Experiencia negativa: My boyfriend was in rehab at this facility . He got a uti snd cellulitus which was weeping . Finally after talking to director they wrapped his legs . The only shower he had for 9 days he was there is when I gave it to him . I could hardly ever find nurse or cna to help him . He was baker acted on 08/29 so he left there thank goodness . I couldn’t find his cell phone looked at bill someone was using it all hours of night and used almost all of my data . Not very happy about that . Would not recommend this facility to anyone . Very disappointing

I change his sheets when he showers. I don’t think they’ve changed any yet. Some nurses are very nice others need to find new jobs . Place smells like poop on some days. I don’t recommend this place at all !

Experiencia fantástica: My mom was a resident here for 4 years. She settled in well and loved being there. The care she received everyday was awesome. All the staff members were very caring, professional and the building was always clean. The nurses were always friendly and spoiled my mom. They all called her granny and of course she loved all the attention. The activities director, Mrs. Faye, is a awesome lady and she does an amazing job with all the residents to make sure they always have a great time. She makes sure every holiday that the building is always decorated. I am very satisfied and would truly recommend others to The Terrace.

Experiencia fantástica: The building might not be brand new, but the staff was wonderful to my father during his stay. Marshianna Vickers was always attentive to him and she always responded to my text and calls, for even the smallest of questions. She was polite, helpful and reassuring. Overall, the staff really made my father feel at ease which was helpful to his recovery. The staff is caring and compassionate and I am thankful to them for their care of my father.

Experiencia negativa: My uncle was in this facility a few days prior to being returned to the hospital. A prerequisite to his acceptance per HCA NFM he had to supply one of his very expensive meds. I took a full bottle and a half of his medication. Since his return to the hospital I have called and spoke with someone requesting his meds, I have left two messages and a couple of days ago I spoke with his nurse who was unable to find the meds and promised to call me back (He was assigned to room 301). As of this date almost a week later no one has contacted me to tell me to come to pick up the medication or to let me know that they have misplace or stolen it. Most definitely he nor anyone in my family will ever be placed in this facility.

My Husband is a Retired GPD Sargent. After Talking with His Nurse Ms Minnie on the 100 Hall she made it possible for us to be Moved to the 700 Hall. Thanks to Ms Minnie for the follow up . Upon the Arrival to the 700 hall every thing began to make us feel good about having to need Rehab. . The Entire Staff was very Supportive starting with Admissions Assistant Her name is Marshanna Vickers , The Rehab Staff Richard was very good with his Therapy. Overall the entire stay was good… THANKS TO EVERYONE FOR ASSISTING MY HUSBAND TO REACH HIS GOAL ! I WOULD RECOMMEND FAMILY MEMBERS TO USE YOUR FACILITY IF NEEDED THE CASONS

Experiencia fantástica: I have been blessed,extremely for my stay here. With getting stronger and my endurance as well. I have been fortunate to have certain staff members to assist me. I have to start with the CNA’s they work extremely hard. And are diligent about their work. There are certain aids that do their job extremely well! Ms Stephanie on 700 hall has been one of my aides she shines so brightly! She is all about patient care ALWAYS FIRST! I never seen a CNA like herself, she makes sure that I am well presentable and keeps my room and belongings in tip top shape. I feel like my room is as clean as a hotel when she works! My night crew aides Hailey and Tasha are the Dynamic duo! And they are the best with helping me get back to myself! Tasha makes sure she keeps me going as well they are such a blessing. All these aides should be recognized for their outstanding abilities Their names are as follows Janice, Tonya, Latecia and Rasheeda and Mrs Shirley, And Mrs Christina who is the Best on the midnight shift! Mrs Stephanie is Amazing I cannot say enough about her I have never met anyone as caring, thorough and efficient as she is I had to talk about her again, she is definitely a Golden ! Now for my Nurses, they have all been sweet and giving me the best care I cannot thank them enough. GiGi is phenomenal and such an outgoing personality, I bet she can cure the most dowdiest person on in the She knows how to make a frown turn upside down! She makes every patient feel like they matter and that no needs are too small I myself and other patients definitely look forward to having her as their nurse when she is here and things are not the same when she isn’t here. Now for my Physical therapy crew, Mrs Debbie OT! Has helped me tremendously, by strengthening my arms, shoulders, and back! The exercises Bahr has given me has been nothing short of Amazing! She has been so kind to come at bedside and help me with my exercises, I would also like to recognize Zac PT. He is very much so Amazing, I cannot speak highly enough of this young man! He works with me on my weaknesses, he I always brightens my day and uplifts me with kind encouraging words. I have the tendency to be hard on myself sometimes, but Zac has a way of being so positive and his encouragement uplifts me and keeps me going ! If you are blessed to have him as your PT you are in for a treat he is extraordinaire. To sum it up Terrace as a whole! Being here at Tereace the care I received has been phenomenal and I highly recommend this facility. From the house keeping, Drs, kitchen staff. Nursing staff, everyone plays such an important role and they have truly made my experience here Outstanding! And I can’t give them anything less than 5 ⭐️ ⭐️⭐️⭐️⭐️ They come highly recommended! I would tell any family and friends to come here for rehab.

