Experiencia fantástica: In February 2024, my 89 year old mother entered Spring Meadows Health Care Center with serious, debilitating health issues (post-surgery for a fractured hip and advanced dementia). My sister, Sheena, and I feel truly blessed to have found such an amazing facility for our mother. The highly professional Administrators and entire staff's love and care for their residents is exceptional with daily, engaging activities and delicious, nourishing food. The facility is extremely clean and well maintained. We have witnessed from day-one, Spring Meadows Health Care Center go above and beyond to ensure their resident's safety and high level of care.

Experiencia negativa: Do not recommend at all 0 stars Doctor over medicates to keep patient from being workable for physical therapy and to keep there long term Neglect of our loved one resulted in death and they get by with it

Experiencia fantástica: Today was my first time visiting my uncle after he had a stroke a while back. I found the facility to be clean, and the staff to be friendly & courteous. I took my aunt (74 yrs) to see her brother (78). They were accommodating of her handicap as well. We had a very nice visit. We both felt better after our visit. My uncle looked better and about 10 years younger. Thank you Spring Meadows.

Experiencia negativa: This place is a dump. They are very neglectful of their residents. They were verry abusive to my father who was there for rehab from a stoke. They didn't check on him at all for 3 days till I called and asked to speak to him. They allowed him to fall out of bed 3 times and fall in the bathroom 8 times. They didn't answer the call light for help at all. I will never allow him to go back there.

Experiencia fantástica: My mother was at Spring Meadow for 1 month and had the best care. Everyone at Spring Meadow was thoughtful and caring and treated mom like family. The Administrator is very active and involved and was always checking to make sure we had everything we needed. The office staff was top notch, so friendly every time we came to visit. I can’t say enough about Spring Meadow, I would highly recommend them to anyone!

Each of those places we were concern with his treatment medically, mentally and physically. Now he has never been so happy. My 85 y/o father states, "they run this place like a well tuned machine." My family and I can rest assured his needs and wellbeing are taken care of. 5

Experiencia fantástica: My family and I are eternally grateful! My mother Carmen M. Alicea Castro passed on May 16. Spring Meadows in Clarksville, went above an beyond caring for my mother on a daily basis for over 10 years. I knew her caregivers by name .Even though my mother was bed-ridden, she never developed skin ulcers, was always clean and they accommodated her and didn't force her when she refused her meds. She was always treated with respect. Spring Meadows nursing home made an extremely difficult situation bearable when she was dying and we ended up staying with her 24 hours for 5 days until she passed. They fed us, housed us and offer us whatever we needed it. It meant a lot to us. This is a good place where the staff cares and the residents are treated with dignity.

Experiencia fantástica: I had a a tour through another facility but I realized my father was going to need more than assistant living. So I walked into Spring Meadows hoping someone could give my some paperwork on this facility. They were so nice. Immediately they had me setting down and asking what kind of care my father needed. Which really I don’t have a clue? I started talking about my dad being an army vet and she stopped me right away saying that there were places that my father would be able to use his benefits. I absolutely love that. She was more interested in finding a place for my dad rather than just sticking him into this facility. She was so great I just wish I could remember her name and tell her nowadays most people are only looking out for themselves.

Experiencia fantástica: I was delighted to see that there was a secure entry and exit point. The friend I visited was in a clean room and had an amiable roommate. They are able to choose between choices on the menus for each meal or not choose any and have someone bring them something from the outside. The nurse who came in to retrieve the dinner trays was very friendly. There were plenty of hand sanitizer machines on the way out.

I will admit that I didn't want to be there, but even though I was a little depressed the staff and the patients that I came in contact with helped me make it through my stay. Ms Vanetta has called me several times since I've been home to see how I am doing. That me realize that once I got home I wasn't forgotten.

I got a call inviting me to attend her care plan meeting tomorrow before I could even get there to see her after work. Excited for my mom to get the care and attention she deserves.

From Ms. Bell bathing her and putting lotion on her feet. The physical therapist took her time with my mommy. Her lunch that was hot. I truly appreciate them. It was just a joy seeing my mom smile.

Experiencia negativa: A facility where they make you sit in a room for 20 min to not get a report or keys. Left that facility to go work because they weren’t even ready after a shift had started. Showed up 10 min before my shift was supposed to start. They claimed they didn’t have a schedule ready. Yet, they had confirmed me for a NON Covid shift, asked my name & title, opened the schedule book, & denied they had a schedule. I left because of the poor planning and having to pass meds late was not gonna be an option for me. Then they tried to say I was a “No Show.” This place is disorganized. The scheduler did not greet themself. No one told me their name. I was the only DAY shift person to show up on time. I don’t recommend this facility. Pass. A picture for proof that I did show up and got played. Don’t go to a facility where they want to lie. Oh and they have Covid all in the building mixed with the general population and they didn’t let me or others know about this. Met another agency & they told me they were not told they had COVID until they were told to place all PPE on. Crazy. After my review was posted they sent out a mass info that they had it. . . & they refused to apologize to me & my horrible experience. Side not: Same day I posted, they tried to intimidate me by telling a 1099 company that I needed to take my review down. Yet, they never took down my “no show” or apologized. 2 weeks later they want to talk. . .

