Experiencia negativa: Don’t bring your loved ones here. This place needs to be reported and shut down. Negligent, unclean, uncaring staff for residents, the list goes on. Definitely looking into reporting this facility and administration for the lack of care my loved one receives. That reply seems so automated. How can you investigate and you’ve asked no questions? You all clearly don’t care about the safety and well being of these patients and that’s the only thing thats clear. Shame on you for that response knowing you all aren’t trying to really help these people and actively neglecting them. Just remember, one day it may be you in need.

Experiencia fantástica: My mom brought Tommy who is mentally challenged to AHC Clarksville several years ago as his medical needs exceeded the care she could provide for him at home. The staff have cared for him like family ever since. He's now 75 and it looks like he's probably going to outlive us all. They have good food served each day, take care of his laundry, bathe him when he lets them lol, keeps his haircut nice, and their communication with us is outstanding. We are so grateful to have found AHC and would encourage others who are seeking this kind of care to contact them.
Experiencia negativa: AHC is such a failure and disappointment in the care it provides. Not to put every employee at fault because Ms. Lilly, Breonna, and the front desk receptionist with the twist lock hair was always helpful and polite and I didn’t experience every employee personally. So I hold the administrator responsible for the operation lack of care. I spoke with the administrator twice about my concerns so once you’re aware I can hold you accountable, example of failure.. no shower given in 10 days, dietary restrictions still given, proper medication not administered, occupational and physical therapy not provided regularly. If I didn’t show up every morning to assist the patient dialysis appointments would have been missed. Over flowing toilet with blankets placed to soak up the mess. Blankets removed but floors not mopped immediately . The hallway smell of urine most days. Initially I thought ok the administrator may not be aware of the lack of service thinking that she has her hands full so I spoke with her to shed some light on the situation but after reading the reviews the facility is totally aware and it’s sad that it continues. This facility was used for a two week therapy program to help the patient adjust after a long hospital stay. If you’re using AHC please check on your love on regularly, chances are they’re not.

Experiencia negativa: DO NOT put your loved one in this facility. They do not clean rooms or bathrooms. Had another picture of the bathroom but I will not post it. On our first visit when we arrived our mother was dirty and said she asked repeatedly for someone to come help her but no one came until we arrived and questioned what was happening. While we were there they also failed to give our mother her lunch at/around 12:00 we asked at 2 pm and they said “looks like we forgot.” They began to blame it on her lunch was usually on top of cart but someone didn’t put it out??? They could’ve cared less. This facility should be looked into and shut down. We pulled our mother out of this facility as soon as all this happened. I do not want to hear from AHC, I am simply notifying review readers before you make the mistake of putting your loved one here.

Experiencia negativa: A loved one was sent to this facility for rehab as he had no insurance and they had an open bed. This place is the exact sort of place that gives lawyers business for nursing home neglect. My person was transferred there on a Thursday afternoon. I visited on Saturday morning. As I was walking to his room (I had a wagon with a rollator in a box that I was planning on assembling there), I was basically stopped by the nursing station (all just up there chatting and enjoying themselves) asking if I was making a delivery. I said yes and they started to stop me, and I told them I was going visit too, and they stopped talking to me. (It was a strange and unexpected interaction.) When I arrived in his room, first thing I noticed was it was extremely warm. The ac unit in the room apparently was not working. My friend said they would not respond to the nurses call button. He took to calling them on his cell phone when he needed something. Still if they responded it took quite a while. When they came in to help in to the toilet, their tone and words were just really disrespectful..absolutely no dignity given to the people in their care. But all of this could even be worked with…it was after all free to him since he had no insurance. What was unforgivable was the next day (Sunday) my friend complained of not being able to breathe. He was able to text me and they sent him to the hospital down the street (a whole other horror story). No one from here called me as his point of contact. He returned back to the facility and once again was having trouble breathing on Tuesday. He was finally taken back to the hospital down the street in respiratory distress. The hospital transferred him to a hospital in Nashville on that Tuesday. He was admitted to the ER there and immediately intubated and put on a ventilator. Once again, no one called me as his point of contact. I did not find this information out until I received a call from Palliative Care Services at the hospital on Wednesday afternoon. He passed away a week later. When I called to make arrangements to retrieve his personal items, I was asked to leave my name and number and someone would call me back. No one called me. I called back and basically said I am coming for his things. I will pack them when I get there. Someone finally called me back, when pressed for why I did not get a call when he went to the hospital (I really was trying to give them the benefit of the doubt) they said I should have been called and they were sorry. Then the administrator agreed to meet me there to retrieve his things (they would pack them because they had some coming in to the room on Friday) . She even gave me her cell number to text when I was on my way. I did so, and she was not there when I showed up. She asked someone with a key to retrieve his things from her office. Seems no one wanted to talk with me about their shortcomings. If you have any choice in the matter, DO NOT SEND YOUR LOVED ONES TO THIS FACILITY!

