Experiencia fantástica: I hope this doesn’t turn into a novel, but if it does, so be it. My experience at SHARC will stay with me for the rest of my life. With a few exceptions, the staff were all fantastic. The results were what I was hoping for, and what I needed. The experience overall, was five stars, and probably six. Dr. Merkel Was invaluable with her expertise, and guiding me to some good decisions for my recovery. The OT, Ada, And the PT, Jen were also invaluable. In fact, when I was granted the opportunity to go to acute care, a separate facility with more intense, physical therapy, I declined. The level of care and the progress I made with the help of Ada and Jen Can’t be overstated. I knew I was better off staying in their care, and the progress I made after that decision confirmed that I made the right decision. Ada and Jen regularly went above and beyond their job description and what is required to them in order to help me in a few different situations. A few other names, and I wish I could remember their last names… Skye, Kira, Francisco, all of whom were CNA‘s. The nurses were fantastic also. Vanessa is the only name I remember, and she was great, but every nurse that served me deserves a pat on the back. The wound care nurses, Denise and Andrea, were also incredibly helpful. Emily Kaylie and Hannah from PT also went above and beyond. And unfortunately, I remember faces better than names, but those were a few. The front desk staff, including Autumn, were friendly, and always available to help answer questions. I hope I never have to go through anything like this again. I was on a bicycle, struck by a car at a high rate of speed, and spent six weeks at SHARC. If I had to do it all over again, though, I would want the same place in the same people taking care of me. I’m leaving here in a few days, and I’m going with confidence that I’m the best I can be to go home and finish my recovery. These people are absolute angels, every one of them, and I wish I could name them all. I highly recommend SHARC as a place for somebody to complete their physical rehab rehabilitation. THE ACCOMMODATIONS WERE FANTASTIC. THE PLACE IS MORE LIKE A HOTEL THAN A REHAB FACILITY. THE FOOD WAS FANTASTIC, FOOD SERVICE EMPLOYEES WERE GREAT, AND IT WAS A COMFORTABLE INSTITUTION TO BE IN. Experiencia fantástica: I am writing this review on behalf of my mom. She just spent three weeks at Surprise Health and Rehabilitation Center. Everyone was amazing. She was so well taken care of from doctors to nurses, CNA‘s to physical therapy, case managers, front desk and specialty care teams, everybody! The communication was exceptional and the people were so caring. I hope we never have to use a rehab rehabilitation center again but if we do, this is the only place I want my mom to be.

Experiencia negativa: I had a friend in there and she fell out of the wheelchair. You would think the staff would’ve said something to family , they never did know what happened to her except she had bruises on her that never got pictures of x-rays or any quality of care it was horrible

Experiencia negativa: My family member was admitted to Surprise Health & Rehabilitation Center on November 20, 2025. Just 12 days later, on December 1, 2025, she suffered a full-blown seizure in the hallway while in her wheelchair. I screamed for help at the top of my lungs while multiple staff members looked directly at us. It took approximately 8 minutes for anyone to come help. I was the only person physically holding her in the chair to keep her from falling and hitting her head. She was rushed to the hospital by ambulance. In those same 12 days we had already filed a formal written grievance about: • No feeding assistance (despite written promises from management) • Being seated directly across from coughing residents (she quickly developed respiratory symptoms) • Blank care-plan whiteboard • Meaningless “2-hour checks” None of the promised fixes were ever made. Tonight the hospital is discharging her back to the exact same facility. I am terrified for her safety. This is supposed to be a Medicare-certified building. No family should ever experience this level of neglect in less than two weeks. Dear Jessica Booher and Michael Nabea, Thank you for your update this morning regarding the orders for an X-ray and Doppler on my mothers right leg. I remain deeply disappointed with the ongoing level of care and attention my mother is receiving at the facility. This swelling in her right foot and leg has actually been present for approximately 5 or more days. It should never have required me, as her son, to bring this to anyone’s attention—it is concerning that such a visible and significant issue appears to have gone unnoticed by the nursing staff or providers for this length of time. I am left wondering whether any doctors or care team members are routinely performing thorough physical assessments to identify abnormalities, or if family members are expected to micromanage her healthcare while she is in your facility. This type of issue should have been identified and addressed at the onset by the professional staff responsible for her daily care. For clarity and documentation purposes, please confirm in your response the exact date when this swelling was first noted or assessed by facility staff, as well as the current status and any preliminary findings related to the ordered X-ray and Doppler. Mr. Derek Bowen, CEO, under your leadership, this facility has demonstrated a troubling failure to provide the vigilant, proactive monitoring that vulnerable patients deserve—especially in a rehabilitation setting where timely detection of complications can be critical. Families should not have to act as constant advocates to ensure basic care standards are met. I strongly advise others to exercise extreme caution when considering this facility for loved ones needing rehabilitation services. The neglect and inaction in my mother’s case raise serious questions about patient safety and staff accountability.

Experiencia fantástica: The staff has been awesome. One of the CNAs, Sienna , is very thoughtful and caring. When we asked for something or needed help. She was right there. In addition, for PT and OT, my family would like to thank Jace and Amanda for working so hard with my husband. They listened to us and worked with us to help my husband. We appreciate late their hard work with and for us. Their support has made all the difference because they allowed us to be active participants in his care - taking him for walks, to the gym to ride the bike, etc. they are a great team and we are grateful. Lastly, we’d like to shout out Thomas, the speech therapist, and Adam. Thomas worked hard upon our arrival to ensure my husband could eat and drink properly and not regurgitate on his food. Adam is our meeter- greeter. He came the 1st day there to evaluate my husband. He then provide him with simple exercise he could do while waiting for therapy to begin. The amount of growth from his admission to now has been amazing and we are grateful. Thank you all for the lovely care you provided.

Experiencia fantástica: I was treated very well, felt like a home. My care was wonderful at this facility. My therapist Shawna and Adam we very kind and compassionate but very knowledgeable. I was pleased at the progress I made. I would recommend this place to friends and family.

