Experiencia fantástica: I've had the pleasure of working with Citrus Nursing for 1 year, and I'm consistently impressed by their exceptional care and professionalism. From (A Gone Transportation) perspective, the coordination and communication with their team has always been seamless and efficient.

Experiencia negativa: This is the worst rehab I have ever seen! I could not believe how uncaring most of the staff is. There were only a couple of nice and hardworking employees. No Dr on site. And if you try to find a nurse seems impossible. We took my friend there and she almost died. They let her to scream all night “help me”and she did not eat for days. The food is so disgusting and greasy. I would not feed that good too my worst enemy. I can tolerate a lot of rudeness but when it comes to elderly nope not doing it. That’s why I’m writing this. Don’t go there if your loved one can’t speak up for themselves. We almost lost our loved one. I told staff that they needed to call 911 because she needed help and was sick. The nurse said that her and the other nurse did not feel it was urgent but that a non emergency transport was coming. When we got to hospital Fontana Kaiser. They quickly took her in and said she was very sick and needed nutrients. Dr said it was good we came in. Now she is safely in hospital trying to recover. But 4 days without no food and no sleep because of how was treated at Citrus nursing in Fontana will forever be a horrific memory for me. Oh and the nurse laughed at me when I asked if patient got sleep. And said no. I did not know what the heck was so funny! That urked me! Dirty place. And By the way patient is in good hands now best staff over here in Ontario Kaiser! She does not scream help me. Food is great here! I have more to say but will not. I don’t want to ever talk about this situation again. And No Fontana nursing center I do not want to talk to you. I’m done with that place.

Experiencia fantástica: Took every good care of my husband. Was always there for him and I appreciated it so much. Area was clean... And the nurses and staff were amazing, friendly and always there for us..

Experiencia fantástica: My dad has been at this nursing home.just like any daughter or son and patient are nervous about it. Since he arrive all staff made welcomed him well and made his stay super easy and comfortable. Any concerns or questions they always get answered with a fast timing. My dad left the first time very grateful for everyone and even made friends. Second time he had surgery and had to go there for a couple of weeks. He wanted to come back to this nursing home and with the grace of God he was able to. All staff remembered him and made him feel comfortable once again. He left the nursing home yesterday and let me tell you he didn’t want to leave. lol He made sure we gave this place good reviews and 5 stars. He is thankful for all the staff and residents. I hope he can stay as healthy and God protects him but if he ever needs to go to a rehab with no hesitation we will sign him to citrus nursing home once again.

Experiencia positiva: Update: Because I was there so often visiting my mom, I started to see the bad things going on. There are amazing employees but there are employees that should not be around people who cannot defend themselves. I saw 2 of my mothers roommates being mistreated or not taken care of properly by certain employees. I spoke up and they stepped up at that time. They need to replace a lot of employees. Original review-My mom has been there for several weeks now. They have taken good care of her. Any time we have a complaint, it's addressed quickly. My mother has been given physical therapy regularly and is improving quickly after an infection made her extremely weak. The reviews scared me but we have had a good experience with my mom so far. They are going to get her home soon.

Experiencia negativa: My grandmother got admitted to this facility after having a hip surgery the room that they gave her it smelled like urine the bed was all messy we told the staff and they said, will take care of it, we ask to change the room they were rude about it, they also wanted her to walk when she just had surgery 2 day ago she 87yrs they should know how to take care of older people if they are Certified for this. I will not give them a star. Really bad experience People review the place were your love ones are going to I didn't had the chance to check but if I had the chance I shouldn't bring her here at all.

Experiencia fantástica: On my first day when I came and I’m admitted in CitrusNursing Center I am amazed.I know that I will had speedy recovery from my Lession Surgery and I need to PTEmployees are very friendly and always had a good smile Once u press the call they came in to your room.I am giving more than 10 stars. The facility are clean and the food is good To all employees you found good nursing home to work with.

Experiencia fantástica: Let me start by saying, everyone’s entitled to their own opinion. I give a Big thank you!!! To all the staff that took care of my Dad Celestino Martinez jr. I can’t remember everyone’s name. The ones I can remember are Billy at the nurses station, Maria and Maria (Grace). Great Job!!! He didn’t want to come home. The times I went he was always clean shaved and always had clean clothes!!! Also he was always in good spirits!!! Once again thank you and Blessings to each and everyone of you!!!

