Experiencia fantástica: My wife is 86 and broke her ankle. All of the staff were helpful attentive and courteous. The Therapists, and nursing staff were very helpful in giving me tips and suggestions to care for her when she was discharged and sent home. If something happened to me I would definitely choose Park Avenue Health and Rehabilitation Center.

Experiencia fantástica: Horrible,my mother stayed there for 2 weeks and cried every time I spoke with her. She mentioned how she was treated by staff, very sad to hear that Profesionals treat their patients who are there for a reason. Physical therapy was a joke for her .what's even worse was their meals. Most of the time you didn't know what you were eating so instead she wouldn't eat. There's a serious plumbing issue that hasn't been resolved, because she was told that's been an ongoing problem for quite some time.

Experiencia fantástica: My visit here was worth the most blessing in my life never met so many kind people that came in to my life. The staff motivated me and gave me the strength and courage I needed to keep going. There was days I felt like giving up because I felt weak but the staff didn't let me give up and gave me their strength to keep going. It was a blessing in disguise to come here and meet all these wonderful people. Special thanks to Melissa RN, Pami, Robyn, Tami, the student from therapy, the operations manager, Rosie, John the driver, and the weekend night nurse, one that wears glasses and the other one with a pony tail sorry I can't remember most names.

Experiencia fantástica: You guys are very good and take care of your people. Pam was a very cool lady and a hard worker. Everyone takes really good care of us! Melissa is very nice and helpful also. Eliseo, Erica, Cheryl, Tara, Robin, Mary, Connor were all amazing and so many more! I hope to not come back no offense and thank you all! And I also would like to thank Dominic someone who I call youngster. He was also very polite and generous. There’s another person I forgot to add Matt I think the all they’re so nice and generous and very helpful.

Experiencia positiva: This is a long one but I know how important information was for me when choosing the right facility. I have a parent that is paraplegic from the waist down. For awhile she had been able to live in an assisted living, however during the pandemic my mom acquired a bad bed sore forcing her to go in to a skilled facility until healed. My mom has been in her share of these facilities due to her ailments, so we had an idea what she may be in for and were trying our very best to find the best facility for her. In reading the reviews we decided to give park avenue a try. I was then put in contact with their admission representative who then immediately met us at my moms old facility and helped answer question and get things started. During admission the team made sure we were all set up and had what we needed for a smooth transition, they even continued to check in with me to see if there was anything else she needed as time went on for a few weeks after. The wound care team was amazing, and helped in the healing of her wound tremendously. There are days where patient care would fall short, a lot due to the lack of staffing available, but to be honest, is something my mom has experienced in the other facilities she's been in as well. What I found to set this place apart from the others was my ability to contact the facility with an issue with care I might have and see them trying to fix it until resolved and not just "yes-ing" me and then blowing it off. In speaking to my mom she has stated that there have been great cna's that have worked, but unfortunately end up leaving due to wage issues which then leaves her floor short staffed. Nursing staff is good. PCP and counseling is available to her when she needs it which has been great. They are so warm and welcoming to family as well and are sure to keep family informed. The food at the facility is not appetizing at all so I end up sending ready made dinner meals on days she doesn't want to eat. The rooms and parts of the facility are old and could definitely be upgraded. Overall my mom herself tells me this facility has been the best from other facilities she's been in and happy with her care.

They are knowledgeable about what each patients particular medical care needs will require and are very accommodating to most care situations. Their efficiency is unmatched in the Tucson area.

Experiencia fantástica: I wish to give a strong shout out and well done to two hard working professionals in the Social Services office at Park Ave. Health and Rehab in Tucson, AZ. A close friend of mine was a patient there for over 3 months in late 2021 and early 2022. Throughout the stay these two staff members, worked diligently to help her and me navigate the often confusing details, rules and regulations associated with Medicare and private insurance. Though they often told me that they were just doing their jobs to serve the patients under their care, I know that they regularly went above and beyond the call of duty. Through COVID 19, supply chain issues and all the restrictions of the times, they got us what we needed and steered us in the right direction. Our thanks to Becky Middleton and Veronica in Social Services for all you did for us. You are both Angels and we appreciate you so much. God Bless you both and job well done.

