Experiencia negativa: Giving this facility 1 star is too many. The Skilled Nursing portion of this facility is dirty, disgusting, under-staffed, and sad. My mother was released to this facility the last week of August 2024 so my experience is recent. The CNAs are doing their best, but are spread extremely thin. The food is beyond disgusting and nothing was covered - who knows what got into the food before it arrived. There is no central A/C, they have large commercial portable A/C units in the hallways. Spare equipment and garbage is everyone making it a hazard for anyone walking around. We finally got to the point where we DEMANDED discharge or we would just take her home. AND - here's the best part, she went home from this facility with a bacterial infection known as C-Difficile. This infection is transmitted by physical contact and is linked to fecal matter! She got so sick she was re-hospitalized within days and eventually went into septic shock. AVOID THIS FACILITY!

Experiencia negativa: First off the things that these ppl are posting on account of Windsor Elk Grove Care, are true!! It's dirty, there is little to no air flow through that place so it smells. The hallways are carpeted, and the rms are linoleum so they look disgusting & are stained from only Lord knows what at this point. !Just For Sanitary Reasons, All Flooring Should Be Linoleum! My relative called for a 'nurse' for a 1hr ½ to get help W/the potty. Finally, a 'nurse' showed up, too little too late, unfortunately. Someone though was having a good ol' time down the hall @ the 'nurses' station @ 10 p.m. from the hooteren & hollering they were doing it was someone's birthday party or something more exciting than doing their jobs. But no one else could hear anything from the sick & elderly that the 'nurses' were supposed to be taking care of @ 10 p.m. I'm all for the 'this land is my land, this land is your land' thing but when I, the voice of my sick family member who could not speak for themselves @ that moment, is not understanding you because you're now deciding to speak in a foreign language to your colleagues, apparently abt what I had just asked you. They standing next to both of us; their English was just fine, I assure you. That's just disrespectful & not classy. I've spoken with W/a few Kaiser nurses, (Kaiser is affiliated W/Windsor) upon their discharge from this place & none of them, not one has had anything even remotely nice to say abt that place. The 'nurse' didn't know how to reset the call light, located on the wall behind the beds, in the middle of the other occupant. These pictures & video were from the beginning of May so it wasn't too awfully hot outside yet but I was starting to perspire just sitting there visiting, so to know that the oscillating floor fan is the only real air circulation that the patients get is or should be an issue. The bigger air circulation 'system' you can see in the picture only circulates the air already inside the building. No open or cracked windows', why because if one of their patients were to fall out of their windows they would have a major lawsuit on their hands. The windows' should be up to code for that sort of thing. Wouldn't ya think? In fact, the only nice thing I have read or heard come from there is from the ppl who work there. The pictures say a thousand words...

Experiencia negativa: This place is dangerous. My elderly mom fell two times in four days. She just had hip repair surgery and they left her alone in the bathroom twice and she tried to get up and fell. She ended up in the hospital with a brain bleed. Do not let your loved ones end up here.

Experiencia negativa: If I could give this place 0 stars I would. My mom is currently a patient here and they are extremely negligent in her care and transportation. Olivia the nurse had said there was transportation for her dialysis three times a week, and numerous occasions they have had my mom call us last minute at 4am to pick her up. I tried to talk to who ever is in charge but they said too bad wait for Monday, no one here. As I got there this morning there was another patient being transported so what’s the problem? This morning she had scheduled transportation for the wrong time so they canceled and had her call us last minute at 4am when she should already be at dialysis and that’s on Olivia the nurses mistake. Hold yourself accountable.

CANT WALK DUE TO TINGLING in feet BCUZ OF NUERORAPATHY NO PHYSICAL THERAPY..NURSES WON'T GET MY MOM(WITH BACK PAIN) OUT OF bcuZ THEY HAVE NO IDEA WHAT SHE n do must talk to therapy..ummm??!! shes there for physical therapy SO SHE CAN COME HOME&b ae to wlk..im thinking they should know by now! Experiencia negativa: Highly annoyed and just wasted 1 hour of my time waiting around for Alma the DON to tell me they had no positions available for certified nursing assistants. I called last week to ask about CNA sponsorship and I spoke with precious and she said they were hiring (specifically told me to come in and fill out the form).. Had they just told me right away and not have had me sit around filling out two applications I could have been going elsewhere. A big waste of my time and energy— I was sitting around just sitting for 1 hr and the new front desk lady didn’t know what alma looked like so she walked by a few times before she stopped her to say I was there for her -this was updated bit happened a 2 months ago

Everything from the staff to the administrators should be lined up like ducks in a shooting arcade and used for practice dummies. Experiencia fantástica: Finding a quality skilled nursing facility in this area is hard to find because there isn't that many, but Windsor took me by surprise with how attentive and dedicated everyone is in each of their departments. A fine well oiled machine from admission to discharge. I will be recommending Windsor to my family and friends in Elk Grove if ever it's needed.

