Experiencia fantástica: My husband has been here several weeks now and has been making progress toward regaining enough mobility to come home hopefully soon. The staff have been top notch in caring for him, arranging transportation to medical appointments and in every other way. And everyone has been super responsive to any concerns we expressed.

Experiencia negativa: Night time crew are mostly lazy sitting chairs watching their smart phones. I asked blanket to the guy sitting chair answering "I'm not your nurse " Asked another nurse, it was 5 steps from the guy's chair. Patients staying here are difficult to speak so nurses don't have the responsibility to patients. I was transferred from Hospital , nurses qualify are big difference . Some Assistant nurses are not nurse like security or janitor. But some of nurses are very good.

Experiencia fantástica: Posting on behalf of my wife: My dad has spent almost 3 months at Sunnyvale Post-Acute after his hospitalization.  He received a ton of care there: Physical therapy, Occupational Therapy, Speech Therapy (including Swallowing), and even music & aromatherapy.  He has improved to the point that we can bring him home soon.  The primary doctor saw my dad once a week, and the doctor of rehab saw my dad twice a week.  Then there were so many nurses, from RNs to LVNs to CNAs (it is a skilled nursing facility after all)!  They also individualize the meal plan per the hospital discharge orders.  For my dad it was low-carb vegetarian, and pureed food only, with 1/1 feeding assistance.  They also crushed medicines.  The staff was amazing: not only are they highly skilled, but they have a lot of empathy towards the patient.  They even got to know the family, as my mom & I were there a lot.  We also enjoyed meeting the nursing students.  A group of the them came to my dad's room to sing Happy Birthday on his 90th birthday! This was our first experience with a SNF, and should we need one again, we would request this one!

Experiencia fantástica: I am a patient here and everything went really well. I am getting discharge and it is hard to say bye to everyone. From the nurses to the therapy staff, everyone has been so helpful. The therapy team helped me get back on my feet and were very patient with me. The facility is very clean and odor free. I will miss the staff here.

Experiencia fantástica: My mother recently completed her stay at Sunnyvale Post Acute, and I cannot speak highly enough about the outstanding care she received from this incredible team. From the moment we arrived, Jessica in Admissions made what could have been an overwhelming process feel completely manageable. She explained every step of the intake process, helped us understand what to expect, and ensured all paperwork and assessments were handled smoothly. What truly stood out was how she was always on top of everything - proactively reaching out to check if there was anything she could do to help our family during the transition. Angelica, our Kaiser case manager, went above and beyond to ensure my mother's case was handled with utmost care and diligence. She expertly bridged insurance requirements with my mother's care needs, advocated with insurance, ensured coverage for necessary services, and helped us understand available benefits. Her dedication to getting every detail right was exceptional. Kaiser is lucky to have her. Renee, our RN, handled the crucial transition from hospital to facility with exceptional thoroughness and attention to detail. She ensured all the complicated medical information was transferred correctly and nothing was overlooked during this critical handoff. The daily nursing care was provided by a dedicated team of RNs who monitored medications carefully, kept our family informed about medical status and progress, and were always available to answer questions about the treatment plan. The clinical expertise of the entire nursing staff gave us tremendous peace of mind. The CNAs, including Alvin and Maheen, provided daily hands-on care with incredible patience and kindness. They assisted with personal care and mobility while treating my mother with dignity and respect every single day. Their warmth and caring nature made such an impact that my mother ended up considering them part of her family! I know there were other CNAs who cared for my mother during different shifts, and I'm grateful to each of them as well. The therapy team - Gargee, Kaavya, and Joe - designed individualized PT/OT programs that were key to my mother's recovery. They worked patiently to rebuild her strength and mobility, taught safe techniques for daily activities, and celebrated every milestone toward independence. What made Gargee and Kaavya especially wonderful was how they found creative ways to keep my mother occupied even during off days - suggesting biking exercises or activities she could do in bed. The special relationships my mother formed with them made therapy something she actually looked forward to. Dr. Rao and Dr. Lively provided excellent medical oversight throughout my mother's stay. Dr. Rao's thoroughness was remarkable - she maintained constant communication with our family through emails and phone calls, treating my mother as if she were her own family member. Dr. Lively brought such empathy to her care, spending generous amounts of time with each patient and approaching every interaction with utmost care and thoughtfulness. What truly set Dr. Lively apart was that she not only focused on making the patient better, but fully understood the caregivers' needs and addressed them with anticipation. Both doctors carefully monitored her condition, adjusted treatments as needed, and communicated clearly with our family about progress. Matthew, the social worker, was invaluable in discharge planning. He connected us with community resources and helped navigate the complex process of ensuring my mother had everything needed for a successful transition home. And Revathy at the front desk deserves special recognition for being such a warm, welcoming presence throughout our experience - her friendly face and efficient coordination made every visit feel more comfortable. If you're looking for skilled nursing care that combines clinical excellence with genuine compassion, I highly recommend Sunnyvale Post Acute. This team truly made all the difference in my mother's recovery journey.