I want to take a moment to express my deepest appreciation for every one of you, your kindness your warmth your healing presence have carried me through one of the most difficult chapters of my life.There are words that heal what hands cannot and each of you has spoken those words into my life when I needed it the most. Front desk team; Jaalaiyah, Jonee and Taria- Thank you for greeting me each day with patience and compassion your smiles and gentle understanding set the tone for healing before I even walked past you. Nurses: Kelly, Tonya, Andrea and Felicia - Your care has meant more to me than I could fully Express during my hardest moments you saw me, supported me, and remind me that I mattered. The comfort you gave me during the 6 weeks I was here lifted my spirits when everything felt heavy, you are the heartbeat of this facility. Nurses Aids: Ziena- your dedication and attentiveness never went unnoticed thank you for the dignity and kindness you showed me in every interaction. Laundry: Heather -thank you for the pride you take in your work and the comfort you bring through something as simple but essential as clean character clothing and linens it matters more than many realize. Management team; To the entire management team thank you for going above and beyond when my world seemed to fall apart you found a place for me when I had nowhere to go even after being away from the program I came from for past 6 weeks. Your willingness to step in during a vulnerable moment change everything for me. To all of you, I want you to know how deeply your work impacts lives. Mine is one of them, I felt cared for not just in body, but in mind and Spirit. this is a rare place, one I would trust to take care of anyone I love!! I would be crazy not to refer someone here because I know without question, that they would be treated with the same compassion, dignity and dedication that you showed me. thank you for being the reason this Medical Rehabilitation Facility runs the way it does you are the difference!! you are the healing, and I'm truly sincerely grateful thank you. Experiencia fantástica: My elderly mother had to stay for awhile. The PT staff was the best! Kind, caring and helpful. Listened to us. The entire facility was clean. Staff was attentive and caring. This facility made a very uncomfortable situation. Comfortable. They were great to us. We greatly appreciate them.

Experiencia positiva: I was very happy with this faculty EXCEPT the terrible medical treatment I got from their inhouse doctor. I developed a UTI the first few days. They gave me the wrong medicine. I complained after 3 does that it wasn't working and they kept giving it to me then waited another day afterwards. Finally I had to demand to go to the ER at Del Webb because I wasn't getting any better. All of a sudden "Yes you were on the wrong medicine and we'll give you the correct one". While this was going on they had me on diuretics round the clock and I wasn't getting any sleep because I was peeing ALL night long. The care I got was horrendous and their inhouse doctor should be fired. I ended up taking 4 kinds of antibiotics to get rid of my UTI and now my GP wants me to see a Urologist for the chronic UTI I had. I'm still peeing "ALL night long.

Experiencia fantástica: After breaking my ankle, was referred here for rehabilitation, both before and after my surgery to repair it. All of the staff were absolutely excellent, from reception to nurses to physical therapy to case manager. I left the facility stronger than before. Highly recommend if you need rehabilitation. 10/10

Experiencia positiva: I am only giving 3 stars because of the Nurses and CNA's. Otherwise it would be 1 or 2 stars. The food was pretty good. The cleaning lady was great but I didn't get her name. The following were outstanding: Daytime CNA: Bianca, Skye, Autumn, Paige, Keke, Pedro. They were great at their jobs. Nighttime CNA: Joy, Gabriel. They were great and always there. Nurses: Tiana, Maria, Vanessa, Cliff. Always got my meds on time. Sandy was great always listened and help with issues. Now the negative: I pray for anyone that gets Angela as a case manager. She lies all the time. Document anything you talk about. She told my Mom that I said things that were not true.I gave her an envelope with info from the hospital and she said she would bring it back. Never saw the envelope again. I asked her and she said it wasn't her. I know it was her because she has negative all around her and she is always frowning and I remember that about the person that took my envelope. She keeps threatening to send you home because of insurance but I called my insurance to do an appeal and they said I didn't have anything to appeal because they weren't sending me home. She depressed me several days and didn't want to do anything. She was going to send me home with no equipment and I can not stand or walk. We went to Angela's boss, then she was nice and helpful. Physical Therapy acted like when you were sick that you just didn't want to do physical therapy instead of understanding and try to help other ways instead of rolling eyes. Good Luck.

Experiencia fantástica: My husband spent 2 weeks at Surprise Rehabilitation Center after his back surgery. He received excellent care there and felt the staff members were very caring. The food was good but he felt there was too much food. He is not a big eater. The physical therapy helped him very much!

Experiencia negativa: Never take your loved one to this facility. When you do a walkthrough with emissions they are going to take you to their trophy case they brag about all their trophies they're putting lipstick on a pig the quality of care there is unbelievably the wrong as a patient you ask for something they say they'll be right back and 4 hours later you have to ask again the food there is absolutely terrible I wouldn't feed it to my dog my wife had a seizure there and had to be hauled out of an ambulance when she started having a seizure they had no clue what to possibly do the Medics that came and picked her up told me they wouldn't put their own mother in this institution word to the wise don't put your loved one in this place it's absolutely terrible to care that they give these people. Call my wife doctors cleared her to go home she's up walking great no issues with all for some crazy reason PT wanted her to stay there for a long weekend when they are not even there for those 3 days absolutely ridiculous all they wanted was her to stay there for 3 days so they can get more money from Medicare what a fraud do not take your loved one there

Experiencia fantástica: Let’s start at the very beginning I came to this rehab place scared out of my mind because I was I didn’t know it. I am 79 years old and if you don’t know some thing you do get anxious, but I met my aide named Paige she reassured me that I was just fine here and I didn’t need to be nervous. I realize there are many other aides that then take care of me, but it it seems like Paige knows what I need. She’s a wonderful young lady with a smile on her face. That makes you feel much better..! I could name of a few others that I am close to, but it would be too much for me to write but as far as me living here for the time being, I am on OK so if you ever need to have an aide just reach for Paige!

Experiencia fantástica: Surprise Health & Rehabilitation Center Wonderful place! Five Star! My mother was so happy she was moved from Bella Vita Health & Rehabilitation Center which in my opinion didn’t deserve one star filthy and hot and they take a long time to tend to patience my mom called 911night shift lol keep your loved ones away because it takes 4 days to process a transfer to another facility. Mom went in at night first thing in the morning I requested she be moved. I was being nice I told them I wanted her closer to my house. Bella Vita is a dump!

