Experiencia negativa: I’d give this place zero star if I could. DO NOT SEND YOUR LOVED ONES HERE. Management is a joke and employees do bare minimum. They do not care about the people. These are horrible people. They do not care about your family member. This place should be shut down and management should be prosecuted.

Experiencia negativa: If there was a negative 5 stars that’s what I give this place where my healthy strong uncle needed help and literally after virtually begging he never got help like rehab or entertainment or company. It was truly like a prison. They talk a big talk but never follow through.

Experiencia fantástica: The facility have 5 star quality measures and 5 star quality of care. Nurses are very nice and helpful, they treat their patients with love and care. The facility is actually nice and clean. The rooms are very nice, smells really nice, and clean!!!! They have good different activities for the patients and good therapy department. Highly recommended.

Experiencia fantástica: This review is mostly for the nurses, CNUs, and staff working directly with and caring for the residents. My aunt has been staying here for about 5-6 months and every time I come in, the people caring for her are so friendly, so positive, so respectful, and sometimes very humorous. They do a great job making her feel supported, comfortable, entertained, and cared for. The food could use some help though.

Experiencia negativa: They are overworked and can’t provide the care needed. My sister developed a c-difficile infection. It took hours for them to change her when she needed changing. My sister has insurance and they didn’t want to give her the provided care. They refuse to give her physical therapy and claimed that it was because of her insurance. They tried to send her home and say that she was able function without acute care when she is unable to walk or use the bathroom or eat food on her own. We noticed that she wasn’t looking well and she was not talking right and was throwing up a few times a day and had diarrhea. We told them that something wasn’t right with her and they should call emergency. She ended up back in the hospital with an infection. Don’t send your love one to this place.

Experiencia positiva: 1/19/15 I've been in this facility several times meeting with residents. The staff at the front desk are very welcoming and helpful. I arrived early this morning for a 10 a.m. meeting with a resident and the woman at the front desk was cheerful and happy to call the care provider assigned to the room so that she could walk me back. The facility seems very bright to me - there is a lot of natural light and many big rooms for the residents. The place is also clean and spacious - the walkways are wide, as are the rooms, making mobility in a wheelchair that much easier. Most of the patients I saw were in wheel chairs, though not all of them. Windsor provides care and recovery for many different types of injuries. I asked the family I was with if they were happy with the care that was being given and they really were. They said the staff truly cared about their father and he definitely seemed in good spirits when I met with him. Recovering from a serious injury at almost 90 years old would be hard - you have to really trust the facility giving your loved one care - I feel like this is a place that makes it easy to do that.

Experiencia negativa: Is there a place that resembles a rehab from a third world country? Yes there is...here at Windsor Manor. Nurses do not follow patient rights and the doctors and nurses do not practice patient centered medicine. My husband was in pain, but when they asked him how much pain he endures they said, "by his expression on his face, he is not in pain even if he says he is" This is making a mockery of patient responses. DO NOT GO HERE.

Experiencia negativa: A two at best. Specifically for their choice to subject these poor folks to the garbage news that they have on the television. I asked who was in charge of the programming. Its DEPRESSING enough to be away from home. The NEWS, ANY NEWS, is the last thing these folks need. Not to mention right now someone may have used the bathroom in their pants. If I can smell it THEY can too. So sad.

Experiencia negativa: I would give this place a 0 they don’t deserve any stars. Do not come to this place through registry, there are complications with shifts being verified and being paid. The scheduler is so rude she speaks over you constantly. Go elsewhere.

Experiencia negativa: My father was there to get better he had been there since September of last year he had dementia and cellulitis on both of his legs, after being in there for that long I would think he should have gotten better, well infact he did not he was admitted into the ICU at John Muir where he passed away a day later due to sepis, I can't understand how if he had been getting the correct meds he should have gotten better not died. I went to Windsor manor a couple days after his passing to get his belongings the lady at the front desk checked me in and then showed me where I had to go as soon as I walked into the section my father had been I wanted to cry the smell was sobad it was crowded and very unsanitary the patients there looked so sad all I could do once I left was cry knowing that my dad had been in this place just before he passed, my dad was suffering and no one cared I'm just glad he's in a better place and doesn't have to go back there .R.I.P DAD love and miss u

Experiencia negativa: Worse rehabilitation center ever! Do not let your love one be admitted here, as the staff is incompetent and there are no doctors to take care of emergencies. My brother was admitted to this place in October and died less than 48 hours later. No no physician was there to help him when he went into cardiac arrest not to mention the other two times before that when he was getting sick and us as a family was not told. They even let him get out of his bed and he couldn’t walk. Do not let your loved ones be admitted to this place. Also no one will call you back, no matter how many voice messages you leave. This place needs to be shut down!!

