Experiencia fantástica: I recently had a two week stay at Avantara. Everyone was wonderful to me during my recovery. Nurses, CNA’s, PT and OT staff treated me like a friend. The facility is very clean and the food is good.

Experiencia positiva: I would give 5 stars to a nurses aide by the name of Tamika Herring. From what I saw, she ran circles around any other staff member. She was constantly busy going from room to room. Tamika was particularly attentive and kind to my 93 year old Mother. When my mom would push the nurses button, if Tamika was working she came right away!! On the last day of my mom's stay, Tamika used her lunch break to give my mom a much needed shower! I would also like to thank Sayra Nava, the Social Service Director for expediting my Mother's discharge from Saturday to Wednesday. Cutting through the "red tape".

Experiencia negativa: Where do I begin the staff there is rude. They have no communication. They lie about things that they do to the residence. My father was staying there. They did not rotate him. They barely checked on him, and because of them not rotating him. He had a open sore on his bottom and he also had a catheter that whatever nurse put it in did it completely wrong and had my father backed up and also had a blood cough going on that he was peeing blood I do not recommend this place. This place is horrible. They’re rude they’re arrogant they don’t know how to communicate when things need to be done. They feel that you are I do not recommend this place. This place is horrible. I mean the list goes on and on.

Experiencia negativa: this place has given out other peoples personal information to me like their social security number and DOB, they also cant seem to set up the appointments that are needed for follow up. They dont accept responcibility for their lack of actions, making it so thatt i have to do their job in scheduling. i would not recommend this place for anyone. i noticed that someone else previously complained about staff taking a long time to respond to patients, its wonderful to see thatit is still a problem at this facility The staff that checked my father in didnt know how to spell the word "jacket" and asked me to find it for him on the inventory list... how is this place aloowed to opperate still?

Experiencia negativa: It should be noted that my wife left a review about this place shortly after we placed her mother into the facility. I had an administrator contact ME and try to get my wife to remove the review she left. Why would an administrator want me to convince my wife to rewrite, or remove a negative comment unless the place was as bad as she suggested it was. Further, judging by the other reviews I've seen come pouring in over the last year or so, I see nothing has improved. On the contrary it's somehow getting worse. Hire some staff and maybe don't make QUITE as big a profit as you'd like, and I'll bet every bed you have will be filled with happy residents, and families will leave glowing reviews. Otherwise accept the poor reviews for the poor job your doing.

Experiencia negativa: I decided to take my father there from Sherman Hospital to this place Aventura in Elgin so my first complaint was that they do not clean at all that’s why I decided to move him like right away after that the next day that I went to pick him up they didn’t give him breakfast so I finally take him out of there like right in the next day or too a new facility and when we get here we notice that they gave me another patient’s medicine so that’s not right if you’re So if you’re thinking to take your parents to this place I’m just saying it’s not safe for them

Experiencia fantástica: I had my hip replaced on 4/22 and went to Sherman West Court on 4/25. I never heard of this rehab center and was very leery of what was in store for me. I have never had such great care! My therapy was 7 days of week. All of the Physical and Occupational Therapist were top notch in their job! I stayed almost 3 weeks and because of their rigorous therapy and care, I am now walking by myself without a cane or walker. The nursing staff, Especially Marge, Winnie, Ann, and Reyna, were the best of the best! Best of all, the food was great! Thank you and hopefully this review will help others make the right discussion for their therapy at Sherman West Court. Thanks to all, again...

Experiencia negativa: Do not send anyone here. My husband was here and I needed to take him out after 2 days because of extremely poor care. These people do not care about their patients. Husband had to call my Daughter at home to come to Avantara because he was in so much pain and no one would come to help him. One lady walked by and said - Oh - he's in hospice and kept walking. When my daughter got there he was sitting in his wheel chair with nothing but his underpants. She dressed him. He cannot walk or get out of bed, so someone had to put him in the wheelchair in that condition

Experiencia negativa: Poor service. Staff does not return phone calls. They can take 4-5 days to give an update about a loved one. There is no transparency. Pictures do not reflect current facility. Walls are dirty and run down.