Experiencia positiva: Giving 4 stars cuz there’s always room for improvement. My brother was transported here a week ago and so far so good. I really like how everyone has been kind, attentive, and helpful! The first weekend here was scary, he was transported on Friday night, so it was discomforting and I was on edge about the transfer to a new facility for my brother. However, come Monday my worries did subside! My brother’s assigned RN, LPN, CNAs, and therapists for speech, occupational and physical therapy have all been amazing. It was very comforting to hear the staff give each other genuine praise and helping one another as a team and most importantly helping my brother. I have been fortunate enough to have the resources to be at my brother’s bedside during his first week at Surprise Health & Rehabilitation; I was there from like 8AM to ~3PM his whole first week. It’s been rough traveling back and forth from Tempe to Surprise, but it’s better than a 4 hour drive from the Navajo Rez to Surprise. He is in a minimally conscious state and has been receiving healthcare for his needs. I do wish I could continue sitting by his side only to help him with sensory stimulation. I trust the RNs CNAs and Therapists completely, I just wish there was more staff for the little things. We saw this facility based off of scores and its overall rating with a high score of 5 stars from Medicare.gov (Health Inspections & Quality); even though it had only 2 stars in one category (all of our options for the sub acute facilities in Phx area were scored on Medicare.gov with 2 stars for “staffing”) we chose this one. It is SO CLEAN, the custodians are so organized! If anything tho, I wish I had the resources to hire a nurse to perform the smaller things that I’ve been trying to do with my brother, especially range of motion exercises and the sensory stimulation. I completely understand the 2 stars score they received and I see how much more staff it would take to have a CNA or RN perform range of motion exercises with every patient labeled in a minimally conscious state. The current CNAs for my brother are doing God’s work and I appreciate how they do their rounds every few hours to change his soiled bedding. I look forward to his daily therapy sessions so that we can get him on the next stages of rehab. A very long road to recovery, and I’m so fortunate for Surprise Health & Rehabilitation, big thank you to all that you do!

Experiencia fantástica: Amazing friendly staff from the time you enter throughout your stay. They have a great energetic therapy team and helpful case management team that responds to the needs of their patients quickly.

Experiencia negativa: My Mom went stayed in this place for nearly 2 weeks after Heart Surgery. All they needed to do was rehabilitate her ability to get out of bed herself and stand on her feet. After 2 weeks when I picked her up she needed a wheel chair to get to the door and literally had no strength to be able to be on her own in her home. They do not make sure that she is doing the exercises required. They "check in" and that it. Do not take your loved one here if you think that they will rehabilitate. All this place does is buy you some time. They will blame insurance as the reason why they cannot get the job done even when they know insurance will always seek to cut the time off quickly. Insurance covered 10 days so there was never a plan developed to get the job done in 10 days. In my experience if they were only paid if they did the job hired to do, they'd be out of business. They get paid whether they do a lousy job or not. They failed my Mom.

From admissions, therapy department, , dietary, housekeeping, nursing, case management and the CNAs. Experiencia positiva: I give the facility 3 stars and 5 Stars for Ruth(Mueni). I want to express my deep appreciation for CNA Ruth (Mueni) at the Surprise Health & Rehab center where my mother-in-law received care in late April to early May. Mueni was not only professional and attentive, but also incredibly warm and personable. Watching her interact with our mom, it was clear that she had developed a genuine connection — one built on compassion, patience, and respect. Mueni went above and beyond in making sure our mom felt seen, safe, and cared for. Her presence brought comfort not only to our mother-in-law, but to us as a family. Thank you, Mueni, for the love and dignity you showed our mom. We are so grateful for the role you played in her care at your facility.

Experiencia fantástica: I came for Out Patient Physical therapy with Jessica and it was a great experience and she is very professional. I will highly recommend her to my family and friends.

Experiencia positiva: It was a nice place , the majority of the people were nice. A girl named Bianca was the best, also there when you buzzed, she did a very good job and she always had a smile on her face, this girl should get some kind of recognition.

Experiencia negativa: I arrived at the rehab at 12:45pm from the hospital, and did not get situated comfortably in a bed until just after 8pm. During the day I'd pushed my call light to ask for assistance. After about twenty minutes, a maintenance employee asked if he could help with anything, as he'd noticed my call light. He was able to get me water, but as he was maintenance, not nursing, he was unable to help me with my needs. It took a couple of hours to get the assistance I needed. At the time, I was bedbound and needed to be transferred to another bed. Two women came to assist. They grabbed additional help from the kitchen. Moving is excruciating for me, because of my health issues (cysts growing on nerve roots in my spine). The ladies gave it their all, but it took five tries, and I was half hanging on both beds in devastating pain. At 200lbs I am by no means light, but am also not morbidly obese. My second day there, several people came to evaluate and make a plan of action moving forward. During the day, as long as I asked, they followed my medication schedule. Unfortunately, the same cannot be said for night. I was due for scheduled meds at 6 PM. When I asked, I was told they would eventually get to me, because they had to do rounds first. Three and a half hours later, the evening nurse showed up with my medication and a huge attitude. Ruth, the nursing assistant on duty, was polite and professional. However, the rudeness, judgment, and disrespect shown me by the nurse, far outweighed the kindness Ruth exhibited. I understand that a pleasant bedside manner is not required to be a nurse, but following a patients medication schedule is. After missing a dose and then having the next one be 3 and a 1/2 hours late, my pain was such, I felt I needed to go back to the emergency room. I was feeling very sick as well as in excruciating pain. The nurse did not agree; clearly stating as there was no need for an ambulance, I'd have to call myself. Had my vitals, been checked regularly, they would have known my blood pressure was extremely high, and I had a high fever (probably caused by the terrible UTI I aquired, after being left to sit in my own excrement for more than an hour and a half). I actually remained in the hospital several days before being released. I was treated for sepsis and an acute UTI. So in fact, there was a very serious medical need for me to be taken to the hospital. For those of you that may not know, "sepsis is a life-threatening condition that occurs when the body's response to an infection damages its own tissues and organs. It's essentially an extreme immune system reaction to an infection, and if left untreated, it can lead to organ failure and death". I am not being overly dramatic. These are simple truthful facts. In summary, I was in this facility less than two days. The day staff were polite, professional, and appeared to do their jobs, despite being understaffed. The occupational therapist I met was lovely. The "day" employees seem to have good working relationships with one another and promote a positive attitude. If a patient is there strictly for therapy, it's probably a pretty decent place. However, if you require scheduled medication or assistance with daily living skills, this is NOT the place for you. I have written 21 reviews in total on google, only two of which we're negative. As a patient, my energy should be focused on healing and moving on, not on fighting for my care and self advocacy. I hope this review can help people avoid the same terrible experience I had. I do not write this review lightly. If you or a person you know, requires care in a rehab, please look elsewhere, especially if the patient is unable to advocate for themselves. As I said, I was released from a hospital to this facility. l was at this rehab, less than forty eight hours, and their negligent care, could have cost me my life. It is only a matter of time before there is a fatality.