Experiencia negativa: DO NOT SEND YOUR LOVED ONES HERE!!! They all hate working there, so you can obviously guess they will give your loved ones terrible treatment.. they never change the resident.. they allow them to sit in their own feces for a couple hours because changing them is an inconvenience. When residents are being cared for and saying what the staff is doing is hurting them, they don’t care they just laugh and continue to do it rough. They constantly laugh, mock, and make fun of the residents to their face. They avoid residents call lights because they all hate working there. They will purposely not walk past certain rooms because they know certain residents will ask for help if they do. The pictures don’t match the inside, the place is honestly a dump and very depressing. SEND YOUR LOVED ONES SOMEWHERE ELSE! Especially if they have dementia/Alzheimer’s because they know they can do anything to them and they won’t get in trouble. Also this is coming from someone who did their clinicals there… so I got to see the place for what it really was. Don’t even know how this place is still a care facility. State should shut this place down..

Experiencia negativa: Worst place ever! They are NOT taking caring of our sick and elderly at all. I should of never let my mom go to this facility. This is my second review My mom lost weight and was 60 lbs then finally taken to ER and died in the hospital days later. They make you jump through hoops just to communicate with your loved one! This place needs to be checked out by the county! Even elderly on SS or county shouldn't be treated this way.

Experiencia negativa: I’m a patient in this rehabilitation facility for three days. My allergy is getting worse and make me sick. Very Nasty and filthy place. So disgusting! Horrible and Scary place.☠️ Most of the day shift CNA are lazy and not doing their job very well. WARNING⛔️ BEWARE❌ OF THIS PLACE!!!! ☢️⚠️

Experiencia negativa: My father has been there 3 days and they are not providing him his diabetic care as need. They are not giving his insulin as need as well as last night they completely dropped the ball and did not give him any medication! I will speaking to whose in charge today! Disgusting display of care and they are negligently understaffed!

Experiencia negativa: My family member was transferred there after surgery for rehabilitation physical therapy, before she was ready to come home. I wish the surgeon even more honest about her discharge, so I could have taken time to find a place, I would have picked differently. Her first day there was a Sunday, no PT on Sunday, she was left lying in bed all day, not even assisted up to use the bathroom, forced to wear a “diaper” (her words) and soil herself. That happened more than once, due to no one responding to her call for help. I received more then one phone call, that she was in pain, and unable to get a response, for more then an hour. Getting to talk to a nurse, for an update, or get her pain medication, was difficult at best. The physical therapy, I believe was good, they were responsive to my calls, being she needed that, she endured the rest. Also, as far as COVID, she always had a room mate, and was moved to a new room (new exposior) I think three times in three weeks. They did test everyone acne a week.

Experiencia negativa: My Aunt sat in urine for hrs. She was fat shamed and medication for diabetes was withheld. Nobody answered her calls or mine. Worst place to ever put a human being. This place should be shut down and charges brought against the staff. I tried to call and NO ONE has answered my calls. We had to call 911 and get her out of there. This place is a disgrace

Experiencia negativa: My mom was sent here after her hip surgery. She has been there since Wednesday. She is telling me she is in a room covered in plastic and the food is bad. She's telling me that they don't answer her. She's not getting her meds as she should be. They have left her soiled for over an hour and not helping her use the bathroom. Yesterday she told me she is throwing up and has diarrhea. I have called a number of times, only to be put on hold for over 20 minutes then disconnected. None of them have returned my call or even bothered to call and let me know how she is doing. Now, looking to have her moved ASAP!