Experiencia negativa: This place needs the TN State nursing home regulatory board to shut them down!! If your people are there contact insurance and request change to other place immediately. My father was sent from hospital for tx after a broken hip. He fell out of the bed and yelled for help for over an hour. No one came to check on him. Another patient across the hall got out of bed, into her wheelchair, and went to get him help in the night. They never sent him back for Xray to check his hip surgery hardware. They never informed us of any accident he incurred while under their care. I spoke with Tom Sawyer NP he was never informed of accident. After I discussed all the mistreatment of patients he stated "this is not acceptable and neglect to patient". At that point, he brought down an RN and Janine. I gave them a list of issues we were having. They took notes and I asked for a copy the RN stated she would have to transfer them into her computer. She would give me a copy on Tuesday. That never happen. My father was there for 59 days. SAWYER NURSE PRACTIONER only seen him 1 time. I complained he did not receive a bath for 12 days. What he received was a cold shower at 1am in the morning by Destiny. Father had a broken hip had not walked in 6 weeks was told by Emmanuel (CNA) to walk back to bed. Dad fell, cut arm, hurt shoulder and heavily bruised. Ewmmanuel never reported the truth. He told Fellie LPN dad only scraped his arm. During the fall he hurt his shoulder and needed tx. Did not receive anything until 6 days later. An RN came in with her dog, I explained about the fall, and had requested SAWYER to do a shoulder X-ray (Florida LPN sent him the message). Finally, the RN got the x-ray so we could rule out a fracture in his shoulder!! While under AHC care my father's health declined immensely. We have moved him and he is getting excellent care at Spring Meadows. We are seeing a huge improvement in his health and receiving the care he deserves. I filed formal complaints to state, ombudsman, and insurance Thank God the insurance stepped in and allowed a transfer. All the CNA's, some of the LPNs, and patient care management should be reprimanded. This place is horrendous! I pray for all the patients there, especially the ones that have no one to fight for the treatment they deserve. I will not stop with this rating. The word needs to be spread on this establishment. Stop taking any patients until you learn to take care of the ones you have!!
Experiencia negativa: My mother is in the facility!!!!! Absolutely the Worst ever! First off, the speech therapist comes in my moms room and tells her she is going to die. Who tells someone that She isn’t there for that! She is there to walk!!! Then give medication that’s not needed, can’t even tell you who wrote the order! Dropped my mom, never said 1 word about it, my mom told me. Luckily she wasn’t hurt. When food trays are given, the food is so gross and very small portions, not enough for a child to survive. They are given about 10 minutes to eat, and the trays are picked up. My mom was there 2 weeks before she got a shower!!! We were told diaper changer happen at 10am and 4pm!!! Wow, so you lay in urine and poop in the mean time. Everything you need there is a waiting game, no need to push the call button, they sit up there and talk and cut up while people needing assistance. I strongly do not recommend you putting a loved one in here!!! Therapy is a JOKE too. Their therapy consists of sitting in a chair! Place is a JOKE. I highly recommend you check on your people daily if they must go here. And also no one ever answers the phone up front, sit there and let it ring!

Experiencia negativa: The nurse practitioner tom Sawyer is a very rude person with a god like complex that's going to cause a lot of people to be harmed .the a lot of the CNA staff and 3 of the nurses I dealt with are remarkable .Dr white ,the administrator, social worker and the assistant social worker along with other staff are crooks talk about elder abuse I WAS ABUSED IN THAT PLACE ! God knows I need a lawyer

They don't change them nor feed them food is nasty and they don't wash their hands clients fall all the time management is horrible and only care for self will over work you if you let them old milk giving don't care about the diet of the people rude to them as well.i feel sorry for the people that has to be there.