Experiencia negativa: My sister is now in this facility and it again has no real working air conditioner, just a fan in the room. This was the same situation 3 years ago when my husband was in here. Isn’t this a concern for staff and state auditors to be concerned with? Amazing how the HVAC works when out side people come to observe and check on efficiency and then it goes out about 4 pm and you need the fans again to just get some air.

Experiencia fantástica: My father, George was in Windsor for about two weeks. Every employee was professional, treated my dad with great care and all around just nice people. Special shout out to the two CNA’s who did so much, Lori and JP. You ladies are a class act.

Experiencia negativa: My uncle reside here for a short time until he passed from his terminal cancer in early 2020. He was quadriplegic with cerebral palsy and had an intellectual disability. However, he was very alert and oriented x5 and can communicate quite well. He would let us know that some of his CNAs treated him poorly and were rude because they assume he didn't understand them because of the way his disability made him looked. He was placed in a room with 2 other roommates. This room was quite cramped and old looking. He had a roommate that was verbally aggressive and yelled constantly that it would terrify my uncle and put him under so much distress. We requested for a room change many times but management kept saying they had no other rooms available. I ask if they may accommodate one of their short stay rooms for my uncle to rest in peace when he was placed on hospice care with his terminal illness but they declined and refused to make any accommodations to help him alleviate with his dicomfort as he was passing. We couldn't find another placement in time and were devasted when our home couldn't be an option as it was not adequate enough for his equipment and needs. Our family could barley gather and visit in his room due to the tight space and his roommate would be verbally aggressive or yelling at the staff while we visit. It was uncomfortable to hear and to also bare the fact that my uncle couldn't get the peace that he needed while he was passing. My recommendation is to not take your love ones here for any comfort or long term care. They have a nice rehab for short term care but I wouldn't recommend staying any longer than the recommded 14days. Save your 100day of Medicare SNF days for a more appropriate facility. The hospital social workers can give you resources to placement agents that can better find you a proper place for your loved ones for longterm care. Please do your research and make sure your love ones are properly cared for.

Experiencia negativa: Husband sent there for rehab for fractured rt hip, rt ankle, bl ribs and left arm so he pretty much was bed ridden. He had a small sore his buttocks when transferred there. He had an air bed kept deflating, power kept going out 3 times a day, no air conditioning. He would lay on medal of bed frame but kept telling nurse his buttocks hurt. He need pain meds and kept getting worse. Instead of finding the cause of pain they kept giving pain meds. Finally after a week doctor checks and he has an infection causes him to be taken to Emergency and he may have tonhave some bone removed in his buttock. Whenever I asked why didn't the nurses check the root of his pain before that instead of giving pills, they tried to.say they thought it was pain from his fractures. I said no cause he wasn't even taking narcotics when he arrived. If he started asking for stronger meds then you should see why. They left him in his poop and pee for.over an hour which I believe help lead to the infection.

Experiencia negativa: They need an option to give zero stars. This place needs to be shut down. It's extremely dirty. When we arrived my Dad had been signed up with them for 2 days by the hospital. The transport was 2.5 hours late, yet when we got there they didn't have any information on my Dad and the intake nurse was MIA. There was one lady who had an RN on her tag but that is highly debatable - I told her there were registration forms I needed to sign and she didn't know where they were and said she would get the intake nurse. I waited and then she turned to me and said can I help you with something? This happened 3 times before I lost it. She was walking around saying so many things are going on right now, there was nobody around and she was just sitting at a desk. She told me they had 7 people coming in that day and I looked around and said I'm the only person at this front desk and you aren't doing anything and you don't even seem to know what is going on. My Dad's room was dirty, used gloves were on the floor, wrappers from some kind of medicine were on the floor, the sheets were stained, no pillow was provided, his dinner was there in his room when we got there which was 8pm so I had no idea how long that had been sitting there. He is bed ridden and had to go to the bathroom and we couldn't get anyone to help us, I had to ask the "RN" lady at the front desk for a plastic urinal and she looked lost. We only got someone to help when I started throwing a fit. One patient was wheeling his chair down the hallway asking a nurse to turn on this tiny oscillating fan that was attached to the wall about 8 feet up and looked like it was from the WWII era and said it only cost pennies to run it and they could afford to run the fan. There is no A/C, they had some awful portable AC unit in one of the hallways. Red flag at the door was a notice posted asking everyone to go and write 5 star reviews and then they would get a taco party. When we checked the place out earlier that day the guy giving the tour took us to the communal area where people are supposed to eat or spend time with family, there was something wet on the floor and he just looked at it and said "well that's a workers comp claim right there" and then proceeded with the rest of the tour. I highly doubt that mess will get cleaned up. This is a case of taking insurance money and I would guess the owners are using it to take a yacht cruise or something because no money is being spent on staff or the facility much less the patients.