Experiencia fantástica: One of the cleanest places I've ever seen. Nurse's are pretty in too of things. Response time is decent. Everyone was very nice and helpful. Took care of me and my mom very well. Especially my mom. Very patient and kept her very clean.

Experiencia fantástica: I was a patient at Park Avenue Health and Rehabilitation for just over a week and the care I received there was amazing. House keeping always kept our rooms and the facility so clean! Veronica and Becky from social services where so pleasant and helped make sure I was set up for therapy and home care once I left. They also made sure I had a walker to help me be able to get around. The nursing staff that helped with showers did an amazing job helping me get clean especially Mirna. Matt, Allison, Matthew and Andrew from physical therapy and occupational therapy helped push me and motivate me to do as much as I could. The nursing staff especially Donna, Cici, Marvin, Angela, Angelique, Josh and the wound care nurse where so attentive and really cared about our well-being and did a great job keeping up on all of our needs. Even the director of nurses Jaleesa would come by to make sure I was doing ok. Michelle from activities did so good at planning and helping out with events and always made it so fun. Also I want to thank the admission team especially Erica and April for getting me in and always making sure everything was taken care of and that I was always ok.

Experiencia fantástica: I enjoyed the rehab techs a lot! I also really loved the nurses, everyone was very friendly. No complaints and the food was good. I was glad to be able to gain 2 pounds. Everyone in the facility was very polite, good to talk to and fun. I will miss everyone at the facility.

Experiencia fantástica: My dad was here for serious wound care and dialysis. Everyone involved in his care has been helpful and compassionate. There are some very intelligent and skilled people working here. The food was much better than what we had anticipated. My dad finished all of his meals which gave him the strength to heal and participate in PT and OT therapies (also an amazing team). I feel confident that if my dad needed additional health care after a hospital stay, I would choose Park Avenue Health and Rehab again.

Experiencia fantástica: Therapy was great, listened to me well, nursing was, for the most part, great, social services was outstanding, but not obviously accessable, facility maintained cleanliness throughout the stay, small details could've been more attended to. Overall, I found I was provided an environment condusive to and focused on me healing and getting stronger... you know, being well and healthy enough to go home I mean, that's the end goal right? Loved the live music on the 4th of July, daily activities (though I'm not sure they were EVERY day so don't quote me, lol), snacks & drinks upon request, and best of all, during my 5-6 week stay, I was asked 2 or 3 different times what I thought could be improved about the facility. I also noticed other obvious signs that they were constantly taking actions to try to improve what they were about.

Recently I had to be in town to have my father sign a document and a wonderful lady named Wendy came in on a Sunday because she is a notary and we needed the signature to be notarized. That is above and beyond type of service

Experiencia fantástica: A haven of support during the pandemic. Unfortunately my husband took very ill from Valley Fever in the late spring of 2020. After a stint at Banner University Hospital, he was eligible to go to rehab. The social worker gave me a list of possibilities and Park Avenue was the one I chose. From the first phone call (Donovan was my contact), I felt this rehab would be the support my husband needed as well as keep me updated as to his status. As visits were not possible during the pandemic, they were able to provide an ipad and we were able to face time for the first time in weeks. Ultimately he was gone from home almost 3 months before he was able to return. I cannot tell you the level of comfort he (and I) received with the communication from Park Ave. Although he had further complications and a return trip to Banner, the staff of Park Ave., walked me through what was going on and how to work within the Banner and Park Ave. restrictions during this time. Although I hope neither of us ever have a need again, I would not hesitate to recommend Park Ave. If they did what they did for my husband, and myself, during a pandemic, I can only imagine the support would be even better if patients can have their families support during such a stressful time.

