Experiencia negativa: That was absolutely horrible, I quit going there play Ray Roman fresh out of surgery he immediately bent my leg backwards under the table at a 95° angle that injured me from the get-go it took me out of the game completely couldn't do any exercises on my phone at home went back to him for the third time and he did the same thing after I asked him not to do that this is my second surgery so I kind of know how the therapy goes you set your goal and work towards it you don't start right where the goal ends went back one more time and he did the same thing again 3 and 1/2 days on the couch with ice and agonizing pain now I have to take a whole week off stay off my feet except for three small walks a day and walking back and forth to the bathroom I now have a new therapist at a new location

Experiencia fantástica: After my heart surgery in 2019, I was here. The care is great and a fantastic therapy center and devoted therapists with great and caring floor staff everywhere.
Experiencia negativa: My mother has parkinsons abd was sent here from the hospital after havin a seizure for rehab. This has been the worst experience ever. Not being familiar with Fl laws concerning rehab centers and skilled nursing we had to speak with the administotlr and director of patient care. My mom is a risk fall patient and they say thwy are not allowed bed alarms so their solution was to put her in her wheel chair and sit her in the hall way. Came in one day everyone bustling around doung their job as she sat there with snot draining out her nose down her front all over her hands. Another incident was getting her medication down her. Laws concerning the forcing of a patient. My mother also has dementia and macular degenerative disease and cant see or find the nurse call button. We found her 2x lying in her feces and once was so bad it had harden on her skin.. They lost her glasses and we still have not gotten word if they found them. Communication between the staff and us was bery strained. Nurses on duty did different things or were unaware of what she was getting and how we had to proceed to give her stuff..We had talked 3 x times with the staff and head dietician about her food and every meal came up with food that we discussed she would not eat and was lukewarm at best. Finally gave up talking about it. There was total confusion between us and staft concerning her care plan and we finally got one scheduled 14 days after she had been here. My mom was not typical rehab and required more attention which as far as Im concerned she didnt get. If we had not been there everyday to make sure she got what she needed she may have died in here. As I sit here typing this we eneterd her into hospice and her release will be the exact number of days her insurance pays for this facility and had the care plan been done 7 days after she entered we would have been long gone from here. I have seen the reviews and ratings. Great for all that had a good experience but I would not recommend this place to individual who have limited cognitive function and requires assistance for eating and bathing and dressing. Worst experience Ive had.

Experiencia fantástica: This time around I had great physical therapy and occupational therapy. The staff was great (nurses and aides were great) very clean, the food was just okay but all in all I’m very happy with my experience
Experiencia negativa: I was here for shoulder replacement therapy and 5 years later I still don’t have range of motion. My therapist Tim was more concerned about cleaning his sneakers everyday with alcohol wipes and discussing what was for lunch. (Ask anyone) I’m now going to outpatient therapy in near my home and finally getting the therapy I should’ve had years ago. Still don’t have range of motion & it hurts. My right shoulder I went to Tarpon Springs Peninsula and I have great motion in that shoulder. I should’ve went back there but I thought how bad could it be? Well I got my answer. BAD The nurses (besides one) were all great and the staff. Loved all of them!

