Experiencia negativa: This facility is ran by an out of state company. They do not pay their bills. They currently owe my company over $6,000 dollars in unpaid invoices for plumbing work dating back to October 2025. We just had to send them to collections for non payment. Joe the administrator has fed us lies for months. Do not trust this business. I’m leaving this review up until we get paid!!!

Experiencia negativa: Do not recommend for rehabilitation. The neglect we have seen has been sick and disheartening. We shouldn’t have to walk in to see our family member hanging out of bed, diaper not even fully on, and covered in food. We know he won’t make it long but having his life be cut even shorter due to neglect? How disgusting.

Experiencia fantástica: My mom‘s been there twice now for rehab and we like it there she does too. Everybody’s very friendly. You have good food options. Someone one’s always checking to see if Mom would like a beverage or snack. The rooms are nice and roomy. The room are also a little bit cold Like a hospital usually is but she’s got lots of blankets and she’s heading to play bingo. Today I took my Chihuahua in and she had a great time meeting all the people just bring your vaccination records and they’ll let you bring them in last but not least it’s very clean they’re always cleaning

Experiencia fantástica: I have been coming to visit my friends here for over a year now and noticed how much cleaner and homey it is than a visit I had at Eagle Cliff in the heights this past week. It’s important to have a facility that doesn’t reek so visitors will come! Sad enuf so many elderly are forgotten or avoided by family because of where they live. Yellowstone River is super friendly, organized fun, and smells clean! Thanks for taking care of our loved ones

Experiencia fantástica: I toured the facility and found it very nice and clean. The rehabilitation gym was very large and had lots of equipment. The interior courtyards were beautiful and even had a nice fountain. The private rehab rooms was very spacious and included a large TV. The staff was friendly and helpful. Such a pleasant experience.

Experiencia fantástica: Yellowstone River Nursing…I would like to take the time to say “Thank you” to Brandy, Micky, Heather F, Nurse Irene and Heather S for making the move of my dad from Kalispell to your facility easy and a blessing! After visiting with everyone, I knew this was the place for my dad. The next day I stopped in to see how my dad was doing and he said, “this place is so much better!” So, this made me feel good to know my dad approved. He did say that he will have to plan his escape for the spring cause it’s to cold right now…LOL! He is in the Pearl Care Unit/Memory care. Thank you to the staff for all you do! This is not an easy job but they put their best foot forward. God bless you all for an excellent job!

Experiencia negativa: Complained about my dad asking for help and using call button too many times…he just had a stomach tube surgery right before going to them and needed help with things and the “CNA” staff would tell him do it himself and walk off…when I asked to speak to the nurses they never knew anything about it which seemed to always be the story…they wouldn’t give him the things I sent to him and the “doctors” which u never get to see or talk to would never return ur call or answer questions…because they told my dad he used the call button too much he ended up falling and luckily is ok but this place seems to be not in it for the residents there …don’t recommend

Experiencia negativa: they lost a christmas package intended to be delivered to resident who is down syndrome and recovering from a stroke. the receptionist hung up on me upon inquiring about the package's whereabouts. I wouldn't trust them with my life or recovery.

Experiencia negativa: Dad-in-law is there for 2 days and we are already very unhappy with the care there. It took them 30 minutes to answer the bell when Dad needed to use the restroom. And they took his food tray before he had a chance to eat. All while the nurses sat behind all their computers giggling. Not impressed. I hope they get their crap together.

Experiencia negativa: As I sit her with my mom at the doctors office I hear a conversation about an individual waiting to be picked up by this facility, with no response, and she has been waiting at least 2 hours. I am paying attention because while my mom was at this facility she was taken to an appointment and left for over 4 hours, until she called me and I had to get ahold of them. I was furious, because at the time my mother was a fall risk and not to stand or move on her own. Seems to be the same with this lady. The food was sometimes in identifiable, most of the staff was ok, the patients made it good. One individual has more knowledge of the need of the patients and advocated for the, she herself was a patient.

Experiencia negativa: Been visiting a family member for the last couple days. Nurses don't attend patients in a timely manner or even check if they are ok when the lights come on. After waiting an hour I or another family member would have to get a nurse. Had one nurse showing signs of narcotic use one day. But tonight on 7/5/2024 there was a patient screaming for 45 minutes for help that I had to go find a nurse for because of the way they staff and attend the lights. Would not recommend this place and go somewhere else. If you can't avoid it please visit your family member everyday day all day. Would rate a zero. Hope other places are better than here. It takes less that 60 seconds to check for lights and make sure patients are ok so you can finish with another patient in more need.

Experiencia fantástica: I’ve had the privilege to work with the amazing team at Yellowstone over the last few years and can say that their devotion, compassion and dedication to their residents far surpasses others! I feel that the leaders at this facility are always working to navigate obstacles, they are open to anything that benefits their residents and treat their residents truly like family.