Experiencia negativa: I hate to give this place even 1 star I was sent there after having bilateral knee replacement. The first night I was there thy did not feed me. Every time I put the call light on it took at least 2 hours before anyone would come. In 8 day never got a bath my wife did for me. I could not even get a bed pan it took my wife 3 days to get me one.the last day I was there they did not give me a lunch. The rehabilitation is the only reason I gave them 1 star they rehab workers was great. Every thing that got done for me at AHC my wife did for me they did nothing.

Experiencia negativa: I hate to give even 1 star. My husband was there for rehab. He couldn't get up or reach things. He was suppose to be on his own doctors prescribed meds. Instead, they chose what to give, one of which was to drug him. One day I couldn't wake him all day long.He told one person he had to have a bm and she told him he'd have to go when they are in the hallway!! The food was absolutely horrible and was always brought very late. The only praise I can give is to the PT crew. My husband ended up in the ER a week later and was there over a week. I brought him home with hospice and lost the love of my life a week later. PLEASE DON'T TAKE YOUR LOVED ONE TO THAT PLACE. THERE IS NO COMPASSION THERE.

Experiencia negativa: I've never even been to this nursing home or have any relatives admitted here but after seeing so many of y'alls reviews about loved ones dying in this nursing home due to neglect and the fact they are literally responding with the same uncaring automated message to y'alls concerns tells me that this nursing home needs an entirely new team of people or just needs to be shut down entirely.

They ignored her so bad she suffocated in her own saliva because she couldn't get up to clear her lungs and they ignored her calls for hours and by the time they went to her room she was non responsive and had passed away..... ABSOLUTELY NEVER SEND YOUR LOVED ONES HERE AT ANY COST UNLESS YOU WANT THEM TO DECLINE OR WORSE DIE.... Unfortunately it was to late for us as a family to get her out so dont be the next victim....
Experiencia negativa: My Mother-in-Law was here for 3 weeks for rehab. Afterwards we moved her to Ahava. Night and day difference in the quality of care. We saw a complete turnaround in her demeanor, appetite, and overall happiness once she was moved. At AHC she would push her button and wait 45 minutes and someone would come to her door and say 'what do you need'. At Ahava she pushes her button and someone is there within a couple of minutes and comes to her bedside and talks to her with kindness and respect. Tennova hospital did not offer Ahava as an option when she was discharged. Do your research and tell them where you want your loved one sent and definitely not AHC.

Experiencia negativa: My family was placed in AHC for rehab after a injury. He put call light on was not answered numerous times 1 to 2 hour's at a time. Very poor care all around. He was left in urine and feeces for hours . My family member broke their hip. They were there 6 weeks with very little rehabilitation. He had lost 10lbs in less that 2 weeks. Food, care, Nursing care response time was extremely poor. His appetite decrease, over all demenor and health had declined. When we asked for help to get him up the nurse said it was not her job and refused to help. Then the nurses went to the nurse's station and started playing on phone. We transferred him to another rehab. He was there 2 1/2 weeks. Excellent care and returned home. I'm concerned for patients that can't speak for thereself and no one to talks for them. I would never recommend my family or freinds to this place with all the problems we went through. It is not a safe place.

Experiencia fantástica: I'm thinking of possibly putting my father in law here for rehab. My mom was in rehab and long term care here around 3 years ago. I have nothing but positive things to say about them. While scrolling through these reviews I've noticed a common thread. Most reviews have bad grammar, poor spelling, make no sense and most are very repetitive. In my opinion, I believe they are fake reviews. Just my 2 cents worth. I'm thinking of possibly putting my father in law here for rehab.

Experiencia positiva: My mother has been here since first part of January 2022 after leaving hospital and I was extremely leary of placing her in a facility. Well my fears have been alleviated! Momma loves the attention she receives from all the staff from her OT...PT..speech therapist..and her nurses and care givers! We had a small problem with h er phone in her room but this was handled with utmost courtesy and kindness. I am sleeping soundly knowing she is being well taken care of. They keep me updated on her progress and I really appreciate that. These workers are doing so much and just wanted them to know they are appreciated! Thank you for all you do!

Experiencia negativa: Middle school management took COVID vaccine had side effects could not come in was terminated. Way to treat your front line workers. Management are bullies everyone afraid to speak , they rule with threats and intimidation. So much to say not enough room.