Experiencia fantástica: Had the pleasure of touring the facility and I was very impressed. Ms. Samantha was professionally personable, and was able to answer my questions. Upon entering the facility I noticed that there was no odor which is often the case in some facilities, No odor, professional, personable, staff to client ratio visible, and clean, and the fact that many of the staff have been there in excess of 10 plus years is a positive especially for upper management. When having to place our loved one in the care of others family members want to be at peace and know that it is a collaborative effort . Blessings

The staff know my mom and have been wonderful with helping with her health and physical needs, but also with being friendly and concerned. Everyone is friendly and helpful whenever we talk to them. The nurses and staff always seem patient and caring---I wouldn't want someone impatient working with my mom. We are thankful for the care she receives.

Experiencia fantástica: My brother has been at Spring Meadows since 2014. I chose Spring Meadows for him because of the first impression that I got when I walked into the foyer. Some nursing homes have a bad smell when you walk in but this place has a homely feel as you walk in and it doesn’t smell bad. Rusty has had the best nurses taking care of him. Rusty enjoys the activities. His room stays clean. He gets medical care as he needs it. He is safe there. His room is on the back side and he sees wildlife all year round. Love it there and love the Adminstrator!

Experiencia fantástica: I’ve been apart of the Spring Meadows team for a very long time and I wouldn’t trade it for the world! I’m also a family member because my grandmother is there as well so I get the best of both worlds! From an employees point of view theirs definitely a lot of hard workers in every department, everyone loves theirs job and they are great at it! So much teamwork! From a family members point of view, it puts me at ease every single day knowing I can go home and know my granny is in the best hands, she loves her nurses and CNAs and they take such good care of her! I would definitely recommend Spring Meadows to everyone!

Experiencia fantástica: It was a tough day when I realized I could no longer care for my father at my home. Despite my best efforts to find him a good place, his first destination was not a good choice. It was obvious it was not going to be a long term solution and I needed to find another option quickly. The morning I called Spring Meadows and spoke to Melissa, I knew I had struck gold. She was understanding, compassionate, and caring. She ended the call by saying, "Even if you choose not to send him here, if I can help you in any way, just call me back." A few weeks later my father was transferred to Spring Meadows and my family was able to breath a sigh of relief. Due to CV19, visits with my father were limited to window visits and telephone calls. It was a tough time for us, but what made it bearable was I knew he was well taken care of by the staff at Spring Meadows. I never worried about him the entire time he was there. He was always clean, feed, and treated with dignity. My father made his final transition a couple of weeks ago, but to Melissa, Amanda, Emily, Sierra, and the other staff at Spring Meadows, I say, 'Thank You for a Job Well Done'.

I’ve visited this facility and saw first hand the positive changes they’ve made with their patient-focused care, and with their staff retainment. I would definitely recommend this nursing home as a place to work or live, and you can rest assured you’ll get a 5-star experience.

Great group of staff who work off of each other’s energy and the inspiration they get from the residents every day. I know physical therapy well, and this team here is the best. They’ll help get you/your mom/dad back to themselves in no time. I couldn’t recommend this facility more for your loved. Especially these days with limited visitation - they’ll really take good care of them. They have all sorts of technology to help with FaceTime/zoom/Skype calls and will go out to their make to always make sure their residents and staff are having an epic experience.

Experiencia fantástica: The administrator and Director of Nursing are such a great help navigating people who find themselves in sudden need of care for a loved one. They make sure everyone is welcomed and feel comfortable, The rooms are clean, and spacious. Frequent and well planned activities help during this pandemic. They will navigate you to all the fun you can handle! I cannot tell you how much worry and stress they taken off your shoulders when leaving your loved one in their hands. I don't know what we would do without this community and communities like this.

Experiencia fantástica: We had my mother here, the staff were friendly and helpful. Many of the workers have been there for years. The facility was always clean and they had activities going on most days for the residents. It’s not the newest facility but the rooms are spacious and have large closets compared to the new places and the building is well maintained. The grounds are very rural so you see deer and turkey wandering often if you look out the windows.

They made every excuse they could to try to cover themselves up from what they done to my love one I would not recommend this place to my worse enemies. Channel 4 needs to investigate them
