Green Valley Rehabilitation Health Center: Opiniones
Experiencia negativa: Pathetic!....I was there for 3 weeks. Words can not describe how awful it was! Horrible care, horrible food, CNA's with horrible attitudes that couldn't care less about you & your legitimate needs, being told "oh well" when things went wrong like lights being broken or medication concerns. Please, if you care about your loved one or yourself, go with a different facility that actually CARES for their patients! It's hard enough to try to heal. It's MUCH harder to heal at all in a dumpster fire of a place like Green Valley!
Experiencia negativa: My husband was a patient at Green Valley for much of August. He was given minimal care; his dietary needs were minimally met; his call button was not answered in an acceptable amount of time; he was taken off of some prescribed meds for unknown reasons; he developed a UTI & was released to home without confirmation of the urinalysis, resulting in emergency care from my primary care. He also was released with undiagnosed cellulitis in both legs which required IV antibiotics & IV hydration fluids due to severe dehydration at the time of his release from Green Valley. We feel all of these things should be brought to the attention of Green Valley's administration & dealt with so that other patients do not suffer as my husband has.
Experiencia negativa: Terrible care at this place. They were going to let my Grandma Poop her self instead of helping her get up out of bed "because they were busy". They also "For got" about her (even though they served the Lady in the room with her food) and didn't serve her food, and after we told them she didn't get her food and they gave her cold soggy food. They also refused to help her shower and she had no restrictions against it so they made her stay in the hospital gown she came in. They also didn't turn her so she had got bed sores in just a matter of two days.
Hi Emma, we wanted to let you know that we have received your concerns. We take matters like this very seriously and are working with our onsite team to review further.
Experiencia negativa: this place has horrible food and is WOEFULLY understaffed and many of the staff are inexperienced and indifferent to what residents experience, the CNAs selectively withhold care. they never introduce themselves and are often rude, as if resident needs and requests interfere with their need to amuse themselves. it's like a mean girls reboot. Some staff are wonderful, but others like (like Tayler or Heidi in particular) are living proof that some folks shouldn't work as caregivers.I workedi in healthcare for more than 35 years and as a resident here for a year, i have never experienced a facility that is less compassionate and staffed by people more focused on their own benefit and self interest. this place claims to be a rehab center, but I have recieved less than a dozen sessions with a physical therapist and have never discussed my care plan and treatment goals with my alleged treatment team.. The night shift(10p to 6a)is particularly tough, turnover is very high.l do not feel safe here at night and experience neglect that borders on abuse. STAY AWAY.! THIS PLACE IS SOUL SUCKING. FIND A MORE CARING ENVIRONMENT. SHOULDNT BE HARD BECAUSE THEY CARE SO LITTLE HERE.

Experiencia negativa: If you have a loved one who lives here, my advice would be to seek care elsewhere. It seems they do not have their ducks in a row. Particularly the evening shift people. Be wary of these nurses. My mom has came pretty close to death THREE times the 1st time they took her out of their system declaring she was dead before even knowing if she had made it or not..messed up my mother called me from the hospital saying she needed a ride from the hospital back to green valley because they hadn't set up transportation "because they thought she died" before any confirmation! After I get off the phone with her green valley is calling me two hours after my mom left in a ambulance I was like yeah why is my sick my calling me for a ride back to you guys? The lady on the other end was shocked "she's like your mom called you from where" um the hospital! I go pick my mom up from the hospital. I go in to get her electric wheelchair and noticed her room was different like all her bedding was not on her bed it was bear. They really thought she wasn't going to be back they took her out if their system my mother went without her medication and food for four days... Most recently they took her wheelchair to put new tires on it "they did put new tires" BUT THEY SWAPPED OUT HER BATTERIES IN HER CHAIR WITH BATTERIES THAT DONT GET HER OUTSIDE AND BACK TO HER ROOM AND THE STAFF WILL NOT PUSH HER BACK TO HER ROOM! IM STILL WAITING FOR THE MATAINCE SLASH COOK NAMED WILL TO CALL ME ABOUT THIS BECAUSE RICH OR RICK SAID TO MY MOM IT WASN'T ME THAT DID IT IT WAS WILL.. he put them in another patients wheelchair and put his batteries that won't stay charged in my mother's chair.. I can't afford to replace her batteries. They need to make him put her batteries back And he needs to apologize to my mother. This place is not good.. They are unorganized I ask one person something they send me to another person I'm fed up with it. They switch my mothers medications all the time they don't give her baths like they are supposed to they don't come when she pushes her light button and I've been there when she has pushed it 1 hour and 25 minutes they come in I see you have your light on what's up? Um no not okay! I feel like they're trying to kill my mother! She's there to get better and has only gotten worse since she got there! DO NOT SEND YOUR LOVED ONE HERE IF YOU WANT THEM TO BE TAKEN CARE OF PROPERLY & RESPECTFULLY! GREEN VALLEY STAFF THE ONES WHO ARE THERE JUST FOR A PAYCHECK YOU SHOULD BE ASHAMED OF YOURSELVES BECAUSE THOSE ARE HUMANS WHO DESERVE TO LIVE FOR AS LONG AS THEY CAN.. STOP GIVING PATIENTS MEDICATIONS THAT SHOULDN'T BE GIVEN TO THEM! STOP BEING LAZY IF YOU DONT LIKE YOUR JOB FIND A NEW ONE BECAUSE NEGLECTING PEOPLE WHO CANT CARE FOR THEMSELVES IS DISGUSTING. KARMA IS STILL VERY MUCH ALIVE REMEMBER THAT WHEN LIFE HITS YOU WITH CHAOS! AGAIN I DO NOT RECOMMENDED GREEN VALLEY...

