For individuals in need of short-term, intensive rehabilitation after a hospitalization, surgical procedure, injury, or illness, we offer a 45-bed, post-acute rehabilitation wing with nursing staff available at all times.

Our full continuum of services also offers independent apartments and assisted living services, Medicaid certified residential care, and long-term care in our skilled nursing facility. Elim Park’s health care programs and clinical collaborations have earned us the honor of being a preferred provider for the Yale and Hartford Healthcare hospital systems.

The Elim Park Rehabilitation and Health Care Center offers a unique blend of rehabilitation services and clinical care located right on the Elim Park campus. Inpatient and outpatient services are available to both residents and non-residents to help address a wide range of medical conditions.

Key to these services is an interdisciplinary team approach to a patient’s care. This multi-disciplinary approach includes professional specialists in nursing, rehabilitation, recreation, and social work- all of whom work closely with the patient, physician, APRN, nurses and family members to create a personalized care plan.

Our spacious rehabilitation pavilion is fully equipped to meet the needs of old and young alike with new facilities, diverse programs, state-of-the-art equipment, and a highly trained and experienced professional staff, all available up to 24 hours a day, seven days a week.

Our Physical Therapy services include mobility retraining, balance restoration, therapeutic exercises, physical conditioning, and neuromuscular retraining. Bracing, orthotics and prosthetic services are also available.

We offer Occupational Therapy that includes self-care skills, activities of daily living, kitchen and home management, a bathroom and home assessment – all to enhance a patient’s independence and address physical limitations.

Our Speech Therapy and language pathology services include swallow evaluations, verbal and nonverbal communications techniques, cognitive and memory retention programs, and dysphagia training.

Post-acute rehabilitation programs are a great choice following a hospitalization with the goal of going home. When a patient is unable to return directly home from the hospital, rehabilitative care may be recommended by the hospital’s team of healthcare professionals, usually as a brief stay. Elim Park’s post-acute rehabilitation unit offers a consistent therapy program addressing individual medical needs in a supportive environment. Our short-term rehab team sets the main goal of getting each person back to their baseline level of functioning and discharging to the most appropriate level of care. Long-term care is a great choice for someone who needs 24-hour skilled nursing care and more assistance with activities of daily living and supervision than can be provided at their home or by home care aides. The resident usually lives permanently on the unit. Both levels of care focus on individual needs and care for a variety of conditions.

As of January 2022, most insurances are accepted at Elim Park. Elim Park is a Medicare and Medicaid-certified facility that also accept most of the more popular plans in CT. We are in-network with most plans through Anthem, Aetna, Connecticare, Cigna, Harvard Pilgrim, and Care Partners of CT, UnitedHealthcare, and accept all supplemental policies following Medicare. We are currently out of network with WellCare, and Humana. All insurance authorizations, copays and deductibles are determined by a patient’s insurance plan and not by Elim Park. The average length of stay for short-term post-acute stay is 1-3 weeks. Insurance plans often have guidelines about how long they will cover an inpatient stay based on a patient’s medical needs and progress. Elim Park has a dedicated team to assist with the discharge process. When coming to Elim Park for post-acute care whether a patient is recovering from an illness, accident, or surgery, they can expect to be cared for by a team of professionals whose goal is to accelerate healing & improve quality of life promoting independence. In addition to receiving rehabilitation services such as physical therapy, occupational therapy, and speech therapy if needed, patients are also provided with around-the-clock medical care including provider coverage 7 days a week.

For short-term rehabilitation, patients are admitted to the facility with the ultimate goal of getting home safely and quickly. With that goal in mind, discharge planning begins on day one of the stays. Elim Park’s initial process includes reviewing physician’s orders during medication reconciliation, implementing an individualized baseline care plan, establishing goals, and identifying barriers. During the stay, our collaborative team of therapists and nursing staff will be assisting with activities of daily living while the patient is working towards their discharge goals. Prior to discharge, supportive care services such as outpatient therapy or homecare services are set up for them to continue therapy and nursing care while they’re transitioning back home.