Experiencia negativa: This place is not a good place to be for your loved ones. There's long wait for the staff to respond on call lights and even if they do attend your call lights they will just turned it off and will promised to come back and you'll end up waiting for another hour or even longer. I understand that they are understaffed but whats even worse while some staff seem to worked their butts off, others are busy flirting and displaying their affection sitting on the lap of this young dude. C'mmon do the management allowed these PDA at the workplace. I've come to this place many times and will always see these staff teasing at the back of the nurses station doing nothing. Residents need help and would like to be assisted most of the time. I dont think the administration knows what is going on. It's just so sad there are so many wonderful people working there but there are still so many that are not supposed to worked there. Just be cautious sending your loved ones at avantara because they cant take care of your family members the way you wanted them to be taken cared of.

Experiencia negativa: This place has poor conditions. The rooms are dated and old. Paint is pealing for the walls. The beds and hospital table/tray are rusted. The staff is not very helpful. They do not share knowledge amongst each other. Nobody seems to help each other and “it not my job” is a common phase. I was walking around the facility looking for the manager after requesting him to see me, after 10 minutes of him not showing up. I had asked 4 workers if they had seen him and they didn’t even know who the manager was. Their was garbage on my mothers room floor from Friday, when I was there until Sunday when I returned and had the manager and housekeeping come and empty the trash and clean up the floor. My mothers nurse let her sit in a diaper for an hour and a half because she was “going on break”. My mother has not been bathed in over 6 days. When I asked they said “we do that on Tuesdays and Thursdays”. She was there on both of those days without being taken care of. She had the call light on for the nurse for almost 3 hours when I showed up to have to go and get a nurse. This place in NOT RECOMMENDED IN MY OPINION!! I have also reached out to the health department for these conditions. It is an Absolute disgrace how they treat patients.

Experiencia negativa: After my surgery my doctor recommends this facility to recuperate. I honestly would say they have poor service. 2hrs respond to call lights, severely understaff and rooms dirty. The only good thing about this facility is the wound care team and rehab. They were wonderful they would change sheets, bring meals and even assist me to the bathroom. They kept me active and moving. They go above and beyond and surpass my expectations to make sure I'm well taken cared of. And the wound nurse has a good healing hands and her laughter is remarkably contagious that eases my pain and worries. They were all amazing.

Experiencia positiva: I would definitely give this place 3 stars on service and overall atmosphere. I’d give them 4.5 stars on the medical staff and physical therapy though. My mom stayed here for a couple of weeks and overall, she was happy but there’s definitely improvements to be made. The place is not as luxurious as it looks online. The rooms are just okay. We also thought you could customize your meals…spoiler alert: you cannot. You get what they serve you for the day. Also, they are severely understaffed. So while my mom’s nurse and physical therapists were incredible, the other staff who changes sheets, brings meals, etc. were just lacking. It would take them up to 2 hours to respond to her calls, which is crazy given the level of help patients require. The weekend wait times were even worse! And finally, there’s a hair salon for patients but when my mom was at this facility, the beautician only catered to white women’s hair. I spoke to the facility director about this and how it’s basically discriminatory (charging everyone the same price but only providing perks to some) and she said she would fix it so that Black and other patients (even men) could take advantage of the salon. I don’t know if that’s been fixed yet. My mom stayed there in December of 2021. Let me also add that I called every single day to check on my mom and visited her in person every 1-2 days. For those who don’t have the time to check on their loved ones as frequently, I’m not sure I’d want my family member staying here.

Experiencia positiva: My brother came to Avantara from Sherman Hospital following a Tibia Fibula fracture. He was there for 2 weeks. Everyone was friendly and welcoming. He was made comfortable and assessed by the staff. He enjoyed the food and interaction with all of the staff. The Physical Therapy department kept him active and moving. We were thankful for your care and compassion. One concern I had was his hygiene needs. In the 2 weeks he was there he only was given 2 showers and one bed bath. He rarely had his teeth brush, and with his dementia and physical limitations he would have benefited from someone assisting him or at the very least, set him up with a basin and washcloth. The evening of his leg surgery at Sherman Hospital, the staff was unable to get anyone to answer the phone there to arrange transportation back to the facility. The family transported him back to Avantara and waited many minutes for someone to answer the doorbell and come to the locked door. It appears in general that they are short staff and chaotic. I hope that they are able to find additional qualified staffing soon.