Experiencia fantástica: Surprise Health & Rehabilitation Center is a wonderful facility. The doctors, nurses, CNAs, housekeeping and all staff were knowledgeable and always helpful. The therapists got me up and moving so I could go home sooner than expected. The food was excellent and always arrived on time. Although I was happy to go home, I will miss everyone. Experiencia fantástica: "SHaRC" as I recall. This place made a HUGE difference in my rehabilitation, they ALL went ABOVE and BEYOND the call of duty. The staff ( Dr. Sandy-Dayton-Krystal-Lily-Miki-Rejoice-Ray-James-Francis-Gio-Hanna-Jennifer-Angela) AND SO MANY OTHERS...I'm grateful to EVERYONE there, that made my stay very comfortable and tending to my needs (ALWAYS POSITIVE ENERGY AND VIBES FROM EVERYBODY) you ALL are #1 in my book. The 1 lady coming to my room everyday for the menu, was VERY GOOD to me, and making sure I got what I needed and wanted. Housekeeping was very PLEASANT to me (always telling me to feel better) and keeping my room clean. THANK YOU TO EVERYONE AT "SURPRISE HEALTH AND REHABILITATION CENTER"

Experiencia fantástica: My stay at SHARE was pleasant. From the nursing staff, meals, cna's, &PT/OT all were very good. I especially enjoyed PT/OT as each time was good. The PT/OT were both knowledgeable, encouraging, &pleasant. The PT Shawnna spoiled me by bringing me coffee at 6 in the am each day. I also liked the exercises given by the OT. One last note was regarding the CNA Ruth. She was pleasant, helpful, & very easy to talk to.

Experiencia fantástica: I had the best healing experience. My nurses treated me with the up most dignity and respect during my stay there. I was feeling real depressed, but each person that helped during my stay there be it my Nurse Todd and Nursing Assistants John and Lucille to my Physical Therapist Amanda and Jessica who worked me so that I would be able to go home stronger and able to live my best at home. I can say that I was never bored. The Activities Director Tori made it great, I played Bingo, Scrabble and Trivial Persuit with other patients there. Tori was alot of fun and very friendly. She made everyone comfortable and we introduced ourselves to each other. I made a few new friends during my rehabilitation there. Julia part of the housekeeping staff kept my room very clean and was very quiet cleaning when I was sleeping. When I was awake I enjoyed talking to her. My over all experience during my stay at Surprise Health and Rehabilitation Center was GREAT! Thank you for my Plesant stay there.

She enters the room with a positive attitude and spring in her step every morning that she works. She asks about your previous day experience and your requests for the next day offering options. She listens to your needs and wants carefully noting them all. Kind conversation blends in her time with you leaving you, as a patient content and fulfilled. On a daily route to a swift rehabilitation experience, Andrea is a perfect experience to start that day of physical therapy. If Surprise Health and Rehabilitation Center continues to hire such proactive employees, I see more investors looking to be a part of that facility's growth. Experiencia negativa: I'm surprised this place is still open. My mother had a horrible experience. They left her dirty, I had to go shower her. Water for showers was lukewarm at best. 1 therapy session only after heart surgery and they gave her bacon and other not so healthy foods. She would've been better off being at home with someone watching her. They bury the bad reviews by having people they post positive ones..see yelp, and the buried reviews. Reported them but no one seems to do anything as usual in this state.

Experiencia fantástica: My mother stayed here to continue to heal and regain her strength after a hospital stay. She stayed 3 weeks and came out a new woman! Kelly, Lorraine, Joy, and Peace were so patient and good to her. Judith, her case worker, helped with all the behind the scenes details, and the OT and PT staff helped her regain her strength to better than it was before she went into the hospital. I would definitely recommend this place for you or your loved ones!

Experiencia fantástica: Posting for my Dad. My Dad cam e in with a fractured hip and no surgery. The room was very roomy and the meals were yummy. The Nurses, CNAs Physical Therapist and staff were all kind, patient, helpful, knowledgeable and encouraging to get me where I need to be to go home . I want to especially thank Giselle CNA- she always had a smile and always happy very helpful, kind and encouraging. It really helped a lot through this time. I am glad to have been able to be here and even more happy to be going home. Thank you all! Room 217

Experiencia negativa: Where do I start! December 28 2024 I had fallen down my stairs one of my knees gave out and I tumbled downstairs. I had neck surgery to repair 3 disk! In Boswell for 9 days then I was told I have to go to rehab hospital and to pick one on the list covered by my insurance. I have never been in a rehabilitation facility so we went with Surprise because of the rating and I get my own room. The staff for therapy are great ADA and Shauna are the best. However the ratio of nurses to patients they need to work on 1 nurse and 1 CNS to 21 patients. It funny they have this huge sign about the ratio I laughed because that is not true!! You turn your light for help and it takes anywhere from 30 minutes to an hour to answer, because am not the only patient I would hear! The CNA are nice but need more training in how to help their patients. It takes special people to help the elderly especially when they/we are in pain. The day I was discharged 1/29/25 the facility was to make sure I have equipment like bed, walker and other thing along with in house therapy. None of this happened. Their excuse was your insurance didn’t ok it.. So I called my insurance case worker to verify after being home 9 days with no equipment no therapy! Per my case worker she started to email my case worker at rehab to find out what happened Judith case manager went back and forth with insurance and I have all the email trails. Bottom line is it took 13 days when I was home to finally receive my equipment. The therapy is another headache! Again Judith blames my insurance however my insurance sent them the list of therapist that are in network but their intake person who was to set this up sent to other places NOT in network and it prolonged getting me some therapy. I am not one to sit around so I did my own therapy that I learned from ADA and Shauna and kept doing them everyday. Thank God for this team! Anyways the rehabilitation dropped the ball. Judith called me to tell me she set the referral to all facilities on the list and that was just 2/25/25 I been home with no therapy so now we have to go with outpatient meaning I go there and not them come to me, because of my doing better by myself I have to go with outpatient. Here’s the issue I finally getting it however not until March 11 2025 is my first appointment!!!! It took me going back and forth with the case worker at Surprise Judith and another person Yolanda which BTW was no help that I have no therapy for now 41 day! I even called and sent email to the CEO and left message to call me and he never responded Mr. Kevin. I left a clear message of how disappointed I am of this process. Am a lot younger than the elderly that are in this place. I can’t imagine the frustration for those going through something like this if they didn’t have family to help them! I rarely complain or even write a review but this needs to be addressed and all I hear is your insurance is at fault! I beg to differ my case worker at insurance and I couldn’t understand the lack of communication why this was not set up prior to me returning home! Why did I have to send emails and calls after I came home. Please before having your family leave this place make sure they set up everything prior to coming home so you’re family member does have to go through this headache’ They should just have to focus on getting better and not have to do other people’s JOB! To the CEO and staff at Surprise health do better please peoples lives are in your hands it’s not just about making money!! The facility maybe nice and clean but it’s not just about the facility it’s about those that work there too. Experiencia negativa: ⭐ 1-Star Google Review for Surprise Health & Rehabilitation Center I am beyond disappointed and heartbroken by the treatment my mother received at Surprise Health & Rehabilitation Center. Over the last nine days before her discharge, I noticed severe neglect, including her being left in soiled clothing for hours, increased confusion that was ignored, and unexplained bruising all over her body. Despite bringing these concerns to the staff, I was met with dismissiveness and no clear answers. On January 26, 2025, I visited my mom to pack her belongings for discharge the next day. During my visit, she spilled a Pepsi, and while a nurse helped change her sheets, they left the sticky soda on the floor. The next morning, I arrived to take my mom home, only to find her sweating profusely and in agonizing pain. She told me she had fallen, and a staff member had picked her up. However, no staff member admitted to this, nor did they document the incident. Instead of taking responsibility, the nurses, including Gina P., RN, were defensive and dismissive. They showed zero compassion for my mother, even raising their voices at her while she was in visible distress. Despite my repeated demands for medical evaluation, the staff delayed taking X-rays until after 4 PM—leaving my mom in severe pain all day with a broken hip! If I hadn't insisted on calling an ambulance, they would have discharged her in this condition without any medical intervention. The paramedics, were shocked by the facility's failure to provide care, noting "red flags" and inconsistencies in the staff's explanations. They even stated they would be documenting this as abuse. My mother, a 76-year-old woman who was supposed to be in rehabilitation for pelvic surgery and then while under their care suffered another broken hip due to this facility's negligence. She was left in excruciating pain for an entire day because of their unwillingness to acknowledge the incident. If you love your family, DO NOT SEND THEM HERE. This facility is a disgrace, and I will be filing formal complaints with the Arizona Department of Health Services and seeking legal action. No one deserves to be treated the way my mother was. Shame on this facility and its leadership!