Experiencia negativa: My mother is there and she can't get anyone to help her. She gets left laying in her own feces and no one will check on her. She went home in such horrific condition we had to have her readmitted To the hospital in less than 24 hours. The facility was better when I could check on her but now that she has no one to keep an eye on her they leave them alone to rot and die

Experiencia negativa: Horrible place. 80% of staff is uncaring and rude. They are happy to let the residents lay in bed on their back in wet diapers for many hours and it takes forever to get call lights answered. Food is always cold and served in styro-foam containers with plastic utensils. Management seems to try to help but there's only so much you can do with staff who doesn't want to take care of their residents. My mom was happy to leave. The 20% of staff who do care are amazing people, they need more of them. Whoever owns this company needs to go unexpectedly and investigate what's really going on here if they don't already know.

Experiencia negativa: Horrible Horrible Horrible!!! My beloved Mother passed away only four days after being admitted here after she was on the road to recovery. Staff, leave a few are caring but even the call to be informed of my mother being non responsive was from a rude nurse . I'm sure my mother couldn't wait to fly home to heaven with God than be in that place of torture and neglect. So grateful to God for saving my beloved Mother from that place. Nothing worse than being neglected and this place get 5 stars for neglect.

Experiencia negativa: I’m trying to figure out how this facility is still open. 1. Barely any staff they’re always short staffed whether it’s 2 CNA’s for each side or only 2 nurses for the entire building. 2. Just look how many management positions are available because everyone is quitting left and right. Look at how they are always hiring nurses and CNA’s 3. Corporate doesn’t care they walk in and don’t talk to anyone. They don’t ask the staff how they’re doing or anything. 4. The administrator is HORRIBLE she doesn’t care about no one or anything she’s clearly there to get a check! It shows. She doesn’t leave her office. Only time she gets up is when it’s time for their morning management meeting. 5. The residents are on the call lights for HOURS. 6. They barely give them showers the residents have to threaten to call state or call the news for them to receive a shower. 7. This place needs to be shut down or this place needs a new administrator not a administrator who’s friends with corporate. Now look at all the reviews below and you will see how down bad this place has gone.

She called for help more than 12 times and then messed herself and the restroom and had to clean most it up herself. Don't put your loved ones at this site. Staff does not answer patients calls.

Experiencia positiva: I was there for 3 wks for rehab and couldn't wait to get out of there, staff is rude uncaring there is a few that are excellent ,I had a nurse lie right to my face I asked her if my pain meds were in the meds she just gave me she looked like a deer in the headlights needless toosay she never gave them fPhysical and occupation are awesome Stephanie and Diane got me walking again I or days, the Crare lazy took me 2 hrs to get a bed pan,

Experiencia negativa: The lady answering phones at the front desk is extremely rude! I called wanting to apply for the dietary aid position and she was so rude I never even put in the application. I wouldn't want to work anywhere that allows staff to be so rude. Called today at 2:20 and didn't get her name.

Experiencia negativa: Only take your loved one there if you want them treated horribly. There are a limited number of nurses and techs that actually care, for the most part the call lights are ignored and patients are treated really bad. My loved one died due to their neglect. If zero was an option that would be my rating. Meeting with management is like talking to the wall.

Experiencia negativa: My mother’s stay there was a disaster. I would not recommend this rehabilitation facility/nursing care facility to anyone. Communication from the facility to the next of kin was totally lacking. If I had not contacted them I seriously doubt I would have heard from them at all. I never got a report or information from a doctor regarding my mother’s state of health, prognosis, or prescribed medications. Care went from a conference call meeting with patient and therapists one day, with a scheduled follow up meeting in two weeks to track progress to a call - a week earlier than the scheduled follow up meeting - saying “Hey! Your mother is being dismissed to her house alone.” And don’t even think for a minute your family member gets USPS deliveries or Amazon shipments in a timely manner. Worst food, worst staff, worst place ever.