Experiencia fantástica: My aunt discharged from Windsor after spending a week and a half there receiving physical and occupational therapy. The staff were attentive, professional and friendly. The staff in the rehabilitation department were especially impressive. My aunt loved James and his sense of humor and Mary Jane who assisted her with preparing for discharge was great. In preparing for discharge, the social worker, Marisa and the R N, Kristine, were so helpful. They explained everything well, took time to newer all of our questions and made sure we had everything we needed for her to return home safely. This may be an older facility, but the staff are wonderful and caring and the place is well kept. Food wasn’t bad either my aunt said. I would definitely recommend Windsor Elk Grove if you’re looking for a facility in Elk Grove.

Experiencia negativa: This place is awful and really needs to be shut down. First red flag is the garbage and junk all over the grounds. Next was the old wooden door that barely works, no handicap button to get the door open. Next, right inside as you wait there is a poster on the wall asking people to write a 5 star review so they can have a taco party. I don't know what they are bribing all the 5 star reviewers with but those people should be ashamed of themselves. When I tried to look those reviewers up they have no photo and no other activity on google except for their 5 star review of this place. Listen, if you love the person that needs a skilled nursing facility don't send them here. I would recommend ACC in the pocket area or Advanced Healthcare. Even if someone I hated was being transferred here I wouldn't let them. It's dirty, there is no A/C and I highly doubt the nurses working here are actually licensed nurses. One of them didn't even know what a plastic urinal was when I asked for one, I would hate to see how they reacted if I actually asked for something medical.

Experiencia fantástica: 5.0 star rating 3/29/2016 We rate Windsor Elk Grove as the best of the best skilled nursing care facilities, both short and long term. A few months ago my elderly mother fell out of bed and broke a couple of bones. After 11 hours in Kaiser Emergency, Windsor opened their doors to helping my mother rehab. We had visited several facilities prior to this happening, and none of the other places had the cleanliness, team of caring staff members, activities, great food, physical therapy dept., doctors, nurses etc., to the degree we saw at Windsor. Then, we were saddled with having to deal with her medical coverage and paperwork. At the same time we felt lost, and devastated, from mom hurting herself, and didn't know a thing about how to put things in order for her. Then, thank God, we met Carrie Moua the Director of Business Development at Windsor. She took control of the situation, and spent hours educating us and hand-carrying us every step of the way, to get all the necessary paperwork properly completed. We could not have done it without her. That was just the technical end of things. She was also giving us all of her sympathy and compassion, of which made this transition so much easier. There is no way we could thank her, and the other Windsor staff members (the really care).. They are all wonderful. Then, we had the pleasure of meeting Bonnie Gordon, the Administrator of Windsor Elk Grove. We then could clearly see that this compassionate, easy to talk to, caring woman, was probably responsible for a lot of the hiring and coordinating, with Carrie, to make things as good as they are for the care of my mother and so many others. It should be understood that no one is paying us to write this review. We are writing it to let anyone out there know that if you are looking for the best care for your loved one, at a skilled nursing facility, don't look any further. Go to Windsor Elk Grove and look around the facility. We thank God we were led there.

Experiencia fantástica: I was a patient at Windsor Elk Grove C & R Center and just was discharged. I cannot say enough good things about the facility and staff. The staff was just incredible. Caring, hard working, and doing a job that few people would be able to do, and a job that matters. They kept me clean, addressed any issues that came up quickly, and with care. They got me right of ICU from a coma and not being able to even stand, to being able to start walking again. And callout to the physical therapy team as well. They got me back on my feet. The key was participating with the Windsor team, from doctors, nurses, and PT in my own care. I would highly recommend WEGCRC. And side point - everyone hates the IRS and hospital food. I'm here to tell you the food, especially lunch and dinner was really good. I was prepared for the worst, but experienced the best. Thank you Windsor staff for getting me back on my feet. You are more appreciated than you know. You make a difference!