Experiencia positiva: Surprise health and rehab has a very welcoming reception staff. The best experience was with the CM/DC coordinator Ashlyn and her assistant Paula, durning discharge. They made sure we had everything we needed for home, including prescriptions, Home health care and equipment. It was a pleasure working with both of them. The evening staff seemed to be very slow in response. Only some of the evening staff demonstrated a positive attitude. The wound care staff was very dissatisfactory!!! Some staff members were very lax in changing bedding and bandages on leg wounds Despite the fact that they badly were soiled. On a happier note, the food was good and delivered in a timely manner with a smile. The Physical therapy staff was very positive and engaging. They were good in delivering new techniques to the patients that was useful in the healing process.

Experiencia fantástica: I cannot say enough positive things about the therapy team. Sarah, Gina and Adam have a level of exuberance beyond belief. They are always upbeat and go above and beyond to provide a positive experience for everyone. I am grateful for all their encouragement and expertise.

Experiencia fantástica: I had a great experience with Gina and Sarah both. They are very knowledgeable about physical and occupational therapies. They knew when to push for results because they got to know all of their patients. The music and singing made therapy bearable. Thanks for everything.

Experiencia negativa: If you're looking for PT/OT, this is probably the place for you. If you're looking for the "Nursing" services they advertise, go elsewhere. Anywhere else. Neglect is not a strong enough word. A patient, bedridden with a catheter, unable to move left leg at all or roll herself called the CNA on duty that she had an accident (bowel movement). The CNA took 32mins to respond to the call button. I was there at the time. CNA said she needed to finish rounds before she could help. 1hr and 12mins after the initial call, the CNA arrived to clean the patient. It's embarrassing enough to be in a condition like that, but the humiliation and uncomfortable situation, and the smell she had to suffer for over an hour, is absolutely ridiculous and unacceptable! The CNA was extremely sweet, but seemed to be following a policy. "Sorry, you're the last room". The nurse on duty the night of Jan 15, 2025, was about as nasty as anyone could be. The patient, in extreme pain, discomfort, running a fever, and felt like her pulse was racing, asked the nurse to call an ambulance. Not only did the nurse refuse, the nurse had an attitude about it, told the patient to do it herself, that she didn't need an ambulance and was just wasting the paramedics time, AND was just seeking meds! Wasn't able to catch her name, but she was very short in stature and, for lack of a better description, medically obese. Pretty sure she was the only "nurse" on duty, so shouldn't be difficult. Paramedics arrived, found the patient's blood pressure to be 174/106 and was running a fever over 101. After two days in the hospital, she has been treated for the discomfort, antibiotics for the infection, and has been diagnosed with Tarlov Cysts. No, do not use this facility if you need nursing care. I wouldn't wish this place on anyone!

Experiencia negativa: Had the worst experience here with my mom who just got done with a hip surgery. She left her call light on for nearly two HOURS and no one came regardless of her inability to put any pressure on her foot. When I was there, the nurses practically scattered and left me to do everything, transferring my mom in and out of her wheelchair, getting her up from the bed/toilet, tracking down the nurses who needed to give my mom medicine. Yes I do understand these nurses are stretched thin, but when the other nurses can’t even get ahold of your own CNA and those nurses are REFUSING to help, that is when I get mad. The facility is beautiful, truly, but I will ALWAYS prevent anyone from going here. This place needs to be shut down with malpractice. My mom couldn’t do anything herself, the doctor told her to stay off of her foot, and you guys just leave her to her own devices? This will be reported.

Experiencia fantástica: I’ve been going to Jessica at the surprise health and rehabilitation center. She is excellent and every time I would leave the facility after seeing her for an hour my torn tendon would feel so much better. I have made a lot of gains with her help and my help doing it at home And we worked together as a team. She is wonderful and I will miss her greatly.

Experiencia positiva: Judith, the case worker didn’t send Antonio’s prescriptions in and the one causes seizures if taken off all at once. She caused him to be off for 2 days before going to dr for prescription. Also forgot to call in pain meds.thankful nothing happened. Everything else was great.

Experiencia fantástica: I went to check out this facility for employment purpose.All I have to say that, I was impressed with the customer service received , from the front desk to the medical staff everybody so friendly, courteous and , genuinely attentive. Another thing that stood out to me it is the cleaningness of this place , immaculately clean and, " Smell" free, compared to the others one that ,do a distinctive smell to them . 10/10

Experiencia positiva: Very clean, PT team is very good. they do group exercises and make it fun to where you don’t know you are doing PT. Sandy the nurse was very helpful. You do have remind them of things but once you do they are all over it. Ashyton was very helpful with discharge. Thank you!