Experiencia negativa: I just had my 1st visit with my Mama Gail aka Ms. Linda Daniels this past wkend. However while there, a staff member(a new employee) finally came in to change the bed sheets. Then I noticed she's about to put clean sheets on WITHOUT spraying disinfecting spray on the bed! So I asked her if or not she's supposed to & she tells me,NO!! WTH!! I told her it was gross concerning the situation & I've always known it's protocol, too!! I said if she was back home in Lawrenceburg, they would've.
Experiencia negativa: The staff is rude, unprofessional and I've personally heard them insult elderly patients to their faces. The " medical staff " is a joke, I was woke up at 5:30 am under the guise of drawing blood, only to have the door to my room thrown open and fluorescent light turned on abuptly !! Needless to say they could not draw the blood after all that Hummmm. DONOT COME HERE OR SEND ANYONE YOU KNOW HERE....EVEN TO DIE .

Experiencia negativa: My mom's been here twice, the first time was great. However, this time has been horrible. I don't know what happened in the last year, but something is off. Would not recommend.
Experiencia negativa: They offer awesome therapy at the facility! The physical, speech, and occupational therapists are awesome! They are attentive, professional, and communicate well with their patients and their families. That is all I can say that is nice about Clarksville Nursing and Rehab!
Experiencia negativa: The rehabilitation therapists are great and seem to really care. The nurses and CNAs are either short staffed or don't care one. Would not use this facility again for my loved ones.
Experiencia negativa: If you have a loved one here make sure you double check the medications they are prescribed, call your loved ones every day and call the nurse if you can't get a hold of a loved one. My mom almost died from medication issues, if I paid attention to the changes they did she may not be as bad off as she is right now. Besides that very bad issue, there is a lot of wonderful staff at this home who really care about the residences and treat them like family.
Experiencia positiva: The nurses and staff work hard and do an amazing job. They do care about the residents. My mother in law has been a resident there for 3 years.
Experiencia fantástica: As a paramedic who visits here awesome, this is probably the best nursing home and rehab center in Clarksville or Montgomery County. I would have no problem with a family member of mine being here.
Experiencia negativa: I am not sure where to put my mother. I am looking for a pretty good place where my mother will be taken care of by the staff that she and my family will count on. It must be somewhere inexpensive please can somebody give me good a recommendation.
Experiencia negativa: The admissions lady is very rude and never answers the phone. She needs to understand that she works for a patient care business, but not a corrections facility.
Experiencia negativa: An elderly family friend of mine was being taken care of at this center. Each of the times that I visited, there seemed to always be a problem and the head nurses didn't seem to care, much less look at my face when I asked them a question. The first time, I needed a thickened liquid for her tea, the nurse didn't seem to know how to do that. The second time I visited her at around 2pm, she was distressed and wanted to lay down, I asked the head male nurses, and they told me that there were no CN's available. I asked again, they stared at me with an attitude and again we waited. It was 6pm, when a food person walked in to bring her dinner, then I asked the food lady to help me get her into bed, by this time my friend was crying. A CN walked past and the food person asked for help, and she seemed scared to come inside of the room. The last time that I visited my friend, it was close to 3pm, and she was laying in her bed, crying and trying to lift her legs out of bed. I felt her legs and they both had a huge knot that were throbbing causing her so much pain, that she was yelling out in pain. No one came to assist her. What if I hadn't gotten there when I did? She would have been ignored! I immediately asked for help from a male nurse named Michael, he was also in charge of dispensing medication. I told him, that my friend is in a lot of pain, and she's crying. He suddenly SNAPPED at me! Got all red in the face, crossed his hands over his belly, and gave me the nastiest attitude I have ever been given in my life! He said "and, what?! She told YOU that she's in pain?! Because, she can't talk, all she does is grunt!". Then he sharply walked past me, to her room, still talking about how she's not in pain and just making noise, in front of her, such a lack of respect. He touched her leg and soon realized that her pain was real, and that her cries were not because of frustration. He tried to apologize to me, I honestly didn't want his apology, I just wanted her to have pain medication and feel better. These staff members are awful, they don't care, they don't have compassion for the elderly that they are being trusted to care for. They leave the patients to fend for themselves. I would not recommend this place to anyone.
Experiencia negativa: The management is only out for whatever is in their best interest. Pretty sad that when they have a good employer they treat them like crap. Management cannot not make a schedule either. Working nightss, why would you have someone work 3 off 1 work 2. Pretty stupid cause the one day off you sleep. Theb your up all niggt while the rest of your family sleeps and then tou have to sleep the next day cause.tou work that night.