Experiencia positiva: My husband was at this facility for almost 8 weeks recovering from a heart attack and major heart surgery. This was a difficult time for our family and I was thankful to meet Jessica when I visited the facility for a tour. The place smelled clean and the staff were friendly. I had visited a post-acute facility prior and it was HORRIBLE. Jessica made me feel comfortable and at ease to have my husband continue his care at SPAC. During his stay the CNA’s & Nurses were helpful. It was not always smooth; however, once we voiced our concerns and needs, they responded quickly. Angelica is a good Case Manager. Thanks so much Angelica for all your help and guidance. The Physical Therapy Team (GG, KK, Angie, Brandon, Camille, Jason, Joe and all the others) are AMAZING, a 10++++++ and everyone is very nice and helpful. They know what they are doing!! We cannot thank them enough for all their help and teaching us everything they did over the 8 weeks. We recommend this facility for post-acute care. Thanks again to everyone that assisted in the recovery process.

Experiencia positiva: I am very happy with the staff and the facility. My mom stayed almost 3 weeks there and she loved the place. PT/OT people were very supportive too. Few time some unpleasant experience happened during night time (with night staff )but overall it was very good experience.

Experiencia fantástica: Hello! I just wanted to make comment about all the CNAs especially Jamuna CNA. She spent about 40 min getting the knots out of my hair to braid it when I got here. She fed me, and gave me a shower. As far as a Representative of Sunnyvale post Acute Center she has really given me an excellent impression of the facility and some of the necessary functions it provides customers. As a customer I have all positive experiences including far above what my expectations were when I arrived. I am so happy that things worked out the way they did as soon as I arrived. I am impressed with the kind of help I received as soon as I arrived because i really needed it ! Thank you .

Experiencia negativa: If I could give this place zero stars I would! My brother had fallen and broke his hip and after his surgery he was sent to SPAC. He was improving slowly but in January he started choking and was rushed to El Camino Hospital where he was diagnosed with pneumonia. On February 8th he was sent back to Sunnyvale Post Accute Center. On Monday March 10th, he called our mom at 5:15 in the morning, pleading with her to pick him up and bring him home because he couldn’t breathe. Mom called and when she didn’t get an answer we drove there to find out if he was okay. When we got there there we several nurses standing around but the horrific situation we saw when we got to his room will forever be etched in my head. Mt brother was unresponsive and the nurse who just found him with his eyes rolling to the back of his head and foaming at the mouth was trying the revive him. We tried to call his name to get him to wake up but nothing. The nurse was also putting a hose down his throat to clear his airwave and we were asking if the had called 911 they said they had called several minutes ago. I then started to ask why the staff was just standing around and not helping and why the ambulance wasn’t there yet I got no response. The first responders finally got there and they started to try to revive him, thank god he still had a pulse and was still breathing. Once we got him to the hospital and they were able to assess him we found out that he had pneumonia again plus he had a bladder infection as well as Rhino Virus. This place needs to be shut down before they actually kill someone. PLEASE DO NOT TAKE YOUR LOVED ONES THERE.

Experiencia negativa: I would never ever send anyone to this location if it was the last place on Earth. My roommate went there to rehab after being in the hospital. She got sick there and they did not pay attention to her and she ended up back in the hospital for another 3 weeks don't ever send anybody to this location

Experiencia fantástica: I've been there twice, and received great care and PT/OT both times. Their care really made a huge difference in my life. I can't recommend it highly enough.

Experiencia fantástica: Sent my grandfather here since the facility we were at before was under construction. But we are so happy with this facility because they had a private ortho wing for my grandfather after his surgery. The facility smelled and looked really clean. Which, thank god because the other facilities we toured were not. Would recommend this facility to others. Staff were very friendly and helpful.

