Experiencia fantástica: I am impressed on their discharge process. The Social Workers & Case Manager follow through until after discharge. They make sure that the patient and family has support at home, making sure that patients are safe with their loved ones or caregiver at their home to try prevent patients to bounce back to the hospital. The SWs & CM have constant communication with the family/caregiver during the discharge process to ensure that patient/family have what they need at their home. I highly recommend Grossmont Post Acute Care

Experiencia fantástica: I don't even know where to begin. But my Mother had a massive stroke 6 years ago and when the hospital was ready to discharge her they gave us 2 hours notice. My brother and I scrambled to go look at options. My brother and I walked into GPAC at 8:30 at night (long after regular hours) and the kindest person in the world showed us around. After talking to the administrator the next morning we knew THIS was the place to take our mother. Sadly my mom did not benefit from the physical or occupational therepy (the effects of her stroke were just too great) and my Mom became a resident (I don't know if that's what it's called) but as of Janury 2024 she has been there for 6 years. GPAC is not perfect, but the facility is clean and the people are fantastic. They take such good care of mom and are so kind and caring. I tell my friends that if I can't have her at home with us then I'm able to sleep at night knowing she is at GPAC. The facily is not new (although they are buidling a new building across the street that they will move into in December 2024) but it's SO CLEAN, smells nice and they have staff that provides activities for the short term and long term patients (see photos of recent car show!) Although I compliment the entire staff--there are more than a few staff members who are nothing less than 'Angels on Earth' that have take extra special care of mom and we will forever be grateful to them for how they have cared for our mom.

Experiencia fantástica: The care and caring provided for my Mom by the staff here is wonderful. I've visited her at least 6 times per week for the past 2 years. So I have a lot of observations to base this review on. Staff members stop by numerous times per day checking up on her. They're always happy and helpful, although I know that they're working HARD. And that's not easy. Mom gets plenty of conversation and hugs. And the few times when Mom's gotten sick and I noticed it before they did, they were extremely quick to respond and take appropriate action. The atmosphere is one of healing and optimism which is much appreciated.

Experiencia fantástica: Six weeks ago I fell, which resulted in a compound fracture of my ankle. After surgery and a few days in the hospital, I was transfered to Grossmont Acute Care. Since it would be 8-10 weeks before I could put weight on my broken ankle, I needed a lot of care and intense Physical Therapy. Of course I was scared, but soon I was reassured and challenged to work past my fears and do the work. I have nothing but positive things to say about the entire staff at this facility. I have grown to love them and consider them my angels. Always respectful, helpful, encouraging, funny, well trained, informative, and extremely skilled in each of their positions on staff. They turned a very difficult time in my life into a joyful and learning experience. I have grown as a person from this experience and would recommend this facility for rehab and PT. I hope I'm never in a situation where I need this again, but if I do, this will be my first choice.

Experiencia fantástica: A wonderful place to help my mother-in-law during her physical rehabilitation journey. All of the staff is courteous and on top of things. The occupational physical and speech therapists are on top of their game. They treat patients graciously and yet push them to do better and succeed. They made the transition easy for our family, my wife especially seeing her mom go through this difficult time in her life. The staff made us feel welcomed and brought comfort,and peace of mind knowing that they were giving her the best care. Greatly recommended for those in need of rehabilitation ????
