Experiencia negativa: Update: after the matters were brought to the facilities attention nothing was fixed or even acknowledged. Brittany responds with call this number and we'll fix the issues. I shouldn't have any issues first of all if the rehabilitation was run properly. I decided to transfer my mom out of chandler acute to another short term facility. My advice do not let them talk you into bringing your loved one into their facility for recovery. My mom was not treated well, she was left unattended in the bathroom hanging on to the handicap bar when she was suppose to be supervised. Secondly my mom arrived Monday didn't shower at all even when it was her day. Their reasoning was they didn't have enough cna's on the floor but yet a rn couldn't be bothered to be a team player and give a helping hand. The Food the food is horrible, the food looks like what you'd serve someone in prison. No matter how many times I expressed the diet my mom is on and foods/drinks she can't have they still continued to ignore the modifications I made and gave her the opposite of what she can't have, my mom was roommate with another patient who would lay in her soil brief 8 hours of the day I know this because I visited my mom everyday and had to face a can come in to change the roommates brief. This is no exaggeration. Housekeepers do not go in your room daily to sanitize sweep or mop patients rooms. That alone is very unsanitary. Receptionist tabby rude doesn't say hi doesn't say welcome to so and so place. If you like to have the ability to reach out to your loved ones after hours by speaking to a nurse you won't be able to, it's nerve wrecking not able to call the nurses station without interrupting your loved ones rest or therapy. The facility offers over night stay but it's impossible when your loved one is sharing a room. This facility is not updated it's chaotic Please take what I said to mind and do right by your loved ones when looking for a short or long term rehabilitation. Experiencia negativa: The employees at this facility don't give a crap about their patients. My mother is there for rehab after back surgery. She needs help getting out of her bed to get to the restroom. The nurses ignore the call button for EXTENDED periods of time to where she will urinates in the bed and has to lay in it until they choose to show up. The other patient in my mom's room spilled lemonade on her self and all over her bed. Her call button was pressed by her daughter. After an hour her daughter left without anyone checking on her. When I left 45 minutes after that, I stopped by the nurses station and mentioned it to them. They were snitty and said yes, her daughter already came by, we are aware, we will get to it. They didn't care at all, they new the call button had been pressed almost 2 hrs prior and they just were not going to be bothered. KEEP YOUR LOVED ONES FAR FAR AWAY FROM THIS PLACE.

Experiencia negativa: I would give zero stars for this horrible place if I could. My husband was sent there after a foot surgery for IV antibiotics, due to dementia he needed to be watched to keep him from pulling out the PICC line and to keep him off his foot. He was there 8 days and pulled his picc line out 4 times and his urinary catheter out at least once that we know of, and was allowed to wonder around without anyone looking after him. My husband went into kidney failure on the 8th day and was sent to to hospital and unfortunately passed away 10 days later. Nursing care was totally not there, we found him when visiting one day curled up on his bed in fetal position on a bare mattress any only in a brief we inquired as to when he was last bathed and no one knew. The room he was in was so small his bed was shoved up against the wall ac unit and a wheelchair could not fit between the 2 beds, I think this room was a converted closet. We took him 3 new sets of clothes and when I picked up his belongings there was only one pair of shorts the rest were missing, he was sent to the hospital in a pair of sweats that were not his, he was dirty and the dressing on his foot was black, I was so embarrassed for him. Please if you have a choice do not send your loved ones here.

Experiencia fantástica: My mom was at Chandler Post Acute for two weeks as she recovered from serious stomach ulcer bleeding. We were very nervous about sending her to a rehabilitation center due to all of the horror stories we had heard. I am so happy to say that Chandler Post Acute blew away our expectations. They were kind, organized, thoughtful, professional, and we left grateful that she had been there. Before she left, Mom started to make a list of all of the employees who had been friendly and helpful. The list was so long she thought she might try just making a list of those who were not helpful and kind. Turns out there wouldn't be anyone on that list. For anyone who is looking for a reliable rehabilitation center, based on our experience, we highly recommend Chandler Post Acute and Rehabilitation.

Experiencia fantástica: REGISTERED NURSE NHI is the BEST nurse I have ever had since 2020, she cares about her many patients everyday like they are her own family, which includes me, she shows her love, support and encouragement not only to her patients, but to the patients families as well, she goes above and beyond for her patients, if I could give her a award and a gift card to wherever she wanted to, I would, but most of all, she has experience and is very professional about her work, so if you or your loved one comes over here, request REGISTERED NURSE NHI, I also recommend the Facility continually support her and award her as employee of the year.