Experiencia negativa: If there was a 0 star rating, I'd use it, they neglected my father's medications, simply because "We forgot, because he's a new patient", and there was only 3 staff, he pressed the emergency button, they didn't respond for 30 minutes, we got someone, who worked on a different plane, and she was making it about herself, saying stuff like "I'm going to be honest, I'm retiring, I have no reason to lie, the nurses here care, blah blah blah, I was on my way to (whatever store), I need to feed my mom, i was just getting off shift", excuses, excuses, she was trying to make it about herself, while we were concerned about my dad, right when we were leaving, there was a patient, on his wheelchair, sleeping, at night, while no one else around, oh, and they had the audacity to ask "Can you turn that button off" after they responded after 30 minutes, like they didn't care about him, just the button, this was only the first day

Experiencia negativa: HR has no problem throwing you under the bus. Melissa will create reasons to fire you. I stopped working for them when she got in my face and screamed at me. All I did was not getting to her bathroom before I clocked in for work. And that's how the employees are treated. The neglected older generation that pays to stay there get treated even worse. The mice and ants will crawl into the patients bed and the facility has to hire traveling nurses that all take their breaks at the same time and just let the patients sit in their filth calling for help. Ive had patients beg me to give them a shower because the place doesn't have enough staff to provide showers. 13 days is a lot to go with out at least a wipe down. The mice are really a problem, I used to have to shake my cart to make sure the ones sleeping in the bag would scrawl away fast. The turn over rate is bad, no one really stays unless they have no other skills. But even then, the rooms are cleaned only for show. The dust and fecal matter that gets smeared on the walls are only just taken care of when someone calls state on the place, which happens fairly often.

Experiencia negativa: 10/10 would never ever recommend this place for any family member. I am a CNA my self and my grandpa was put here so he can get surgery so he has to do some rehab. The things my mom tells me from going and visiting him is terrible she leaves in tears every time. I can’t even make myself go and see him because I know I wouldn’t keep my mouth shut. This place needs to be shut down. I wish I could rate this with 0 stars. The first night he was there they didn’t help him into bed and he slept on top of the covers because he was struggling and was freezing cold. Food served so late and COLD. Lost his clothes found them in another residents room. TERRIBLE TERRIBLE. Only there because of insurance sh!t.

Experiencia negativa: Put your family here if you want them to die sooner rather than later. Horrific how much neglect is allowed to go unpunished at this facility. Administration absolutely deserves to have every license stripped for allowing this facility to still function. Also just so everyone knows I don’t know if they were bought out or are trying to get away from their previous name but this was AVANTARA OF BILLINGS!!!!! They may have changed their name but the abuse that goes on here has not changed DO NOT PUT YOUR FAMILY HERE!

Experiencia positiva: My husband has been in Pearl section which is memory care. He has gotten very good care. Is it perfect NO! But I have spent more time there than most families. Those CNA'S have their hands full. Just think what it would be like taking care of a room full of 2 year olds that are adults. Need all their needs met diapers, food, clothes etc, some help eating. They a great job!

Experiencia negativa: My aunt was there and I really upset about the care I heard she was getting. She had to sit in bed till 10 or 11. One time she didn't even get Breakfast. I'm glad she was able to get out of there soon. Some people were great help. But some did nothing. Please work on policing your workers better

Experiencia fantástica: Just came home after a stint in the rehab wing. I personally have nothing but good to say about the staff that took care of me while I was there. Other posts talk about staffing problems. That may be true, but all the staff that took care of me were as professional a crew as I have seen. I never missed going for a shower, I never missed a meal, my meds were brought to me on schedule, the Physical Therapists made sure I learned what I needed to go home. A comment was made about people calling for help being disturbing. Yes, I had that happen both in the hospital after my surgery and in my wing of the facility. It was explained to me that it was because the patient was in some stage of Alzheimer's / Dementia and was in a strange to them place and they were very confused. Taken in that context, it was completely understandable. I listened to a poor lady call help for some time, and finally pushed my call button to ask what was going on, and was told when the aid came that someone was sitting with her and trying to keep her calm, but the person didn't recognize her and was still being emotional. It wasn't because the patient was being ignored. I personally would have no problem if the need arose to spend time in the rehab facility of this operation.

Experiencia negativa: I would not recommend this place for anyone. I spoke with the social services director and she is rude and does not know how to speak with people Not even sure how she got the job. My family and I came to visit our loved one in here and he just seemed miserable. Please avoid at all cost if you can. The cnas don't care they are short staffed. They are all rude here.

Experiencia negativa: If I could give zero stars I would. This place should be shut down. The nurses cannot tell you what medication they are giving, scold the patient when the staff neglects to place compression stockings, had to have the bed replaced 3 times before getting one that worked. One CNA actually tried to use the surgical leg she was holding to steer the patient in the wheelchair. Meals are deplorable. PT schedules not adhered to. People yelling out all night. This is a profit above people facility.