Experiencia negativa: Two years ago I gave this death trap 1 star. I held off on any explanation until we had thoroughly discussed legal action with our attorneys. Because we are not going to go through with any legal action, due to my father's advanced age and a legal system stacked against it, I feel free to share our story. If you or your loved one checks in here, you will quite possibly not survive the experience. I don't say this lightly. And in Florida, it is EXTREMELY difficult to bring a lawsuit against places like this. You have to be able to prove beyond a shadow of a doubt that their lack of care actually killed the patient. It's difficult to do, in part, because there is both benign neglect and minimal care, but no overt malignancy. My mother was apparently in stage 4 cancer when she was admitted for uncontrollable vomiting after what was supposed to be a routine gallbladder surgery. To be clear, her doctors and Morton Plant Hospital (who owns Madonna Ptak) only treated the nausea and vomiting, with no real attempt to discover the source. None of us knew what was happening, and we all found it bewildering. Eventually, it was implied that Mom was somehow psyching herself into it, but they inserted a feeding tube (aka "peg" tube, which is a direct port into the stomach through the abdomen). The feeding tube helped, but only slightly. Because the vomiting continued when she was home, Mom was repeatedly readmitted to the hospital, and then sent back to Madonna Ptak. This happened for many, many months. It appears that Madonna Ptak, like Morton Plant and so many other hospitals, is deliberately understaffed to maximize profits. Mom would tell us of alarms that would go off at night, and the nurses were so short-staffed that it could take hours for a nurse to respond. The woman across the hall from her would often have screaming fits in the middle of the night and keep everyone up, but since no nurse would show up, it would go on and on. Mom also told us that during her entire stay (off and on) of at least three full months, she never saw a doctor. She would see a physical therapist, per the doctor's instructions, on an almost regular basis. However, unless we stayed on top of it, the physical therapist would frequently forget to put her through her therapy. They were delighted to leave Mom in bed and keep her heavily medicated, as they did with other patients, to minimize the manpower it would take to actually work with her. If they had paid more attention to her, chances are they might have found what was wrong with her before it was too late. Even though it was stage 4 cancer, she certainly could have been given chemotherapy or other drugs to make her life less miserable in those last months. From anecdotal evidence, we believe that Madonna Ptak releases patients as they're dying, which keeps on-site death at a minimum and makes them look far better than they really are. In other words, patients decline to the point of death, the staff at Madonna Ptak declare that there is nothing more they can do, and they release the patients to go home to die. This is exactly what happened with my mother. Mom was the author of Growing Older, Growing Wiser, a book about the aging proces. Although Mom was an expert, she had no idea that the elderly could be so disregarded in this era, nor did she realize that a horrifying death was awaiting her. My mother was ejected one week before her death, and came home to be nursed by me as she died in agony, due to the fact that nobody had ever figured out what was wrong with her until it was too late. We finally discovered that it was stage 4 cancer two days before she died. Sadly, she was fully in her right mind, so she was brutally aware and suffered greatly.

Experiencia negativa: My Mom of 86 years was sent to this place after she had a stroke. She was admitted there on Feb 11,2022. There days later they decided to finally weigh her and she was 94 lbs. In the hospital she was 106 lbs. And she was put in the covid unit for 10 days. They just moved her to the regular rehab wing yesterday. Today they weighed her and she is now 84 lbs. My Mom is a Britel Diabetic, which means they can't control her sugar levels. They screwed up her menu as well. I wouldn't send my worst enemy there. And I am very surprised being that it's part of Morton Plant Hospital.

Experiencia fantástica: I have only words of praise for the team at Madonna rehab! My aunt went thru so much since she found out that she had rectal cancer at 87 yrs.old. Since the beginning of this journey the staff were caring, friendly, willing to answer all my questions(and believe me they were a lot of them) with patience and professionalism. Especial Kudos and Thanks to Amanda Johnson,Helen from OT dept.,Leny, Nurses Geo,Ela and the CNA's that have the hardest part of the job ,Angie,Sherlynne and Linda.Sorry that I forgot couples of names.But you know how good you are.And to Maya Perez for her attention to my request and questions.THANK YOU!! GOD bless you all and give you the courage to keep going in this hard times we are living!
Experiencia positiva: My mom has been there since Jan 8th. She just passed tonight. I can't say enough of the wonderful care she got from the aides-especially LULU who I seen mostly on duty when I visited my mom, the nurses who called to let me know everything, the social worker Amanda who helped so much. You guys are amazing and I appreciate everyone.
Experiencia fantástica: Very clean. The whole staff was very pleasant his whole stay. They even allowed him to have his dog sleep in his room . And if he wanted to eat outside they delivered his meals in the courtyard. The food was delicious.
Experiencia negativa: THIS IS A HORRIBLE PLACE!! The DON and administrator do not address residents complaints and concerns. When asked on multiple occasions for a corporate number was denied. Dietary messed up my father's diabetic meals EVERYDAY. Lenny the case manager doesnt communicate and only warms the seat in his office. They are under staffed. My father had his call light on for over 18mins one time and an emergency call light on for 11mins. The rehab director has no personality, no customer service skills at all. Do to my outspoken mouth I was banned from coming to the facility. The DON lied to get me banned. The supervisor falsely documented my father's wishes for a DNR.