Experiencia fantástica: When I first was admitted to citrus I was frustrated and concern related to my stroke and weakness. Nursing,case manager,social services,therapy,business office manager,dietician, maintenance all departments were cool and treated me fair. Very attentive to my needs.Staff provided friendly astomphere,knowledge,and assistance to all my needs and concerns. I would recommend Citrus to my family and friends. I am very pleased with the outstanding care I received at Citrus Nursing.I thank the entire staff for making my stay memorable and positive.

Experiencia negativa: My friend was due to discharge today. I helped her get her things together and she has been waiting all day for the staff. She gets notified at almost 5 pm that she is not going to be discharged today. Waisted my time as well as poor communication. They need to do better.

Experiencia negativa: This place is disgusting, smells, is under staffed and the staff is negligent. A report of concern has been filed w licensing. The staff are argumentative and rude to patients. The food mimics that of a correctional facility. Patients are left sitting in feces for hours. The doors are locked after hours—what if there is a fire? Medications are NOT administered correctly. Just yesterday 8/21/22 my brother was given a medication that should have been administered over 2 hours in 30 minutes. When he questions the charge nurse who did it, she replied ….what are you gonna do sue me? Perhaps that’s is what needs to happen. His diabetic diet was not followed and he was told that he had labs that indicated he would need to go to ER. Upon arrival at ER there was absolutely nothing wrong with him. An official complaint has been filed with licensing. If your insurance company tries to send you or a loved one here please challenge them. You are gambling with your health and safety. He was there 72 hours and the police were there on 4 separate occasions. Anytime patients question the lack of competency the STAFF call on the patients. This place needs to be shut down. Below is a picture of a bag or urine that was emptied from a urinal by a STAFF, thrown on a shared bathroom floor and left for more than EIGHT HOURS THIS IS NOT A PLACE CONDUCIVE TO HEALING OR RECOVERY….just a place to die slow without dignity!

Experiencia negativa: My poor Dad was sent there for rehab after surgery and they allowed him to lay there soiled and fecal matter got into his surgery site wound and he ended up with a horrible infection with wound vacs and all. The infection never got better my 65-year-old father ended up dying. they lost all of his personal clothes even though we wrote his name in them. He waited forever to someone to come in and feed or clean him up. it was just a horrible place all the way around nobody spoke good English and they didn't seem to care about my poor dad at all. He ended up getting a really bad infection while staying there and passed away 2 months later. He was fine before he ever got surgery he went in for a simple operation got an infection and died! I only gave this place one star because there wasn't an option to give them zero stars.

Experiencia fantástica: My Grandpa was a patient at Citrus Nursing Center and my family and I had a great experience. All of the staff was kind and really seemed to care. I would recommend this place to anyone who wants for their family member to recover quickly. The only complaint that we had was the strict visitation policy but towards the end of his stay at the center they opened their visitation and we were able to see him without making an appointment.

Experiencia fantástica: This facility is how all facilities should be. The staff provided prompt, skillful and caring support to my dad for his short term stay. He was living on his own prior and the transition to needing more support went smooth, thanks to the nursing staff at Citrus. My family and I are so appreciative to everyone there.

Experiencia negativa: I don’t understand how all these place loose their patients clothes, after family members pay a lot of money for their stuff all of it gets lost then when you see your loved one they look like a homeless bum!!!! These elderly don’t have much and for thier clothes to get stolen or misplaced

Experiencia negativa: I wish I was able to post photos here, they would certainly turn your stomach! My sister passed away at Citrus in 2018 from repeated neglect. It is worrisome that visitors are kept out during Covid and cant check on their loved ones hygiene and general well being! So many nursing notes were falsified, I didn't discover it till after her death! Please! Please! Be vigilant!

Experiencia negativa: So sorry that we let our dad get transferred there from the hospital for "rehab". They just let him lay in bed 23.5 hours of the day. We called several times a day as we were not able to get in to see him for several days after he was admitted. The staff would tell us what we wanted to hear and made everything sound like it was great. Then we would talk to dad and get a completely different story. He said the food was horrible. Without proper nutrition and exercise, he was never going to get better. After a few days we checked him out against doctor's orders. We can provide better care at home.

Experiencia fantástica: We were referred at this place for my mother in law’s rehab needs prior to Covid and it’s an awesome facility, very clean and inviting. The Administrative staff is excellent and the girls in the front office are very helpful and sweet. Great therapists and nursing staff, very competent and attentive to the patient’s needs. I will definitely recommend this place for any rehab needs.

