Experiencia fantástica: Pavilion is a great Rehab place. The people are friendly; they care and listen to patients needs. The facility is clean and the ambience is uplifting and bright. Gayathri is the director; she exemplifies a great leader. I always find her cheerful and hear out her staff when they talk to her. I saw her personally working with patients. Kyle and Magaly are awesome OT and PT! My mom cannot thank them enough for the therapies and training they had taught her. She says it's because of them that she's able to recover fast and walk again so soon after a fractured hip. Thank Magaly and Kyle! Elena was a wonderful CNA as well as Mercy and Victoria who were admirable in ensuring my mom had clean beddings after a shower. Pavilion also allows a family member to stay overnight with our loved ones. Thank you Pavilion for such a wonderful stay!

Experiencia fantástica: Great, clean, facility. A family member recovered there for 2 weeks. She received PT, OT, and help with bathing, etc. She said the food was pretty good. She received excellent care from Gloria, Erva, Christine, and Chris (in PT).

Experiencia fantástica: Thank you to the wonderful staff and healthcare professionals at The Pavilion at Sunny Hills for taking such great care of our mother while she was recovering from surgery after a difficult and unexpected diagnosis. We are especially grateful for CNA Elena Bello, for her wonderful professional care and kindness. We always knew Mom was receiving the very best care with Elena. We can never thank her enough. CNA Mercy was wonderful as well, and we're grateful to Jackie for all her help. Most of the CNAs & nursing staff were very helpful and professional, and for that we're giving a 5-star review, especially because of Elena and Mercy. (Sometimes the response time for the call-light from some of the other CNAs was unacceptably long in the evening, 35 minutes or longer, very concerning for elderly patients at risk of falling.) We're grateful for the nursing staff, administrators, rehab therapists, custodial staff--everyone. The Pavilion was clean, comfortable, and pleasant. We loved being able to have family members visit often and in comfortable circumstances. Thank you!!

Experiencia fantástica: My Dad was a patient here for a little over a month and they took great care of him. He was moving every day and got to know the staff. He loved each worker and everyone was very kind and caring. The facility and environment itself was really nice and very clean. Every well taken care of. Experiencia fantástica: My wife did rehab here after her surgery. She was there for a long time—well over a month. The experience here has been wonderful. The rooms are large which was so important and great because my wife has dementia so I or a family member needed to stay in her room with her at all times so we were able to sleep on a cot there. The staff was also really friendly and professional—especially our CMAs (Elena and Mercy) who were so amazing—-so friendly, so kind, helpful, compassionate, understanding, very very patient and made my wife feel happy and cared for. They got to know our family and were so helpful to us during this difficult time. We are very grateful for them. Elena was really instrumental in making this such a pleasing experience for my wife—my wife really took to her. Also compliments to the sweet maintenance lady—she’s so sweet and always said hello to my wife and me. Dr Park and all of the nurses and physical therapists were all so great and friendly too. My wife had improved greatly under the care here. I highly recommend The Pavillion at Sunny Hills if you’re looking for a skilled nursing facility!

Experiencia fantástica: I currently have my father in The Pavilion at Sunny Hills, he has been here two weeks and I can not say enough about the staff and doctors. Our CNA Elena Bello is the most empathetic and caring person. Elena takes the time to listen to my dad as well as me, she is attentive to all of our needs. Elena has made this unfortunate situation as comfortable and pleasant as possible and can not thank her enough! Dr. LaMotte is wonderful and can't thank her enough either: she definitely was the reason my dad will recover and go home. Should you find yourself in a position to need a Rehab facility I highly recommend this facility. Thank you to all of the Maintenance team, Nursing team, CNA team, Food Service team, Case workers, Social Workers, Rehab Team and Receptionist team!! Thank you!

