Experiencia negativa: Nope. Gotta tell ya folks, my mother has been in a couple of "skilled nursing facilities", after hospital stays or falls. This does NOT qualify as a skilled nursing facility. New aides, first day, NO training, (right out of their own mouths), working alone, caring for patients. Constantly being told they're short staffed. THEN QUIT TAKING IN MORE PATIENTS THAN YOU HAVE STAFF TO CARE FOR! Cleanliness is definitely NOT a priority in the rooms. We were told several times, "Oh, that's not my job". Like the other review says, RUN, don't walk from this facility. If I could leave a minus stars, I would.

Experiencia negativa: Be extra cautious allowing loved ones to stay here. On numerous occasions nursing staff administered double the dosage of medication prescribed. Luckily my mother was aware enough to refuse even after they argued with her. I was present for one interaction. Staff could be heard yelling at residents to "get dressed" or "leave me alone". My mother didn't get a shower for 4 days when she first arrived because they were "short staffed", then we found out an orderly just put in her chart that she got one but she didn't. Then they denied having told me that. Clearly records are being falsified. While visiting, I heard another patient being told to shut up after begging for a shave for hours. I could clearly tell he needed one. Then someone else lied right to him telling him he just got one so stop asking. A disgrace. We reported everything to the Director who seemed annoyed and just kept blaming things on being short staffed. It didn't look short staffed when staff were hanging out at the station talking about their "baby daddies" and where they plan to drink after work. During the month long stay bed sheets were only changed twice and personal laundry was lost weekly. Please be careful if you have no other choice than to use this facility.

Experiencia negativa: My grandmother was discharged and out 6 or 7 outfits she had there with her she had 1 pair of pants and 2 pairs of socks when we left. No bras, no underwear, no shirts. Do not recommend

Experiencia negativa: Most companies that can’t keep employees close there doors I truly believe that’s what this facility should do. Some of the help was great but many are very new and really don’t care. Very unsatisfied with the care.

Experiencia fantástica: You can not rely only on Google reviews. You will notice some facilities have most good reviews done by employees, which is a conflict of interest. You can tour any time without notice, which gave me confidence that they have nothing to hide. What is really important to consider is the facility rating with Medicare. This facility is rated all 5 stars. Medicare does inspections of facilities to make sure they are in compliance with drug, food, and safety measures, etc. They also do inspection based on complaints. Everyone we met with and all the care providers have all been very friendly, helpful, patient, and respectful. When mom pressed the call button, they were there in a very reasonable time. Even their employees say the food is very good. They eat the same as the residents/patients. Mom raves about the food. Rooms are spacious and clean. If you choose to take your loved one's laundry home to wash, you can. They have nice dining and social areas, a salon where your own hair stylist can do your hair or they provide one. They also have a physical therapy room that is well equipped and roomy. Some folks from church have rehabbed there and rave about it. I strongly recommend this facility.

Experiencia negativa: Don't just walk, run from this facility. They failed in all areas. Too much to detail, but from administration on down - not proper care or assessments, medication errors not reported, lack of documentation, no admission paperwork until one week later when they presented the Medicare discharge sheet and I informed them that there was no admission paperwork completed, no communication at all, no care conference planning/fraudulent discharge record, no accountability, loose pill on the floor in the hallway for 2+ hours (even though reported to nursing right away when observed). My loved one was in the ER less than 48 hours after the Transitional center discharged him - diagnosed with septic/UTI/pneumonia. He was at the care center for 11 days after a total knee replacement (they listed him as Independent w walker, but he had excruciating and extreme back pain, not himself at all, and was assist by one getting out of bed and chair). It was reported several times, but the Nurse Practitioner stated that it didn't warrant more testing because she saw him walk to and from the dining room with his walker that morning. They have no care or concern about someone's baseline, nor any consideration to take care of them. They have to do better for the sake of their patients and per state/federal regulations. Do not trust your loved one here unless you have time to spend 24/7.

Experiencia negativa: Bad bad bad! Not impressed with this place at all. A lot of the issues are the same as what other people are saying. The location itself is very nice, but that is where it basically ends. My 87 year old father was there for rehabilitation on a broken ankle and the staff there just don’t want to be bothered. The huge thing is staff not responding to his call button calls. Like others mentioned, it takes 45 mins or better to respond to these. And for a person who needs assistance with going to the bathroom, making a person who has to go to the restroom that long is inhumane. There is hardly anyone ever around at either the front desk or at the stations to answer questions especially on the weekends. Like others mentioned it’s like a ghost town. If by chance you are able to find someone, they have no clue what is going on or are busy with something else. For a newer facility you would think you could or would want a better staff that cares working there. The ones that work there seem to hate their job or are where the bottom of the barrel gets sent. I hate giving such a bad review, but I want to make sure other people know what they are in store for before choosing this option for their loved one to go to.

Experiencia fantástica: Was placed here from the hospital after breaking both of my arms and couldn’t move them. I was nervous but the nurses and CNAs (as well as all the other staff) made me feel so comfortable. It’s difficult to rely on others to take care of all your needs, especially at the age of 35, however I never felt like an inconvenience no matter how many times I had to hit the call button. When I did, they would come quickly. Everyone that works here is SO nice and good at what they do. Definitely would recommend trusting this place with your family members. Even though I’m excited to go home, I will actually miss the people here!