Experiencia negativa: My sister was transferred there from Kaiser for rehab yesterday she a had a stroke and paralyze The ac was not working I believe it’s down for over 2 weeks Portable unit was provided for nursing station but only fans in patients rooms It was cooler outside And the smell is Unbearable Amazing how they could operate a rehab Center legally I decided to take my sister home and take the risk I am Surprised that Kaiser health insurance would send their patient there This place need a complete renovation Please don’t send your love one there

Experiencia negativa: Menu is not followed and they serve lousy food. Staff is very slow to respond most of the time. Doc shows up once a week. Assisted bathing time is missed by staff, grounds are so-so. Mix up scheduled visits to hospital.

Experiencia negativa: The staff is great. They have been kind and caring with our dad and answering our questions or concerns. The AC is not working properly and nothing is being done to fix it. This is not acceptable for the patients or the staff. We complained to the maintenance and the manager. The manager claims the AC isn’t broken. It just can’t keep up in the heat. Well then it’s broken. They have a unit that blows on the nurses station and break room which is a bandaid for the employees only. Owner needs to put some money into the facility asap.

Experiencia negativa: I received the worst care with this facility. The facility was incompetent and insensitive with everything dealing with my care and getting situated. The admission and discharge was unprofessional and unacceptable. The entire facility was gro test and filthy. I would not recommend this nursing home to anybody seeking nursing assistance or recovery in their health.

Experiencia positiva: My mother received excellent care while at the Windsor. The nurses were very kind, caring and attentive.

Experiencia fantástica: Having already had my mother stay at a different SNF for rehab after a hospital stay, I can say that Windsor is far superior to any other SNF in this area. The staff seem happy to be here, they are easy to talk to, and they are great with my mother. The facility is smaller than the prior SNF we dealt with and it feels much warmer and friendlier. My mother is recovering from her most recent hospital stay much quicker than before and it is definitely because of the excellent care she is receiving.

Experiencia positiva: My father’s recent stay was good. The Nurses and CNA’s were attentive and caring as well as the Physical Therapy staff. He was provided 6 days of therapy per week with an occupational and physical therapist. They also arranged for home heath visits and a meeting with the home health manager. One more thing his dog was allowed to visit.

That's a lie! She went to go downtown Sacramento to get a CT scan and the hospital never receive paper work so they couldn't give one! My moms having the worst experience

Experiencia negativa: Apparently not a place you want to go if you have broken your back in a motorcycle wreck. As one who has wrecked bicycles and motorcycles, I can assure you that laying in hospital or rehab situation is not where we want to be. Denying us care does not motivate us to do anything but write bad reviews. Trust me, we want to be back to our homes, our stuff. From the reports so far, I'm disgusted.

Experiencia negativa: So, the staff at, Windsor Elk Grove Care and Rehabilitation Center, won't even answer the patients call light. Patients are having to explain why they are using it. What was their question??? Why was there a question???!!!) This is absolutely horrible and horribly frustrating for patients. I am going to make sure the Google request for reviews is bombarded with this information--that the staff fails to answer call lights and even asks the patient why it is being used.

Experiencia negativa: My grandmother is currently here. Push the nurse's button... they'll come in 15-30 minutes. They take so long the patients here poop themselves before staff comes to assist with the bathroom needs. FOOD IS AWFUL!! Dinner... two slices of wheat bread with a Kraft single slice... super nutritious!! Complaints are ignored because most patients here are not long term. Monitor your loved ones here.

Experiencia negativa: As in any corporation or department, you have those individuals that are "less than" and impact the rest of the staff. Windsor Elk Grove has some very dedicated and wonderful individuals as evidenced by the care I witnessed them conduct with my father. But they also have a couple of individuals who lie, falsify records and violate HIPPA Unfortunately, with the care of my father, I came into contact with a couple of those less than ethical individuals. Their actions resulted in negative results which impacted my fathers health and necessitated a trip to Sutter Emergency and a very long night. If you can be a strong patient advocate for you loved one and can connect with the ethical professionals, then it can be a good SNF; otherwise stay away. The meals are decent but their breakfast is very lacking, especially for a 6'2" guy. They did allow us to bring his dog for a visit. Windsor Elk Grove obviously needs a review of their policy and protocols and ethics training with their staff.