Experiencia fantástica: My husband was their for rehab 17 days. With the great clinical staff and wonderful PT/OT Dept, he is stronger and able to walk w his walker at home. His Parkinsons Disease is still progressing but a big Thank you to all of you. We appreciate you. Curtis & Linda

Experiencia negativa: Not a place to do rehab. The PT and OT staff changes daily. Most CNAs are students on a fast phase to earn credits. Kaiser Doctor was very inconsistent and out of site. I was overdosed and hidden for the most part. Pain management meant I had to request for meds for hour to get them. I did have a couple of positive relations with one RN and CNA. Do not send a love one here if you want them to deal with things in a positive and passionate way.

We are sorry your experience did not meet your expectation! In looking at your 8 day stay, you did have consistent therapists but on their regularly scheduled days off, you did have those services provided by another therapist! Our staff C.N.A.'s are not students as suggested however our site is utilized by a nursing school for training of students who will shortly be graduating. They do work on the floor in partnership with our staff C.N.A.'s and we are happy to partner with these students. In regard to your physician, that is your physician provided by your insurance company as we don't employee physicians. If you have specific issues with them or what they have prescribed, please contact your insurance company. We wish you continued success in your recovery and happy you progressed with the care of our staff to transition home.

Experiencia fantástica: We has the best experience at Pavillions. All the staff including the cna's and nurses and physical therapist and occupational therapist helped our mother recover fully after her fall. The facility is amazing too. It is very clean.

Experiencia fantástica: Shout out to everyone at The Pavilion at Sunny Hills. My mom broke her femur and needed rehab after a brief stay at St Jude. We looked at numerous Rehab facilities in the area and decided The Pavilion was our top choice. We were told it was full, but with help from Carlos and John we got mom in. The admin staff here are phenomenal. We had no idea what we were doing, but Carlos, Eve, Mariana, G, and others (I'm sure I'm forgetting) we felt welcomeand got all our questions answered. The nurses or aides were awesome. There were numerous great aides (with 24/7 care) that really stood out. Some that we really owe a great deal of gratitude to for taking care of mom are Maria, Ryan, Ronnie, Moises, Cecilia, and Hannah. We/she could not have done it without you guys, THANK YOU! It was a bumpy road getting mom well enough to go home, but the nurses helped make us as comfortable as possible under the circumstances. The day to day care, rehab, PT, OT, bathroom needs, medications, were all handled by these wonderful people along with many others. The nurses even helped us celebrate Mom's bday in the facilities activity room. The only con would be the food, sometimes it was below average, but it sufficed with as many beds as they have. There is no doubt that if asked, we as a family would definitely recommend The Pavilion at Sunny Hills.

Experiencia negativa: The personnel of The Pavillion did not give my husband's blood thinner to him for the first week and a half he was in The Pavillion and for which he had a standing order. Thank goodness his Kaiser medical doctor was monitoring him while he was in rehab there. She called me and informed me, and she ordered him to stay in rehab a week in order to monitor my husband while he was given his blood thinner by injection. The most infuriating part is: my husband questioned them about not giving him his blood thinner medication and they shut him down and ignored him. The Pavillion put my husband in a life threatening situation. And the woman at the reception desk was rude. In the middle of my sentence asking her what room my husband was in, another staff member came up and interrupted me, and she asked the reception for some information. After standing there several minutes I intervened and asked for her to tell me my husband's room number, which she did. But she didn't give me any directions to the room. I asked someone in an office further down the hall. Just amazing to me that neither person cared about me at all. I can see a short question-answer for the two of them but my info took about 15 seconds. I do not recommend this facility.

Experiencia fantástica: Our grandfather was taken well care of and content. Not only that, but the doctor and staff help him be able to go to a family wedding while there. They went out of their way to help us get a ride for him and his wheelchair. It was so impressive… Very satisfied. Her grandfather was able to get a haircut, and the staff dressed him so he would feel happy to go. The doctor, front desk, Social worker , and Case Worker were all very very helpful and attentive to our needs. The nurses were very friendly, and our grandfather felt he made friends with them.