Experiencia negativa: This is a Legacy Healthcare facility that is under staffed and very much in need of additional caregivers who can attend to the needs of their clients housed in their Elgin facility (Avantara Elgin). The quality of care is not what is promised and implied. Complaints get ignored until one of us get very stern. What will it take for change to occur and our issues to be addressed?

Experiencia negativa: Administration didn't have a clue as to how to do things with Medicare or ambulance service. Can't get through to patients on the phone. You get transferred around the building and then they hang up on you. Grandfather went there for a month and a half for physical therapy and was in worse shape when he got out.

Experiencia negativa: My 95 year old father went for "a week or two" of rehab after a fall. After a month of "rehab" he was not improving and he was losing weight because he would not eat the food. Even though my father is 95, he is still decisional. He did not want to do rehab and he wanted to be home with family. He absolutely hated the food, he said it was the worst food he has ever been served. Of course the facility was against discharge and encouraged another month of rehab. I seriously question the ethics of this facility. I have been a CNA for 15 years and I have seen a lot. I would never recommend this place

Experiencia negativa: This facility is unprofessional and and unethical. My mother got a clearance note from her surgeon to exit the physical therapy program. We were told that she could not be formally discharged until she saw the doctor at this facility. The doctor was in two days that week but did not go to see her, so that she could be formally discharged. They were purposely stalling so they could hold her over the weekend and continue to make a profit. When we called to get my mother out of the facility, we were ignored, sent to voicemail, and hung up on. My mother tried to walk to go see the case manager and was told it wasn’t in her best interest to do so. It wasn’t until we physically came into the facility to get her that they would respond to us. Furthermore, she had unusually leakage with her wound and she asked to go to the ER. They would not take her until we again showed up and demanded it.

Experiencia positiva: My mom spent her last six months of her life at the Avantara Elgin nursing home. She was 90 years old and well cared for by the Avantara staff. The facility was clean and the staff was professional and respectful. When hospice was called in, both the hospice team and the Avantara team worked well together to provide my mom a dignified end of life. I would recommend Avantara of Elgin as a public aide facility.

Experiencia negativa: My 90 year old grandma was sent here after having a hip fracture and we were concerned about the care she was receiving. Her call button didn't work at first. She had an isolation sign and PPE outside her room but only some caregivers wore the proper PPE. Other times the isolation sign was still up but no PPE outside her room. When I asked about it on multiple occasions nobody was able to give me an answer. Oxygen was left on in her room when she wasn't even supposed to have oxygen. We had concerns about her actually receiving therapy. Once she was left laying on a urine soaked gown. Many times we would visit and there would be a full cup of water left on the table out of reach and no straw in the room. We had to go find straws on multiple occasions. The last day she was there they were rushing to give her an IV because she was dehydrated. My grandma was there for 20 days and at no time did they provide any kind of overall progress updates to my family. We had to request a call with the social worker to gather specific information and get an overall progress update. It is understaffed and nobody was around to help or answer questions. I would be cautious of sending your loved one here. If you do, make sure you ask lots of questions, get the answers you're looking for and advocate for your loved one.

Experiencia negativa: IF YOU LOVE YOUR FAMILY DO NOT SEND THEM HERE!! This 1 star is being generous!!!! My father was sent here from the hospital for a week or so as a part of his cancer treatment to work on his physical rehabilitation. He wasn't thrilled about going to a rehab facility but agreed for the sake of regaining his strength. He arrived there in the evening, and by morning he was not acting like himself and was desperately begging to be let out of the facility. His medications usually made him very drowsy, but that morning he was extremely alert, hyper and claiming to be feeling very VERY bad. After hours worth of us calling the facility and him desperately calling for help (we were unable to visit him because of Covid restrictions), he was finally released back to the hospital ER for evaluation. Within MINUTES of him being in the ER they found that he had 2 strong medication patches on! Not only was he given a new patch a day too early but no one even bothered to take off the previous patch!! This tells us that he was very poorly evaluated (or someone simply didn't care) as the patch was clearly visible!! Thank god that he was screaming for help like he was because if not then we could have faced a huge tragedy due to the medication overdose!! He has been through so much with his recent diagnosis and this trauma should have never EVER happened! He was lucky enough to be able to fight for his discharge, but what can be said for all of the elderly patients who cannot fight for themselves?! This whole situation is absolutely unacceptable!!!! Please please PLEASE check on your loved ones and believe them when they call for help!!