Experiencia positiva: I don’t care what anyone says about this place because we all have our own opinions. My uncle was here for several years and they treated him like a King. We made and developed many relationships with nursing staff. They were always good to him, answered all our questions during the Covid-19, provided hot meals, very clean and made sure he had his cell phone charged to receive phone calls from us. The nursing staff would notify us right away if anything wrong. My uncle passed away due to old age, and underlying conditions. This convalescent home will always have a special place in my heart for taking care of my loved one who is now with the Lord.

Experiencia fantástica: My grandma was a patient here at citrus and unfortunately it was around the time citrus got covid thankfully my grandma recovered and is feeling a lot better.. the staff was very good with keeping me in the loop.. I knew when she had good days and bad days. My grandma has no complaints the staff did what they could and I couldn’t be more grateful.

Experiencia positiva: I am a covid 19 survivor. When I came for physical therapy here at Citrus Nursing Center I couldn't get out of bed. Within 2 weeks I have relearned walking and my oxygen requirements have dropped from 6lpm to 3lpm. CNC has an expert staff of trained pros working 24/7 to help achieve independence. The food isn't bad either. I highly recommend this facility for recovery from hospital stays.

Experiencia fantástica: I enjoyed my stay at the Citrus Nursing Center for several reasons, which I have stated below. First, I should say that I spent two weeks at Citrus due to suffering a stroke on my left side (I have never had a stroke before). When I arrived, I had no mobility in my leg and arm. This is the placement my health provider provided thus, we did not have chance to research, it was a true learning experience. Positives: Caring Nurses and CNA’s (Certified Nursing Assistants) even during the most difficult times Many bilingual staff Clean basic living and facility areas (beds changed daily) Food is what you would expect, low salt/fat. Appropriate portions Access to management when needed Excellent therapists, very knowledgeable (A+ for me) Excellent Covid-19 rules are in place for all who are in the building Staff always took my wife’s phone calls and deliveries Concerns: Time it took for Nurses or CNA’s to come and help/answer questions, I had to be patient. (I will add, the average CNA had anywhere between 12-18 patients to attend too at any given time. What I found, some of these patients required constant monitoring, thus other patients where left to wait, this also pertains to the nurses. I highly suggest to the management that more staff is hired due to the type of high-need patients Citrus serves) This facility serves both high and low functioning patients. At times, the needs of the low functioning patients had a negative effect on the ability of the staff to attend to the high functioning patients. Inside gray wall paint is very institutional (dark). Wall pictures of old Fontana…why, who cares! All staff wear gray institutional scrubs (yuk). I viewed this atmosphere as counterintuitive to patients wanting to get healthy. Suggestion: a new paint job with positive colors, scrubs for the staff that are bright, cheerful, place slogans of positivity on the walls and hang pictures of famous people who have overcome tragic health/life events. Make sure to educate all staff about the slogans and pictures so they may share and possibly inspire their patients. Final thought: I have read the other reviews, I see their point of view. What I observed and this is from my point of view, families are dropping off family members and thinking that the Citrus staff has a magic-wand that will automatically make their loved-one well. This is not the case, the patient has to want to try and families need to be involved as best as they can (I understand, Covid rules make this difficult). For me, my goal is to be healthy, self-reliant and get back to what I consider normal. The Citrus made a difference in my life… now I’m getting better! Many thanks to Citrus and thanks for reading.

Experiencia positiva: I've been a resident at citrus for about 5 years now and during my experience I've had a lot of ups and downs but mostly ups. Unfortunately when I first came in the staff wasn't very welcoming.. now that I have warmed up to everyone I've grown to have great friendships with them, staff and residents. Citrus felt like home right away. The people here really care about you. Like any facility you know your good nurses from your bad ones. At citrus I was able to go out In the halls to talk to everyone, I was able to go to the stores, I participated in activities like bingo and bowling and much more. Since covid started, all of that is on pause now. Things are different but I hope it's temporary. The facility itself is very clean and organized. As for the food, it's not going to be the greatest but it is definitely edible! Out of my whole experience and 5 years here I'd have to say I would definitely recommend.. no one's perfect but this is the closest you'd get to it.