Experiencia fantástica: The best thing about my very elderly dad's visit was the terrific job the physical therapists did in getting him back to baseline that he was before his accident. The assisted living he was going to had a red line in the sand for admission about his ability to do a one person assist transfer. Green Valley actually got him to unassisted transfer. That said, they are sometimes slow to answer the call light, the food doesn't look that good, and the rooms are very utilitarian. But the staff is all very good and very caring.
Hello Jackie - Thank you for your review. It is our pleasure to serve you and your Father. We will continue to strive for improvement so we may continue providing the very best care possible.
Experiencia positiva: My aunt had wonderful compassionate care in her final days. Lots of employees. Friendly and informative. I'm very appreciative of the care my aunt had there.
Hello Kay - Thank you for your kind words. It was our honor and privilege to care for your aunt.
Experiencia fantástica: Very caring staff and so many fun activities for everyone. Ex: Mom insisted on a lemon with her iced tea, kitchen folks found one for her (it’s the little things that help normalize being away from home). Physical and occupational therapy folks have been wonderful too. Thank you all for your hard work.
Hello Stephanie - Thank you so much for your kind words! It is our pleasure to serve you and your mother.
Experiencia fantástica: Best place to work.. great rehab team
Thank you Gail! We appreciate you!
Experiencia negativa: Witness a caregiver or nurse not witness or understand even with personnel telling her the woman's leg was under the wheel chair n being dragged. Disrespectful even with the client telling her to stop and just "stop". Please if your client can't be respected how do you expect me to respect you. Edit: Also my resident has been there 16 days n finally got a shower on her 15th. This place doslesnt care for there residents.

Experiencia negativa: Nightmare of a Place- Neglectful, Dirty, Disorganized If you care about your loved one and want them to receive adequate care, please do not send them to this facility. I am a career caregiver, and I witnessed unacceptable neglect of my family member at Green Valley Rehab. If you want more details feel free to contact me, but out of respect for the privacy of my family member I am not posting the story publicly. I have made a formal complaint against this facility. They need to be looked closely at by a 3rd party entity, not their own little compliance line. They should be on their toes because they are going to be watched now by those above them who make the laws. If you have a loved one who lives here, my advice would be to seek care elsewhere. It seems they do not have their ducks in a row. Particularly the evening shift people. Be wary of these nurses. The lobby is nice, but the rooms don’t look like they are cleaned out between residents. I sat on the floor due to lack of chairs and saw so much grime, dust, and crumbs. The underside of the meal tray was absolutely filthy, with dried crusted old food smeared across the bottom. Seriously heed my words. Look elsewhere for care. This place should honestly be shut down.
Hello Audrey – We have received your concerns. If you would like to discuss further please feel free to call our compliance hotline at (855) 302-1100, thank you.
Experiencia negativa: My mother in-law was a resident/ patient at Green Valley for almost 3 weeks she was there because she had a stroke. She did not receive the proper physical therapy she needed to be able to function and was not helped to use the restroom this place is a joke and I wouldn't recommend it to anyone.