Experiencia fantástica: My mom stayed in Sunnyvale Post Acute for 35 days after the left knee replacement surgery. She was pretty happy with the care and service she received there. Her concerns were addressed promptly. For example, when the roommate was so loud all the time that my mom could hardly sleep or rest, my mom complained. The facility found her a compatible roommate the next day. The food was good most of time. The doctor in the facility made sure that the surgery cut was healing well. The therapists took my mom to do proper and adequate exercise. The communications between the facilty and me (my mom’s guardian) were smooth and effective. The admission officer, the nurses, the nurse assistants, the doctor, the case manager, the social worker had all done their job well.

Experiencia fantástica: The Facility is very clean and odor free. When we visited we always saw people cleaning the rooms and the common area which was nice. The staff are also very friendly and easy to communicate with. I would recommend this facility to anyone who is looking.

Experiencia negativa: Worst place to put your love ones here they force you to do room change and my mom was room change 8 times in 1 month. the staff are too loud and the food is not for human it’s tasteless and the staff are always on their phone talking. Takes forever to answer call lights and the Management does not even wants to help or answer some questions.

Experiencia fantástica: Been visiting this facility since 2021. Some family members have been here for rehab and also visiting family friends who has been here in the long term area. Its just sometimes very disappointing when I read reviews which are unreasonable. I OBSERVED that some staff are very helpful like Director of Business Development and Admissions. I also see that some staff needs more training on how to handle patients and how to answer questions. It's difficult to be in this type of setting for you to succeed is you need to have a heart, the passion and patience to take care of elderly. Some staff needs to understand that they are taking care of human being and should be treated with dignity and respect. I have also seen the renovation of the building and its been very appealing now that before. I still give them 5 stars because no matter what they say the staff here takes care of our love ones. It's not perfect but we can help . Hope the management will look and find ways to see who they should keep as their valued employees. I will still bring my love ones and tell friends to bring their relatives here.

When I contacted the admitting department, they were quick to respond and answered all my questions thoroughly. Additionally, when I took a tour, I was impressed by how clean and nice the facility was. You could tell they really care about keeping things tidy and making visitors feel comfortable. Based on the great service and friendly atmosphere, I definitely recommend Sunnyvale Post Acute.

Experiencia negativa: Place reeked of urine. Burned my throat. Shabby peeling mismatched thrift furniture. Cheap beds that dont adjust. No automated monitoring of vitals. Just a sparse thin bed with a steel folding chair in the corner and a shared TV. Lobby has two cloth chairs full of stains that you can see from the door.I took one smell and looked around and got my grandma the heck out of there even though I had to get a Lyft because of my broken foot. I would not let a dog spend the night in such a filthy depressing place. The bed rails had dood and dried blood on them and the walls had weird stuff stuck on them too.

Experiencia negativa: This is where you come to die, not to be rehabilitated. They ignore the call button. They give pain meds 2 hoys late and if you are coming out of hospital after surgery they ignore the “as needed”one has. It is dirty worn down the food is horrible and the staff is horrible. If you want to dump someone here to die bring them here. If you want professional care run as quickly from here as possible.

Experiencia fantástica: Our 84 yr old father is being cared for here and our father can not walk . We are so happy with the level of attention this facility gives to our dad. The staff are very helpful and include him in all activity’s. The Halloween party was amazing ! There was pizza, staff karaoke for the residence, a Halloween costume contest, and they even brought in dogs for the residence to enjoy. We are able to video chat as many times a week as we want with pre booking . It is currently being renovated, so it isn’t the most modern but the care level is what’s most important. The food could use a bit of work but not always to bad. Thank you staff at Sunnyvale post acute center! ❤️

Experiencia negativa: Horrible!!! Sometimes takes an hour to get help to get to the bathroom. Patient left on toilet. Patient left in wet towel while CNA took a break. Meds are administered late causing pain for patient. Should be getting physical therapy and received none! The staff lies about things that are or are not happening. They had Covid cases, and did not tell visiting family members. The manager has no idea of the Covid protocol and/or how long they need to quarantine. Don’t bring your family member to this place!

Experiencia fantástica: Visited this facility back in 2020 and wasn’t overly impressed. I toured the facility again at the beginning of October and was blown away with all of the improvements. The facility was spotless and inviting. It was far more safe and secure than my original visit. The staff was very courteous and professional. The change in management has made some wonderful changes. Currently my top choice for my father.