Experiencia fantástica: My mentally and physically handicapped son recently spent two weeks in this facility to get rehab for a broken hip. I was with him daily for several hours. It is not the Hilton, but the best place to recover I have come across in my 82 years. I would highly recommend it. The staff is dedicated and friendly to everybody. The place is light and clean, the meals are appetizing. I am grateful for the good care my son received. As I mentioned before I can highly recommend this nursing home. I do not think there are any better ones. At least I have not seen any when I had to visit several of my elderly friends.

Experiencia negativa: *NO STAR* WOULD RATHER DIE THEN TO GO TO THIS PLACE, I WAS THERE 3-3 1/2 WEEKS WORST PLACE EVER WAS VERY LOUD ALL DAY ESPECIALLY AT NIGHT. THE STAFF TREATS THE PATIENTS VERY BADLY ESPECIALLY THE ELDERLY WHO CAN'T DEFEND THEMSELVES. I COMPLAIN ABOUT EVERYTHING TO HEAD OF NURSES MARILYN, I COMPLAIN TO SOCIAL SERVICES, EVERYONE I COULD COMPLAIN 2 & NOTHING EVER GOT DONE ABOUT IT. SUPPOSE BE THERE 6 WKS & SO BAD I DISCHARGED MYSELF IN 3 1/2 WEEKS I TOLD HEAD OF NURSES MARILYN I RATHER TAKE MY CHANCES AT HOME W/NO MEDICAL CARE. BLOOD PRESSURE UP EVERYDAY, STILL THINKING OF REPORTING THEM 2 MCARE& MCAL.. THIS IS NO 5 ***** PLACE.. IF YOU LOVE YOUR LOVE ONES DO NOT TAKE THEM HERE

Experiencia negativa: First impressions are lasting. No one at front desk to start, had to ask administrator for assistance, this facility needs to have better housekeeping, dust on furniture (alot), trash on floors, etc. The only good thing was staff. Very friendly and accommodating. Don't have much confidence in this place.

Experiencia positiva: My mom has been there twice for a period of 6 weeks. She is happy staff is very loving and freindly with her.

Experiencia fantástica: My 94 yr old father is currently at Citrus, and is receiving excellent care. Their pureed food is delicious, I've tasted it. Pork loin with herbs, mashed potatoes with gravy, green bean salad, chicken chili. He is learning to eat and swallow. Kaiser would have let him die, drugged and starving, because "he's 94 after all". WHAT?!!! Dr. Constantino was the only Kaiser Dr. that believed my father needed respect and proper care, and sent him to Citrus. He will be coming home soon, within a week. He is clean, shaved, receiving physical therapy and is doing awesome. I see him every other day, and am always impressed with the staff and the cleanliness of the facility. This is a good place for your loved one should they need to go to a facility like this. Highly recommend.

Experiencia negativa: When you speak of quality of care,leave this place off that list. Their incompetence is only outmatched by their inability to perform basic services for the patients. Things that we take for granted that we trust these people to handle they don't. I would Strongly recommend that anybody that maybe considering using this facility,NOT TO. Be aware that if you leave your loved ones at Citrus Care it's like leaving them in hell. Here is a short list of things he don't do, properly bath their Patients, brush their teeth, change their catheters, we won't even mention whatwhat don't do for their patients that are incontinent. Once again in capital letters DO NOT USE THIS FACILITY YOU ARE JEOPARDIZING YOUR LOVED ONES LIFE.

Experiencia negativa: My grandma was here from June -mid September of 2016 and it was horrible. The staff is usually playing on their phones, watching tv or gossiping the whole time and they steal clothes/belongings from the patients. Technically anyone can walk in because there's never anyone at the receptions desk and the employees never ask where your going. On top of employees not doing there job the a.c. is usually out in most of the building so it gets really hot inside and what makes it worse is they don't always wanna change the patients that soil themselves so the smell of feces and heat does not mix too well in a building with little air flow circulating. My grandma would receive dialysis 3 times a week and the van would come pick her up at 5 in the morning, the driver would ring the bell so the nurses could let him in but most days the nurses would see the guy through the window and walk away because they didn't want to open the doors. Overall this place is horrible I don't recommend sending loved ones here!!

Experiencia negativa: The worst service ever. The RN's belittle all staff around them and Infront of visitors as well. No security what so ever. No one is on the same page when it comes to patients care. Many patients are rooming around the faculty with no supervision one almost left the faculty in the wheel chair complaing they were hungry.
