Experiencia fantástica: I am so glad to find a place that my dad can be comfortable with, staff more or less are bilingual which is helpful to us cause my dad can only speak Korean and he was also able to make friends there. staff was really nice and very patient with my dad has he has somewhat of a temper.

Carrollton Health & Rehabilitation Center exceeded our expectations from the moment of admission. The warm welcome extended to both our father and our family allayed our initial skepticism, facilitating a smooth transition for Dad from home to the center. The staff consistently kept us informed of any changes or updates regarding Dad's care through timely phone calls and emails. Experiencia negativa: Food is absolutely disgusting. My Mom has lost too much weight being at this facility and the slop they serve is part of the reason. Cannot get room temperature regulated. I have been asking since Summer and it’s now December. I was there today. It is still ridiculously hot in her room.The hallways and individual staff offices are cool though. Promises made and promises not kept. Call on weekends and no staff answers the phone. I can call 5 or 6 times in a row and the phone will ring until it cuts off. I can do that continuously and no one will answer. I have brought this concern to management. This Staff has no sense of urgency. No one really cares because nothing changes. Staff (not nurses)operates with a chip on their shoulders. Although I spoke to a male nurse recently who didn’t know what a fan was! It took me 10 minutes minimum to try and get him to understand what a fan/box fan is. We bought a box fan and it was missing from Mom’s room and finally found. They have lost Mom’s cellphone and wheelchair as well. Phone was FINALLY replaced after I asked numerous times. Today, her toilet is plugged and won’t be fixed until Tuesday (so they say - we will see). Not a nice aroma for the next 2 or 3 days. I complain and nothing changes! All they offer is excuses. Thank you for Annette G in business office. She is fantastic. This facility needs a total makeover!! We need staff who cares and is not afraid to make much need changes and improvements quickly. Also, it took 4 days before I found out that Mom had pneumonia. And, please spare me the obligatory “I’m sorry for the mistakes” verbiage. Actions speak louder than words.

Experiencia negativa: Place is dirty and understaffed. If you press the call button, don’t expect to see anyone for hours. Nurses don’t know what medication they are giving. I asked multiple times if they were giving my grandmother her medication and could never get a straight answer. They don’t even know what is needed for doctors appointments. Oh and if it rains, there are leaks everywhere. I’m surprised someone hasn’t fallen - specially on Sunday when it is like a ghost town.

Experiencia negativa: My Sister and I. Went to go and see my mother who has been in the hospital then she was transfered here. Don't come here. Blood on bed control, when I walked in it smelled like urine, rude employees, dirty floors,patients sitting down in a row just staring doing nothing. not very nice rooms.very loud.this place this place doesn't deserve 4stars. Never coming back here again.

The nurses are kind and helpful. Things seem to be going well. Only thing negative I see is the paint color in some rooms is dark gray. That is a little depressing but hopefully can be improved to happier colors sometime soon.We will be decorating to hide this from being so noticeable.

Experiencia negativa: As of now they have a lot of problems don't have nearly enough help and some of them act like they could care less and the food is lousy I don't see how some of the patients eat that food the rooms look nothing like what's posted on this picture but I will say this the Good nurses that they have a lot of them are leaving or have left

Experiencia negativa: DO NOT GO HERE! Luckily I was able to transfer out quickly after I arrived. Noise all night long, employees were good/bad depending on the person. I never saw the inside of the therapy room and I was across the hall from it! The place is shabby. My loved ones didn’t want to leave me there because they felt it was unsafe. Do not put anyone you care about in this facility!

Experiencia fantástica: I had a two week stay for rehabilitation. The staff, nurses and rehab technicians were all super patient, attentive and compassionate.

Experiencia fantástica: My grandfather has been sick on and off this year. He has stayed in 4 different rehab places and said this was by far the best with the most friendly staff. He still calls to check up on them :)

Experiencia positiva: My grandfather was sent here and was treated really nice by the staff and nurses. Even the therapist treated him very nicely, however, the reason why our family is giving this a two star is because there was one specific nurse, who is Korean and has short hair, was very impolite and unfriendly toward us whenever we visited or needed assistance with our grandfather. For example, she would get very impatient with us whenever we ask certain question, gets upset, mutters to herself, and sighs very loudly for us to hear because she is annoyed with us. We asked for the hospital phone number and she said she was going to give it to us, and didn't. We asked kindly if we can have the number again, however, she was like, "I already given you the number." And still did not give us the number at all. By the way she said all of this in Korean which we could understood. The other staff and nurses were very nice and friendly, however, it is because of that nurse that made our family very uncomfortable and displeased with her service. She even gives us a glare at us as if why are we even here whenever we visited our grandfather.

Experiencia fantástica: Extremely Clean Facility! I was sad because I thought I walked into the Ritz Carlton Hotel on accident. Then come to find out I was in the right place- Carrollton Health & Rehab. And the staff was equally as good as the facility.

Experiencia fantástica: The staff all away around from the nurses to doctors. From records to grievance counselor. Admin to management. They treat you like family. The are providing great care for my father-in-law, who has been there like 8 years, and when my mother-in-law was there, 2 years. When she passed recently the staff went out of their way to make my father-in-law to us the family feel comfortable and very well respected. It's a great facility with even better staff.

Experiencia fantástica: I feel welcomed at Carrollton Health and Rehab. I have lived here for a little over a year and I feel very loved by the staff. I feel like I am able to participate and converse with everyone here. The facility is clean and there is an alternate dining menu in case you aren't in the mood for what's offered that day. I receive my bath on time and always have clean clothes. I have enjoyed living at CHR!

Experiencia negativa: Please be careful when going for an interview. I live quite a distance from the facility and Monica called me about an interview, I asked if I can conduct the interview the following Monday and she said I could and she sent me the address to the facility along with her number. I drove there the Monday as discussed, and spoke to the receptionist and she gave me forms to fill out and I waited an hour. Skylar the administration spoke to me after he concluded with another interview and told me the job was filled. I asked why wasnt I contacted and everyone said they were new. Which that is a poor excuse, I called Monica and I got another sorry. If you want to work here call first to save your time.

Experiencia negativa: My dad had knee surgery and took him there for therapy. Not a good place to take your loved one. Nurses ignore calls, small servings of food, rooms are old and TVs are horrible to watch any channel. I didnt see any interaction with patients such as activities. Sad, depressing place.

Experiencia fantástica: No place is perfect but Carrollton Health and Rehab comes pretty close. I visited 3 other places before deciding on Carrollton. 2 of the 3 I did not like and the one I liked was full. I noticed Carrollton had just as good a rating as the one I liked that was full so I called Carrollton. I needed to get my mom in a facility quick because she was wasting away in the hospital for 2 weeks waiting for her insurance. She needed to start rehab ASAP so I called and left a voicemail with the director of admin, and he called me back quickly within 1-2 hours and then after another hour my mom was approved to move in that day! I visit my mom daily, some days I stay all day and the director of admin, the head of admissions and the nursing director all came in to visit my mom personally several times starting her very first day. I have personally met all of them and a lot of the nurses and staff and everyone genuinely cares about her and I feel safe having her stay there. I have called the director of admin a couple of times asking questions about insurance, etc and he always gets back with me in a timely manner and always has a positive, can- do attitude and I can tell he wants to help with any issues that arise. The communication is excellent at Carrollton rehab and is very important in helping my mom get well. I wish other skilled nursing/rehab facilities were like Carrollton. I thank them so very much for taking care of my sweet Ma. ❤️