Experiencia fantástica: Registered Nurse Nhi is very compassionate for those in care. Especially how she dealt with my care. When I was in pain, she would be concerned and ask if I ok or needed meds. Plus she was very patient and dependable. Even if f I needed essential just like water, ice pack, meds or help she was there. She's a hard worker and she would make sure I was taken care of and got what I needed. Around the clock she was asking how I was doing and just checking in on my leg pain. Her job requires so much and she does a great job at what needs to be done. I hold her at high regards of her position because her job skills reflects her heart. She recognizes the people and knows how to do her job, but does her job with love, compassion, patience and care.

Experiencia negativa: I had to rescue my husband AMA from this horrible facility. I found him on the toilet naked,shivering, and without his Oxygen. I m fortunate he didn’t have a heart attack or stroke!!!

Experiencia negativa: My mom is there right now. She's 86, and is recovering from two fractured discs in her spine after a fall. She needs help getting up, which is why she's there. The last two nights she has waited 45 minutes and ONE AND A HALF HOURS for someone to come help her to the bathroom after pressing her help button. Last night she finally managed to get up on her own after laying in pee and screaming for help. She could see and hear the staff at the desk talking and laughing, standing around ignoring her. She could have fallen. My sister talked to the head nurse yesterday and my mom was treated worse last night. My other sister tried calling the director 4 times, hasn't been able to reach them yet. -Zero stars for the night staff. -One star for the food that gave her and her roommate food poisoning, which days later is still not resolved. -Four stars for the daytime nursing staff. Daytime students are great, too. -Five stars for PT. UPDATE: After posting this review, my sister was able to speak with someone in charge that said they are on top of getting the lack of care at night resolved. Hopefully my mom will receive more timely assistance getting to the bathroom tonight and for the rest of her stay. I will update this review once we see how it goes. I'm glad we were at least able to get ahold of someone that could address the problem.

Experiencia negativa: This facility is a conundrum - the staff who deal with the patients are wonderful. Very nice and professional. They were outstanding. But something is not right - at 2:18am, my wife was awakened by a very loud rough voice talking outside her room. After it finally stopped, she closed her eyes to try to go to sleep and after few moments, heard her curtain open and opened her eyes to a man in a wheelchair sitting next to her bed staring at her. They did move her to another room later, but was given her roommate's medication instead of her own at one point. The food was horrible - never even warm when it arrived. There is potential for this facility, but there need to be some major changes.

Experiencia negativa: I recommend any other place go if you had recent surgery, are really sick or need pain meds unless you can get up to help yourself. This is a long term facility not fully staffed for skilled care such as 1 to 2 1/2 hours wait for call response. Staff are well hired and nice but the nursing mgmt culture seems to ignore calls because patients are mixed from dementia to surgical. There is a huge difference in care needs and CNAs can’t keep up. Get the number of an administrator before entering. You may need their help. I was sending a photo of a dirty bathroom which got housekeeping. Bed light was out till my last day. I went there for PT and that staff is very good. OT is oriented to long term elderly not surgical. Balloon games won’t help a new knee. Male staff respond the soonest. Needing night meds is tough because they aren’t prepared for non routine pain management. If pill is due at 2am… you can wait till nurse is free regardless of need. No special diets and food is well plated but always very cold.

Experiencia negativa: Most of the staff I experienced was exceptional - Randy RN & Mary RN. Cristabel, Shanna and Juanita were fabulous. There were a few others that were good. It was very disappointing when Case Manager Theresa dropped me like a hot potato because the hospital was no longer covering days for me to be there. I was released with no additional PT or OT, all the things she said she would help me with, she did NONE OF THEM. Her entire demeanor changed when I had no insurance. They couldn't wait to get me out. BTW, I did get insurance coverage back dated to march 1st. If Theresa would have taken more time perhaps the facility could have gotten paid more.

