Experiencia fantástica: The therapy team is amazing especially Mike and Patrick. I went to the hospital for 9 days and transfer to Park meadows Health and Rehab Center and I met those two guys. I could not walk, stand and did none on my own. It takes 1 1/2 week and I was able to stand, use the restroom by myself, and walk using a walker. After three weeks, I’m using a Cane to walk. And getting dress on my own. That! Is amazing.

Experiencia negativa: The worst nursing home I have ever seen patients are not being taken care of at all. The facility is so dirty and no house keeping. Wounds are not being cleaned getting infected. The staff sitting around doing nothing. No supplies are in the rooms or available. This place need to be shut down now.

Patients wandering the halls . Nurses and staff busy on their phones , laughing and joking around instead of taking care of business. Staff walking around with pants below their but , hats, hoodies covering part of their face Can't visit family in their room nowhere to sit. Roaches running around . Was there 3 hours a elderly cried for help no one checked on her she was still trying to get staffs attention when we left. I could go on and on etc... Something needs to be done about this place

They didn't give or offer a shower or bed bath and he was there a week Had just recently had surgery and his wound needed more attention than they we're willing to give. Wanted to leave him in urine covered clothes and just put a pad under him for next shift to take care of!! Didn't wipe his behind good and left him dirty!! Terrible place to send anyone I would NOT recommend!! IF I COULD GIVE ZERO STARS I WOULD.

Experiencia negativa: This place is outdated, buys the cheapest food, does not meet dietary requirements for residents, run like a Concentration Camp. None of the food is appetizing or edible. The best dedicated employees are not compensated for their work. None of the photos online are of this old building, or employees. Maintenance is nonexistent, owners don’t talk to or see deplorable conditions. The State guidelines aren’t adhered to, the only correct statement is it is an LLC, for profit of the owner. God help the residents.

Experiencia negativa: I was there for 10 hours before I had to call 911 because the concentrators they had didn’t work and when the alarm went off no one came I went without oxygen twice once for 15 mins and once for 30 mins and went thru 5 non working concentrators they don’t answer the call button. And I forgot to mention I was paralyzed and could do nothing for myself. I call Department of Children and Families to investigate DO NOT PUT YOUR LOVED ONE HERE. I tried to put no stars but it won’t let me.

Experiencia negativa: I would give this place a zero stars, but it won't let me. My father was here for a few weeks after brain surgery for recovery. In those two weeks he developed a major bedsore. Everytime I visted he was in a dirty robe and wet, everytime. The cockroaches were entertaing to watch scamper across the wall however. The only positive reviews are from the staff, trying to keep the place open so they can continue to have a job where they are allowed to slack off. The staff reviews with selfies are hilarous and should be the red flag youand the authorities are looking for. It is sick the waythey treat people and is criminal. I have been inside alot of retirment homes and none have smelled of urine as much as hole.

Experiencia negativa: I am a current patient, the staff are very rude. And I wouldn't recommend this place to anyone. It's bad enough I have to be here but to endure the constant rudeness of staff is very sad. ... Unfortunately I came back to Park Meadows because it the facility closest to my apt. I don't even know what to say. Getting someone to answer my buzzer is ridiculous. 1 to 2 hours. Getting medicine 1 to 3 hours. Getting medicine from pharmacy is ridiculous. Over 24 hours and still haven't received all my medicine. They play pass the buck. No one takes responsibility for errors and the fact that patients aren't getting there medicine.

Experiencia negativa: I was recently her for short term rehab. I can think there were at least 2 days maybe more that i did NOT receive my IV antibiotics. I waited hours for call light to be answered. One evening i was literally hanging off my bed and no one would answer my screams for help or my call light, my roommate ended up walking out in hall to get help, my fall was preveneted. Many many times I asked for social worker/case management and got no one. I wanted to come home, they'd say I needed something first, so I got it. Then they'd say i needed something else repeatedly. The food GOD awful. The only thing I got there that was good was fresh fruit, chefs salad and cereal. Half the time they were out of these things. They sent tartar sauce with chicken. Awful. The ONLY good thing I can say about this place is the PT/OT people were good. Good to me. The equipment they have to work with ... needs updating badly. Don't be fooled by the pictures on the website. This was mostly why I was sent there. Of course they stopped having me on their schedule the last 10 days I was there. I don't know who's fault or decision that was, what I do know that's why I was there and it didn't happen and I got excuse after excuse why I couldn't leave from administration. I was held against my will,because the ONLY way they'd let me go was if I signed out AMA (against medical advice). DO NOT send your loved ones here. DO NOT go here.