Experiencia negativa: Had my mother at Surprise Health & Rehab from Nov. 12th to Nov.25th. She had her own room (#158) and the room was cold as the thermostat in the room was broken. Also there was a window air conditioner that had open gaps to the outside and the nights were getting down to 45 degrees F. My mother made complaints constantly about the room being much to cold and her meals always being served cold. The maintenance staff finally removed the window AC unit on the day she left (Nov. 25th). Many of the nursing staff and nursing assistants were busy having conversations and talking on their phones. There was no PT (physical therapy) or OT (occupational therapy) on the weekends, so patients just laid in their beds or sat in a chair. After making complaints about the lack of care on the weekends, my mother finally got some PT on her final weekend. I would not recommend this facility for rehab after surgery for anyone.

Experiencia fantástica: My dad had a great experience being a patient here. He loved most of the staff and felt like he was getting the help he needed. Bree from speech was amazing and really worked with him to gain strength. Same with all the pt people! The place was really beautiful. However I did notice some pee on the walls that I wished was cleaned better.

Experiencia fantástica: I spent over 3 weeks there as part of my tongue cancer recovery. I was in Rm # 160 and found the following people as ones that all went beyond their duties. Each went the extra mile to help and was a true joy to work with. I have such high praise for the following people.: Armondo, Lindsey, Isaiah, Cedar, Christina ,Jen in PT, Hannah in OT, Rosie in PT, Erica, Nathaniel, Becky . I believe all these folk were from the "A" side of the building. Please fell free to contact me directly if you wish. .

Experiencia fantástica: This facility really took great care of me! I was probably the youngest individual in their care. I was really able to assess and see what was going on daily. Everyone has a smile and it’s clean. I enjoyed the daily activities and PT was phenomenal! The food was excellent and I couldn’t have asked for a better medical staff. This is an “A” facility for sure.

Experiencia fantástica: From the moment I entered thru the front doors I was welcomed with smiles and positivity! The facility is pristine, offering a safe, comfortable environment. Private rooms with your own bathroom with walk-in showers! The staff is all about giving each person the personalized care they need to get back on their feet-literally! The staffs dedication to providing exceptional care & support is truly remarkable. I had the pleasure of being able to work with several of the PT's & OT's mainly being Amanda-pt and Adam-ot, but also Amanda-ot, Rosie, Sarah, & Gina ( if I missed anyone I'm sorry!) All the RN's were exceptional but one in particular really went above and beyond to help me and that was Todd. Had really great care from the CNA's Juan (the Juan and only) made me feel at ease when I arrived. Sanji, Danielle, Tori, Brandon, Michelle, Autumn, Elijah were just a few that I can remember. Three others that I really want to mention for going above and beyond are Renee , she really took the time to keep me going with her positively. The other two young ladies went above and beyond to take care of me and make sure I was showered before being discharged and those two ladies are Sienna and Davae. I also wanted to mention Andrea from food services she was amazing and the food was pretty good! Loved that we got fresh fruit almost daily! The housekeeping staff is excellent especially Julia. And last but not least I wanted to mention Ellie who runs activities . I had the pleasure of attending Bingo and a fall craft session and had so much fun. They have therapy dogs that come to each room to visit. Last but not least my amazing case manager Francisco and CMA Ashlyn. I highly recommend this rehab facility to anyone seeking top-notch care! I would definitely stay here again!!

Experiencia negativa: Please do a walk through before putting a loved one here. I believe our problem was due to being understaffed. so many patients for just a few workers 30 minutes or longer for someone to show up after pushing help button. Couple times no one came. My father's experience during a one month stay was very disturbing. Oxygen generator Humidifier cup o2 hose fell off dads o2 dropped into the 80s. Dropped in one day dad had been sitting in a wheelchair watching TV i checked his o2 80s again empty tank on back of chair. He was finally discharged on a Wednesday. Saturday had to call 911 back in hospital with pneumonia and a MERSA blood infection. I can say the cna and PT staff do the best they can but seem overwhelmed. RNs, docs, and management need improvement.

They had me up and walking quickly and without pain, within a week had stamina to return home. Will be doing outpatient to keep improving. the nursing staff was attentive to any needs and always available. Was Know as the quiet guy at the end of the hallway, would HIGHLY recommend them to improve your quality of life..... gjkjr

The nursing care, the food, the physical/occupational therapy, all of the persons on staff that I interacted with; everything was just wonderful. Cleanliness is king there. I can't say enough about how great this place is!

Experiencia fantástica: My mom was there for almost 3 months and received great care from almost all the staff in the building. We're going off my memory here, so I'm sure I'm leaving people out, but here are the ones that stood out and that I can remember (i should've made a list). Sandy and the docs made sure Mom had the meds, specialists, tests, and other things that she needed, taken care of quickly. Nurses Regina, Tara, Cathy, and Brian were all so kind and gentle with Mom. CNAs Brandon, Beulah, Cedar, Bianca, and more who's names escape me were very attentive. From the kitchen, Andrea and Ruby were very sweet when they visited daily. PT/OT Jen, both Amandas, Kaley with her sweet boy Hennessey, Adam, and more were very knowledgeable, motivating, and encouraging. I hate that I don't remember the front desk lady's name. I know it starts with a T. She is so helpful and wonderful. Ellie works hard to schedule fun activities and brings good girl Gypsy to visit on Mondays. Laura put up with my many many emails and scheduled appointments and safe comfortable medical transport for Mom. Ruben and Ethan can fix anything and are so very kind and positive to speak to! I know I'm leaving amazing people out, but I wanted to list as many as I could to show just how wonderful the staff at SHARC are! Also, I want to give David a special shout out for being such a bright spot whenever we ran into him in the lobby. What did the pig say when he got hot? I'm bacon! We want to thank everyone for their care and attention while we were there, and would recommend this place to anyone who needed a little extra help in their recovery... with one exception. They weren't able to provide a sitter and my mom needed someone with her 24 hours a day to keep her from getting out of bed. Fortunately, our family was able to take shifts, but I know not everyone has that flexibility. Edit: I remembered more names of wonderful people - Sage (and the other wound care nurse is great also, but my old brain doesn't remember her name), Rene, Francisco... and the housekeeping staff, too! They were all very kind. I really wish I made a list. So many should be recognized. ⭐️