Experiencia fantástica: This is the best nursing home I've ever been to out of 7 in my life. The food was great and there was a lot of it. The housekeeping staff was very friendly and always there to keep the room clean. The CNAs were so attentive and the nurses and managers were amazing. They made me feel warm like it was home and I don't usually nice things about places. The staff here really makes you feel like they care!

Experiencia fantástica: My mother in-law-had a fall which required nursing care and rehab. After that she needed full nursing home care. We searched the internet and found Park Ave had a wonderful review. She has been there for more than a year and a half and could not be happier. (This is a woman who said she would never leave her home!!!) The care she is receiving is excellent. The communication we have with Jordan Monson the Administrator has been superb along with the social worker, The Director of Nursing, all the dedicated nurses, Activities director and all those involved in her care. During the Covid period, which has been very tough for everyone, she has been protected as well as all patients, to the fullest degree. She enjoys her meals, her comfort, the birthday celebration last year she had and is now in a few months ready for another birthday. Did I mention that she will be 105 years old in June!!!! My mother-in-law is alive and well BECAUSE of the good care by all the good folks at Park Ave. We live in New Hampshire, she lives in Tucson, and yet we are always kept informed as to her condition. We can now rest comfortably just knowing she is in a good place and in good hands. Thanks to EVERYONE at Park Avenue for caring for our family member. You are the best. Experiencia fantástica: Upon arrival at Park Ave Rehab I was terrified, seriously injured, fresh out of spinal surgery, stressed, and in so much pain. I had no idea what to expect. I was completely caught off guard by the level of care I received; that of the highest quality. I was treated with dignity, respect, compassion, attentiveness, and professionalism by every single staff member. Besides a personalized custom treatment plan, they have been impeccable at prevention and spreading of covid -19, and every need being met, each staff member made it a point to truly establish a positive relationship with me. I was overwhelmed by how much everyone cared. They inspired me, were patient with me, challenged me, coached me, motivated me, and strengthened my faith in God and humanity, ultimately, giving me the hope I needed to not give up. Rather, to thrive. They treated me like family. I never imagined I would feel so loved. It was the catalyst I needed to recover so quickly. Within 2 weeks, I was able to walk again without assistance. They healed more than my body. They healed my heart. I will always be thankful for this place... and everybody in it.❤️

Experiencia fantástica: There comes a point in time when you realize that you may not be where you want to be, instead it’s where you need to be. From the moment I walked into Park Avenue, three years later, and counting I was, and am welcomed with open arms. Having a dedicated team, outstanding support, and most importantly the freedom to make my own decisions is what truly allows me to keep faith. The diverse culture, multiple personalities, sets Park Avenue apart from the other facilities. Although, Park Avenue is a liberalized facility, small, alternative requests are always accommodated by the kitchen, as well as other departments depending on the situation. Aside from that, bake sales, potlucks, as well as activity events bring a lot of fun, excitement, and most importantly comfort to an unusual circumstance which more often than not no one signs up for. Overall, I must say the pleasure is all mine.

Gabby is a wonderful and caring CNA she also goes above and beyond for everyone facility is very nice ..well recommended..thers just so much more great things about park Avenue rehab.

Experiencia fantástica: Its extremely hot today and when I walked through the entrance I was offered a glass of cold water and the chairs are very comfortable in the lobby. Everyone has a positive attitude. My family is happy and will be home sooner than expected.

My friend loves it there and tells me often about how amazing the care is there. I looked them up online and they are a 5 star according to medicare and recently had a perfect survey. Experiencia fantástica: Yesterday when we visited my father we noticed he wasn't acting normally, very lethargic, very weak, staring straight ahead, eyes red, rumbly cough. We alerted Nurse Dawn Ratliff and she quickly did his vitals, called the Dr. and got a chest xray. He had pneumonia. He's now on RX and oxygen, has had breathing treatments and his vitals were up this AM. Dawn is an exemplary nurse and we want to thank her for probably saving our dad's life. He's 95 and pneumonia hits that age group hard and fast.