Experiencia negativa: Do not send your loved one to this facility unless you do not care about them. They are understaffed especially for nights and weekends. My loved one spent 2 weeks at this facility and was very vocal about his quality of care. He told me of residents screaming for help. Call buttons are often unanswered for up to an hour or more. I personally called the front desk several times to ask for a nurse to go to my loved ones room to give him his meds, to clean him after a bowel movement or to help him to get back into bed from a wheelchair. He was afraid to eat many times because he didn't want to be left possibly sitting in his own waste for up to an hour or more. I feel our concerns were not addressed at all by management at the facility. I thank god we were able to get him out of there and he is happily recuperating at home now. Even this was a struggle.

Experiencia fantástica: I overall love the facility my number one reason is they put the residents first. They have a beautiful therapy gym wonderful staff my interactions with the staff is very welcoming. There’s not a lot of places where the family is offered to reach out to the administrator and others on their personal phones. They’re willing to go the extra mile to give the families relief doing such a difficult time. Such a beautiful clean facility and the work they continue to do is amazing.

Experiencia fantástica: Let me start by saying. This is my first time having a loved one in a rehabilitation center. I went in not knowing what to expect other than what I read on Google. Hands down the worst thing to do. Yes absolutely the first few days was rough. But once thing's got going. My husband was more comfortable the staff got to know him. Now I feel he is going to be taken care of. He will be home to me in no time. I don't want to mention name's only because I don't want to leave anyone out. Please just remember everyone has the same thing in common. The health and well being of our love one's and for their Pacient.

However, one common thing everyone has is to ensure we give the best care to our residents. It is an ongoing and daily process but we will never get tired of thinking and implementing ways to make the experience great for our patients and staff.

Just walking in I noticed how nice and clean the facility was , staff was very friendly and helpful with any questions or concerns I had . The rooms are have great space , clean, comfortable and gives my father everything he needs as he transitions into the new facility. Nicole Jones was most of all the most helpful with helping me and my father with all questions and making sure that all the needs he require are met and keeping my father happy at his stay. Experiencia negativa: Not a place to send a loved one. My grandmother was there and during her time there I had staff argue with raised voices in front of me, I had nurses tell me how to skate around head nurses because they knew the care my grandma was getting was poor, I heard other families yelling at staff because of the care for their family. It was a sad place and dirty place. (See pictures taken ONE DAY after my grandma was there Edit: could not figure out how to post pics but there was liquid on the floor, empty cup, there was tape, and a cap and middle part of a syringe (no actual needle) on the floor) once my grandma started walking better they pushed her wheel chair up agains the “nurses station” and locked her in place as their version of watching her, she tried to get up and fell with “no one to witness ”. Unacceptable. I am not saying all the staff are bad, they are understaffed and cannot care for your family members the way they deserve.

Experiencia negativa: I would recommend that management pay closer attention to the nurse/patient relationships. I think weekly check-ins from management with patients is a must. I would recommend families of patients to be fully aware of what medicines are being administered and make sure none of the pills are "swapped" for a lesser or different brand. Ask questions. Always. If you or your loved one never took, say, a big pink pill, and one starts magically showing up, question it. Definitely an understaffed facility, quite possibly not a well taken care of staff, and as a result, bitterness and laziness may sometimes show from certain nurses and aids. Not all though, many are very kind and the patients seem to really take to them. Unfortunately, it only takes 1 or 2 bad apples to spoil the bunch. I agree that this is a far better facility than most, but it is in dire need of a staff overhaul.