Experiencia fantástica: Yelp review on Sunnyvale Post Acute Center. My relative was just admitted to Sunnyval Post Acute Center ( SPAC ) for near a month. It was excellent and I ranked top one ( 5 out of 5 stars) without hesitation. 1) The Center was all excellent team work under the Adminstrator ( Mr. John Bennett ) excellent leadership. 2) All other top management ( Zaldy, Marketing Director and Director Misti, and Matthew from Social service are also wonderful and helpful. 3) The entire environment is nice, quiet, and clean. 4) The nurse team under Cecile ( Nurse Director) are very knowledgeable, experienced, and thorough. My relatives has wounds and wound care team is excellent, details, changing daily. 5) My relative also needs iv treatment. The treatment team is excellent and on time. 6) The CNA ( Certified Nurse Assistant) teams all are really efficient and excellent. Quickly responding and changing diapers and feeding and bath patients gently and nice. 7) The doctors ( such as Andrew Chi, MD) are very knowledgeable, experienced, efficient and earn my family's respect. 8) The location is near freeway 85, very convenient, nice decent neighborhood. It accept most insurance, Medicare, MediCal, Kaiser etc. The insurance and billing office also help a lot to quantify the admission. 9) The rehab program ( PT/OT) with manager Asha, are also impressive. They all are doing great jobs and help patients quicker recover. 10) This is my top 1 choice of short term skilled nursing homes and long term nursing homes. You won't regret you choosing here.

Experiencia fantástica: My review means opposite, nobody writes good reviews as they have no purpose for it, but here is bad late July-August-September why no norms were organized, and out brake for senior occur, and some died after getting covid infection in emergency or regular room later?

Experiencia fantástica: My wife stayed here for 3 weeks and the place is the best nursing home I've ever seen. Every staff member (maintenance to administration to the caregivers) demonstrates a genuine concern for the patients. Everyone treated my wife like a family member. The meals were varied and quite delicious. I am so impressed with this place. I would recommend it to anyone who needs their services. If I had to be in a home, I would want it to be at the Sunnyvale Post-Acute Center.

Experiencia fantástica: I have been here for an entire month now and I have to say that this place really surprised me with their level of concern for each of their residents here and their attentiveness and attention to detail on a patient by patient basis is outstanding. I wouldn’t say any of this if I hadn’t witnessed it first hand. There was a time where I had a problem with the transportation to my doctors appointment and Sandra from Admissions took complete charge of the situation and stepped up tot fix the problem herself and got it all figured out right away. That was very impressive to me how quickly she made things happen for me and I will always be grateful for her and her efforts! Thank you Sandra I hope you read this some day! I am going home from here this Friday and of course I am beyond happy to be going home but, I must admit it’s a tad bitter sweet. I will miss some of the staff here that I have grown close to in my short time being here, and Sandra is definitely one of them! She is awesome! And hilarious!!! The nurses are kind, the CNA’s are timely, and the PT & OT are truly amazing!

Experiencia fantástica: Sean Barksdale. Is a Rock star at his job. I transferred from Sunnyvale back to VMC Hospital 1/2 my belongings made it w/me. 1/2 didn't. Then i got the other 1/2 of my things...yay! But no purse or my personal walker ( which has my name written on it in like 5 different places on the Walker Michelle Roach) BUT SEAN. RECOVERED MY PURSE!! INCLUDING: my birth certificate my bank card my current ID my driver's license my everything. Are the very difficult to recover all these items and there was no cash missing. So a big hug and a big thank you and God bless you to SEAN BARKSDALE. worked relentlessly AND KEPT HIS WORD.. recovering my purse I'm so grateful

Experiencia negativa: My dad entered the facility 2 months ago and it was not a good experience. They are under staffed, no communication, and don't check on patients with urgency. Things that belonged to him always went missing and no where to be found. They do not follow instructions that are given. My dad went down hill rapidly and passed away. I have a hard time to not think that if he was cared for better he may still have had some more time.

Experiencia negativa: Listen folks if the place has any bad reviews when it comes to your health or the well being of a family member. I had the worst experience in this place First off I was lied to at the hospital about what my freedom and boundaries were while staying there. I got there late at night and was roaming the halls and asked a nurse a simple question and was answered with go bother someone else I'm busy. Second the next morning I was chased down when I went out front like I was a prison inmate . Not all the staff are bad but are you going to chance it to the odds of one of your loved ones to get the disrespect of someone who they're looking for comfort and your giving your trust in someone to take care of your loved one while your not there. A place is only as good as there worse employee when it comes to your family. The extent of ones insurance should not matter on the level of treatment and compassion when your too I'll or weak to look after yourself or the ones you hold dearest in your heart I hope that this is helpful in making the right decision