Experiencia negativa: This place is a joke. 1 nasty. 2 the run out of food regularly. 3 the physical rehab is a huge joke!! Most not all folk there are not nice. I never got better. I'm now in a top notch rehab in Jacksonville. Better than Park Meadows. I'm getting better with the proper rehab!

Experiencia fantástica: This place is unreal.. My mom LOVES IT AND ALL THE STAFF!! They do EXTRA things for you that other Rehabs dont..They have strict LPN's and the care is awesome.. Even when my mom goes to dialysis they pack her a lunch!!!!!!!!!!!!! also the place DONT SMELL LIKE PEE like all the others. I knew a nursing home didnt have to stink!!! and this place PROVES IT. YOU ROCK PARK MEADOWS!!!!

Experiencia positiva: We have been happy with the care my mom has received at Park Meadows. She is new to the facility. She came from an awful place called Magnolia Ridge. That place may be new and beautiful but the lack of care is horrible and the nepotism is in full effect. They only want your money!!! I would NEVER refer anyone there! EVER! Park Meadows may be an old building (I hope for renovations) but at least every time I walk in everyone I see in the hallways smile and say hi. The residents are always socializing and out and about throughout the facility. At Magnolia Ridge the hallways were empty, like a ghost town. Good job Park Meadows!!! And thank you! I would have given 5 stars but the food and the building definitely needs work.

We chose Park Meadows Rehab because the person in contact with us was extremely knowledgeable and helpful with her wellness. I praise the staff and my grandmother has fully recovered. Everyone was helpful and and having them follow up with was a plus! Thank you Park Meadows staff and administration.

Experiencia fantástica: I came to park meadows as an agency nurse, which I had been for a few years. After almost a year I committed to a full time employee. I can say that I am glad I made that choice. I had been wanting a place to call "home" and this place felt like just that. I like the way that they aren't micro-managers. You do what you're supposed to do and you have no problems. Your also acknowledged for the work you do as well. I have since been able to get a promotion into management and I am loving it even more here. I can now say that I have made Park Meadows my "forever home ".

Experiencia negativa: I give 5 stars to Shalaka, Mrs. Santiago , and all who work in the Physical Therapy Department because thus far they have gone Above and Beyond to help me in anyway humanly possible. The majority of the employees I have received service from show they have extremely poor work ethics. Thankfully there are some that will help you with a smile . Most of the nurses don’t care or have compassion. Ebony is one of the nurses that is mean and cruel.. Unfortunately because of the treatment received from them I was forced to write a grievance letter, in hopes others don’t have to suffer in the hands of the folks receiving a paycheck to do their jobs. Today Debra the CNA refuses to remove the bedpan it’s been sitting here since 8am. I spoke to Genesis the nurse and I spoke to Dr. Martinez however the urine and feces is still sitting in my room. Today is Saturday August 27th of 2022. The feces has been sitting inside the room since 8am est.

Experiencia positiva: Park Meadows has come a long way. The pandemic hit the facility really hard causing a lot of staff shortages. The staff that remained have worked over and beyond to provide the best care possible to the residents and patients.

Experiencia negativa: We have filed complaints with the state about the lack of care and communication at Park Meadows. In just two weeks my uncle has developed bed sores so severe that his health is at risk. We called and emailed numerous times and have never had one return call or email. Bianca, the social service director , doesn’t have a voicemail according to the front desk and has been in a continual meeting apparently for two weeks straight or out of the office even when her name can be heard being paged to different locations. We knew instantly this was not a facility we wanted our loved one in but getting our loved one out has been nearly impossible. Just go see and smell the facility for yourself…

Experiencia positiva: Staff has been very supportive, and they work well with your school schedule. Competitive pay rates for Nurses and CNAs no facility is perfect but this one has the staff and the management team that is willing to put in the work to get it there.

