Experiencia fantástica: My mother is almost 90 year old and has stayed the facility for more than two weeks. It takes great care for my elderly mother. Every one in the facility is friendly, helpful and very patient. We are very happy for the nurse, MAs, social worker, dietitians.

Experiencia positiva: SECOND EDIT: My 3 weeks there were not great. They refused to consistently adhere to my dietary restrictions leaving me with many meals I could not eat. I had to order from Amazon and survive on protein powder. They were not consistent with my medications, over and under-medicating me several times, gave me my roommate's meds, and gave my meds to other patients....this was one nurse who was finally barred from my care. Sam was excellent with my PT - the ONLY reason I was there in the first place. I was not treated like a rehabilitation patient, but like I needed a nursing home....I DO NOT! I'm certain I'll have to go back here with my Right knee replacement and I hope they will give me the level of respect and care that Sam gave my PT. EDIT: Things are getting better now that I have a new PT and someone addressed my medical issues. They have tried hard to accommodate my situation and I'm grateful, but the VA dropped the ball by not preparing me. They sent me here saying it was a rehabilitation hospital after a knee replacement...this is a straight-up nursing home...I was completely unprepared for that, and there are some issues, but the facility says they can rehab my knee. The staff has been friendly while also being difficult and inconsistent. The Admins tell you what you want to hear, but the follow-through is not always the best. I will give it a shot and see what happens.....

Experiencia fantástica: We tried a previous facility in the San Jose location. This facility was absolutely horrible. This place was absolutely amazing. The staff was amazing what they love my mom. She loves them. She actually wants to live there, but we told her she can’t, but she is so happy and thankful and grateful that we found this place for her this place you cannot go wrong the staff there’s no smell. People are amazing there and supportive and loving to your parent. Definitely try this place.

Now, my in law is there after surgery and she is having the same experience. It is nice to see the same familiar faces still working there. Thank you for taking care of our family.

He had great nursing care and so yesterday when he needed to go somewhere, that was my first choice.

Experiencia fantástica: I went to visit my friend at Cedar Crest Nursing and Rehab Center today. She had just transferred over from the hospital after suffering a broken arm and leg from a fall. I pulled up and was very impressed by the exterior and landscaping. Walked in and it was just as inviting inside and smelled great. All not what I expected. It was bustling with happy workers and clients enjoying their days. My friend was also very hesitant, and is now so happy with the staff and her lovely roommates. And Dogs can visit! Fun place to recover!

When my grandmother first entered this place she was recovering from her hip surgery and weighed at 155 just about. The next time I saw she had weight put on her and weighed about 170. Ever since my grandmother has recovered from this place I had not once heard her complain about the food or refusing to eat like she did most places plus she gave the staff such glowing compliments and how they made her feel happy here. Plus she’s getting better and on her feet walking now. Which made me feel happy. Thank you Cedar Crest for all you do!

At first glance the facility appears to be in good condition and we are hopeful that we will an experience consistent with the referrals and positive feedback received from numerous sources.

Experiencia fantástica: The PT and OT here are the best I could find out of 13 SNFs for a loved one. He has been there almost two months after 4 months in hospital at Stanford undergoing a brutal BMT stem cell transplant. He is immune compromised and Cedar Crest Rehab was the cleanest facility I saw of 13 and the largest rooms with only 2 beds per room to give patients a little more comfort than being stacked on top of each other. The CNA Staff on the Rehab side are extemely attentitive with a 6 to one cna to patient care ratio. The PT and OT are in House not contracted. PT even has a kitchen and bathroom to help you or your loved one learn to re-navigate being home and transitioning back to independence. Sam and Shao are extemely skilled with advanced degrees and the PT OT aids are all excellent. VIcki and Maria are the RNs and oversee the nursing staff. They are easy to talk with, detail oriented and care about each patient and recovery. Katerine and Lenie in Social work are very knowlable and go above and beyond in help filing care appeals, getting the care needed and working with the billing department as well as helping your loved one transition home. The admissions team javier, Lauren, Ella are all very caring about the room mate arrangements and comfort for your stay there. The Nursing staff and CNAs are all very caring and listen to the patients and the families. The food is actually really good too. CJ does a great job with variety and nutrition including some customization of preferences. The soups are home make and really good. I have been going almost daily for the two months and the experience on the Rehab side is really positive. He will be going home to his apartment in about a week and is able to now walk with a walker, navigate the bathroom and is mostly independent now!!! A miracle he is alive and a blessing I found Cedar Crest for his rehab journey. Katherine also helped with finding a home care provider for his first 2-3 weeks home to help m-f. I cannot speak to the Long term side as he was only there about a week until we won the Medicare appeal (that was a huge fight and be aware Medicare and HMOS can be mean and refuse more that 2-3 weeks Rehab). If you have a loved one or yourself in need of SNF Rehab PT OT and want the best place I found of 13 in Santa Clara County, tour Cedar crest. I dropped in the first visit and Vicki walked me through. The second time Lauren the admissions coordinator walked me though again. Note: They have a nice lobby, a large activity and dining room, a smaller more private dining room off the kitchen, and great outdoor patios and spaces. Many facilites have nowhere for patients to go to get out of thier rooms. I bring my dog in and he loves visiting here. He is a natural and everone loves him. He brightens the day for many people. Good luck on your healing journey as it is long and can be difficult. a clean and caring place will help you or your loved one get back home.