Experiencia negativa: I wish I could give this place a zero star. Giving a one star is too modest, given how unprofessional they have treated our situation. My grandmother was sent here, for rehab. The staff and doctors were very nice and helpful. My family and grandmother really enjoyed the care and service. Once my grandmother healed, she returned home. Unfortunately her condition worsened and she needed to return. For some reason my grandmother was “banned” and could not return to this rehab center. This was shocking news to my family and I. Apparently my family was “mean” to the staff and “were not allowing” the nurses to do their job. There was no discussion or warning from any staff members of this “behavior” nor any documentation to support their reasoning. The obvious goal of sending her to rehab is to heal and get better, therefore, why would my family be “mean” or “not allow” the staff to do their job? I come from a large, supportive family so I can speculate it has to do with the amount and/or frequency of the visitors but this is not a reason to ban anyone, especially without warning. It is unprofessional from this rehab center to instantly ban someone from treatment, especially with the lack of documentation and reasoning to support the ban. This action impacts and hurts my family tremendously. Now that she is banned, locating a rehab center has been challenging.

Experiencia fantástica: The Pavillion was surprisingly wonderful! I didn’t know what to expect with a rehabilitation facility for my father and it was excellent. The facility was so clean, Dr. Park was communicative and kind and the administrator Mike Pierson was friendly and really seemed involved with patients and staff. My father received regular therapy (the PT was amazing), meals, CNA support, and wound care. He looked so much healthier and an existing would for over a year actually healed while he was there, even though we were told it wasn’t possible. The wound care nurse took incredible care of him. We actually wanted him to stay longer bc the care was so well rounded, consistent and told notch. We are grateful to the team at Pavillion!

Experiencia negativa: Mom did okay in skilled nursing. Was walking 35-50 ft with help. June encouraged us to let her stay in custodial care for 2-4weeks, so that she could exercise 5 days a week and get a little stronger. We reluctantly agreed. In those 4 weeks, she was exercised twice, not 20 times. She wasn't getting her food cut and started losing weight. In 4 weeks, she went from 168lbs to 142lbs. She ended up in the hospital and was sent home on hospice due to severe malnutrition. She is paying $200 a month for a year, to pay for that abuse. She needs 24hr care now and doesn't have money to pay for that. She has been failed every step of the way. Update... she couldn't rebound from being starved for a month, she passed away

My mother said she wouldn’t have a problem ever going back to this location if ever needed. Experiencia positiva: We felt that the rehab scheduled for our loved one was not enough for his particular needs. Scheduling was not reliable or dependable. He was oftentimes waiting for rehab hours after the scheduled time and sometimes not showing up at all. We spoke to the director of rehab stating our concerns to no avail.

Experiencia positiva: There are many good & not great things about any facility, the issues of ‘too many contacts & dangerous parking are our experience so far. 1: So many different points of contact to get updates on status: why so many? Multiple calls required. Front desk often overwhelmed. 2: Parking lot is unlit, dark, on a slight hill, loose rocks & dirt- so frightening as a single female to walk to parked car in pitch dark after 5:00 pm!! Unacceptable! What about older clients? Light that Parking lot!!!

Experiencia negativa: If you love your family, do not send them to this location. They have an inexperienced staff. They over sedated my loved one in such a manor and ambulance had to be called! I will advise everyone that I come in contact with regarding the care at this location.

Experiencia negativa: My father received the absolute worst care. I would not recommend this facility to anyone. I am extremely disappointed in the care provided to my father. So sad when you put your loved ones care in someone else's hands who could careless. Horrible experience. Disappointing staff and nurses. I do not recommend this facility.

