Experiencia fantástica: Hands down the most caring and amazing staff!!!! My relative moved in at 500+ lbs and bed ridden. The nurses and the aides have the biggest and most caring hearts in the industry. I visited weekly and never once was my relative dirty (mind you they were there for 2 years and were bedridden the entire time....never dirty!!!), never once did she state she had a bad experience, never once did she feel shamed for her condition, and she always spoke about how nice and funny and wonderful every staff member was. She grew especially close to a few and I appreciate so much the friendship and love they showed her. Even though I was there weekly they were with her everyday and showed her love, laughter, andexperiences, every day. The nurses and aides are one of a kind. The love they have for their residents is astounding. Their office manager is amazing. She is a fierce advocate for her residents and has a big heart for their families. She has to put up with alot of crap being the mediator between residents, family, staff, administration, state requirements, and so much more. She does all this with class. I am not sure how they managed to get so many angels in one place but I highly, highly, highly recommend this facility. 2024

Experiencia negativa: It has taken me some time to write a review of this facility since my mother's passing this summer, but at this time I feel it necessary for me to express my concern. In short, this facility is not equipped to care of its residents due to limited staffing, inadequate facilities and supplies. The photos of this facility on the internet are grossly misleading. There is limited staff who are stretched between floors and are working with limited/suboptimal supplies. During the summer the air conditioning was not working for almost 2 weeks (or perhaps more) and the patients had to be moved to different rooms due to the heat and they were given fans. How is this a plausible solution for people are expected to heal. On one of my visits, I transitioned my mother to her chair to make her bed, only to find that there was no linen to change her bed, this happened on multiple occasions according to my aunt who lives locally. The reason for mother's admission was for rehabilitation post surgery and wound care. My aunt had to advocate for rehab services to begin. The wound care nurse that I witnessed, did not have the correct supplies nor did she practice aseptic technique during her dressing changes. My mother's doctor's office called Gem City to ask if they had a certain device for incontinence and the nurse at Gem City told them that they had this device however, we were later told they did not; and they did not have the staff to ensure that she was changed in a timely manner. The house physician never visited or rounded on my mother. The administrative team and social work was only interested in placing my mother in their long term care instead of making sure she was receiving the care she was there to receive and transitioning her back home. This facility is inadequate and unsafe for patients. However, there were a few aides who were kind and caring with my mother and I cannot remember their names, but this does not change the fact that they are working in a facility with no support. I am a licensed healthcare provider in another state and have never seen a facility that operates in this way.

Experiencia fantástica: I was unsure & had my doubts about Gem City, but when I tell you that the staff here is amazing. From the nurses, the aides, P/T, O/T, speech therapy, activities, the DON & the administrator have shown me, my daughter & my family nothing but Love. I am so glad that I made the decision to choose this place. Everyone here makes me feel so welcomed & send nothing but positive views for my baby. I would definitely recommend this place to anyone. You guys rock

Experiencia negativa: I highly suggest that u don’t send anyone that u love here. I recently was here the staff had no knowledge of how to take care of my tracheotomy. The room I was placed in had no call light which was a huge concern because at night my cuff on my trach is inflated so I’m unable to talk. So when I gt sick the first night I was there I could call no one to help me I had to text my mom to have her send someone in my room. The D.O.N was extremely rude to my mom and fiancé. Also the ceiling fell in the my room close by my bed. They rarely ever checked on me….Please do yourself a favor and just don’t ever go Here… We definitely will be going to court soon ….

Experiencia negativa: I would not send my dog there. My husband went there after the hospital and I went and pulled him out 3 hrs later. If you love the person, DO NOT SEND THEM THERE. THEY ACTUALLY DESERVE 0 STARS.

