Our team will help optimize your health, improve your quality of life, and reduce your risk when you have had a heart attack; undergone coronary bypass surgery, valve replacement, or catheter-based interventions within the last 12 months; or have congestive heart failure, peripheral artery disease (PAD) or symptomatic lung disease such as COPD (chronic obstructive pulmonary disease), emphysema, interstitial lung disease, or chronic bronchitis.
We’ll customize a physical activity and education plan to meet your needs, and a physician will monitor your condition and progress. You’ll also have access to an exercise physiologist, physical therapist, and dietitian to help support you along the way.

Jamie Connors, LCSW offers supportive counseling to assist with psychological needs related to financial issues and creating a stable environment. She also assists patients in building coping skills to help lower stress and improve day-to-day life.
Jamie earned her master's degree in social work from the University of Maryland, Baltimore, as well as a master's degree in public health from Johns Hopkins Bloomberg School of Public Health.
Kally Mohlke, RD, LD, CSOWM provides individual nutrition therapy to patients to meet their specific nutrient needs based on their diagnoses, lifestyle, and goals. She has experience in both inpatient and outpatient settings. Kally’s passion is educating clients about improving health through nutrition and lifestyle changes. She has a special interest in plant-based eating and concentrating on the prevention and management of chronic diseases and optimizing health.
Kally was born and raised in Boise and earned her bachelor’s degree in nutrition and dietetics from the University of Idaho. She is a board certified specialist in obesity and weight management. In addition to her passion for nutrition she also enjoys snowboarding, hiking, reading, and spending time with her husband and dogs.
Jennifer Shalz, MD serves as the medical director of St. Luke’s lifestyle medicine department. She develops and directs programs that use lifestyle interventions to help prevent, treat, and reverse chronic disease. Dr. Shalz is passionate about helping people not only live more years, but put more life in their years. Current programs under her management include cardiac, peripheral vascular disease, and pulmonary rehabilitation (referral required); nicotine dependence treatment; care transitions; the St. Luke’s Complete Health Improvement Program (CHIP); and the St. Luke’s Center for Lifestyle Medicine at the South Meridian YMCA.
Dr. Shalz is a diplomate of the American Board of Lifestyle Medicine. She earned her medical degree from Tufts University School of Medicine and completed her internal medicine residency at David Grant Medical Center, Travis Air Force Base. She practiced at three different VA medical centers after separating as a major from the US Air Force. She was most recently a hospice medical director and worked in supportive oncology at a cancer center prior to joining St. Luke’s.
Nancy Taylor, NP-C, CCNS focuses on using lifestyle interventions to help prevent, treat, and reverse chronic disease. Her areas of practice include cardiac, peripheral vascular disease, and pulmonary rehabilitation.
Nancy is a diplomate of the American College of Lifestyle Medicine. She earned her bachelor’s degree in nursing from York College of Pennsylvania, and her master's degree in nursing and post-master’s certificate in nursing as a family nurse practitioner and clinical nurse specialist from Gonzaga University.
