still never returned a call) and her imaginary medical team (who never visited her once to set up a care plan) we were at the end of our rope. If that neglect wasn't bad enough, they repeatedly gave her too much blood pressure medication - to the point she was bedridden and a fall-risk and we were done with Cassena. She finally developed pneumonia and was THANKFULLY transfered to Standford Hospital (after my cousin had to contact the nurse and tell her my Aunt sounded horrible because no one had bother to check on her for hours.) DO NOT PUT A LOVED ONE IN CASSENA... IT'S A VILE PLACE AND THERE IS NO PATIENT CARE IN 'CASSENA CARE' Any other facility would be a step up - even the gutter
Experiencia negativa: Do not send someone you love here !! My mom went and I found her to be coughing . I was told she was fine . Well she wasn’t !! After my daughter in law told the Nurse Practitioner to check Her vitals, my mom’s oxygen level Was very low. She had pneumonia! Maybe because they washed her with ice cold water!! They also allowed my mom to sit in her filth for long periods of time !!
Experiencia positiva: EVERYONE was polite and greeted me. Patients looked clean and pleasant ... mild strong smell throughout the hallway. Update: The nurse Therese on the 4 th floor was very kind to speak to me and update us on my auntie's progress. Praying everything goes well with the recovery and stay.

Experiencia negativa: My wife asked to see her Doctor, but he or she is never around to check up on her she is homeless the supervisor said that they will help her with finding housing for her she's been here for a good 3 months and now they want to get her out back on the streets only because we my wife and I have a voice we speak up for ourselves unlike other patients that don't the food is horrible and some nurses have big time attitude problems supervisor told my wife a lie saying she's good to leave from here that supervisor is not a licensed doctor she is just a lazy supervisor that doesn't do her job in reporting to us I can go on and on about this supervisor name Erika

I was impressed by the exemplary team of nurses, aides, housekeepers, PT, social worker, billing office staff that all went above and beyond to make a situation that initially overwhelming less stressful. The nurses, billing, therapists, social workers were informative, professional, and kind. The aides were attentive, pleasant, helpful, and encouraging. I can only speak for the short term experience of less than 3 months on the 2nd floor. You are all appreciated. God bless. Best, Michelle Ackee

Experiencia fantástica: The facility was clean. The intake for visitors was seamless yet careful. Staff was communicative and helpful.
Experiencia negativa: Shut this place down! Patients are belittled, disrespected, and treated very poorly. Cleanliness is a word that staff has never heard of. Besides being disgustingly filthy and horrific treatment of patients, the facility is very outdated (by decades), and very strong medications seem to just “vanish”.
Experiencia negativa: Had a very mediocre experience here. Place was decently clean but extremely outdated and run down. The nursing staff was less than competent many times. Some were nice, some were very not nice and lazy. Would never leave a family member here long term. The discharge part of the stay was probably the worst - nursing director was VERY incompetent for someone who is in a director position. Handed a bag of 15 medications with no instructions, we had to ask every question, including what medications were already given earlier in the day as no one bothered to write it down for us so we knew. Had we not been able to be there for discharge we would have been in major trouble at home caring for a severely ill person. Very unprofessional for an organization to be run this way.

Beautiful room, incredible staff, great food, wonderful rehab in their gym. The room is cleaned several times a day, The entire place is spotless! We are so grateful for this healing opportunity! RRP

Experiencia positiva: When we first arrived at Cassena Care I had my doubts as it was fairly bare bones. What it lacked in physical appeal was made up for by a genuinely caring staff. My husband was given so much special attention by every department. Although I never saw a doctor. The PT gym is quite nice and they are very patient. He had cognitive check and therapy every day which I thought was useful to gage his cognitive state and to keep his mind active. Food is average like most of these types of places. When my husband would not eat they did their best to find something that appealed to him. The PA and nursing supervisor were quick to clarify my husband’s diagnosis with me. There was a mistake from the hospital. Thank you everyone for taking your jobs seriously, and doing your very best.