Experiencia negativa: Where to begin? If I could give 0 stars I would. This place seemed to get worse and worse throughout my grandmother’s 3 week stay. My family started calling it Sunny Hell. The communication from the staff to the patient and patient’s family is HORRIBLE! Good luck getting them to answer the front desk phone. You have to call repeatedly before anyone picks up. One time they finally picked up on the fourth call, but all I could hear was the staff laughing/goofing off and they hung up on me—TWICE! Also, they never call back or will “transfer” you and leave you on hold indefinitely. The worst thing was that there was a COVID outbreak and after my grandmother tested positive, it took them 2 full days to get the Paxlovid treatment to her. Worst of all was that they committed a CRIME by recording in the log that they had given it to her the day she tested positive, when they hadn’t. They even LIED to the various family members that called to see if she had received the Paxlovid yet (my grandmother had told us she had not received it). This UNETHICAL and DANGEROUS CRIME of lying about medication that was not given (and going so far as to falsify medical records) was discovered by a Kaiser rep. There should be some serious repercussions for what they did! One night my grandmother told me she had not yet received her pills. When I called they said that the only person that had the key to the medicine storage had left, so they couldn’t give any patients their meds until they could pry open the med storage. SERIOUSLY?! She did finally get her meds later that night. The food is so bad that grandmother could not eat most of it. I started getting doordash to deliver food because I was worried she was not getting enough nutrition. Each time I tried this was a nightmare. They would not bring her the food until hours after it was delivered and cold/unsafe to eat. Even when I called ahead of time and they assured me they would bring it to her right away, they left the food sitting for 2 hours. One time someone even ate the food I had delivered. Though they later sent someone to replace the chicken that went missing, my grandma didn’t end up getting the food until 4 hours after it was initially delivered. In conclusion, this place is an ABSOLUTE DISASTER. They are unprofessional and uncaring. They neglected my grandmother’s needs. I’d like to add that I was nice each time I called, but dealing with them was an extremely frustrating and stressful experience. I would not trust them to give proper medical care/treatment to anyone. DON’T SEND YOUR LOVED ONES HERE!

Experiencia positiva: My husband had a stroke was paralyzed on right side was put there for therapy. I told them he may try to get out of the bed because he didn’t understand he couldn’t walk. He fell out the bed the first night they said they would put a pad down and they didn’t. The next week he fell again then they wanted to put a bed alarm on the bed, but they never did. He got sick because of the narcotics they kept giving him for pain. It’s a nice place but they are truly understaffed. I also don’t like that the wall is there and you have to go in the patience’s room to see them.

Experiencia negativa: My mother's stay went terrible, she was having a heart attack and was calling for a nurse and nobody came in. She called my brother who called the facility and told them are mother was having issues and to go check on her. My brother drove there and when he got there nobody had still gone in to check on her. He asked them to call 911 and they tried to talk him out of it. She had open heart surgery yesterday !

Experiencia positiva: I am unhappy with the time lapse of attending to the patient. Some of the staff passed off obligations of care to other staff members. I was there visiting everyday. I understand the staff shortage but it is their job to make sure a patient is tended to in a timely matter.

Experiencia negativa: On an early Friday morning, I unfortunately fell on my right side fracturing my hip. St. Jude Providence in Fullerton confirmed I had a broken hip bone. Saturday morning I was operated on and sent to the Pavilion’s at Sunny Hills across the street from the hospital. In the beginning all was terrific, each day I was making progress. With the assistance of my physical therapist, I was able to walk, transfer from my wheelchair to my bed and restroom. On one morning a week later, my therapist was assisting me and I fell. Unfortunately my therapist did not catch me and I dislocated my injured hip. I was sent back to St. Jude Hospital to have my hip placed back into the socket. (Painful) I then returned to The Pavilion’s to recover and start over again. At 91 years of age, recovering from the Anesthesia was difficult. My injured leg was attached to a foam wedge to keep it from moving and in position. Your not going to believe this… Four days later while receiving a visit from my daughter-in-law and my granddaughter, two aids came into my room to assist with lunch. The two ladies grabbed my torso and yanked me upward, I screamed as my hip was dislocated again. I again was transferred to the hospital, put under general anesthesia, and had my hip put back in place. My third time back at the Pavilion’s to recover did not go well. My body was not recovering from the anesthesia, was heavily medicated, and was unable to eat. Not long after, I was sent back to St. Jude Hospital for a UTI, a colon infection, and had lost a great deal of weight. My doctors were extremely disappointed with the care I had received and placed my on hospice care. Sadly, three days later I passed away at 1:30pm in my room at St. Jude Hospital. As I look back at my stay at the Pavilion’s, it appeared as if the organization was understaffed, many of the nurses aids were not informed of my injury or why I was even there. My son was told by the Pavilions management how sorry they were for the unfortunate recovery experience. The manager started there staff would be receiving additional training and have learned from their mistakes. I hope my death was a walk up call for the facility and may no one else have to go through the same experience. Frank Kvaska