They are very efficient, they motivate me. they treated me like a person and not a patient and took the time to get to know me. They were so supportive throughout my stay. They went above and beyond every day with my therapy needs. Thank you Sarah and Gina To my nursing team: Tara, Whitley, Nakisha, John, Michelle, there were many others and i apologize if i forgot your name. Experiencia fantástica: My whole stay has been like I've been with family, everyone was so friendly. Sarah OT and Gina PT were absolutely amazing, they are so good. they treat people so nice, they help them with all their needs, and the amount of encouragement they gave me helped me push myself. I am so grateful to have had them as my therapists and to have met people like them. We need more people like Sarah and Gina, I love my homies. The nursing staff and CNA staff is so great. Experiencia negativa: My mom was sent to this facility and had to be taken back to the emergency room yesterday. There was one particular CNA and one nurse who seemed to be making things much worse for my mom. The nurse never came in when requested and then said something that made me realize she confused my mom’s room with another patient, then tried to cover that up. I never got any answer on the phone nor a response from the email I sent. I spent all day and night doing the job of the staff by getting her something to throw up into and cleaning up the throw up. They never changed the sheet that had throw up on it and took pictures for evidence. It is no wonder there is now an issue with a particular fungus my mom had been cultured for, and is still waiting for the results from that. I am not wanting her to go back to this same facility also because I was given wrong information, not given requested information and the way Autumn treated me yesterday. On the other hand there was good energy from another nurse, a CNA named Elijah and the physical therapy. Yet the physical therapy only started yesterday and wished it would have started sooner. Also other people were complaining of cold food and being charged multiple times for food bought in their store downstairs.

Experiencia positiva: My father was here after dealing with pancreatic cancer and chemo reducing his abilities to move. The staff was wonderful and very responsive. Right down to the maintenence gentlemen. My father ultimately passed away here and our family was treated with the utmost kindness. The facilities were very clean, food was good and my dad felt taken care of. Thank you

Experiencia fantástica: I can't say enough how great the trainers are at this rehab center! Especially my trainers Gina and Sarah! So very patient and encouraging! I am doing better than I was even before I was I'll and needed to go! And if these ladies weren't available there was always a kind person to assist! I would definitely recommend this facility!

Experiencia positiva: My father was here for 19 days after a hip replacement. The facility is inviting, clean, smells good and we loved the private rooms with private bathrooms as well. This was a huge plus! The nursing staff was warm, caring and attentive, and he had a good experience with OT/PT in their wonderful gym as well! From the front desk to the case workers and billing department, all were friendly, kind and helpful. The only reason why I gave it 4 stars instead of 5, was the food. Providing more nutritious meals that are not cooked to death would be a refinement for SH&R. Other than that, it was a fantastic rehab facility with excellent staff! I will definitely have my dad go there again in the future, if a medical need arises! Thank you to the entire staff at SH&R! We appreciate your hard work and please know you made a positive impact on my father!

Experiencia fantástica: My father spent 10 weeks at Surprise Health & Rehabilitation Center, which was the second rehab he stayed in this year, and I will say it was FAR superior to the first one. The facility was clean, the food was decent and cooked well, and most of the staff was pleasant (although sometimes they seemed short staffed). We understand it is a sub-acute care center, but we do wish he would have received more PT & OT during his stay, as it was far more limited than we expected. I would especially like to highlight an employee by the name of Brandon, who really went above and beyond for my father (Conrad). “Thank you, Brandon”, for assisting my father, even when you were short staffed, or it was the end of your shift. We appreciate your willingness to help and the care you provided to him!

Experiencia fantástica: My wife searched several Rehab facilities but the hospital recommended Surprise Health and Rehabilitation Center. What a pleasant surprise. It is true every facility has a couple of staff issues, but the vast majority of the individuals that work here are exceptional, caring and professional. I can't praise them enough. I came here after emergency surgery and laying in ICU for two weeks. I had major surgery and literally could not do anything thing. The Rehab staff were compassionate and also demanding but more important my biggest cheer leaders. They are truly great at what they do. Thanks to the exceptional care and help given by Amanda, Jessica, Tyler, Emily and Adam. They are all excellent and I don't think I would be walking or any of the many achievements I have made without them. Medical is a major part of Rehab also and I received excellent care with compassion, caring and guidance the best people that went out of their way to give me excellent care CNA'S Francisco (exceptional), Elizabeth, Michelle, and Renee. LPN's Jessica(excellent)Regina ( exceptional), Sage, Brian and Leena. NURSE PRACTIONER Sandy was absolutely excellent. She is compassionate, caring and gives the feeling she care about she is a Fantastic lady and very caring. RN Lisa. Excellent Nurse, compassionate and caring and a good teacher. Ethan. In maintenance a nice young man who came and fixed my bed and visited once. He went out of his way to get an air mattress installed in my room to avoid me being out of bed longer than necessary he worked and put portions of the bed to in the hallway and that reduce my time out of bed and discomfort.. These people care and do and excellent job. I am going to have surgery in a few month to reverse some of the things done in the surgery to save my life. If I require Rehab I am coming back here. You guys are an example for other to strive to meet your standards.

Experiencia fantástica: They took excellent care of my husband who has a broken neck. The entire staff from nursing, PT, OT, housekeepers, chefs, front desk, administrative, everyone was fantastic. Respectful, kind, compassionate and were on top of everything. I wish all facilities and hospitals were like this facility. Everyone here seems to "want" to be here working and doing the best job they can. We can't say enough fantastic remarks about this facility and all it's people and protocols!

Experiencia fantástica: Surprise Health & rehab center is nice and clean. Ruth the CRN working in the 2nd floor was kind and caring. She was gentle with my 93yr mother. Mom appreciated Ruth's patience and kindness towards her. Ruth is a great CRN!

Experiencia fantástica: My husband was admitted to Surprise Health and Rehabilitation after spending two nights in another facility that was not up to our standards. I came to research Surprise Health and Rehabilitation and decided that this is where I wanted my husband to be for his Rehabilitation. His CNA Nykiesha is the best, she is caring, nurturing and cared for his over all being. She talks him through every move she makes, while caring for him. She even "attends" to him during her passing by his room. She will stop in to check on him. She is a treasure to keep. We will miss her after we are discharged

Experiencia positiva: No rehab is perfect although Surprise Health and Rehab is one of the best in the area. My mom has been to several rehabs in Surprise, Sun City, Peoria, and El Mirage. SH&R is clean, smells good and offers private rooms with a bathroom and shower. The staff is mostly polite and courteous. We especially enjoyed having Ruth the night nurse take care of my mom. She was always very helpful and had good bedside manner. I would recommend stopping by for a tour if you are looking for a rehab for yourself or a loved one.