Experiencia negativa: Green Valley needs to follow Health Department, Avalon, and CDC guidelines during this pandemic. The staffing coordinator, ADON, and IP seem to think they can make up the rules as they go. First they require testing in the form of a PCR and quarantine, then (after 10 employee positives when they are struggling for staff) they want to mandate that employees who have not recovered and are still symptomatic come into work around the residents. That takes a lot of nerve. I forsee calls to the media, the health department, and the state. I know where I won't suggest anyone to go....as a resident or employee.

Experiencia fantástica: The staff and facility did an excellent job keeping residents as safe as possible during the pandemic, and you can tell they really care about their residents!
Thanks for your review!
Experiencia fantástica: I have been a staff member at Green Valley for nine years and it has been a great nine years. The bulding is beautiful and our staff is friendly. We are not perfect but we strive everyday to learn and become better at taking care of the loved ones that enter the building.
Experiencia fantástica: Best place to work hands down. We are appreciated and treated with such high regards. Management team is amazing. CNAs. Nurses and other departments are the absolute best.
Experiencia fantástica: Green Valley is the best! Every department has great leaders. They take great care of their residents and treat them as family. The facility is clean and up to date. This facility is beyond amazing!
Experiencia fantástica: An amazing place to stay if you're looking for a skilled nursing facility with an amazing and hard working staff! I recommend Green Valley Rehab to anyone searching!
Thank you for your review. We're happy to give you a good experience.
Experiencia negativa: Not the same place that it used to be, the new company is running it into the ground, had to change my review.
Hello Jammie - We would like to know more about how we may serve you better. Please give us a call at 541-683-5032.
Experiencia fantástica: Green valley has a great team that takes pride in caring for their patients. I have been blessed to find a pace with such great people to work with.
Thank you for your review. We're happy to give you a good experience.
Experiencia fantástica: The best in the west
Thanks for your review!
Experiencia fantástica: The facility has great staff. So friendly and helpful.
Thanks for your review!
Experiencia fantástica: I was transferred to Green Valley after surgery for a broken hip. Everyone was so nice, and I felt at home right away. During my stay, the caregivers helped me to learn to do everything: to get in and out of bed, to use the restroom and the shower, to get dressed, all the necessary skills I'd need later back at home. Since my discharge, I'm using the techniques I learned. They taught me how to use the special equipment I was going to need. PT was very helpful and gave me extra exercises to do because I wanted to heal up as fast as possible. The food was also good, and I looked forward to each meal. I could ask for some ice cream at night, and they'd bring me two flavors. I made a new family there and almost hated to go home after 3 weeks. I've got a new outlook on this type of facility. The stereotypical ideas some have of rehabilitation centers do not apply at Green Valley. Everyday my room was cleaned. New towels were brought in each day. They even provide laundry service. The center also smelled nice and looked clean and attractive throughout. The entire staff seemed very pleasant and clearly enjoyed working at Green Valley. If I ever need this type of wonderful care again, the only place I'll go is Green Valley.
Thank you for your review. We're happy to give you a good experience.
Experiencia fantástica: I love this building! The staff is phenomenal and works so hard in every department. They take amazing care of residents and strive to make sure they are comfortable. I highly recommend Green Valley.
Experiencia fantástica: I want to announce my appreciation for the therapy staff and their dedication to superior patient care! They go above and beyond to provide care for short term patients and long term residents as if they were family. It has been an honor to be an integral part of this facility and leading the best therapy team in the best SNF in the Eugene / Springfield area!
Thank you so much for your feedback. We really appreciate it!
Experiencia fantástica: Like a family! Treated my mom like family! Great care and therapy. Mom came home a new person. Both times!
Thanks for your review!
Experiencia fantástica: I have worked at Green Valley for little over 11yrs and I love it. We are like one big family here. I was also a patient in 2014 for 30 days. I got into a motorcycle wreck and sustained some broken bones. The care I got was amazing. I love working here and don't plan to go anywhere else.
Experiencia fantástica: To start I've been a happy employee of Green Valley for the last 25+ years. I've also been both a family member and a resident at Green Valley. My brother passed away at the facility on hospice and I saw first hand the hard work and excellent care my brother was able to receive in his last few days. The comfort to him as well as my family was the best. I also was a resident / pt. at Green Valley within the past year or so. The rehabilitation department and the nursing department both were gung ho and got me to the point I could go home. When you are at an all time low Green Valley can bring you back up with their expertise and experience.