Experiencia negativa: This is terrible. Christmas day and they're closed! For those who are here 24/7, they can't have family or friends visit on the Lord's birthday??? Doors locked, no one answering the phone. Terrible.

Experiencia positiva: The reviews seem to reflect the problem with most of these so-called rehab facilities-some good interactions and others neglected. My mom was transferred from NFMC and got very little of the rehab we were hoping for as she had already regained her ability to walk before being transferred. I don't know what the expectation is for rehab patients at Terrace, but we're disappointed that she remained so dirty for a week and was only given a little time alone on the pedal exercisers, with little instruction or assessment. We were hoping to learn and apply exercises that would help her improve and maintain her health.

Experiencia negativa: We have a patient at rehab currently. We have been trying to get a hold of a doctor or nurse for 3 weeks to no avail to find out how our loved one is doing. We have contacted the facility by phone, talked to them in person and we have yet been able to talk to anyone. Stay away from here if you have other options. They will not make it easy to care for your loved ones and know their status.

Experiencia negativa: My husband was in Terrace for several weeks due to Congestive heart failure. His blood tests were "lost" four times in a row. He gained 30lbs of excess fluid in three weeks, developed horrible rashes/oozing/weeping legs that he did NOT have going in. He died at home. So-called nurses knew nothing about his treatment and I have no confidence in their staff. If you have a loved one, DO NOT LET THEM GO HERE.

Experiencia negativa: My husband arrived yesterday 10/26/2022. The time now is 6:45 p.m.. I have been trying to call ALL . he has not had his meds. His blood sugar is low. This is patient is neglect. The date is Dec 10. My husband is back in this place I have been trying to call my husband for a week. Phones don't work., no one is helping me to talk to him.. I give a p star review. I can't get thru. It feels like they are holding him hostage.

Experiencia negativa: The nurses, CNAs, other staff seem to be caring, although short staffed. The transportation person, Shantel is great. The problem we’re having is getting any information from social services and administration for a plan of care for my brother-in-law. My calls and messages are not returned, for days. Today we were told that our request for a copy of his hospital discharge paperwork and diagnosis could take up to 30 days??? Seriously? I gave them our email address. We’re trying to arrange some kind of home health care for him. But we need an evaluation on his condition. Forget trying to get social services to help. Our frustration level rises each day! He was automatically transferred here after being released from NFLREG. We had no say so in where he was transported. Good luck to others dealing with administration here.

Experiencia negativa: Our loved one was placed at The Terrace as it there were a limited amount of rehab facilities that accepted the Medicare advantage plan. The facility is very poorly ran which starts at the very top with administrator Truvette and director of nursing Tabetha who are a very poor representation of healthcare and to be transparent are a laughing joke and embarrassment the the healthcare industry. They will defend and/or deflect any concerns you bring to their attention. If you do not advocate for your loved ones and have no knowledge of how proper healthcare is to be provided please watch out closely for your loved one as they will most likely be neglected and treated poorly when attended to. Most days the facility is short staffed and the staff that are working are overworked and have no empathy for the residents they care for. Burnout and elder neglect is clearly evident in the facility. The turnover of staff (CNA’s and LPN’s) there is a constant revolving door which speaks volumes of the facility and the leadership of it. Just try asking a question to a staff member and most times the response is I am new and only been here (x) amount of time a which leaves you with no answers to your questions. This place needs a complete whole administration overhaul and start over or should be shut down by the state.