At various points in her stay she was given the wrong (double) dose of her medications, on repeated occasions was left for a few hours after asking for help, given a pitcher of water but no glass, and once found a fly in a glass of milk brought for lunch. I believe there were one or two genuinely considerate caregivers, but it seems they were the exception. If you or a family member are scheduled to be sent here I strongly recommend you seek other care settings.

Experiencia negativa: The patient has told me that the CNA who was assigned to his room brought some trainees in one day and he was very inappropriate in. Touching and handling the patient in an inappropriate manner. Showed little to no respect for the patient's privacy. He also made very inappropriate comments to the training group about the patient. Also during his transport the patient's ice machine was either not loaded into the transport vehicle or somebody kept it because he never had it and we had to Get another one. Other than that everything was satisfactory.

Experiencia negativa: Don't send your loved one here. The administration is unprofessional and extremely hard to get a hold of. Call the only listed number and no one answers. Where did the receptionist go during business hours? Call the number, talk to the receptionist, ask for a case or business manager, their voicemail box is full. Ask to leave a message and no one calls back. My calls were time sensitive, involved insurance questions or care related. After 9 days of zero communication I took a day off of work to visit the facility. The person I needed was conveniently not there. When money was due my phone received 5 calls in 2 days. The actual nursing staff and support team were fantastic but very short staffed. I waited 45 minutes outside, in my car, for someone to bring out a wheelchair for my mother. I reminded the receptionist that we were waiting and she acted like she forgot and said they were short staffed. My mother fell during the end of her stay, resulting in multiple staples in her head and major skin tears. She was sent to ER. I called to get information and no one was available to discuss what happened. Accidents happen. The communication experience makes me very concerned. If you need to get a hold of your loved one make sure they have a cell phone close. The phone they provide is often physically out of reach.

Experiencia positiva: We had an overall positive experience at Pavillions Acute Rehab. They handled our mom's short term rehab needs very well with PT & OT and presented helpful options for home care at the end of her stay. I would say that nearly every time I called the facility the phone rang dozens of times before anybody answered, both during regular business hours and in the evenings. Obviously, this was a bit disturbing. But, generally, we had a positive experience.

Experiencia fantástica: We admitted a family member approximately 4 months ago and was recently discharged to an assisted living community. From the moment we arrived to Pavillion at Sunny Hills, we were greeted with kindness and assurance that our family member would be treated with great care....and to the day of discharge this was completely accurate. From the amazing PT/OT staff that worked hard to accomplish our goal, to the administration...Marianna and Joanna, to the cleaning crew (especially Leticia who constantly had a smile on her face and brought joy every day)..we are grateful! By far though, a special "shout out" goes to Elena. Never have I seen a more consistent, dedicated and compassionate worker than Elena. Daily she greeted our family with the same work ethic and complete knowledge of her job as well as the same friendly attitude that genuinely resonated to our family. Elena always made us feel safe and confident in choosing The Pavillion. If I could, I would hire Elena TODAY! Others that showed "over the top"care and pride in their job was Alejandra, Dan, Joy-OT, Sarah PT, Daisy PT and Jacob. Experiencia positiva: The therapy staff was great and took awesome care of my dad. Most of the nursing staff was very attentive. There were times he was waiting a long time for his call light to be answered or someone would say they’ll be back and didn’t come unless he called them again which frustrated him since he had limited mobility. Experiencia positiva: I’m very pleased with how my 86 YO Mother was cared for at the Pavilion. This is a very friendly staff and patient, even when a patient is being difficult. I cannot give praise enough to the Physical Training personnel. They are very capable and have excellent people skills. The PT training room is excellent. The facility is clean and inviting, spacious and hospitable for visitors. I gave a 4 vs 5 rating only because the food could use some improvement and response time can be a bit long sometimes, but I am very pleased with the quality of the staff and facilities.