Experiencia fantástica: Working for Green Valley Rehabilitation is AMAZING! I love the environment and my coworkers. Great synergy with community partners. We get the best residents and families who are a joy to serve.
Experiencia fantástica: I’ve never had a bad experience when visiting Green Valley. The staff is friendly, professional and accommodating. If I ask a question and they don’t immediately know the answer I am pleasantly surprised with a timely and most important accurate response. Everyone will have to make their own choices but for my family and me, Green Valley is the right choice.
Thanks for your review!

Experiencia positiva: Never have had any experience here myself. But I’ve recommended it to a few church friends.
Experiencia negativa: I don't like it they let my sister leave there and then she passed and wouldn't let her stay the night with me.bad review

Experiencia negativa: My mom was recently switched to the long term care wing at green valley and since then her mental and physical health has declined rapidly due to lack of care. I couldn’t see my mom for two days because I was moving and in those two days my mom has become severely dehydrated and her lips are swollen and bloody

Experiencia positiva: They are very good of taking care of people's needs I see him helping everybody all day long. I watched some of their of what they do tonight very good trying to help a person they were so good to her. I mean how many would take her out with them in and take her to the nurse's station and take care of her that's very thoughtful and kind. God bless you ladies and guys
Thank you so much for your feedback. We really appreciate it!
Experiencia fantástica: Happy helpful staff, top quality care, good food, nice visiting through the windows
Thank you for your review. We're happy to give you a good experience.
Experiencia fantástica: The Green Valley team loves to serve their community. This is a hard-working team who care about the recovery and livelihood of each and every person who comes to them for their healthcare.
Thanks for your review!
Experiencia positiva: Staff is wonderful, food is awful, could be a little cleaner. Is the nicest home in the area.
Experiencia fantástica: I could not have asked for a better place to do my rehabilitation. The medical care was great. Physical and Occupational Therapies were wonderful. I loved playing Bingo and getting my nails painted by the recreational therapist. In fact all. Of the staff was great. They all were very friendly and concerned. Thank you Green Valley. I'll be back after hip surgery in September.
Thank you so much for your feedback. We really appreciate it!
Experiencia fantástica: My mother was there recovering from a severe stroke. She had very good care from everyone in the facility. The entire staff, from the housekeeper, nurses, and therapists were compassionate and competent.
Thanks for your review!
Experiencia fantástica: A dear friend of mine was a resident here a couple of times. I was impressed with the level of care and attention, as well as the facility itself. Top notch. The staff were kind and knowledgeable, in addition to the immaculate rooms and common areas. Would highly recommend.
Experiencia fantástica: Our daughter was a resident at Green Valley for 5 years until she passed away. The staff became family and when Our daughter passed away grieved with us. They made the most difficult situation much more bearable.
Thank you so much for your feedback. We really appreciate it!
Experiencia fantástica: The entire staff took steps to further my well being and daily care. I was encouraged and challenged in positive ways to become a healthier me!
Thank you for your review. We're happy to give you a good experience.
Experiencia negativa: This place almost killed my mother from a lack of care. Do you and your loved ones a favor and do not bring them here.
Experiencia negativa: If I could give this place a zero I most definitely would have my mother went here after having a stroke and brain surgery they did a horrible job of taking care of her 5 days after she was there they called and said ambulance was on there way that my mom had low vitals and was unresponsive saying that she either had another stroke or accidental overdose which my mom can't move and has feeding tube where they give her feedings and meds after scans and blood work in ER test show she had a overdose of pain meds needless to say she is going to a different facility and i will be getting a lawyer
Experiencia negativa: My friend stayed here for a month while recovering from brain surgery. Positives: Many of the staff are kind, and they checked on her frequently. Neutrals: The usual nursing home gripes; inedible frozen food, showers given only once weekly, missing clothing and personal items, overworked/ understaffed staff, nobody knows ANYTHING and you're just put on hold indefinitely if you call for info on your loved one. Extreme negatives: Nobody there gave ANY info or warning as to when the insurance would run out and my friend would be discharged, they literally just called and said "come pick her up", which was extreme stress on everyone. No medical or care discharge info, no sign of her brand new wheelchair or walker that had been ORDERED AND PAID FOR BY INSURANCE and which she NEEDED to get around, and worst and most dangerous of all, they REFUSED TO GIVE HER PRESCRIPTION MEDICATION! When she checked in, they made her transfer all her prescriptions to their "in-house pharmacy" and nurses doled out her meds to her. When they kicked her out (late evening) they literally REFUSED to give her medications to take home. I'm talking life-saving meds to prevent strokes and seizures. It was night and pharmacies were closed, so she could not get her meds. The next day when we tried to fill her prescriptions, insurance would not pay for them since it was "doubled up" for the month, as Green Valley HAD her current months pills!!! Total nightmare, hours on the phone trying to reach staff at Green Valley (transfer after transfer put on hold) and managers (always "in meetings" every time we could get through), with NO RESOLVE!! Who has her pills?? What is green valley doing with them?? This is some illegal pill scheme at minimum, and if I were wealthy I would sue them.