Experiencia negativa: Selective treatment of patients. While I don’t doubt the reviews below, it is my experience that the staff at this facility are selective on the treatment of their patients. They will either help or leave you for dead. My dad was recklessly transferred here by another facility with missing information that put his life at risk, along with both unclear and vague dietary information/medication schedules. Because of this confusion, the staff chose to do nothing until they figured out what to do. Meanwhile, the doctor, instead of taking responsibility for his patient, decided it was better to try to convince my mom it would be better to let my dad go. Because he isn’t going to recover and he can help make that happen if we were okay with it... It was a combination of this confusion and attitude which lead to my dad being rushed to the emergency room a week after initial admission to Sunnyvale Post Acute... Primum non nocere, “first do no harm”. That is the Hippocratic oath that all doctors must take. I know this and was personally there to witness an entire graduating class of new doctors take this oath at my brother’s graduation. Doctor Chi must have forgotten his oath or if he hasn’t, he should have his license revoked. I fully understand my dad’s condition is a serious one, my brother is a physician after all. But how can someone, sworn to care and do no harm, offer to starve someone to death after only seeing them once and barely any evaluation? How can a patient make any progress in recovery if that patient has never been provided sufficient care to be able to recover, especially after such a serious injury? The admitting doctor from the E.R. At ElCamino Hospital was able to admit his initial prognosis was wrong since my dad showed improvement during his stay there. Why can’t Doctor Chi? Arrogance? Lack of compassion? Does he even deserve his license? Despite the objections from myself and my entire family, my dad was forced to be transferred back to Sunnyvale Post Acute. I came in to visit my dad today to see broken a light above his head with a broken housing and shattered fluorescent bulb inside of it. Someone had apparently attempted to change this light and failed miserably. A chunk of the housing was still on the floor behind the bed, behind my dad’s head... This was also right above a running oxygen concentrator, putting both my dad, the other patient in the room, and everyone in the facility at a serious safety risk!

Experiencia fantástica: Megan, the Dietary supervisor is a top star, she always make sure that what I need for meals is what I get. If there are any problems, she will make sure to correct them. In my opinion, she is one of the best employee the facility have. I would recommend this facility as far as food goes, food is okay.

Experiencia fantástica: The nursing and therapy staff are excellent. They are kind and caring. Any issues that came up during our stay were immediately addressed. This is the best place to physically rehab.

Experiencia fantástica: I spent some time at Sunnyvale postacute after a leg amputation of my recovery. The Karen staff there were fantastic! I also do parents and Neil dialysis. Normally I do this at home but after my surgery that was some trouble in placing me a home that could take care of it. Let me share you the nurses are more than qualified to take care of your peritoneal dialysis and all your medical needs they shined above the hospital in ways that I couldn't even begin to describe. The therapy both physical and occupational we're not only patient and knowledgeable made the most of my time or do you seen my recovery would go smoothly and that I would be able to get back to my daily life. I can't thank him enough for all line ever done for me in my recovery. Dan

Experiencia positiva: My Mom has been here a couple of times post care from Kaiser. The staff is great here from what I have seen when I visit but the food is terrible. I know their health and well being is the major focus but the food is equal to that of what a child gets at a school cafeteria, in America. My Moms "enchiladas" were All cheese. A thin slice of tortilla if any. If this was any indication of all the meals provided I am worried. Whoever is the manager or director of meals needs a major overhaul. Healthy delicious meals should be provided, if your family member has high dietary needs I'd be concerned here.

Experiencia negativa: I don't usually write reviews but this needed one so that families can be aware of this place before sending their loved ones there. You can tell they are understaffed because they were not attentive. My father was there for post surgery/rehabilitation. He is diabetic and they repeatedly gave him food he cannot eat despite my family telling the staff he cannot eat the food they were serving. One day he didn't get breakfast (and because of all the pain medications he was on he was not fully coherent all the time) because they accidently deleted his name from the computer! This is not acceptable care! I feel for the nurses as my husband is a nurse so I understand how much work and how demanding the job is. But a quick check in when a patient calls just to see if it's very urgent or can wait and they'll get back to him would have been appropriate and respectful to the patient. Also they need to be more organized. I would never recommend this place.

Experiencia negativa: When on the ad it says 24/7 well had my partner stay and the said its not false advertisemet fix up the issue...pictures of building makes it clean but not couches stinks...

Experiencia positiva: My 90 year old father checked in after a hip replacement at Kaiser and so far so good. He likes the staff, the food and starts PT and OT tomorrow. After five star treatment at Kaiser, its going to be a tough act to follow. I'll keep you posted