Experiencia fantástica: I had major surgery on my right leg on April 20, 2022. I was send to Park Meadows April 24th for rehab. Park Meadows is a very old facility. The building is in great need of renovation. While they are under staffed most of the time, I found the staff to be friendly and caring. I give this part of the facility a 3 star rating. I rate the Physical Training department “5 STARS” for overall excellence. The members of this department far exceed expectations. They are caring and totally dedicated to they’re patients. If you are in need of help recovering from any surgical procedures I highly recommend Park Meadows PT department.

Experiencia negativa: MAJOR staffing issues and problems left unaddressed. Here's what happens: staff either call out or just don't show up. That's bad enough BUT there is NO recrimination at all. It is tolerated since "we can't fire them because we need them". WHAT IS WRONG WITH THAT STATEMENT?! Yes, we need them - to SHOW UP! By allowing staff to show up, or not, this is effectively rewarding them while PUNISHING the reliable, dedicated, caring staff! The result of that is a direct impact on patients. Those upstanding, dedicated staff are left picking up far more work than they can manage. It's no surprise that they are upset! Being over-worked and knowing that management does NOT have their back and that this will continue translates to short, sometimes rude, interaction with patients. Being one of those patients I can assure you that this is EXACTLY what is happening. Care is not what it needs to be, what it SHOULD be, what we all deserve. This facility cannot attract and retain GOOD staff because of the burden placed on them by tolerance of POOR staff. This is absolutely the #1 problem. And because of the staffing problem, so many things are left unaddressed. I've not been able to get results of blood tests tho I continue to experience symptoms! (For almost 2 weeks now.) We've reported having COCKROACHES IN OUR ROOMS and, like other problems, that is "on the list" to be addressed. Excuse me??? PESTS need to be controlled and exterminated IMMEDIATELY. PLEASE! Why are cups for water not available? If you don't save your cup, you're out of luck. And dehydrated. Thank goodness I have scissors so we can make washcloths out of towels. Shall we continue to pour funds into use of agency nurses and CNAs rather than provide properly for the patients? Oh, yes, back to issue#1.... I hate that this is such a negative review. But it is truthful. And I apologize to those WONDERFUL staff who care, who show up, who continue to struggle through the culture established here. You are GREATLY APPRECIATED!

Experiencia negativa: They have a lot of work ahead of them in the improvement department. Struggling with the food service and retaining CNA’s that actually work. Rely too heavily on outside agency help who are only here to collect a paycheck and do as little as possible.

Experiencia fantástica: No one enjoys having to deal with the inevitability of dealing with the declining health of a love one, and certainty not having to place their loved one away from their home and into a facility. Although burdened with this decision and all the issues that related to this, my family and I are very fortunate for Park Meadows. The nursing staff along with the leadership genuinely cared about our family’s needs. They provided care that focused on the needs of our family member, and resources to assist us with making informed decisions about care that best suited our love one’s situation. When we experienced an issue- the administrator, Alexi, personally came to address our loved one about the issue and followed-up with us once the situation was rectified. He even circled back with us weeks later to make sure everything was still okay. I don’t wish declining health on anyone, I will say that if the situation presents itself, you want to be somewhere you can get the help you need from people that care. This placed worked for us.

Experiencia fantástica: Only had my Mom here for 2 days and I love the staff. Very kind and genuine, makes a huge difference. I only had one little complaint and the Director of Nurses took care of it immediately. I was a little weary pulling up, it's an older facility. But the inside looks refurbished and clean.

Experiencia negativa: Nurses do not care, are not friendly. Speak to the visitors just as badly as the patients. This is a place if nowhere else is available, and even then, only until you beat a nurse for mental abuse.

Experiencia negativa: If I could give it a zero rating, I would. My father passed away in this facility, he was only 56. He wasn't being watched enough and he complained all the time about it being awful, and there was nothing we could do but visit until he got back home to us. He was only in there for rehab to walk on his own after he broke his femur, and had surgery on it. Came into the place on a Friday night and took his last breath by himself on the following Wednesday. After his passing they made my mom drive all the way back up to the place just to fill out a blank piece of paper. It was very insensitive to make her drive all the way there, where her husband just passed. And they could have just mailed it or faxed it over. Also the lady that gave her the paper gave her extreme attitude and I was shocked how she handled my mom. Just today, mom asked for his records and the lady told her that she couldn’t have them. After my mom proceeded to tell her all the paper work was done, and how he died there, she caught an attitude and said “He didn’t die here, he died at the hospital.” My father took his last breath there, and wasn’t found for over 10 mins. Even called a code blue on him. I hope they get shut down or maybe even get a grip on emotions. Very insensitive and rude staff.