Experiencia fantástica: My father received exceptional care at Green Valley after hip surgery. The care takers and nursing staff were friendly and knowledgeable. We were always greeted with a smile and compassion. My fathers needs were adequately taken care of. We would highly recommend Green Valley to anyone needing post-surgery care. Thank you Green Valley staff. You guys ROCK!
Experiencia positiva: My daughter was there in October. The staff was friendly and caring. They tried hard to find foods she could eat. Smoothies were available and delicious. Physical therapy, speech therapy, social services, Nurse Practioner were all very helpful and kind. I’m sorry that I didn’t write down all their names to thank them. Nurse Joe and all the CNAs and med aides were so helpful. However, as in most care facilities, the staff giving direct patient care (CNAs and med aides) are overworked especially at night. It appeared difficult to keep up with the night time work but I would say that they tried to do a good job under the circumstances. One roommate was incontinent and kept soiling the bathroom. There wasn’t enough cleaning staff available to keep up with it. People who are ill may find any food hard to tolerate. I observed that the food was fine and looked pretty good to me. Overall, the facility smelled clean, was pleasant and friendly. I would recommend Green Valley to family and friends.
Experiencia fantástica: My Grandmother was here twice after breaking her leg. The staff is over worked and busy. But they do better than can be expected. Social Services (Michelle) went above and beyond in helping me navigate the insurance company and all of their paperwork and red tape. My Grandmother is much improved and we owe it to her stay at Green Valley, and all the wonderful help she got there.
Experiencia fantástica: Great help with 91 year old mother
Experiencia positiva: My husband has been having health issues that last few years and this is the fourth rehabilitation center he has been admitted to after a hospital stay. This facility was the best that we have seen so far. It is still not perfect; mediocre high carb food, a shower only twice a week and it took a week to get his physical therapy started. But the staff was reasonably empathetic, friendly, and generally available when I had questions. It has a large community area with natural light when you walk in the door which makes it feel more welcoming than some of the other dreary places we have been in.
Experiencia fantástica: Convenient location near Costco with friendly, professional competent staff. Multiple staff went out of their way to greet and orient visitors and were seen providing caring service to rehab patients.
Thank you for your review. We're happy to give you a good experience.
Experiencia fantástica: I’ve been at green valley sense August 29th 2018, I have to give a shout out to Tim. He does an excellent job at what he does. Always showing every patient with kindness and care. He is funny and always making us patients laugh. His kindness makes the stay here at green valley worth staying here. His good positive spirit is what keeps us going. Helps us continue our treatment, and keeps our spirits up. If you know Tim your surely blessed. If you don’t know him then I pray that you get to meet him someday. He is great to my husband Bryan and I. Thank you Tim for being you. You rock man!
Experiencia fantástica: The facility was very professional from the intake staff, doctor, nurses, CNA's, therapists, kitchen staff for meals, laundry, labeling of personal items, and the cleaning staff. We could not have gotten any better care for my husband. They answered all our questions and were quick to meet the needs that were requested. The discharge went very smoothly. Everything regarding my husband was discussed from what his illness had been, what meds he would need, what home services and what therapy to be continued at home. All of these items had been put into motion before we left the facility. Thank you Green Valley Rehab.
Experiencia positiva: Attentive staff. Nice locaction.
Experiencia positiva: PT at Green Valley Rehab Center is fantastic. Glad I went there after knee replacement surgery.
Experiencia negativa: The worst place for meals I have ever been in... Including other similar facilities.
Experiencia negativa: Some very neglectful staff w amateur approach to nursing care. Good staff are atretched too thin to cottect anuse or have no talent to do so.