Experiencia fantástica: My dad was at Windsor Manor on two different occasions. The first time he had an emergency surgery while traveling out of the country. It took me a long time to get him home and when I did he couldn't even stand on his own. We were afraid that he wasn't going to be able to walk again but they had such a great team of therapists that he walked out of there! Not only did he walk out of there, he went on to live independently. I can't say enough about the rehab team at Windsor Manor. They did an amazing job. Sadly, he also faced a cancer diagnosis. He lost his battle with cancer this year just before the pandemic and I have been meaning to write this well deserved review ever since. When it got really bad, he wanted to go back to Windsor Manor for hospice care. It was a very difficult time for all of us but the nursing staff and hospice team took such good care of not just him but our family as well. I would like to thank Kris, Cathleen, Joan, Kat, Janet, Rosario, Daphne and many more for the compassionate care care they provided for my dad, especially in his last days.

Experiencia negativa: My mom is in there now 2020 she said the nurses never respond! They take long time to come in the room to give her meds! They leave a diaper on her for a while! It takes forever to get nurses on the phone they don’t return phone calls taking my mom out that place!

Experiencia fantástica: This has been the best place in the county for our mother! The staff is very helpful and they do everything they can to help our mothers recovery and to find an appropriate placement when she was ready to be discharged. If needed we will definitely have our mother come here again.

Experiencia negativa: I have had different encounters with Winsor Manor. A friend of mine had MRSA which is a very contagious virus that basically requires anyone who enters her room to gown up and wear gloves. I watched a nurse enter her room help sponge bath her come out of her room then enter another patients room without changing her gloves or washing her hands. Completely wreckless.!!! To my horror my mother inlaw was sent there. She was there three days before they figured out she has Altzeimers disease. The staff seemed caring but each day she had a different nurse which only confused her more than she already was. They discharged her because she couldn't follow directions for therapy. How could they expect her to. Hello people. Her brain isn't working properly. I was glad to get her out of there. They suck.

Experiencia positiva: This place is down "La Vista" but you need to look for Farm Bureau Road and turn at the light and go down the alley with visitor parking in the way back. Your GPS will tell you to go up Joann and to go into n non-existent back way. The back way is not there! Signs on the road are small and hard to see so look for the lights at Farm Bureau! This is suppose to be a review about finding the place.

Experiencia negativa: Worse place my mom stayed in room 102 the patient on b bed joann was rude mean and used to abuse her and nothing was done she controlled the place saying she was paying big bucks abusive to staff and patients no respect is given to nurses by this patient i felt because of her attitude and behaviour my family memeber was neglected also at one point that patient tried to choke my mom on pm and nothing happened even point finger on her face while talking... Hope they can do something about her... Complains were made but no action was taken..

Experiencia negativa: My mom was living in this place for about 6 years she received horrible care sat in urine for hours got a burn on her outer thigh that never healed my mom could not speak all the time people if you have loved ones here remove them my mom ended up in the hospital where she passed away . Experiencia negativa: Rock hard mattress. Takes up to an hour to get a response to the call-button. The staff is wonderful, but this place is way understaffed. Awesome comfort food (except the vegetables are always mixed) - and the food and milk are served at room temperature. Update: the "owner" has replied to me below, but has still failed to contact me in my room. Update: no word yet.