Experiencia negativa: My brother was put in this place for Rehab due to the fact of his foot surgery. I decided to go down and see my brother for Thanksgiving. Walking in to this place their was no one at the desk to greet you or to give you any information about where to locate a patient. Walked around the corner and all you see is nurses standing around chatting and not doing their job. The smell in the facility was gross smelled like poop all over the area. We had to walk through a dining area where they had people that were consider to in a nursing facility area. We finally found my brother. Only to see more nurses standing at a desk chatting away and not doing their job. After talking to my brother I knew then that this place was a SERIOUS JOKE, that no one that was there needed to be there. Even management could not do their job except for a gentleman named Josh. He is the only one that has stepped up and did his job. Case manager, Director, or head nurse and nurses can not do their job. Your love ones will stave in this place because the food is gross and cold. Not even a human being will die in this place. I strongly believe the HEALTH DEPARTMENT needs to inspect this place and they need to fire and hire new people that have a heart and want to work with patients that are in need. My brother did find 2 night nurses that took care of him only when they worked. If they were not there then that place is SERIOUSLY DOWN HILL. So thank you ladies for taking care of my brother the night of Thanksgiving. The one that filled up his cup of water and the other one for coming in getting his blood pressure and temperature. Thank you Josh for doing what you did. With these 3 being there they are the only ones that have a heart and care about their patients. That place need more people like them!!!

Experiencia fantástica: I'm not a fan of these facilities that house the elderly because of the problems that exist with insufficient staffing. I've read some of the negative reviews and I disagree with them. My most recent unannounced visit to the Park Meadow Health & Rehabilitation facility was in November 2018 with my sister. I found the facility clean, no smells and my sister and I sat in the room that service as a dining room until dinner was served, (what people need to realize is that this is a hospital like setting and they are not going to get their mothers cooking). then my sister and I were given a tour around the facility and we found it and the employees to be pleasant and presentable. We've been making visits like this to different facilities for future references for our selves (just in case) and Park Meadows Health & Rehabilitation Center passed the test.

Experiencia fantástica: They are taking very good care of me here.. ..plus excellent nurses, doctors CNA's, physical therapist for rehabilitation. The physical therapist here really cares for all of us and our recovery . They also have activities to do here daily such as Bingo,movies, nails&hair, etc. Yes also great food as the Chef here Russell(whom we call him Chef) is a great cook and makes sure our meals taste good and are fulfilling to our appetites.

Experiencia fantástica: My mother-in-law died 1yrs ago, the staff and the director was very caring, to her last needs, and my family needs. The director had sent the family side foods as all the family gathered in her room. That was very touching to my wife and I. The director Alexi was very hands on with the transition of my mother and law to her next journey. His staff was very caring, and that touched my heart.

One of the nurses gave me a TB test. I fainted after she did it. Their were bugs everywhere, and dust, and the doors didn't close properly. LMAO managment is a joke. This place should be closed down.

Experiencia positiva: My sister is currently at this facility and the staff has been great so far in caring for her. I haven't noticed the times I've been there any smells or issues, but realize also they have some geriatric patients that have issues.

Experiencia negativa: TERRIBLE, MY BROTHER NEVER COULD GET HIS MEDS, THE PEOPLE ARE JUST THERE COLLECTING GOVERNMENT FUNDING.. BECAUSE THEY DON'T GO OUT OF THEIR WAY TO EVEN TRY AND HELP YOU BE COMFORTABLE.. NURSES ACT LIKE YOUR PUTTING THEM OUT.. JOSH THE NURSING SUPERVISOR WAS THE ONLY ONE THAT ACTUALLY TOOK THE TIME TO LISTEN.. BUT HE STILL COULDN'T HELP WITH WHAT THE DOCTORS ORDERED, AND THEIR TIME BETWEEN TREATMENT.. WELCOME TO OBAMA CARE..