Experiencia positiva: My 86 yr old Mom entered the facility in May and had a 3 week stay. She broke her pelvic bone and required no surgery, but needed 100% assistance. I read reviews, some great, some not so much. Well, I can confidently say that her stay at the Pavillion was very good. Look, all places today are understaffed, and we went in expecting some delay in some services like reaction time for changing diapers…Mom complained a little but understood and it was never that bad. The staff are ALL exceptionally kind and helpful. The PT staff are so good at what they do. They really got my Mom to push through the discomfort and didn’t let her rule the session, LOL. My siblings and I were there with her trading off every day so we spent many hours observing the place. The food is not so great, but seriously what do you expect. ….and I felt there were too many sweet items on the tray, sometimes . (she had no food restrictions at this time.)She did love a few of the dishes, and you can make changes and choose other options. Or even order in. All in all, we were pleased with the facility and would recommend it for your loved ones.

Experiencia fantástica: I cannot say enough about the good care that I got here. Every single person from nursing to PT and OT to the aids, dietary social work case management they all went above and beyond. The care was spectacular. The rooms were great. The food was good and I have lots of food restrictions and my CNA Elena went above and beyond to make sure that every single time my food was correct. She found out what I like to make sure I had extras of it. She made extra time to make sure I had a shower on my day of discharge, which she did not need to do. Really the care was extraordinary. If you need to go anywhere for rehab, this is the place. The physical therapist and occupational therapist listened to the challenges. I have getting in and out of my house and assisted with good strategies. They arranged for a pain management consult for me and I saw Dr Sayer and they were both extraordinary. Again, if I could give it 10 stars, I would.

Experiencia fantástica: Elena Bello cared for my father during his stay at the Pavilion. She was very kind to my father. Although my father didn’t speak English fluently Elena made an effort to communicate with him and made him feel comfortable. He would share with me that Elena was good to him and would communicate to her his gratitude and approval by giving her a thumbs up when she came by to do routine checks during her shift. Elena stood out to our family as someone who truly cared for her patients. My father has since passed away. When Elena heard that my father passed away she came to the funeral to console our family. Pavilion is blessed to have Elena on staff.

Experiencia negativa: Mine and my mom's first impression was we were scared. My mom is scared to be left there and I was scared to leave her there. When she got there she told them she had to go to the restroom. Finally, after 3 hours waiting I had to pick her up and put her on a portable toilet. The nursing staff that were supposed to be helping us told us they would be right back and never returned, they clocked out and went home. The pictures that they show must be a set stage because the actual rooms are quite disgusting. The beds and furniture are old, have chips, wear marks and look like they were picked up off the side of the road and refurbished unsuccessfully. The window that my mom is next to the one whole side has a gap that they tried to cover up with tape but you could still feel the wind blowing in, as for the blinds that are on the window they don't close, they don't open, they don't close. They stay opened in the same position.

Experiencia fantástica: I just wanted to share my thoughts on the Pavilion at Sunny Hills. My aunt had never had to go through inpatient physical rehabilitation before, and honestly, we were all a bit nervous about how it would go. But I have to say, her experience there was really great. First and foremost, the way they treated her with dignity and respect made all the difference. The staff, especially Nurse Aid Eric, went above and beyond to make her feel comfortable and cared for. That kind of personal touch means so much in these situations. The physical therapy and occupational therapy she received were thorough and really tailored to her needs. It's reassuring to know that she was in good hands with professionals who knew what they were doing. Overall, I can't thank the Pavilion at Sunny Hills enough for making a challenging time a bit easier. It's clear that they genuinely care about their patients, and that makes all the difference in the world.