Experiencia negativa: Spent a week there @ approx. 5 hrs a day with my daughter. She had brain surgery needing to learn how to walk & function somewhat normal again. The experience was eye opening and not in a good way. These patients were to schedule a shower and lucky to get one more than once a week. Her roommate was ignored and sat in her own waste all night. Wheelchair Patients in hallway all day, request buttons sounded off non-stop, linens thrown on floor vs taken off immediately. Needless to say, I spent every day cleaning up food trays, helping my daughter with daily hygiene and scheduling her showers. I can't imagine living in that environment for more than one hour.
Experiencia negativa: My husband was in for IV therapy for 6 weeks after hospitalization. The communication is horrible. An medical appointment was scheduled by Green Valley. The physician's office had called Green Valley and rescheduled that appointment with explicit instructions on office location. Green Valley had no record of this. We made a trip for nothing which cost me 2 hours from work. I tried to call Green Valley on several occasions and could never get through. On one call, I was placed on hold for over 9 minutes before I hung up. Medical trash is always on the floor around the bed. The only information given on discharge is: "When can you pick him up?" My husband was not given appropriate instruction on how to use his walker. The speech therapist told me, "You need to learn sign language." Seriously? No information, just an order from someone I just met. I would NOT recommend this facility. Awful experience...
Experiencia negativa: My grandmother was placed here to live out the remainder of her life. I am completely DISGUSTED with the quality of care she received. She would sit in soiled underwear for well over an hour with her call light on. They also threatened to give her an eviction notice if her account wasn't paid, instead of contacting the one in charge of her finances. What is sad is there are good care givers, But, this place is managed horribly, as most of these places are. She died there. Almost certainly from neglect since she saw a doctor that day who was impressed with her rebound from a serious heart surgery. Also, when family came to collect her belongings, they hadn't even cleaned up her room. There were still soiled linens on the bed. No family should have to clean up the after math when they are have paid thousands for the facility to do its job DISGUSTING If you are not in it for the residents, get out of the business. Karma will come around.
Experiencia fantástica: Fantastic facility! Every single staff person working there helps ABOVE AND BEYOND! They have helped our father in his recovery. Every question and concern has been addresed right away. I even asked other residents!! And you know what? They ALL said they are getting fantasic care! So if you are looking for a place for your family member, this IS the place. By the way, my dad loves the food!
Experiencia fantástica: I toured this marvelous rehabilation center for my dad, on a Sunday, and they had a welcome for him Monday afternoon. Our involved caring LARGE family wants the best for our daddy, who had a fall @ home.p And I believe after researching many facilities, asking most everyone that the 5 daughters knows. That we have made the absolute best choice. I personally have had 3 in depth visits with folks here for rehab. They have nothing but good things to say about everything, except the food. My dad has found the food acceptable. The need to check on my father more frequently at night, is my main complaint. It's been only 3 days and we are working on that. L
Experiencia negativa: My mom is 92 has dementia and can be cantankerous! She broke her hip and came there for rehab. I must say, some education is desperately needed, and more staff, every time mom had to go potty the nurse would say," iam in the middle of something, she will need to wait!" Hu? I ask if anyone could help her since she is very weak, I was told no, her nurse has to do it! They complained to me about mom making a bowel mess, well I said, maybe if you listen and help her to the restroom she wouldn't have an accident! I am not happy and can't wait for her to leave that place, the help all seems cold! Yeah, not happy!!!