Experiencia negativa: This place is horrible. Do not sent your loved one here if u want them to come out alive. My dad got bed sores and an infection that almost killed him. I had to call 911 to get him out. They do not do rehab with the patients. They are a disgusting place.

My family member was in here for rehab for physical therapy to walk and while there the nursing staff did not give him/her breathing treatments twice a day - which is needed for COPD. She/he ended up bringing in their own nebulizer medication and the nurse refused to return the medicine upon checking out. Is this legal? Seems meds paid for by another patient should not be used for other patients. In addition, the cleaning solution used to clean in patients' rooms with asthma/COPD are not compliant with CDC recommendations. Furthermore, the phone and call button for meds did not work for at least three days. I finally had to leave a message for the director and then it was fixed.

Experiencia positiva: The rehab here is excellent. My dad was working with Cyril, who is the most upbeat, positive, enthusiastic person. He did a really good job, and my dad really enjoyed getting to work with him every day. The place was clean and well maintained. On weekdays, the nurses and staff seemed excellent. They answered his call button in a short amount of time and did what needed to be done. On weekends it seemed understaffed, and he sometimes had to wait for his call button. The doctor on staff was Dr. Brandt. Not sure if he sees everyone there, but he is a wonderful doctor! He is extremely patient and respectful in dealing with an elderly person and seemed genuinely interested in my dad. The food was not great. The patients are supposed to be able to choose from a menu every day, but for some reason my dad never seemed to be asked about what he wanted, and was given whatever they felt like. Some of the food he was given was inedible, and a lot of it did not seem very healthy. The meat was frequently tough. Since most of the patients there are elderly, you would think they would serve food that is reasonably easy to chew. Most of the rooms are 2 person rooms. If your roommate is noisy or has dementia, you are out of luck. My dad was very fortunate that he was given a private room, because he does not like to watch TV all the time. However, the hall was so noisy that he stayed inside the room with the door shut all the time. You would think they would figure out some way to suppress the level of noise, such as using noise suppressing materials for the walls and ceilings. It is very stressful to have so much noise. Most of the time we washed my dad's clothes, but near the end of his stay he sent a load to the laundry. This was a mistake, because they lost them. We still do not know if he will get them back.

On my last visit, I was so thrilled to see how clean the building was and how much everyone seemed to genuinely care about every one of the patients there! They know all their names and exactly what each person needs. The nursing staff were all so pleasant and took the time to answer all my questions. Thank you for making me feel so comfortable knowing everyone was in your capable hands and for the taking the time to answer all the many questions I had. Britney in admissions was great!

It is absolutely horrible! If you care for your loved ones NEVER EVER have them there and if they are get them out! It is dirty and the staff is rude!

Experiencia negativa: My sister is currently a patient here. Wrong meds, rude staff, equipment needed was not ready at arrival. Understaffed (which I get because of covid) but still not acceptable. Don’t answer call buttons and roommate soiled herself waiting, they didn’t come for hours. I cannot believe a great hospital such as Shands could recommend that she go to such a place. They are aware and still not making changes, just excuses. It is horrifying that now the people “caring” for her, that she complained about, are now spiting her and holding back pain meds in retaliation. The doctor AND nurse. The doctor because she had to come in on a Friday night. I have to listen to my sister cry because she is pain, and I cannot help because I am too far and work full time. Giving wrong insulin as well. So much more wrong, there is not time to write it all. All documented, so should anything happen there will be hell to pay, I will see to it. Please, find an alternative place.

Experiencia negativa: Do not allow your family members to go here. everyone has an attitude at all times. They are not clear on what POA regulations are and how to use them. No one following up with missing medication concerns. And miss-medicating. Staff Seem overwhelmed or lazy, can't tell. The in room communication devices do not work. Claimed the patient didn't know how to use them, that was tested during visit and found to not work. Multiple other issues. CNA's do all the care. Posting this for someone else because they feared their family members care would be affected by public corrective criticism based off the attitude of the staff if they decide to finish rehab there.