Experiencia negativa: My suggestion would be more training and team building for your nurses and CNAs. The administrative staff has always been quite wonderful. However...they aren't overseeing the patients. I suggest more training and resources to support and educate the staff. So many are wonderful people working there, but so many who really are not. My suggestions would be training in reaction. Training in talking to families and patients with an emphasis in empathy. Training in hospice care. PLEASE MORE TRAINING IN HOSPICE CARE. IF NOTHING ELSE, MORE TRAINING IN HOSPICE CARE. There were episodes where our family member was helped into the bathroom and then forgotten. Left on the toilet at night while the CNA and Nurse were on lunch for a half hour. Unacceptable. Now... lets talk bed sores... check your loved one for bed sores. Things will happen, but if you aren't checking...no one else might be either. This is abuse and negligence. There's also often patients in the hallways looking for someone to assist them. The typical response time when the call light is, well there is no typical response time. It can be 10 seconds to 25 minutes in waiting. I don't believe there is an alert system at the stations that allows them to know when the call light is on. They have to notice it as they walk past. UNACCEPTABLE. The kitchen can be negligent to the patients requests and dietary needs. It's bizarre to me that a patient may be diabetic, but still gets a sugary dessert on their plate. Then, nurses come in and act surprised that their blood sugar level is so high. This facility was a bit better when it was known as Sherman Westcourt, but unfortunately it has not changed for the better under new ownership. I understand being short staffed, I understand being overworked...but patient well-being and safety is at stake. Management, owners, investors, whoever it may concern, take better care of your staff so they can take better care of these patients. It is a disgrace to healthcare. It truly is. There truly is no better way to say this other than Avantara Elgin, get your act together. You are failing your patients, their families and the Hippocratic Oath. These patients are not going to get better until you do.

Experiencia negativa: I've been staying here over a month, the care here is terrible. I was originally told I could have a private room, it was a lie. I have to fight to make sure I get the proper medications, including correct dosages of insulin. It usually takes close to an hour for them to answer a call light. Cleanliness is terrible, there are spills and litter all over the hallways. Wallpaper is peeling off the walls. Staff does not follow CDC protocalls, wearing masks on chin, not washing hands, etc. If I speak up about my care, they swear and yell at me, and then blame me and say I'm the one with a bad attitude. I've overheard CNA's talking about how much they hate their jobs and can't wait to leave. If I need anything I have to ask multiple times to get anything, if I get it at all. I've gone 2 weeks without a shower before, usually at least a week (I'm supposed to get at least 2 a week). it takes over a week to get laundry back, and sometimes they write with marker on the clothes, even though they claimed to only use iron on tags. I get no privacy, even when talking to doctors. They claim to have a diatician and personalized and healthy food choices. This is completely untrue. Everyone gets the same menu, and most of the time they don't give you what you ask for. I'm diabetic and they feed me mostly carbs and sugary foods, very little protein. A few of the nurses and CNAs are quite nice and helpful, but most are rude, disrespectful and negligent. The list goes on and on. If you love sombody, DO NOT send them here, it is a living hell. I believe most of the 5 star reviews are by friends of administration, or staff themselves, they are utter nonsense.

Experiencia positiva: I want to know more about what’s going on with a relative that I have in this facility. Because of the Covid crisis we cannot visit this relative, we had one window visit with him. When I call I never get the nursing staff to answer. His doctor says it’s out of their hands. They have had 16 residents test positive and 12 staff members, and they’re recording says that they are following CDC guidelines. But something is wrong because it keeps spreading & going higher. My 90 year old family member can’t hear his phone ringing. He has a skin condition that needs attention, oatmeal bath & application of a antibacterial ointment I can’t even get any information on that. His social worker has been on vacation and I am waiting to hear from her to get his status update. I just feel very helpless. My sister and I are the only family he has left and I know he feels like we have a abandoned him. I am his POA so I should be allowed any of his information. We are not even being told if any of these residents have passed away from the virus.They need to set up some sort of teleconference with the families of the residence to assure us that our loved ones are being cared for every day and their needs are being met, especially when we cannot have a conversation with them.

Experiencia fantástica: Starting with the nurses. There very good at what they do and will go out of there way to get me what i need for my treatments. House keeping also does a fantastic job very polite and. Keeps this place very clean. The kitchen employees are on the top of there game. The food is always great. And when i go to the kitchen in the morning before they open to get coffee. They have no problem getting this for me. And they are all on first name with me. The administration people are great. They welcomed me smiles and made my stsy very comfortable. I would recommend AVANTARA to anyone who needs care of.