Experiencia negativa: I would NEVER send my loved one here ever again! You would think that with the close proximity to the Morton Plant hospital and it's affiliates they would have focused more attention on my now deceased fathers wounds. My father was transferred there from the hospital for rehab after he was in the hospital due to a fall at home with his walker. He was diabetic and had open heart surgery 9 months before his stay at Madonna Ptak. In fact, when he was recovering from his open heart surgery he stayed at Madonna Ptak and all went well, which is why we agreed for him to go back there a second time. Unfortunately the second time was not at all the same. My father developed an ulcer on his heal that broke open and got infected, it wasn't tended to properly and ultimately led to gangrene and he got MRSA and staph which caused his death.

Experiencia fantástica: I broke my left shoulder and left ankle was told I can't put any weight on my left leg or use my left arm for six weeks. I ended up in Madonna Ptak Rehab.The staff here made my . horrible situation not so bad I am treated like gold.From Physical therapy, Occupational Therapy, Nursing staff all the Aides are Amazing and social workers, House keeping , kitchen staff .Every one of them are angels .If you ever end up were you have to go to rehabilitation I highly recommend Madonna Ptak Morton Plant Rehab you are treated like a king or queen .I can't say enough good about the entire staff here ......My name is Frederick Longo

Experiencia positiva: My mother has been here for four months, and, by and large, we (her family members) are happy with the care she's received. One issue that could be easily addressed, however, is the laundry service. As a long-term care patient, my mother's laundry is handled by the facility. One person on staff does laundry for all the long-term care patients -- a number that is increasing -- five days a week. No one does laundry on weekends. So today is Sunday, and she has no clean pajamas, no clean pants, no clean sweaters and no clean blankets of her own. We keep buying her more pajamas, sweaters and pants, but it seems it's never enough because no one is doing laundry on the weekends. One of her blankets also disappeared in the laundry. A suggestion: Hire a part-time person to keep up with the laundry on the weekends. Thanks for listening.

Experiencia negativa: BEWARE.....If you need any special care you had better have someone to advocate for you!!! My mother had congestive heart failure and 2 valves that needed replaced. she retained fluid easily and needed to be monitored closely. they instead of giving her a low salt diet switched her to regular meals it was a week before we caught it. she was filling up with fluid and the "house"doctor , the only one that we were able to see, said she was experiencing COPD, and gave her steroids on top of the salt. we stopped the steroids and we were told by the head nurse that she was so glad that we were medical doctors!! what happened is that they said we needed to leave!! My mother went by ambulance across the street to Morton Plant . My mother almost died because she was drowning in her own fluid!!!!!!!!!!!!! and as of now they have removed over 8 pounds of fluid from my mother. BEWARE ........................................Update!!3-10-2016 My mother is now in Hospice . If they had listened to us she might have had a fighting chance but the end is imminent now.
Experiencia negativa: This facility kept me a prisoner for 30 while they ran out my Medicare funds. I am a diagnosed Celiac--an autoimmune disease which is triggered by wheat gluten food or crumbs or even additives to soap or makeup. The do not have any gluten free food. None at all. Gluten free diets require fresh fruits and vegetables. They would not release me until my Medicare rehab benefits were used up. I received possibly one hour a day of therpy. This is not a joke. After 5 months I am still sick with Celiac's symptoms. Why are they still in business?