Experiencia negativa: Absolutely the worst run facility. DO NOT bring anyone that you care about here for care. They are incompetent and not responsive to needs, do not follow proper medication or sanity protocols. The cna staff turnover is daily, and they have little to no training. The management staff is useless, that is if you can even find them. To say this place is a nightmare is an understatement. I am certain that my Dad would still be alive if he wasn’t in this place. This was the only place that would take my Dad's insurance and he had to go here, and the severe bedsores he got from being here and lack of care contributed to the infection that killed him. Run as fast as you can from this place.

Experiencia fantástica: In my first post alot of things were taken out of context. I was very angry because on the date of my discharge I had no family to live with or no place to go, but the Director and Social Services department came up with a solution to make sure I didn't have to sleep out in the street. They helped me with getting my own apartment. Now I have a place for me and my 10 year old daughter, who I truly miss. So you see, everyone at this facility is not bad, and their are staff that truly care. I have had 2 surgeries before coming to Terrace, and when I first arrived, I couldn't walk, and I was in alot of pain. Thanks to Physical Therapy for helping me to walk again. Now I can walk, not very far but thank God I can. When I was down and weak, the male staff helped me and they were very professional, and made me feel safe. The male staff was also encouraging. I'm not a big fan of women, but most of them will help at times, if asked. In conclusion, Terrace is not a bad place, things can be improved, but in the future things will get better because I see improvement everyday. If it was not for the Director and Social Services, I would have been sleeping outside. I am very grateful that they have a big heart to help me. Thank You....

Experiencia negativa: Residents are generally ignored and treated poorly. There are a few sweet staff members which is why I am giving 2 stars, but it seems that most staff forget that their job are the residents home.

Experiencia negativa: My mother was ignored the whole day and was given 5 mg of oxy's every hr by nursing director Tabitha. That's what she said over the phone so i can only go by what she said. I do know the paramedic driver said if we had not gotten her from over there when we did it would have been trouble!!!!

Experiencia negativa: If I could give one star I would, this place is ridiculous and ghetto. The staff does not have name tags on to identify themselves. The front staff is not professional. They hand you a gown and don’t instruct what to do next. I had to take my temperature when this should have been done by the front desk personnel. My mother complained that the food was disgusting. It was one point they did not bring her anything to eat. The front part of the building has a disgusting odor. I wish I could have found another place for my mother to finish out her rehab.

She was very unprofessional, rude and seemed put out by my inquiry for placement. With that said, like other reviews that have mentioned rude staff, it is probably in my family's best interest to look elsewhere for care. D. Harrell RN

Experiencia fantástica: Very clean. No odors. Patients up in their chairs or mobile. Dinning room food prepare separately for each patient as to what they could tolerate. Guest food good, covered all food groups, colorful in presentation, vegetables well seasoned, Friday fish (suspect fried and oven heated from food company). Only complaint was prolonged fire drill with alarms longer than15 minutes. Fire truck on site. Patients not evacuated nor directions to visitors should evacuation be needed. Patient doors closed as per protocol.

Experiencia fantástica: Have visited twice since family member was transferred here. Minnie and staff have been very accommodating and once I explain some things to my family member about why certain things were being done he understood and was satisfied.

While I was a director in assisted living this facility was always top of the list for referrals when a patient in assisted living needed skilled nursing.