Experiencia negativa: Update to the Below- We were notified that my grandmother fell again the other night, making for a total of 3 falls during her stay of about 2.5 weeks. This time, a nurse called and stated that she fell out of her wheelchair onto the floor. When asked where staff was and whether there was anyone around, the nurse responded “we’re a little short staffed right now so nobody was watching her”. We asked for her to be assessed and report back to us any injuries. We didn’t hear anything until several calls made the next day. This is after I had a meeting with the Directors and CEO after her first two falls. The solution after the first two was to place my grandmother in a “high visibility area” which consisted of her being in a wheelchair parked by the front desk/ front door for most of the day. Thankfully she has left this facility and my family can sleep a bit better at night knowing she’s no longer here. She has Alzheimer’s and communicating has become very difficult for her, however, once back home yesterday, she did find the words to say to me “it takes courage being in a nursing home.” Just heartbreaking. *** Original review: I am appalled at the lack of care my grandmother has received thus far at this facility. She was transported from the hospital and on that same day, I visited. I have attached photos showing the condition I found my grandmother in. She is a fall risk and is unable to feed herself. She was left unattended with a sandwich and coffee on her table. She attempted to feed herself, and instead spilled all over herself. When I arrived, she was in complete distress. I immediately attempted to find a nurse in charge of her room and was told I needed to wait. While my grandmother was pleading with me not to leave her and that she was not okay and felt unsafe. I stayed with her for hours, fed her some dinner, and oversaw her being cleaned up as I was immediately concerned in this environment. On my way out, I saw the actual nurse (rather than an aide) and explained that my grandmother was a fall risk. The response was “I know.” I further explained that my grandmother was unable to feed herself and the response was I know.” I said if you know that, why did I find her like this (see photos). Yesterday, my family received a call that my grandmother fell out of bed. This is after the facility was notified both by the hospital and myself directly of her being a fall risk. I am searching for an alternate facility immediately as the lack of care and bedside manner is APPALLING. I’ve tried to call 6 times to get an administrator on the phone today, and have not been able to get anyone on the phone. TAKE EXTREME CAUTION sending your vulnerable elderly family members here. Never have I experienced this.

Experiencia positiva: The Staff is wonderful and they take good care of the patients. The rooms are kept clean. PT and Ot personnel seem very competent and have a great bedside manner. The TV situation is very antiquated with a tiny tv 10’ away and trying to hear your tv when your roommate is trying to hear his tv. The food is average, sometimes the hot food is not hot when it arrives.