Experiencia negativa: My Father was admitted here yesterday, February 16th 2024. He is on fall risk protocol because of suffering a stroke. We were informed that he is only to get out of bed with staff assistance. The initial time that he had to use the restroom we pressed the nurse call button and waited for help. Finally after 15 minutes a male staff member in blue scrubs arrived, we told him my Father needed to use the restroom. He turned and immediately left the room. After another ten minutes I took him to the restroom. He had urinated on himself and was very agitated. He's a veteran and a proud man, you can imagine how he felt. Later I was attempting to leave the facility and was locked in. I located a nurse and asked for help. You would have thought I asked for her first born child. She told me I would have to ring the bell at the exit for help. I did... repeatedly. No one came, or buzzed me out. Next, I searched for a staff member. They were all busy and would help me in 10-15 minutes. Finally, I found someone to let me out... First impressions go a long way. Sadly to say my initial interaction with Chandler Post Acute and Rehabilitation was not a positive one. PS. I moved my Father to a different rehab facility as soon as humanly possible. DON'T under any circumstances place your loved one in this incompetent place. The difference is day and night between Chandler Post Acute and his new placement. At his new rehab when he presses the nurse call button, help arrives in minutes. Unlike an hour to three hours at Chandler Post Acute and Rehabilitation. Even after posting this review NO one from the facility contacted me about my concerns. They don't care, this place is all about housing patients and collecting the insurance revenue. Patients stood in the hallway pleading and begging for help, many in severe pain, staff passed them by. I found one CNA, Tonya that cared. I would by pass the entire staff and go to her if my Father needed assistance. I actually had ask a member of house keeping to get clean bedding, I'm sorry I don't remember his name. The girl at the front desk is a real winner. When you enter through the front door she doesn't even acknowledge your presence. She doesn't look up from her phone or make eye contact with you. To be fair, my Father loved his meals, they were served promptly and arrived warm. If you are given the option of placing a family member here, decline it. You'll save yourself a monumental headache.

Experiencia negativa: I was there November 2023 for about a week. 1 . It looked like they needed more personal. 2. Not all of your questions were answered. 3. Food was okay. Sometimes hot other times warm. 4. Did not have a diabetic diet

Experiencia positiva: My father recently (October 2023) spent a few days at this facility until he was transferred to another facility. Overall, I give this facility a 3 stars out of 5 and I'll list my reasons. I did spend a significant amount of time there, so I hope this helps you. 1. All patients must share a room with another patient - the room divided by a curtain. Each side has a TV so this can be a big problem as it was for my father. 2. The nurses all seem to be pleasant but understaffed. My father would have to wait a long time when pressed the button for help. Sometimes, the help would arrive 30 minutes or more after the button was pushed - sometimes, right away. 3. The food was pretty good, but often served cold. Again, too many patients and not enough help. 4. I liked the nurses, but other staff members were quite unfriendly. The women (young) who staffed the front desk were especially unfriendly. 5. Overall, I do not recommend this place. It is not good, but may be good enough for some circumstances. I was quite a relief when we were able to get my father transferred to Dignity East San Tan (5 out of 5 stars!). I hope this honest review helps you decide what to do.

Experiencia negativa: Do not bring a loved one to this place! My 80 year mother had fractured her shoulder and was sent there from the hospital. Her experience has been horrible from the beginning, it's a very loud and crazy place, trying to get a nurse to help is almost impossible, the rooms are dirty and so small I didn't have a place to sit, the food is horrible "I've had to bring her something to eat, since the food was something nobody could eat" actually it's hard to tell what it is, they weren't even sure. She has personal things missing a lot even after 3 days. Seriously it's like a loud psyche ward. It's horrible!!! go see different places first, not this one! She really needed a quiet place that would help here.

Experiencia negativa: Wish there were negative stars to award - I was there a couple of years ago and think it should be renamed the "HOTEL FROM HELL." Nothing worked, understaffed and filthy. I can't imagine how they keep a license for care of anyone for anything.

Experiencia negativa: Not enough of pee pads, walkers, nurses/short staff ratio to any patients. Water/Ice machine cruddy build up, no single rooms, it's always the other shift staff! For the money they charge they should always have enough for everyone coming in. The website is a joke!

Experiencia negativa: My father was transferred to Chandler Post Acute earlier this year (April 2022) for some rehabilitation following a month long hospital stay. To say that it was disappointing would be a major understatement, I am angry about the time that my father needed to spend there. Just to name a few of the things that went significantly wrong: 1. My father has parkinsons disease --- he is required to take medication 4 times per day to keep him alive. When he was first transferred there, they went an entire day without giving him medication as they could not find it and didn't alert anyone of this gap. When I was visiting my father, i asked one of the caregivers just to be 100% sure they had enough of the medication ( i would never have thought they just wouldn't be giving it to him) when they said they didn't have any. After i got a bit frustrated by that, they finally did some further digging and found the medication in another patients bin or whatever they store the medication in. Had i not happened to ask, I'm not sure how long they would have just continued to not give him his meds 2. The hospital made it very clear that it was important that my fathers mouth stay clean as he was high risk for pneumonia, i asked the caregiver about this and she mentioned that she had not been able to brush his teeth that day, but had the day before..... I then looked at the toothbrush and toothpaste and it was completely sealed so clearly nobody had brushed his teeth and once again i had to lean on a caregiver to do it 3. Everytime i went to visit my father he was so thirsty and desperate for water, clearly the caregivers were not taking the time to put the straw up to his mouth so he could drink 4. The shared rooms were so obnoxiously noisy and did not seem clean at all, it was a really grim state. My father laid in his bed while the person he shared a room with blasted the TV on his side (not where we could even see it) on full volume so he could not rest --- it was also very hot in that room Given all of these items, i had my father transferred out of there as soon as humanly possible, but i deeply regret the 4 or 5 days that he had to spend there. I understand that for budgetary reasons these facilities can't be perfect and the level of care isn't going to be amazing, but where we had him transferred (Citadel Post Acute) was 1000% better and I would caution anyone against bringing a loved one here.