Experiencia negativa: Prescription meds not available until their Doctor approved or ordered, not even those from your own Doctor. Then meds have to come from Jacksonville FL, four to eight hours. Bring your own, but don't let them know. Went in Monday, got no meds until Tuesday afternoon. Didn't see their Doctor until Wednesday afternoon, for three minutes, interrupted by a call he had to take. Didn't get into their system until Thursday. Some nurses wouldn't dispense as ordered even then. Meals, if you were given a choice, rarely came as ordered, or even as the printout listed. None were served hot or even warm. Three times I got nothing, had to ask. Got snacks instead of a meal. Weekend was worse, didn't get meds as needed, when dinner was missed, was served breaded fish, or maybe a hockey puck, I couldn't cut it. Sunday was worse still. Tried getting morning meds at eight am, CNAs kept saying that they told nurse, after three hours still couldn't get meds I've been on for fifteen years, it's now been ten hours since getting meds I have been on every six hours for fifteen years. I packed up and headed to the door. This is what Medicare pays some $290.00 plus per day for? Go ANYWHERE else!!!!! Should have checked US News and World Report BEFORE checking in

The staff here has gone ABOVE and BEYOND to help my mother who has recently lost her husband. she enjoys all the activity's and loves the food. The staff always greets me with a smile when i visit. I am happy to say that i know my mom is in a good place here and well taken care of. Thanks for all that you do for her..i know she is in good hands... and god bless.

Experiencia negativa: If you have loved ones that need care DON'T put them in here. My mom was only in there for 16 days. it was a nightmare. The nurses didn't actually to any care just pass pills. The CNAS do the physical care. But they are very short staffed so call lights don't get answered, meals are delivered late,patients are not assisted with dressing . If you have dementia you get put out in the hall in a chair to be watched. Luckily I was able to be with my mom most of the day . I helped her shower, get dressed, brush her teeth, go to the bathroom and take her outside in her wheelchair. I started bringing her lunch by the second week because she said the food was bland and nasty.nothing seems to run on a schedule. Sometimes she got her meds at 9am sometimes at 1030 or noon then again at 1 or 2 . On day three I asked when her dressing on her back from surgery would be removed .The nurse responded with "what dressing ". So either no one assesses skin integrity or reads the chart of why the patient is in there. We did not get a schedule or family consulting on her care plan for the first week or even was asked what her status was prior to her injury. They did not know she was very self sufficient and walking a lot with no assistance prior to the injury. For activities one day they had a movie day with popcorn listed on the calendar. When asked no one seemed to know anything about it where it was , what the movie was or where it was. On discharge she had two meds that I needed prescriptions for . The nurse giving me discharge instructions said she would call it in to the pharmacy. I gave her the info of which one we use. Got my mom home my husband was going to pick it up . Nothing . It wasn't called in . I called Terrace got passed around to 6 different people then answering machines. Called again asked to speak to nursing supervisor. They don't have one . They can't do anything. I know when a bed is available its first come first serve. The 2nd roommate my mom got had dementia and yelled" Someone help me " every ten minutes . She didn't understand how to use the call light so she just yelled. The only thing good was the physical therapists. They were helpful. Do some research if you need a rehab facility don't take the word of others who recommend this facility. Mindy Friedman RN

Experiencia fantástica: I was visiting today and had the opportunity to meet the Administrator. He took the time to show me around the center, which is clean and quite beautiful. He answered all my questions and provided his card if I would have any other questions. I was quite impressed with him and all of the people I met.

Experiencia positiva: Went there after a knee replacement surgery on. Thursday and was not to get out of bed unassisted. No one available to go to bathroom even though I could not stand on my leg. Then when someone came they said they didn't have clearance from PT to help me. They said that for 3 days and I asked how long did it take to get clearance? I was moved to a different room next day and they couldn't deal with it. Did not gets Meds or insision dressed all day Friday. They had wrong room number. Kitchen gave up and took name off and said this is your dinner. I saw PT 2 times and my husband was running back and forth to visit and he had to help me do stuff. Finally decided he could do the job better at home and we asked to leave. Obviously we committed an unforgivable sin as they have been uncooperative with arranging for home health care as they said. I still give them a 3 rating as the place was clean and no bad odor as in so many places. The. Nurses and most of the staff were nice and friendly.

Experiencia negativa: My mother was transferred from Nort Florida to this establishment without notice to me as her only child and care giver. They lost her clothes fed her food I would not give To a dog and treated her as a unwanted guest her entire stay. I reached my limit and told them to have her ready for check out with five days notice and they did not even have her medications and oxygen ready. I was firced to delay another couple of days. Do not let your loved ones be brought to this establishment!