Experiencia fantástica: Very clean facility. As well as very friendly, you can ask anyone for help and they are willing to help with whatever issue you have at hand. I can’t wait to see what it looks like after all the renovations are done.

Experiencia fantástica: My dad is here and they treat him real well. He loves the director and nurses especially. The cooks feed him well, sometimes too well but he’s happy and discharging soon. The staff are always really nice to me too.

Experiencia fantástica: Mom had made great strides while at Avantara. She’s doing great at home now. I’m putting this review up because my Mom wasn’t expected to make that kind of recovery. I believe the people made the difference. The therapy, nursing and mangers always go out of their way. The staff there still check up on her because I hear her on the phone with them. Great place for rehabilitation.

Experiencia fantástica: I’m not sure about the comments before but I know they’ve changed a number of things here lately and for the better. They have new renovations and new management. I’m sure it’s not an easy job dealing with peoples health and families, especially ours. They have a great group of people there. The management and nursing are all great to work with. Thank you for the kindness, therapy and care you’ve given our dearest sister.

Experiencia fantástica: I have been here 2 months now this is my new home. Everyone has made me feel comfortable. The cna’s are awesome on all shifts my opinion. Also housekeeping and maintenance outdo themselves.

Experiencia fantástica: Management team is awesome very knowledgeable answered all our questions treated us amazing when we visited. Also wanted to mention building Is getting a brand new physical therapy room and it looks beautiful along with the rest of the renovations being done. I would definitely bring my loved one here!!

Experiencia positiva: Food, physical, occupational and recreational therapy are all great. The menu is high in carbs though. Nursing staff is lazy, putting off everything they can on the CNA's. The CNA's are great but there are not enough of them. The bats ones there are Rachael, Ivan, Hector and Kevin. I am a frequent flyer there due to my MS. Always very clean and staff is friendly.

Experiencia negativa: Worst place ever to have someone you truly love because of that place my love one almost died if it wasn’t because I was there yes it was like I was the nurse with no license. You can push the button many times tru out the day and no one will come in. That’s what I call neglecting. Funny how management hasn't done anything about it!!! They only want to collect money from insurance. Unless you as a patient don’t speak for yourself or don’t speak English you don’t matter to them.

Experiencia negativa: Wound care needs to be better. Short staffed. Staff is great! Since due to poor wound care after5 weeks am in Sherman hospital! Should have been home 3 weeks ago..before u go there ask hospital staff! Don't listen to sales pitch. Elderly people treated worse than me.(57).

Experiencia negativa: I believe the staff here does definitely care, but I also feel the facility is terribly understaffed. Sadly, not enough eyes on people... I do feel that it is very important, that if your loved one is admitted here, to be very engaged in the process. Make sure you have proper power of attorney established, and be very expressive in any concerns that you have. You know your loved one best. It is difficult to get ahold of nurses, when you call past visiting hours. You can wait long periods when you press your help button from the bed. Linens that have been soiled, have gone unchanged for a few hours. If the patient does not have a voice for themselves, I worry for them. I cannot stress enough, that you need to be present. If you are present, then I believe the process can be a bit smoother. The plus side is that there are medicare and medicaid beds, but overall i'm not 100% impressed with the experience. Menu selections can be messed up. Food is often cold upon delivery. The communication from doctor to nurse to patient is not ideal. The sad part is, this is a better facility than most. I don't know if the problem is a lack of funding, or simply poor management, either way... as I said, the people have hearts, but are clearly overworked and sadly the patients are the ones who suffer.

Experiencia negativa: Don't go here unless you are ready to die or are prepared to fight your way out. There is NO middle ground.

Experiencia positiva: This place is clean and does excellent rehab. It's really hard to find to find a cna when you need help. Nurses find whatever reason to not help out, call for transport and then complain about too quick/too slow. Overall decent place out of all choices

Experiencia negativa: Poor quality of care, they do not call 911 for actual emergencies let alone recognize when something is wrong. The lack of attention is pathetic, DO NOT let your family stay here. The stars don't go in the negative soo....

Experiencia fantástica: My mother is there after a fractured pelvis for rehab. Awesome job by staff! A special shout out to Lani, RN. Belle, Sarah P.T. Jamie O.T. U guy's are the very best!!