Experiencia fantástica: Well it's All Thanks To Jay Russell !!! If not for him, my ratings for you would still be 3.5 ! But now that Jay stepped in AND also the fact that the Social Worker name David Alvarez is gone, things are better now, ALOT better. He was uncaring, he lied about alot of things, and was Not concerned about my concerns for mom ( Elizabeth Smith ) in room 226D at Cassena Care at Stamford here. The other Social worker( I forget her name already,) was just as bad as David was, she was very rude. Today ,Sunday, was not good here, when I got here around 10:20 am, my mom still had her left over food on her table, I guess they maybe thought she was going to still eat it ! Do breakfast did not come till 10:45 this morning ! I had to finally get her her tray and bring it to Mom. Only Barbara was doing most of the work, the other Two CNA'S were moving so slow, that if they were any slower, they would have been going in reverse ! My mother's little expensive Scissdors, have been missing for 3 days now. When I Axdressed this issue with the CNA'S , they got upset and said my mother is Always missing something. I went to front desk to report this matter, she was very rude, did not file a report, And just rudely told me that Your Mother Is Always missing something , And why is it Always just her ? She said My mother is the only one that keeps missing something ! I agreed with her, and said Ya, it does seem like My mother is the only one always missing something. She would not file a missing personal belongings report. I said I will talk to Jay Russell, and she just said to go ahead ! I've Never had problem with any receptionist before, even including the one I'm referring to now. If I worked here , things would be alot different. I mean Jay Russell can't be here All The Time. Overall however things have been better since Jay has been more involved with me, he's very caring and concerned my my issues, He Is A major CREDIT To Your Organization ! He told me he also loves his job, and I think I said this before but, Anyone who loves their job , is going to be Very Good at their Job ! Btw, I'm still upset about someone who took my mother's dentures ! I think I know who it is is, but I'm not accusing anyone without proof of course. But this certain person had a reason to be spiteful like that. Long story, it was about " her" taking something out of perspective of what I said, ' She's is a ' Higher up' not mentioning names. And then therewas the time that someone threw out All My Mother's New Clothes I had bought for her ! It was the day before my mom's s birthday ! She was so heartbroken, then in turn, I of course was heartbroken. Everyone said that it was a new worker ( CNA ) , THIS AND MIM' S DENTURES WAS INEXCUSEABLE. AND MY MOM IS BED RIDDEN FOR A FEW YEARS NOW BECAUSE THE PERSON/CNA/NUSTE WAT 7 FEET BEHIND HER WHEN THEYWERE DUPPOSED TO BE HOLDING ON TO HER( DHE WAS IN A WALKER), MOM WAS COMING INTO THE CAFETERIA TO HAVE OUR VISIT ! THEN WHEN DHE WAS IN A POOL OF BLOOD, THE NURSE AND THE PERSON WHO WAS SUPPOSED TO BE HOLDING ON TO HER SAID TO ME NOT TO CALL 911 !! THEY SAID THERE I S " SOMEONE" IN THE BUILDING HERE TO TAKE A ' LOOK' AT HER !!! I MEAN WHAT THE HELL ! I WAS ALSO TAKING PICTURES AFTER I CALLED 911 , THEY TOLD ME NOT TO TAKE PHOTOS !! I SAID WELL SCREW THEM AND PROCEDED. WHEN EMS GOT HERE, I told them how they said to Not call you guys. THE EMS WORKERS COULD NOT BELIEVE HOW IRREPSONSILE THEY WERE. DHE LOOKED LIKE SHE GOT HIT BY A TRACTOR TRAILER ! BROKEN HIL, WRIST, FINGER, DTAPLES IN HER HEAD HER WHOLE FACE WAS BLACK AND BLUE ETC. GOTTA GO NOW ITS 1:30 ALREADY AND I THINK THEY ARE FINALLY GOING TO WASH HER UP, WHICH SHOULD HAVE BEEN DONE 3-12/ HOURS AGO. ANYWAY I WILL HIVE YOU 8.6 NOW AND ITS ONLY THANKS TO JAY RUSSELL ! AND you do not have to keep sending me surveys so very often.... Thanks !

Experiencia fantástica: It was my first time at Cassena Care and didn't know what to expect. When I walked in with my sister it was like a hospital but without the chaos. Very soothing environment and well organized. Everyone I met was very friendly with a positive attitude. Both me and my sister felt comfortable leaving our mother in their care. I was very impressed with their services. I would highly recommend them.

Experiencia fantástica: The nurses were friendly, very attentive, and answered all the questions I had about my mother’s condition and medication carefully and so I could understand. They even called the doctor from another floor so I could meet her (when I had arrived to visit just after she had done her rounds - and the doctor came back to speak with me). The facility is clean and bright with beautiful pictures on the walls. The physical therapists are motivating and gentle with my mother and she has really improved since arriving.

Experiencia negativa: I've lived here since 2011 & the rooms r gloomy& dingy l try to brighten up my room with flowers & pictures but housekeeping does a terrible job cleaning, l own 3 fans which hardly get cleaned cuz of housekeeping! does a bad job! A jerk and doesn't care if they get cleaned my T.V contol does not work. So frustrating. You wouldn't want to live here nothing gets done and the rooms haven't been painted in 15yrs at least not the room all the windows are dirty they are suppose to be cleaned ....but they never are , the bathrooms are dingy, the shower is only warm no pressure half the time and the drain is full if hair so it's always running over. Very frustrating!!!

Experiencia positiva: Have had more experiences with the Nursing Aide staff. They are very helpful and compassionate. I know you have nurse/patient ratios but the nurse’s aides need more help.

Experiencia negativa: If I could give this place 0 stars or asked for it to be shutdown and taken over by new management I would . My mother was there for a year and it was the worst case of neglect that I’ve ever seen. I was stressed, sad and scared to leave every time I left her . She had bruises on her legs and face . The food is disgusting. Knowing that she had dementia they wouldn’t even take off the plastic from her sandwich . It got so bad a hospital told me to report them and I did. Thank god I was able to move her to a different facility . Do not leave your loved ones in this filthy , uncaring place .