Experiencia negativa: My dad got wedged between his bed and wall. He said he felt like he was suffocating. He could not tell me how long he was there but he said he felt like he was dying. When I called the nurse she said the mattress was too big for his bed so when he rolled over the mattress was part way off the bed and tipped pinning him against the wall and bed. Also they did not calculate the number of treatment days he had left for Medicaid correctly and billed me. When I said I would not pay and sent letters everywhere reporting it was not my responsibility to track treatment days and I should not be responsible for their mistake, I was threatened with legal action...classy. When my dad was staying there I dropped off small amounts of some of his favorite foods. When he was discharged and I was unpacking his belongings, they had packed the rotting, spoiled zilplock bags of liquified remains of what I brought to him in with his things. That food sat in his room unopened for him AND they packed the spoiled remnants with his things. I was horrified. This was during lockdown. Very poor overall. It was so awful.

Experiencia fantástica: My family members have sought the services of this rehab facility twice this year. Staff and medical providers are wonderful and caring. Everything about this facility is great with the exception of the heating/cooling system which needs upgrading but the corporate owners have not attended to it.

Things won't change for you or others unless you speak up and let management know there are issues. ***. Update: Avantara and their parent company were very responsive and quite happy to fix all the issues mentioned. Feeling much better about having my Aunt here while she heals.

Experiencia negativa: I have to share a few things about Avantara, this place is nothing but full of hate and neglect. Me as a travel nurse for the very first time, was a horrible experiences. Always short staffed, gives out false statements about a staff, resident that gets room tray, they were to get food or they don’t, they don’t get everything on their request or they get burnt food. I have had few staff treated me horribly but yet management believe their side of story instead. Felt like they did that cuz I’m hard of hearing and they just took advantage of it. Basically. On the main neglect part, those residents don’t get their normal shower days cuz of the bath aide they have but they don’t have enough bath aide to cover all showers so some residents wait like a week or so before they even get a shower….They also even have staff that always shows up late for work. I showed up for work at 5:50a one morning when one CNA didn’t show til 1:30p. Yet that CNA still works there and gets away with it. I’m struggling to find a job due to their False statements and mistreatments..This facility needs some serious help and improvement because those residents pays so much money and gets treated unfailingly. Lastly, this place needs air conditioning! That place was too hot to work there!!

Experiencia negativa: I would not recommend putting a loved one in Avantara for long term care. Our experience has been nothing but frustrating and saddening over the last year. Residents are not getting showered regularly (a lot of times I have to call and request a shower for my mother after a week of excuses). I don’t find it appropriate that I should have to call and threaten to file grievances for a shower. I have monthly conferences calls with social workers which sometimes includes staff, about the issues that have arose including: my mother not being out of bed until after 2 p.m. when she has requested to be up out of bed in the morning, no showers for a week, briefs not being changed in timely matters, food not being given out until hours after their lunch and dinner times. (Sometimes no food is even delivered and the kitchen staff has left for the night so she gets a small sandwich as a substitute.) I understand accidents and mishaps but it’s no longer accidents and small mix ups, because they are continually occurring. Months after these issues have been addressed and I address them again, nothing changes. My mother’s depression has gotten worse and worse over the past year because of the lack of proper care. My voice has gone unheard, unnoticed, and my mother is the one who is suffering. I understand the staffing shortage, I understand stuff happens, but I can’t understand the constant and blatant lack of care that my mother has endured over the past year.

Experiencia negativa: Im only giving two stars because of the way the final process was handled. I was told by the hospital, incorrectly, that my fathers visit would be covered as long as he was making progress. At 21 days I was called and told that on that day I would have to start paying $178 per day for my father to stay. I was given zero notice. I tell them I can’t afford that, and I’m basically pushed into having him out of the facility in 4 business days, INCLUDING the day they called me. They called on a Wednesday, and they expected me to have him out by Tuesday at 2. I requested, even demanded, multiple times for his prescriptions for his durable medical equipment to be processed in an ASAP manner, so that I would have time to prepare him a room in my home. Now it is Thursday afternoon and I don’t have that information. I am expected to MAYBE receive that on Friday, go to a medical supply store, and expect them to have a specific hospital bed, and mechanical lift, in my home, ready for my dad, by Tuesday afternoon. We also live 203 miles from the facility, so I have to pick him up myself because insurance refuses to cover the trip. That part isn’t on them. I understand they have a lot of patients, but there are times when you drop what you’re doing and make things happen.

Experiencia negativa: Terrible. Always short staffed, it always smells bad. The new owners are cheap. At one time we could make sure my grandparent was taken care of but now they’re being held prisoner because of COVID. Stay far, far away.

Experiencia negativa: Air Conditioners have been broken for over a month in this place in the middle of the Summer. It so hot for anyone mostly these poor resident. Staff is over worked not enough CNA or Nurses per ratio of DOH regulations. Switch Administrators all the time. DON seems to not care at all. Money machine for Corporate. I have a loved one in here & I'm very frustrated. Reported them over & over. Night time after hours place does smell of urine, residents screaming for help. No one seems to care. Hopeful to move our loved ones to a better place real soon. BEWARE OF THIS PLACE! Am a Nursing Manager myself in a hospital. This is no way to treat the elderly. Ms. McKinney & Family.