Experiencia negativa: Horrible place: They "NEVER" put my band on-they would take quite some time to get to me "After" I pushed the button. They never asked my my name when they came in to give me my meds, The person taking my temp (with a digital and had difficulties putting the plastic sleeve on), never repeated the temp to me nor my blood pressure, plus he never confirmed who I was either. They were unaware of why I was in there-was asked and had to tell three of them why I was there-Do they not know about their patients when they come on shift? Pushing the button and my pain level rising to a 7-10 any times and once a 12 (at least it felt like it) One Nurse/CNA came on at the midnight shift, came into my room told me she had just gotten there (she had a pen in her mouth while talking to me-I think she was hiding her missing teeth), and didn't have time for my request, right now-I tried to tell her my horror stories of not being attended to and she said she already heard about my "ISSUES", my ISSUES? what the heck! think Because my girls were on their case about their care with me, they probably considered me a pain! On day 2, I needed to go to the bathroom, my walker was across the room and as I was trying to get someone’s attention (even though I pushed the button), by saying, "Hello" as I heard talking right out my door, one finally came in scratching her leg almost up to her crotch, saying sorry I am so unprofessional. I said I need to go to the bathroom and pointed to my walker, she rolled my walker to me and said to pull the string/push the button (can't remember), and I'm like ok (frustrated-didn't say it, just thought it), I hadn't even gotten up or gone to the bathroom without support yet-the least she could have done was ask me if I needed help. I sat on the (dirty toilet-my girls had told them it needed to be cleaned as when I left the hospital, I had a suppository and had a blowout). No one came so I got up and went to the bed myself, I figured what the heck, I got there by myself. The same gal came in and said oh you made it-I said, I didn't have a choice, you never came. As she walked out, I told her the toilet needed to be cleaned and she continued to walk out and told me that is a CNA's job! So the days go on-my daughters called Slocum, and River Bend to get clarification on my meds-From here not sure what was happening, but i was getting meds every 3 hours, my visitors on Wednesday noticed I was strange, rolling my eyes.....I was somewhat hallucinating and seeing a shadow go by me. By Wednesday evening all three of my children were concerned about me-My daughter came to Green Valley at 11:30 PM, banging on their door-got in and said something is wrong with my mom. One nurse stated she was just with me and I was fine. They said they had taken my temp (remember they do not report back to you what your temp is) and well by the time I got to the ER-I was 104.?? and I was over medicated, I also ended up with an infection-Might note I came in on Sunday and by Wednesday my sheets were never changed-I left after midnight by ambulance to River Bend. My stay at RiverBend was of course the care I had before I got to GreenValley, but by Saturday I needed to have a blood transfusion-I was scared and did not know what was happening to me, why....They had so much IV fluid going through me to drain the meds that was given to me at to many intervals at Green Valley that my blood count dropped tremendously. I am requesting them to write off their charges on a NP that saw me maybe three times and a total of 15 mins and the charge of the ER Dr that was out of network, and some pain and suffering.
Experiencia negativa: My father asked me not to leave him alone at night and especially on the weekends because he was taken care of so horribly. The night staff did not know how to work his bipap or the concentrator for his oxygen. He died a couple of days after a night shift said his concentrator was not working properly. If they had been educated they would have known how to use it. My dad was terminal but we were told it was the lack of oxygen that night that took his life as soon as it did. After dad passed away items were missing of his belongings. Housekeeping had taken it home. I just got a call a couple of days ago from a vendor asking about the concentrator because this facility said the family took it. WE DID NOT TAKE IT. THE FACILITY SAID THEY WOULD RETURN EVETYTHING. I would never, ever, ever recommend this facilty to anyone. Jami Sautner
Experiencia negativa: I was in Green Valley for 7 weeks from May through June 2017 due to osteomyelitis - needed wound vac care and IV antibiotics. My nurses were amazing - professional, efficient, skilled, and personable. Had a couple great CNA's also. My room was right by the nurses station, so I could hear all the soap opera drama that some of the younger CNA's brought to their workplace - annoying as hell, especially when your call light has literally been on for half an hour. The food...well, what can I say - see my pictures. My condition required high quality protein and nutrition. I could not get that there...thankful for grub hub! I may have some complaints, but I was in another facility this year (Marquis Care) and Green Valley, in my opinion, is the difference between third world and modern care in comparison. I never had to worry that my nurses weren't following good hygiene practices or didn't know how to properly perform procedures. Bottom line - this is still the best place in town.
Experiencia positiva: Over-all this was a very good experience for my husband. They monitored his meds, made sure he had meals and the Physical and Occupational Therapists were very consistent, attentive, friendly and helpful. The CNA's and Nurses were also friendly, helpful and kind altho they were not always the quickest when the button was pushed. The food was all over the place some was Very Good, most of the time just Good and other times it was Pretty Mediocre. At first the serving times were not on time but they became more consistent and adherent as the week went by. The facility itself is very nice and comfortable the dining room is so nice with the skylights, light is important when you are rehabilitating. Suggestions: The CNA and Nurses could be more consistent about signing the dry erase board everyday and every shift, it was very nice to know the name of the people helping through out the day. I am not sure I would agree with their protocol for delivering the medications. There are certain meds that should not be given with a meal. Specifically on Oct. 3rd 2016 the med nurse gave my husband his 11am dose of Sinemet at 12:00 while he was eating his lunch! This was so wrong. I understand they are in charge of over 40 patients at one time but if a medication works better on an empty stomach shouldn't there be instructions in place to administer it then? When Social Services, Dr's or AGNP's would come in it was very difficult to keep track of who we were speaking to. We do not recall seeing very many name tags they were either hidden or not present at all. I suggest especially the Administrative positions have business cards that they can give to the patients. That way the patient can keep track and they can let their caregiver know who they spoke with that day.