Experiencia negativa: Office staff could care less about you as a patient!! They never communicate with pt… smile to your face then forgot all about what you needed!! The aides & nurses… most of them were kind & giving! BUT well understaffed!

Experiencia negativa: Deb LPn is the only staff member I would trust. They violate HIPPA constantly, they are unygienic, lets just say I would never take a family there ! EVER. One staffer violates HIPPA and she is in Administrative work. Be careful, and choose a different rehab please. SO if you want to be a joke to the staff, but all means go there.

Experiencia negativa: Zero stars. My dad was here for 20 days and was finally released with horrendous bed sores. During his stay he never appeared to be cleaned/bathed, and was kept in a ‘storage closet’ of a room with a problem roommate. He is now back in the hospital with a serious case of a UTI. I would not recommend for family members who need active care.

Experiencia fantástica: I don’t drop these 5 stars lightly but Chandler Post Acute has earned them. I was transferred here from another SNF, and the entire experience was on a whole new level. I came in with the specific objective of being properly discharged and everyone from Admissions(Allison), Case Management(Teresa the Rock Star), Nursing, Dr. Uy, OT and PT were all aligned to help me reach my goals. Don’t get me wrong, there are some things that should be different, but the entire industry of Skilled Nursing Facilities in my opinion is broken. Patient to Nurse/CNA ratios are out of control. Call response times suffer as a result, but Chandler staff were amazing and patient with everyone, even with some patients who were downright abusive towards the staff members. The wound care team of Haley’s and Jennifer were outstanding and caring. My nurses Krystin, Kathy, Monica, Monica and Chandani were all very professional and supportive. If you are looking for a clean, well maintained and comfortable facility for yourself or a family member, I would not hesitate to recommend Chandler Post Acute. I just spent my 2nd Fathers Day in either a hospital or a SNF, and it was actually a good source of memories and enjoyment. Thanks to everyone for getting me better and back home to my chocolate lab and my own bed!

Experiencia negativa: I've seen soo many bad one star reviews of this place, my grandmother was here in 2017, she would call us crying to tell us that her call button was on she would wait soo long for help off her commode (she was an amputee) Do Not Send Your Relatives here. My mom was here for only 1 day, she fell off her bed because they apparently can't have railings on their beds or fall mats. Reading the other reviews she probably didn't get aid right away like she should have, she passed away. ONLY ONE DAY OF BEING AT YOUR FACILITY N SHE DIED?!?! I will NEVER forgive your facility for letting this happen to my mom. She passed away in June 2021. & on top of that my sister n I came for her things n half of important documents that she had in her room where thrown away!

Experiencia fantástica: Amazing. Unreal. I can’t believe how good this place has been. They have helped my family so much. From the admission process, all the way to discharge with the case with the case managers. They were there to help every step of the way.

Experiencia negativa: This place is a disgusting excuse for a “care” center. Already posted on Yelp and will continue to post negative reviews anywhere I can, with the hope that the reviews will prevent others from bringing their loved ones to this horrific place. During the time my grandma was here with a broken hip, the facility lost her dentures, tried to transport her in a van to a doctors appointment after we explicitly told them she was not to be transported, fed her garbage food, did not keep her clean, did not check on her, she fell additional times because staff did not monitor her and did not respond to the call buttons, and the list goes on. The day they transported her, I had to call the transport van to get them to turn around. When they showed up, I found her sitting upright, strapped in a wheelchair, SCREAMING in pain and dry heaving from the pain. It was absolutely horrible to see. This was a frail, 98 year old woman, who refused hip surgery and just wanted to live her final days without pain and in peace. This facility denied her that, every step of the way. Luckily we were able to move her to a different facility where she actually was taken care of. I hope Chandler Post Acute is shut down and that every senior staff member in the place rots for how they treat their patients.

Maintanence, dietary, nurses, cnas and case manager were all great team as part of my progress here and i would 100% recommendthis place or will return if i ever need more rehab Thanks chandler post acute team for such a great expirience!!