Experiencia fantástica: Cedar Crest was awesome helping my mom rehab after a leg break (non-weight bearing). She spent about two weeks here in which she had clear communication and lots of great help from Doctor/Nurses/Staff/Transportation/Social Workers (She is on MediCal), plus a wonderful PT/OT team who took cautious care of her. Her room was clean and nice, with double doors leading out to a very large and calming grass yard. The facility was also nice overall and had good amenities and activities. She has now transitioned to home care, and we couldn’t have figured it all out without her amazing care team. Big shoutout to Alex/transportation - he was so so helpful and even stayed late for us to help drive her to/from a very crucial appointment. And huge shoutout to her Social Workers Katherine and Lenie for all the help navigating everything.

I am also impressed with how clean it is and the fact that you have to wear a mask to protect their residents from any outside illnesses. My aunt is very well taken care of and the staff treats her so good. It's never easy to see a loved one in a nursing home, but it is one of the nicest ones I've been to so if she has to be anywhere, I'm glad she's there. Thank you to the staff. I stayed with my new friend and played bingo in the community room with others. Then went to her room where she chose to have dinner. She will be well cared for here until the move out of state to a care facility. Experiencia fantástica: My mom is there recovering from a stroke. Was skeptical of the level of care she'd receive at a place like this....but they are on top of things! When my mom presses the call light someone shows up within a minute or two. Kindly and respectfully interact with her and solve whatever issue there may be or answer any question, no matter time of day. I also saw physical and occupational therapy in action today. Wow! Mom has made so much progress it's almost unreal. They bend over backwards to make sure patients are comfortable and tended to. I really appreciate the staff. Always willing to help and always friendly. It's contagious!

Her leg infection came back and they did. It address it. They did. Or do the schedule blood test. Sometimes it’s about the humans not the money. Don’t bring your loved one there

Experiencia negativa: We came from out of state after MIL was sent here after experiencing a TIA. She was 94. It was very clear she was suffering immensely. We pushed for hospice care so she could be made more comfortable. This facility pushed backed and slow rolled our wishes. After 5-6 days, 2 phone calls with physician, and many long days of suffering for our beloved mother we told them we were getting her granddaughter, an ICU physician involved. everything changed and the very next morning the dr visited and got her agreement to go on hospice. This is unacceptable!! Medicare $$ over patient care!! MIL was found sitting up in chair in hallway having experienced another stroke. Instead of being made comfortable and able to go peacefully she spent her last hours being transported back and forth to hospital. The administration here puts money over patient care!!!

Experiencia fantástica: I would definitely give Cedar Crest a 5-star review. I recently spent a month there following surgery to repair a broken hip. My choice of Cedar Crest for rehab was based on my surgeon’s recommendation of the therapy department. All of the therapists I worked with were excellent; they quietly persisted in urging me to further accomplishments while always taking into account my existing limitations. Their compassion and good humor helped me past some difficult and at times painful points. Thanks to them, my progress was gratifyingly rapid. The CNAs were very helpful. Some were outstanding in their efforts to keep me comfortable, and they often anticipated my wants. I never found that I had to wait very long when I pressed the button. The facility is quite attractive and well maintained. The grounds are nicely landscaped. The food is remarkably good. Since there aren’t any happy reasons to be in a rehab facility, I hope that I won’t be going there again. However, if I ever need such services, Cedar Crest will be my first choice.

Experiencia fantástica: I had a total knee replacement surgery. I am so happy being here. Because of the activities here I have been getting more excersize than If I had gone home. The staff is very friendly and accommodating. Cleanliness is practiced here as a policy. The food is great. I especially like to eat in the dining room for lunch and dinner. I've met some really great people here. I notice the nurses aides are extremely patient with the patients that need a lot more help than me. My roommate is 99 years old and they treat her with the utmost respect. I'll be both happy and sad to go home. Lol. I highly recommend this facility.

First of all, the staff and the entire team at Cedar Crest deserves all the kudos that they receive from the residents, family and guests. Why do I say this? I have an un-biased opinion since after caring and watching over a family member since 2001 until 2013 - I have been to A LOT of Skilled Nursing Facility and Rehabilitation Centers from way up North in San Francisco and all the way through the Peninsula areas. Hands-On Approach where all the staff takes owndership and demonstrates a lot of patience, love, understanding are honest & true to providing care. If you get a chance to see the facility and the wonderful well-kept patio, it is truly a sight to see. Very positive and encouraging environment for the residents and everyone! Cheers! Experiencia fantástica: My relative had to do rehab after hospital stay. CC-Sunnyvale is very pleasant environment with a great rehab staff and facility. I would visit multiple times a day and each time, staff was helpful from front desk to CNAs. Its central location made it easy for frequent visits, which was helpful in the recovery process.