Experiencia fantástica: The Pavilion Sunny Hills is honestly the best Skilled Nursing Facility in Orange County. As soon as you walk through the doors you can feel the loving atmosphere created by the staff. My grandfather was in there facility for over a month and absolutely fell in love with all the nurses. The nurses and staff responded quickly to call lights and truly made their facility feel like another home for our family. The Pavilion at Sunny Hills has recently been remodeled and is now a top tier state of the art facility. If you want your loved one to be in good hands, and feel at home, I highly suggest this facility!!

Experiencia fantástica: My mother-in-law was admitted to Pavilion from the St Jude emergency room. We were pleasantly surprised with the large rooms and attentive staff. We arrived at the facility by ambulance in the middle of the night. The receiving nurse was very thorough in going through all of the meds and orders. The staff of the Pavilion made every effort to provide the best possible care. The team worked to coordinate medical treatment and physical therapy. My mother-in-law truly enjoyed the staff members and was sad to say goodbye.

Experiencia fantástica: I was scheduled to have a total knee replacement and wanted to go to a rehab facility for one to two weeks before returning home. My daughter and I visited several before the operation. Eve Ankenman, Admissions Coordinator, graciously gave us a tour and answered all of our questions. We observed smiling and friendly staff. The rooms were large and roomy with furniture and accessories that made you feel right at home. Eve assured me she would do her best to accommodate me and make a smooth transition from St Jude Hospital. And she did! My daughter contacted Eve on the day of my operation and she had everything ready for me. The stay provided just what I wanted. I was given Physical and Occupational Therapy every day by a wonderful, caring staff. My recovery proceeded so well during the week that I was there. I will probably be having my other knee joint replaced soon and am planning on returning to The Pavilion to have the friends I made help me on my recovery.

Experiencia fantástica: Our dad has been at the Pavilion for 7 months now. When he first got there, we didn't think he'd ever come home. However, they have really worked with him and their goal is to discharge him. The staff has been very kind to him. Particular shout outs to Susanna C., Efrim, Ya-Ya, Joy, X, Mariana, Mark, Patsy, George, Lucy, Elena, Francisco (maintenance), Latrice, Kimberly, Trish, Jenny.... They have been really great at following up on requests. Sometimes it takes longer than we hoped for, for a response, but considering how many residents they have, they do a good job. They are caring and competent. I have heard horror stories about other facilities and we feel blessed that our dad ended up at the Pavilion. The key is staying in communication with them, visiting in person as much as possible, and honestly showing them a little grace and patience. I would totally recommend the facility!

Experiencia fantástica: Very nice facility with spacious and clean room. Our family membets really appreciated the service provided. We especially thank to two CNA, Felix and Nati. They are really dedicated to patient and frequently come and check to see if any help needed. The kitchen need to be more organized for patient's special orders or menus. We sometimes requested different menus for certain food but most of time we didn't get it. Other than than we highly recommend this place