Experiencia negativa: horrifying!!! these people that run that place need a complete change. they hire people with questionable backgrounds. found out about one that mistreated my dying mother. hr would not give me the name. I was unable to find this person thanks to hr protecting their staff. the employee discovered I was looking for her and she quit and found another job. then hr said they didn't know where she went.........liars. do not send you loved one to this place !!!!!

Experiencia negativa: Hell hole, YES it smells as you walk in and I was embarrassed to have my grandchildren see that. Husband admitted for physical rehab ONLY and sat in bed from Thursday till Monday and then given 5 minutes and they gave up. Just collecting insurance money and I will inform the insurance company of this. Don't need nurses who think they should be respected before the patient gets respected. You are Supposed to be trained to handle patience and you have NONE. CNA's would do better. Told if I take him home it will be against doctors dr's orders. He is NOT sick.

Experiencia negativa: ABSOLUTELY HORRIBLE! do not take your loved ones here! my mother, who had a broken pelvis and could not do anything on her own was left in pain for hours due to lack of response. my mother asked for her medication for hours only to be told that it was not there. when i called and asked them about this, they brought her one low milligram pain pill which she said was probably a placebo cause it never did anything. when she asked for a bed pan and finally got it after having to keep pressing the call button, she was left on it for more than an hour. the ac in the room was broken only making her experience there that much more uncomfortable. to say the least, we took her out of there against doctors orders because we could take better care of her than they ever did. apparently the nurses just can't be bothered. and i have a feeling the nurse who gave her the low mg pain pill didn't even consult a doctor first which goes to show how janky and unprofessional this place is. DO NOT LET YOUR LOVED ONE BE TRANSFERRED HERE!!!!

Experiencia negativa: I wouldn't board my dog in this S@!$ hole! It stinks of feces as soon as you walk in. The facilities are in poor condition and not maintained. My wife was sent there by Florida Blue insurance. They did transport her to dialysis, but other then that they could not have cared less. She was there less than 24 hours because they ignored her calls for bathroom assistance and had not changed her bandages since her arrival . Shands Hospital worked hard to keep an infection form causing my wife to loose her leg. These fatherless scum would have caused her to lose it after all. I ended up calling the Police to get her away from this place as they were threatening us with all kinds of dire consequences if we left without their approval. The conduct of the staff and the policies of the owners and management are criminal. I would give this place negative stars if I could. STEER CLEAR!

Experiencia negativa: I called to check on my best friend and the phone was answered by an unprofessional person.She let me talk to someone who was supposedly supervising him.She was very unhelpful and hung up on me.I suspect they are not as wonderful as their recordings they have praising themselves .I hope they are treating my friend better than the did me,I will find out!

Experiencia negativa: Forgot to give my mother her medication , when spoke to the nurse about this she said she was only human and we need to understand. Two days after discharge home health care orders have not been sent to insurance. My brother in law was able to go to my mothers room at 1:00am, get my sister's purse out of my mother's room walk all around and never questioned. One star only because you have to leave one

Experiencia negativa: My husband was a patient there for approximately 14 days. When you first walk in the facility you smell urine! To me this is unacceptable as I have been in other facilities and you do not smell anything. When he called for assistance they were very slow to respond. When it was time for a shower he had to bathe himself with no assistance. The employees there are not always friendly or helpful and it takes a long time to receive a call back. I would not recommend anyone go there for short or long term care. Also the food was not good and the staff just did not care. The facility was not clean and the members acted as though you were a bother.

Experiencia negativa: This place is a hell hole. It is filthy dirty, loud, uncomfortable beds, terrible food, staff has a very negative attitude and most of them don't want to work. It was the worst experience of my life. Two women,Fritzie and Diane, came to Shands Hospital to talk me into going to this place for rehab. They described it as state of the art, great equipment, great food and comfortable rooms. All of this was untrue! The social worker was the only person who cared about me transferring to another facility near my home. I was 200 miles away from home. After a week of being in this so-called rehab facility, I was transferred to Sunset Lake Rehab in Venice, FL near home. It was just the opposite of Park Meadows. It is a wonderful facility and I was ready to go home in just 3 weeks. Thank you Sunset Lake!