Experiencia fantástica: I'd like to take a moment and say thank you to all the staff and doctors and therapist who took good care of my father after hospital. My father originally lives in turlock, but we decided to go to SJ since we heard good reviews about this place, because my nephew had worked here for several years. The transition was so smooth. They made sure to call me and let me know of my dad's situation. We had meetings and gatherings to discuss the treatment plans. I'm so thrilled we picked this place. Havier was so helpful getting our paper works ready on time. Nurses and CNAs were amazingly kind to my dad and myself. They had amazing events for patients to attend and keep them busy and happy during their stay. They made sure my dad has his showers when I asked and all his meds were given on time. The social worker got my dad's devices ready prior to coming home, such as his wheelchair and necessary items that we needed at home. I don't know, if I continue writing this will take years to finish thank you everyone. I wish good health to all and I hope to see you guys soon to visit and say hello My dad and I appreciate all you did. Keep the great work and the place clean and covid safe as always

Experiencia fantástica: I am home after three weeks convalescing at Cedar Crest. When I arrived at Cedar Crest, I could hardly stand while holding on to a walker. Now, I am safely mobile thanks to the competent and caring therapy staff who continually assessed my condition and planned each therapy session to address my evolving needs. My present condition is proof of their devoted competence and care. The nursing staff were all so kind and considerate to me. Because of them I always felt safe and knew that I was in the best professional care. From the kitchen's delicious healthy meals to the custodians who kept my bedroom, bathroom and the entire facility clean and pristine, my stay was very comfortable and I felt as welcome and at-home as one can during these times. I am gretful for the opportunity to have recovered at Cedar Crest.

Experiencia negativa: My Mom has been here the last few weeks, and they have been very difficult to work with. They are very slow to return paperwork, and difficult to get on the phone. Otherwise, it's fine, not that nice, but not scary or terrible. I would not recommend Cedar Crest and would avoid it if possible.

Experiencia positiva: I was at Cedar Crest from 10 March to 8 April. In a month I met many fine people and particularly the OT/PT group. This group patiently pushed me to recover from knee replacement. Without naming any one in particular they are all great at their craft. One detriment of Cedar Crest is old equipment/broken

Experiencia fantástica: Cedar Crest is a beautiful facility in the Sunnyvale Community. This skilled nursing has great staff, therapy department & a beautiful patio to visit your loved ones.

Experiencia negativa: There were multiple issues that occurred during our father's post hospitalization stay at Cedar Crest in November 2021, including medication that was administered that should not have been given. When his assigned nurses were questioned, Monica and Karla, they confirmed that errors were made. Issues were escalated to Lenie, Clinical Case Manager, and Cynthia, Director of Nursing, who both assured us that corrective measures would be taken so these errors would not happen again. Due to Cedar Crest's negligence, we made the decision to move our father after only three days to a different skilled nursing facility. Fortunately, he successfully recovered after two weeks at the other facility, which provided competent care. A complaint was filed against Cedar Crest with the CA Dept of Public Health, but the investigation has been dragging out for months due to the pandemic and staff shortages. We feel it's important to share our experience so other families can avoid similar issues with this facility.

Experiencia fantástica: Cedar Crest was unknown to me when my mother needed skilled nursing after a hospital stay. I was told by the discharge planning person that hospital staff requested Cedar Crest for their own parents. Both my mother and I were impressed by the response time, competence, and caring of the staff. This was her ninth stay in a skilled nursing facility and Cedar Crest was her favorite!

All of the staff, from housekeeping to management, handled the problem and I’m glad my mom is there. We all feel safe with her there. They did a job beyond 5 stars. Thank you to everyone.

Experiencia fantástica: Recently, our 85 year old mother was placed at Cedar Crest Nursing & Rehabilitation Center. All we can say is that the Team there did an amazing job. Her adult children all live far away and are busy working professionals. Cedar Crest ensured the highest level of care and communications and overly attended to the changing needs of our mother. A high recommend.

Experiencia fantástica: They treated my family so well, like their own! We're really happy with the care there, which included rehabilitation and extensive stay. Everyone is very caring and gentle with the residents. They care about the visiting families too. The pandemic has been hard but the staff at Cedar Crest made it work. They were really creative with the visits so we could do a window visit, video visit or visit outside in the beautiful garden. It helped us keep our sanity. The staff are vaccinated and Cedar Crest is strict (in the best way!) about the COVID guidelines. That's why no one is sick there. I felt safe each time I visited and, more importantly, we knew our family was safe there. We had such a good experience and highly recommend this facility. It is the people who make a place, and the staff of Cedar Crest are so good!

Experiencia fantástica: Cedar Crest is an exceptional facility. My husband has been there for two years. They sailed through the Covid period and now we are looking forward to open visits. The staff is very helpful and caring and the food is excellent. The management truly care for the patients as well as the families.

Experiencia fantástica: I was pleasantly surprised. The thearopy staff was attentive and great. Food was just OzK at first. But they easily tailored it to my taste. Nice they were so responsive. A clean facility that will only get better after an ongoing remodel. Nice outside area in the back.