Experiencia fantástica: The nurses and aids are nice and friendly even the therapist are nice they answer all my questions the food is good I enjoyed my stay there even though my stay wasn't long. And they do take the patience outside. They give snackes and make sure we have I r are ice water tea are coffee, Everytime I hit my call button they responded. I was on the second floor. Experiencia fantástica: Wow, my experience at your facility has given me peace of mind. I was totally stress and concerned about the decision, placement and care of my wife. Your nursing staff, including your administration have been supportive and professional. I'm very proactive and involved with the care of my wife and respectful of the protocols of the institution, but I was surprised at the extra length that your facility has considered to accommodate our request and needs. I would highly recommend GEM City Nursing & Rehab to everyone. Thank you

Experiencia fantástica: I have worked for many facilities in the Dayton area, this facility is by far the most superior facility in the Dayton area. All of the staff is so friendly and willing to help everyone. The nurses, nurse aides, therapist and all other staff treat patients as if they were their own family. The facility atmosphere is always very warm and welcoming. It is comforting to know that no matter what level of care our patients need, whether it is long term care, or respiratory care involving trach or vent care they can receive it here in our facility. I have had great feedback from our patients regarding the food and they love how the menu changes with the seasons. Our kitchen even catered our Christmas party this year and the food was amazing! Our activities department goes out of their way to ensure there is always something on the calendar for everyone to enjoy and they go room to room with a cart for those whom cant leave their room to go to the activities room. It is great to work somewhere that you feel like part of a family not only with your co-workers, but your patients too!

Experiencia negativa: Residents are all fully vaccinated but kept in their rooms (cages) all day because of "COVID" It's been this way for over a year. Awful awful place

Experiencia negativa: I never thought horrible nursing and management at a nursing facility existed until I came across this place. It is the worst place I’ve ever seen in my life. I’m sure there’s other places but this place right here has to be one of the top crappiest places ever! Neglect, mental abuse, and many many residents left in their bowels for hours. If you’re looking for a place this isn’t it. Trust me don’t do it! The food tastes moldy and old and sometimes not given or sometimes missing things. Poor poor poor management and poor nurses that just don’t care! Very few that are good there. Scum!

Experiencia negativa: This place is a joke. They are incompetent, rude, and have no regard for there clients. I'm currently here getting transferred out to another nursing home. The owner treats people like there trash. I was threatened by another client and they did nothing about it and told me that they couldn't guarantee my safety. How am I supposed to get better when I don't feel safe.

Experiencia negativa: If your loved one is a total care patient, do not send them here. My father, within 4 days of being placed here, lost a little over 10 pounds. Was always laying in poop. Never would bring him any water, but would complain because we asked for it. Nurse explained she’s the only one allowed to give the thickened water, and she’s too busy for this. Upon arrival, after accepting my father and saying that they could meet his needs, told us they never knew he was coming. This was coordinated with Ombudsman so I am pretty sure they knew. Come to find out, the administration knew but failed to inform the staff. We were told private room, no. Did not get that. There is so much more I can say about this place. After four days of basically no care, we removed him from this facility. They sent him on with no bath, just a wash of the privates with shaving cream to hide his odor. Don’t send your family here. The only positive is the caregivers are nice and he definitely got his medications on time. The rest... good luck.

Experiencia negativa: After being diagnosed with endocarditis, my brother was transferred from Grandview Medical Center to Gem City to continue his antibiotic treatment. Upon arriving, the facility had lost the order for both his anxiety medication and his life saving antibiotics ( the only reason he was transferred there to begin with). He was then transferred back to the hospital so that another order could be sent out. This is just the beginning of Gem City’s incompetence. There is an old crackhead living in the facility who is on antipsychotics. Multiple incidents have occurred with this man and the staff has done nothing about it. They allow him roam around the building all day and harass other patients. He is out here bodying people for quarters. He tried to rob my brother for his vending machine money. When my brother made a formal complaint about it, the crackhead threatened him and pounded on his door like he was the feds. Scared for his own safety , he approached the director in the hopes that they would resolve the problem. The director was a huge d*** and told him there was no way that he could guarantee my brother’s safety while he was under their care. One of the staff even offered to give the crackhead money to leave him alone (???). How unprofessional. The only positive thing about this place is how kind the nurses have been. Other than that, it is dirty , the food tastes like cardboard, it smells like piss, and there is zero security. Anyone could just walk right in. My brother is currently being transferred to a nursing home in Middletown. He would rather be in Middletown, a city where homeless people are literally getting dumped off, than be here. If you want your grandma to die faster, be sure to take her here.

Experiencia negativa: I've never been so miserable in a place like this place that the VA has sent me to for rehabilitation. I didn't have enough criteria for the VA rehabilitation floor. There is no rules of this center, as the do's or don'ts. No such handouts. The food is way below average. It's short on staff, below average. Smoke breaks are never on time. It interferes with meal time and medicine time's. I rate this center for Rehab a 2 out of 10.