Experiencia fantástica: My father has now been at Cassena Care for almost 3 months and I can’t say enough great things about this facility and the staff. They are completely on their game. I’ve had experience with facilities where the staff was not on point and the staff at Cassena care is 100 percent on point with every aspect of my fathers care. I am completely at ease knowing he’s in such great hands day in and day out. Thank u Cassena Care.

The do nothing social worker has ignired me since the first day when i asked to be moved to ANYWHERE ELSE. MY EMERGENCY BUTTON HAS BEEN BROKE FOR 2 DAYS. GOD FORBID I SHOULD NEED HELP. I ZTOLD THEM ITS NOT WORKING AND THEY TELL ME IT IS. IF YOUR LOVED ONES NEED TO BE IN ASSISTED LUVING. DO NOT SEND THEM HERE. IM PARALIZED ON MY LEFT SIDE AND NEED PHYSICAL THERAPY. GOOD LUCK WITH THAT... THIS PLACE SHOULD NIT BE IN BUSINESS. IM REPORTING THEM TO MY INS.CO AS WELL AS THE STATE. STAY AWAY... KEEP YOUR LOVED ONES AWAY.. TAKE CARE OF THEM YOURLVES.. Experiencia fantástica: My mother was admitted there from Stamford Hospital during the pandemic which was vey hard for me being that she is 90 y/o. I was concerned about her declining because she would not be able to see family members or speak to them but boy I was wrong. The staff caught me off guard with their compassion and care provided to my mother. Recreation was able to accommodate me with a Facetime and calls everyday which put my heart at ease. They kept her active and engaged which I thought was amazing. Rehab also was able to Facetime us to show us her progression. Nursing was attentive and called me with updates periodically.. My mother is a picky eater and the Kitchen staff accommodated her wants , which I think is important. Overall the care was great , I would definitely recommend this facility to other families and its in a great area with lots of parking.

Experiencia positiva: People not the facility make the difference in long term care. The staff was genuinely caring and attentive to my father’s needs. Minette, Anna, Jazmine, Roumana, Shondra & Angela were exceptional. My dad and I developed a very special relationship with them. I will be eternally grateful to them for taking excellent care of my father.

Experiencia fantástica: I am a wheelchair bound patient that came from Norwalk hospital to Cassena Care in Stamford. I was a little nervous to go to a rehab facility because I never been to one before but after being here for 5 days its going to be hard for me to leave because I fell in love with the staff. They are very helpful and they go above and beyond for the patient and they also make you feel welcomed. I have herd that other rehab centers can be a horrible experience but not here. I would recommend this place to everyone.
Experiencia fantástica: The difference from this place a year ago until today is remarkable. I wrote a pretty nasty review of Cassena Care last year. I thought the place was dirty and dated, understaffed and I was just plain unhappy leaving my mother-in-law and best friend. I had to have a couple of surgeries Knee Replacement then had a fall on a faulty sidewalk and had to have it redone. Needless to say it took me a good 7 months to feel comfortable driving from Milford to Stamford to visit mom again. Well this time I was elated! The place was clean there was more staff. A lot of new faces that I felt really cared about their patients the many renovations was a breath of fresh air. Thankyou all for making the place feel more likes home then an institution. Experiencia negativa: Place is very dated, staff not bad ,food was uneatable. My mom was on a diabetic, low salt diet was never served the proper food for this. Never once given a salad or fresh vegetables in her 2 weeks there. There is no interaction with other people, no activities, stuck in there drab room all day. Find another option not this place.
Experiencia fantástica: My mother is a patient there and the nurses are very kind to her. She is well cared for, but I will say this is also a function of her being a very kind and easy to like person herself! :) Still, the staff are very good to her and very competent. Bless them for all they do!
Experiencia positiva: On January 23rd 2017 I was a patient at cassena care and I had very good rehab for a total knee replacement and now I am going back one year later for my other need to be a total replacement the staff was very helpful and very knowledgeable and I appreciated that and for this reason I am giving them for stars for their knowledge and for the rehab
Experiencia negativa: This place is the pitts in nearly every way. More alarming than it's dirty and dated appearance, the whole place smells like urine. The nurses are sweet but aloof and incompetent when it comes to medical care. If you're looking for cheap housing and help with meals and the restroom, this place might work. But if you need astute, skilled nurses to attend to one's medical needs, stay away. We had to pull my father out of here just hours after his arrival. It probably saved his life.