Experiencia negativa: My grandmothers’ premature passing was due to the lack of care she received while Windsor Manor. The intent of my grandmother going to Windsor Manor Rehabilitation Center was for Rehabilitation but she passed a month later due to an infection caught because of neglect shown by rude, incompetent, dishonest, and unsympathetic staff. This facility should not be allowed to care for patients with dementia other diseases that make it difficult for the patient to communicate or help themselves. My grandmother was doing fine on the first floor in rehab, she was walking with a walker and playing the piano in the cafeteria. Once she completed rehab, she was moved to long-term care on the 2nd floor while we tried to find a place closer to home she could live at but her health suddenly declined. She contracted C Diff which is highly contagious infection which we were not made aware of it until my mother asked to see her medication list and asked about some of the medications listed. We noticed staff were wearing gowns and masks each time they entered the room but never offered us protective gear which put my family in a very defenseless position to catch it. Due to her condition she needed assistance eating which they did not take the time to do. Each time family arrived she was in her room with lights out with a full food tray by her bed, when she was in rehab on the first floor the nurses took the time to sit with her and feed her. As she became weaker due to lack of food, water and the infection, she was no longer able to take any of her pills including antibiotics that were needed to fight the C Diff infection. We eventually had to call Hospice, we spoke with him for awhile before he did an evaluation on my Grandma. He said he would take the next few days to find a place for her to transfer to. However, once he saw the condition my grandmother was in and the huge diaper rash (due to being in a wet or soiled diaper for too long) Hospice said that Windsor Manor was NOT meeting her needs and she needed to be moved immediately. Hospice was able to find her a new place to go within the hour. During her stay they told us my grandmother “attacked” a patient, and due to state regulations they had to call the police to file a report. We inquired how she attacked a patient because at this point my grandmother could not get out of bed. They did not have an answer for us as to how she attacked someone. I called the Concord and Clayton police departments to obtain the report records and was told that no such record of the incident existed. Concord police even looked up the address to Windsor and said no call had even come in. I believe most of the staff at Windsor is either not competent or not properly trained to deal with dementia patients. They did not take the time to explain to my grandmother what they were going to do as they were doing it, they just rushed in to get the job done and rush out. I saw no compassion for my grandmother from Windsor Manor long term care staff. I do believe my grandmother’s passing was premature and her sudden decline was due to the neglect shown by 2nd floor staff. If she had been changed in a timely manner, she would have never contracted C Diff (which looking back we think she may have caught from the person in the room before since they just swapped them within an hour of each other and probably did not clean it properly) C Diff required medication that she was not able to receive because she had not been fed properly or given enough water so she started having a hard time swallowing. Had she had proper care, she would have been able to take the antibiotics needed to fight the infection. Therefore, I truly believe she would still be alive today. If you have a loved one or friend at Windsor and are not able to check on them daily like we did, I highly recommend you have them moved as soon as possible. I cannot even imagine the care they are receiving. I hope no other families have to suffer the devastating loss of a loved one too soon, the way we did because of the negligent care at Windsor Manor.

Experiencia negativa: My mother was cared for at Windsor Manor Rehabilitation of Concord in 2013-2014 since she was covered by Medicare, Medical, and a private insurance. Even though mom had excellent medical coverage, and passed away on 2015, Windsor Manor of Concord continues to claim and send a monthly bill to be paid by a non-existent Estate. In 2015 after mom passed away, they attempted to send this claim to collections without success, the agency closed the claim since it was unfounded. In California family members are not responsible for deceased family members debts. We have asked the Administrator, Manager, and clerk at Windsor Manor of Concord for the past two years by letters and phone calls to stop sending any and all correspondence to my address, to date, no one is listening.

Experiencia negativa: They have no idea about giving a patient proper medical care. My mother got to the ER Friday Dec12, 2014, by their transport, not by an ambulance, while she was suffering and time was an issue. Their transport took hours. Then the Hospital Dr. tell me that she had a severe infection for a long time, she also had fluid in her lungs, that her feeding tube was all rotted and for over a month she was not getting the proper nourishment and she was severely dehydrated. They only care about getting their money and their person in charge Ken is a real jek, and one can never ever speak to him, and their billing lady Carrie Townsend is a retarded idiot.They try and save money, and the nurses can tell you that the patient is ok but unless one visits on a daily basic they lie. lie. lie. The place is so far for me to go to and everytime I find another place they cannot even fax over the correct paperwork to get her transferred and they purposely do this so they do not lose patients, ha they only lose patients when they get so sick like my ma did, They are HORRID,a nd do not send a love one there

Experiencia negativa: The care is definitely below par. My Mom developed pneumonia and they didn't even know. I had to tell them she was ill. I never saw a doctor the whole time I was there and visiting her. She did say one would come by but only if I had complained about something. They seemed anxious to get her out of there and kept threatening to discharge her at any moment which made my Mom very upset. We eventually moved her over to a Board and Care facility. Nurses do run the place and don't seem to have the patients best interest at heart. It was hard to get someone to respond unless I tracked them down and asked them to help.

Experiencia negativa: Make sure you pay close attention to your family member. They will say they have a doctor they don't,the nurses runs the place. Some nurses are nice. And sometime they will not tell the truth but they all stick together

Experiencia negativa: I know I live out of state, but if you send a relative here, you really need to be on the look out. My Aunt was placed in the care facility and they made her use the same catheter for THREE MONTHS! She would beg them to put a new one in and they would tell her to stop crying and that she was fine. She ended up in the hospital fighting for her life with a blood infection that was caused because of this. Please look into other facilities and not this one if you are trying to place a loved one!