Experiencia negativa: If this place could receive zero stars- then I believe it would be suitable, realistic, and honest review of this dreadful place. My aunt Leslie Spinella had moved her father William Arthur Sloan III into this facility where he stayed for almost 9+ years. At first, the treatment he received was impeccable. Although with constant changes in management + staff, The “impeccable” treatment grew to be short lived. his time there turned out to be a complete disaster that has traumatized our family since. In the many years he lived there - he was transported to St. Jude on many occasions. My aunt was never notified of his transportation for hours after he had already left. His specific medical conditions / concerns were never addressed either. When visiting, her father had soiled his pants- it looked like he had sat in for days, which led to new issues like infections on his bottom from poor hygiene. During Covid, the family would not let family members visit but ensured families we could see our loved ones via a screen door at facility which they never followed thru with. He was consistently moved rooms to either 2 or 3 people in one room. When he passed, they put all “his” belongings in trash bags when my aunt received them. Majority of the items were not his. Almost all of his belongings he entered or obtained within the time he was there were not included either in these trash bags. He was missing his watch, custom shirts made for him, a fridge, tv, and posters. My aunt Leslie had to fight tooth and nail to track down some of these items and received only 2 of the many missing items. The most horrifying thing though, was the facilities zero regard for ensuring patients would not leave the grounds. Her father had left right out the door one day because he enjoyed being outside… Yes, he did leave the grounds but he freely left thru the front door in his wheel chair with staff all around. Hours later emergency services had contacted the facility stating he fell out of his wheel chair on a sidewalk. Emergency services was contacted by a nurse driving by who saw him laying on the ground. was called by and st jude nurse driving by. Please do not send your loved ones here. Save yourself the heartache. No person deserves this kind of treatment.

Experiencia fantástica: The Pavilion at Sunny Hills was absolutely AMAZING!!! Our Family couldn’t be more Satisfied with the Service my Mother received. Carlos in Administration was wonderful! He went the Extra Mile to assist me with placing my Mother in the facility. My Mother was so impressed with the Nursing Staff. They took excellent care of her. The Pavilion took such great care of my mother that she was able to go home one week earlier than expected. Experiencia positiva: The room and grounds were very nice. My mom had a lovely view of the gazebo area. She really enjoyed the view. Our initial experience with staff went very well. However, the last half not so much. The staff seemed to get weary of helping her out of bed to use the bathroom. At times she was encouraged to just go in her diaper or they used the wheelchair instead of the Walker. One aid even told her she was too large to help. That is a damaging comment and is not acceptable. She is large, however she didn’t require a lot of lifting, more of a spotter in case she started to fall. Several times the aids commented that she didn’t need much help. She was in your facility to regain stability and strength after a stroke and these options were not helpful. She also had times where after waiting 30 minutes after pressing her light she had to have my sister call from the outside to send someone to her room. These issues were disappointing, after it started so well.

Experiencia fantástica: Outstanding Rehabilitation center, staff was wonderful and treated my mom with up most respect, assisted her with all her needs. She went in because she fell and broke her hip, however she has other issues like needs a knee replacement and had a previous shoulder replacement about 4 months ago. This center gave her therapy for not only her hip but also her other injuries I went to 6 different Rehabilitation centers for a walk through, and The Pavillion at Sunny Hills was by far the best place I felt ok with my mother being there. She had her own room, with a refrigerator for snacks. I will be 100% honest she did not care for the food, however that was the one and thing she complained about. She was there for a little over a month. All the nurses came in to give her hugs and wish her well when she was discharged, and this was the fist time my mom actually shredded tears when leaving a hospital. I highly recommend. Experiencia fantástica: I have good news and bad news..The bad news is that I fell in my home and suffered an acute fracture of my right inferior pelvis. The good news is that I was able to have Pavilion available for my rehab. I previously was at this facility and received wonderful care for a prior problem. Even the food is good! I was warmly greeted by many staff members who recalled my earlier stay The overall quality of care has remained impeccably good in all regards. In my opinion the quality and care provided by the CNA's are the heart of what makes any rehab great. They are the crew with whom the patients have the most daily contact. Once again all the CNA staff has proven to be superstar! My daytime contacts (as in my previous stay) have been mostly with CNA's Elena Bello and Merceditas Abayata and they still consistently go above and beyond in providing the care I need. Thank you all for making my current rehab stay successful. I would not hesitate to recommend this facility to any needing the service they provide. Michael Riskin (Here is my previous review From Last year): I have had a series of illnesses for several years requiring extended stays in four nursing/ rehab facilities…Pavilion has been in a league of its own in every aspect of care…Truly Excellent!! They Provide the daily care that makes the stay A “ feel good, healing experience “.