Experiencia negativa: My mom went here after a hip replacement. The food was nearly inedible--they had a "seafood" sandwich which was still frozen the night I went to have dinner there, and when I picked her up they were having quesadillas that were rock hard. When I was there they waited over an hour to serve us. At one point my moms leg was swollen and they kept saying they'd put the compression hose on her and they did not until the next day. They were reluctant to give her the medications she needed so she was in pain. They were rude and took a very long time to assist when needed. Her roommate needed help using the bathroom and the nurse that went in there to discharge my mom told me very rudely that she was too busy to help and told the roommate to press the call button. She was supposed to get physical therapy but all they had her do was try stairs. Overall just a horrible experience. My mom hated it so much that she cried and constantly wanted to get out of there.
Experiencia negativa: The social worker suggested our mother go to Green Valley for rehab for a broken pelvis – this is the start of the problem. She was supposed to be picked up at 4:00 pm Thursday, June 1. The driver was over an hour late, when we got to Green Valley, they did not have a room for her and did not seem to know she was coming – one of the workers said “that happens”. This is when we should have run from Green Valley. They finally got a room and checked her in. Should not be surprising, but the children work, so came after work. Mom was in quite a bit of pain, and the worker said she did not ask for pain meds – for crying out loud, she is not very vocal with her dementia, and a real skilled nurse would have been able to determine her level of pain despite dementia. When her daughter talked to the nurse, she though it was Lois’ shoulder that was the problem – if she would have taken the time to read her chart, she would have seen that mom was there for a fractured pelvis! Then come to find out she did not eat lunch, and they said if she doesn’t eat they would just wheel her out to the dining room. If she cannot feed herself in the room, how in the heck is she supposed to feed herself in the dining room? If Green Valley is not capable of providing skilled nursing to a patient with dementia and broken pelvis, then someone needs to let peace the State of Oregon and family of patients looking at Green Valley for help in their time of need. So either mom’s inept care is an outlier, or it is a trend. We were told Green Valley was the jewel of the Avalon system. If this is the jewel, I wonder how bad the care is at the other Avalon facilities.
Experiencia positiva: I
Experiencia negativa: No doubt the worst run care facility in Eugene, if not the whole state. My family member has been thrown in there twice now by Riverbend. The first time his bladder almost burst because of the negligence of the nurse, who basically told him to shut up and go to sleep when he told her there was an issue. They had to rush him to the ER later and found he was minutes away from his bladder bursting and death. He then had to spend another 3 weeks in this place dealing with all their incompetence. We couldn't transfer him to another home because they were all full. The 2nd time he went there, same story basically. This time though his medication got all messed up because Riverbend took him off everything because his blood pressure was dropping. But of course it was the weekend, and Green Valley's doctor only works 3 or 4 days a week and is not on call, nor do they have a backup on-call doctor. So he had to suffer all weekend in pain. And he has epilepsy, so now he was having seizures without proper medication. But when he started having seizures for the first time in 30 years, he would call the nurse and it would take hours for her to come. Then when they do they say, "well we didn't see it." How could you possibly see it? It takes hours for you to come to the room in the first place! They acted like he didn't know if he was having them or not, even though he's had epilepsy for almost 80 years. And one of the nurses aids said she had never even heard of epilepsy! I could go on and on about the runaround and incompetent night nurses. Each time he went there he got worse. The food is also inedible. And speaking of food, one night a nurse was supposed to get him to go to the dining room for dinner. When she didn't show up, he went there himself. He sat at the table while she passed him by with tray after tray of food. He asked her if he was going to be served, and she vindictively told him that they put his food last and that the entire home would be served before him because he was "late." Talk about a lack of respect. Yesterday when we went to pick him up they had pizzas delivered. But not for the residents. For the staff. And they announced it over the loudspeakers for all to hear, parading it all over the dining room, while those poor old people sat there and ate whatever slop the kitchen was serving. There are a few good nurses and staff there. Some were very sweet to him and took as good of care of him as they could. But they can't make up for the poor management and woeful lack of staffing. When we brought him home he said now he knows what it must feel like to get out of prison. Indeed.