Experiencia fantástica: Just have to send all my love and respect to hailley and Jennifer the wound doctors cuz of the care they gave me I'm doing so much more like walking reaching holding anything with my hands. Also thank u Juan and issec for helping me and looking up to me Browne and proud! Also to all staff that workt withe me thank u for not taking my no I don't want to walk for answer. Thank u Lisa the kitchen worker for my meals and fixing my coffee just the way I like. I hade a helpfully and pecefull stay and positive stay even when I was a lil overreacting. I'm not Heald all the way but u Shure got me more than half way. Thank u for not discharging me without a place I can continue my recovery. And all my long-term friends missing u all (I God is good God is great) thank u Chandler post acute rehabilitation for keeping me safe and well.

Experiencia negativa: My dad ended up there after being in the hospital and they never did his physical therapy like they were supposed to and wouldn’t return our phone calls. We got him out of there as soon as possible and found a better place.

Experiencia negativa: A friend who has TERMINAL METISTATIC BRAIN CANCER, as well as mental deficits was told he needed to leave. With no place to go. He was sent to the curb! He was told many unkind and untrue things, he was belittled, and made to feel less than human at a time he has less than 6 month to live. HOW is this allowed with a "CARE" facility? I am going to be requesting an investigation with the State of AZ re: patient dumping. I work in the medical field in the Great State of Texas. If my friend were only comfortable and able to travel to Texas he would receive the care he deserves here, not be thrown to the sidewalk like trash. So truly disgusted and disappointed in a facility designed to take care of those in need. If you care about your loved ones, please find somewhere else to have them cared for. Experience shows that Chandler Pose Acute and Rehabilitation does little rehab and no care for their patients, your loved ones. Disgusted, disappointed and frightened for all patients, especially those who have no one to speak up for them. *** UPDATE *** MY FRIEND RECENTLY DIED. I AM SURE THAT CHANDLER POST ACUTE AND REHABILITATION COULD CARE LESS BUT MY FRIEND WAS TOLD THAT HE DIDN'T NEED CARE, AND AS HE WAS "MENTALLY CHALLENGED" BELIEVED THAT MEANT HIS CANCER WAS GONE. HE FELT HE DIDN'T NEED ANY MORE CHEMO OR CARE AS THESE USELESS, HEARLESS, UNCARING, SOULESS MONEYGRUBBINGWHORES TOLD HIM HE WAS GOOD. I DIRECTLY HOLD THEM RESPONSIBLE FOR HIS EARLIER THAN NEEDED DEATH. YES, HIS CANCER WAS TERMINAL, BUT HE COULD HAVE HAD TREATMENT IF ONLY HE WAS WHERE HE WOULD HAVE RECEIVED CARE AND COMPASSION. STILL SO DISGUSTED WITH THIS "PLACE"

Experiencia negativa: Today I get a call from my parishioner that he had diarrhea made a mess in the bed they came and changed it asked him to sit in the chair, that was three hours ago.....they still haven't made the bed...and he is still sitting in the chair...Now tell me does this seem like appropriate behavior for a "care" facility, I can see because of the work load, a half hour maybe even an hour is excusable but 3?? You have serious serious problems at this facility, maybe you should get some volunteers to help?

Experiencia negativa: I completely agree with the last review. My friend is in that terrible place. The walls are literally falling apart. My friend asked to use the rest room and it took 2 hours.. I know you have alot of patients. We trust you so please take care of the patients.

Experiencia positiva: My husband is there. The PT seems to be pretty good. It's too soon to know. Be nice if someone was at the front desk as entry is restricted. Haven't talked with nurse or doctor about the plan.as of yet.

Experiencia negativa: As of today Saturday the 28th the building has electrical problems. Staff is no records they can access. They're walking around taking vitals with blood pressure wrist bands, old fashioned temperature reading devices, and oxygen readers that are not connected to anything. Breakfast today consisted of a few cold scrambled eggs, a piece of bread that they called toast. Milk for the cold oatmeal some jelly and some butter. Not a pretty meal. The skilled nursing facility does not seem to understand that diabetics need to eat on a regular time schedule or could result in additional hospitalizations. They do not answer the call button, sometimes for hours at a time. I arrived Thursday around 4:00 and never received anything to eat. My blood sugar was out of control. This morning when the nurse passed meds they didn't seem to be knowledgeable of what meds they were giving and why. There are things added that I'd never taken before like Prozac. Same was true for my roommate. I have no idea what meds she takes it's none of my business but she questioned them as well. I would not recommend this skilled nursing facility. I know things change and there are lots of patients but they're also lots of empty beds in 120 room facility. Blood work which is normally taken around 5:00 6:00 a.m. was not taken today.