Experiencia fantástica: My husband was here for about 11days, from the time you walk in to the reception area to the time you leave everyone you meet are wonderful. Reception ladies are very helpful and answer all your questions. The nurses and aids are always there to help. Therapists work with you so you can go home faster. Thanks for helping him get better.

Experiencia fantástica: Sara & Gina are the best PT staff. They push and support you to do the best you are able. Plus they make it fun. I want to thank my PT staff I was in surprise Health for 69 days. I use what they taught me at home. I also the nursing & CNA staff that went above and beyond. Regina, Brian, Jess, Tammy, James , Todd & Erica nursing. CNA: Francisco: he took such good care of me. He is so kind. I looked forward to the days he worked. I knew that my needs would be taken care of. No matter how busy he was.You knew he alway answered call lights right a.way. He is truly Amazing. Michelle, Zoe & Angie were also Amazing CNA during the day shift. Liz & Zack also truly was also Branon, Juan and 2 other CNA I don't know how to spell there names they took care of me the 1st 2 weeks. When I got there I was very sick and truly believe those 4 boys saved my life. They also were caring and went above . The activities director Ellie kept me busy with activities that the days went so fast. She made it so nice I really hated to leave. The staff is what make this place so special. The food sucked! The desserts were delicious and the Blueberry muffin were the best. I would go back if I ever had to. I for got to mention the head of maintance. Ruben who fixed so many things for me in my room. Like I said the staff was the best. I was blessed that we went to Surprise Health and Rehabilitation center

Experiencia fantástica: We toured this facility prior to my Mom being admitted and we were impressed! Each patient gets their own room! At the front they always have how many nurses and CNAs are on staff and their current occupancy listed! Want to definitely shout out Nurse Regina and CNA Francisco! They have both been so amazing, helpful, and compassionate! The Physical Therapy staff is wonderful also. The meal plan is like a school cafeteria, some meals have been great and some not so great. Mom’s room had some maintenance issues. Once we brought it to their attention, things got fixed ASAP! We always think it’s best to have family at bedside as much as possible to advocate for their loved ones, even at a good facility!

Experiencia fantástica: My Brother was transferred to you facility recently and has received very good care and attention. I want to reach out and acknowledge some of the people on your staff that have gone out of their way to help me and my Brother.. Ruth Musyimi Yonna Jenny his Physical Therapist Judith in case management Sandy his PA was so good to me and went out of her way to explain his condition and next steps.. I had to travel from Colorado and it has been a very difficult journey but thank God there are Angels among us...thank you all

Experiencia fantástica: I want to thank Francisco L for taking such great care of me. He is an amazing CNA. If you press the call button he is there to help you within minutes. The only time you may have to wait is when he is giving a shower to another patient. My suggestion is to have a Floater CNA to cover those calls. Michelle Serna is another wonderful CNA that took great care of me. I can’t say enough about Therapy, especially Amanda & Jessica who worked as a team to get me well. Adam is also great. Though their patience’s and help I have been able to go from not being able to do anything to finally going home because I have made tremendous strides. The Kitchen, Maintenance, Nursing & Respiratory all do a wonderful job. I have been in other Rehabs and Surprise Health and Rehab is far above many many others. You have a private room and bathroom. Housekeeping comes in every day so the facility is clean and pleasant smelling. I highly recommend this facility.

Experiencia fantástica: I had a good experience there considering the kitchen staff was bad But the NA Angie Tercerao was great she mad sure my 3 square meals were served the way I explained it to her and the service for bathroom hygiene and for water was done without a problem and I deeply appreciate that kind of service. The night staff was good but not outstanding and there service was deeply appreciated.

Experiencia positiva: I would like to thank the entire staff for their kindness and their professionalism in my care during my stay. I would like to give honorable mention to Amanda, Gina, and Adam from therapy and last but not least Todd from nursing. CNAs were pleasant. My complaint would be the response time- I found myself waiting a very long time for basic needs to be met and answers to my questions pertaining to my care.

Experiencia fantástica: This rehab was very good for my father-in-law, Rudy Kappe. He received good OT and PT from the two Amandas, Jessica, and Emily. His daily care takers and nurses were great, especially Francisco, Regina, Michelle, Stephanie, and the newcomer Toni. Additionally, PAs Brandon and Sandra were thorough and very caring, visiting him almost every day. Dr. Merkel was very informative and caring. We would like to thank everyone associated with his care for their efforts.

Experiencia fantástica: I was in a terrible automobile wreck where I was T-Boned by another car. He hit me right in the drivers door.i couldn't get out of my car. The fireman came and got my dòor open and extracted me from my car.The rescue came and and tòok me immediately to the Deer Valley hospital where did surgery on my leg the next morning. I was informed that I would be going to a rehab facility which I had terrible nightmares as to what it would be like. I've hread so many horrible stories about rehab facilitates I was very worried. I would like to state that the Surprise Health Facility is clean the nurses are well trained and go out of thier way to keep you comforfortable. I would recommend this facility to any one. The therreapst are well trained. I want to thank Jenn for all she did for me. She worked with me in getting strength back.She is awsume and I want to thank herfor the and effort she gave me. There was another therreapst that helped get my ability to speak fluenty again.i believe her name was Caela (spellig).The accident really messed me up including my speach but Caela worked with me and I'm back to normal speaking. Between Jenn and Caela I owe an awful,but the entire team of nurses and cna's are all professionals. Don't ever be afraid to go to this Rehab faciĺity

Experiencia fantástica: My family chose this location for me and I feel they made a great selection. I can recommend this place for anyone looking for a safe and caring rehab center. The staff is superior compared to other locations, they really care about the patients. The Physical therapy staff was extremely supportive and caring.

Experiencia negativa: Place is a joke. Mom had major surgery, they discharged her twice when not ready, rehab said she could walk 140 feet unassisted.. couldn't walk 10...? Went in healthy, came out with a UTI..?? (sitting in bed wet diapers for hours) Billed for days in advance and post of actual discharge, then sent a past due notice for never having been billed correctly.. Lost items, no communications, Take your loved ones else where!!!!

Experiencia fantástica: Large, clean facility, well staffed. Spacious private rooms. There's a nice area to relax by the fireplace. The outdoor courtyard could use some remodeling. Definitely recommend this facility for a loved one.

Experiencia fantástica: Finding a location to provide the necessary care and accommodations for a loved one is an emotional experience. So grateful for the care provided by the professionals at Surprised Health & Rehabilitation. I appreciate the efforts made to accommodate my father. The staff is attentive and professional.