Experiencia negativa: My dad stayed here after a short hospital stay to get strength to come back home. He's more comfortable in his own clothes and it took a small act of god just to get them to finally stop dressing him in scrubs. I had a couple really great staff I met but all in all i'm just not impressed with remaining 95% of the staff I met. I know they had a lot of temp workers, but in They just weren't worth the money paid. Anytime I talked to someone about anything and almost felt like they were just being interrupted and my questions were an inconvenience. The it's not my job attitude it's very prevalent, If it wasn't for the few staff members that seem to actually remember who I was and care to check in when I'm there to see how he's feeling then I probably would have brought him home much sooner than he was ready. My suggestion stop using the temp staffing company you're using.Nobody knows what they're doing and the bare minimum is done.Again, there were a few that definitely shined, however it wasn't too hard to shine with some of the worthless employees they have. There's definitely more complaints but honestly I think with the right staff some of those complaints wouldn't exist. maybe it's a management problem?
Experiencia negativa: My mother was there for two weeks and was treated horribly. She would have to use the restroom during the night and use the call button. It took and hour for them to come help her up, then they would leave her sitting on the toilet for more than a half hour. When my mother tried to get up and get back to the bed on her because she was tired on sitting on the toilet that long they would get mad at her. She was left sitting at the edge of her bed alone at 3:00am and no one ever returned to help her back into bed. She fell twice when they were helping her walk, she got hurt both times. She hurt her tailbone the first time, and her ankle the second time. No X-rays we’re ever taken. Her ankle swelled up real bad and was all black and blue. There was an overnight nurse that was very rough with her, so much so that my mother told her “you have a bad attitude and should not be doing this kind of work”. Our family complained a few times at all of these instances, but nothing was ever done. They would not put her CPAP on her when she wanted to go back to bed. Mom was on oxygen (one level during the day, a different level for night time) they never set it high enough over night so her co2 level got very high and she became very confused. It was NOT a good experience for her!

Experiencia fantástica: My 89 year old mom just left today after being there for 5 weeks after a fall. The staff was wonderful, the meals were good. Therapy has done so much for her and she is doing better now than before she went in. I would definitely recommend this facility for any kind of rehabilitation.

Experiencia fantástica: My 89 year old mom was in the care center for 5 weeks after a fall. It is a clean facility, 3 good meals a day. They worked with her with both physical and occupational therapy to get her walking again so she could go back home. Staff was very friendly. I highly recommend to anyone that needs this type of short term care.

Experiencia negativa: Most negatives in this review can be attributed to staffing shortages, however there didn't appear to be a shortage of therapy staff, they just lacked in quality. The biggest negative is contributed by the therapy company that the transition care center contracts with, I believe it was Millineum. Their shortfallings were numerous and enormous. There was no investment in patient outcomes and no integrity. Nursing did a good job, the times when there were nursing concerns, routinely involved agency staff. Dietary did not appear to have the ability to provide a diet based on medical needs. My family members room was cleaned 1 time in 17 days.
Experiencia negativa: I wouldn't send anyone you care about to this facility. From the neglect in care to the small details of not having pillows to elevate a leg or ice packs for incision sites, this was a TERRIBLE experience.

Experiencia negativa: My father recently was transferred from an extended hospital stay to St. Luke's Transitional Care Center. Upon arrival, the staff was very unfriendly. No one welcomed us, no one offered to show us around, & no one offered us any sort of guideline about how things operated there. We were ushered directly to his room & there we sat. We tried to get him settled in, but every staff member we asked any sort of question to, it was made very clear that we were a major inconvenience to them. After a couple hours, my mother & I needed to leave due to inclement weather & an extremely long drive. We pressed his nurse call light before we left so someone could help him get in bed & lay down for a bit & to get a glass of water. It was over 20 minutes before anyone responded. We stopped at the office to let them know that dad was very dependent & someone was going to have to assist him with pretty much everything, but yet again, we were met with much resistance & attitude. We both felt VERY uncomfortable leaving him there knowing he was not going to get the care he needed, but felt we had no choice since we were unable to take him home. We attempted to call him several times after we finally got home, but he was not answering his cell phone. So we finally called the facility to make sure he was ok. The 1st time, the phone was not answered. The 2nd time, a staff member finally (& very rudely) answered & told us he had fallen out of a wheelchair & was going to be taken to the hospital by ambulance. Dad ended up with 7 stitches in his face from the fall. No one from the facility ever called to see how he was. We finally went back out there to collect his belongings because he did not want to return to the facility. It was like a ghost town. We were almost to his room before we ever saw a staff member, who told us we had to walk back to the front desk to get our temperatures taken. He was able to get a charge nurse, who we tried to address our concerns with, but she was just more interested in just having us sign the paper saying we came to collect his belongings & told us any concerns would have to be addressed with a "higher up" during the week. We attempted to call on Monday to discuss the situation with a "higher up", but already knew we were not going to get anywhere, so we asked for a copy of the accident report, which was denied to us stating she "didn't feel it was necessary". The building, itself, is a beautiful facility that has a lot of potential! And I totally understand the staffing shortage happening right now!! But if you don't have enough staff to take care of the current patients, than don't accept any new ones in & treat everyone poorly!!
Experiencia positiva: The good: My mom got GREAT care here after her hospital stay. The nurses, therapists and techs are great. The physical therapy she received was wonderful. The rooms, bathrooms and showers are large and clean. The not so good: Be aware that there are NO telephones in the rooms and there's not always someone available to relay calls and provide a phone for patients. After 5:oo there is no one at the front desk to let you in and I had several long waits to get in when visiting after work. I ate a few meals with Mom during her stay ($8.00) and the food was very basic, hard grilled cheese, hamburger or frozen pizza. Suggest to bring toilet paper from home :) ***Not the staff's fault, they were excellent. ***
