Experiencia negativa: Do not send your love ones here. This place is not very clean and they have loud patients that screams all day. Case manager likes to hold patients hostage and go against patients rights. We asked for a transfer and they tried to advise us to have her stay and give her a private room when our family declined their offer. They only care about money and will slow the process just so they can collect insurance money. The transfer process should have taken them a day. Not very hard to do your job correctly.

Experiencia negativa: My father was sent to this facility and we were very disappointed with his care. He was there for approximately 14 days. He was never offered a bath in the entire time. His bedsheets were changed twice-only because we stripped the bedsheets and requested new ones. The call button is just there for decoration as the staff ignores the calls. The bathroom in the patients room was never cleaned. We requested the cleaning staff to clean the bathroom and they stated it was not their job. We are so glad that we were able to get him transferred out of there. Please do not send your loved ones to this care facility! The reason my father was there was for physical therapy. The facility only provided physical therapy 3 times in the entire time he was there. Patients at this facility are ignored and not cared for. I am looking into filing an official report with the state. Hope this helps anyone who is considering using this facility for their services.

Experiencia negativa: Worst facility to ever do rehab in. They do not answer alarms! My grandmother was in this facility for two weeks and she was suffering every day. Constantly sitting in poop and piss because it would take staff 40+ minutes To come and change her. Grandmother fell off bed. She screamed for help. No one never came. Had to crawl to the door for assistance. Grandmother had broken rib and staff pressed down on her chest and all they could say was sorry. Happened multiple times. I would never recommend this place to nobody. when I came to visit, cleaning lady just left the room. You can smell the bleach from the mop. As I went into the bathroom. Bathroom was disgusting.

Experiencia negativa: Terrible horrible horrendous. Do NOT send your loved one here. They don't take care of them, they let them crawl around on the floor, they don't administer medications properly. And they will STEAL your loved ones personal belongings and claim to not know what happened to them! Worst place EVER!!!!! My loved one had to be transferred to the hospital via EMS due to not receiving her proper medication, and being so dehydrated, her mouth was cracked and bloody. They will smile in your face but it's all fake! Thieves and crooks, I tell ya!

Experiencia negativa: This place looks run down, dirty and over crowded. When I was there, all I could notice was duct tape on the floors, ripped screens and unclean looking linens. Patients arguing with staff about their care and I was there for 1 hour. Stsff appeared to be annoyed to be bothered at the nurses station.

Experiencia negativa: Find somewhere else to go they don't transfer the calls to the patients. They are just rude they kicked my boyfriend out after a week when they told us at the hospital he would be there 3 weeks after his stroke and having his toe cut off

My dad was placed here after back surgery the nurses were always late with his meds they gave them to much insulin his sugars dropped so low. It smelled so bad like mildew Where his roommate was the whole roof caved in and that's where the smell was coming from.. do not recommend this place it's horrible

The Activities team, will go around, and get residents up all of the time, just to join in. I love the Activities staff,. They are energetic, sweet, and organized.

Experiencia negativa: My husband and his family had a bad experience at this place my father in law only stayed one night and the next day we took him home when we got there his bed was all the to the floor no one helped him up his bed up the wall are the ugly the A/C doesn’t work they told us we can bring own fan if we want nurses take forever to come help him. His cookie fell on the floor on one came to help him pick it up. The restroom was clogged well he was there when he left they still never came to fix it worse place ever

They didn't offer me a drug test or any typical snf first red flag , next they let you smoke 4 times a day MAJOR BUMMER , next I wasn't prescribed meds as I've been taking them actually they cut the into third IF YOU KNOW MENTAL HEALTH MEDS YOU KNOW THAT'S NIT A GOOD THING AND MY PAIN MEDS WHERE ALSO CUT INTO A THIRD . Next they where gonna make me get my meds crushed NOW I'LL ADMIT I WAS SAVING THEM SO THEY'DE ACTUALLY WORK THEY FOUND MY SAVED MEDS AND WHERE GONNA MAKE ME CRUSH MY MEDS I GAVE VERY POOR TEETH AND I HAVE ROTTEN GUMS IN-BETWEEN MY TEETH I TOLD THEM I CAN'T TAKE EM THEY SAID THEY COULDN'T HELP ME . SO THE LAST 4 DAYS I'VE BEEN A MESS MENTALLY AND PHYSICALLY BUT TODAY DID A VIST WITN MY PROBATION OFFICER AND SHE TOLD ME THAT THEY TOLD HER I'VE BEEN REFUSING DRUG TESTS THEY HAD NO ROY IN THE FIRS PLACE TOO TALK TOO HER AND IT'S CALLED SLANDER WHEN SOMEONE LIES ABOUT YOU TOO MAKE YOU LOOK "BAD" THEY NEVER ASKED FOR A DRUG TEST AND TOO LIE AND TELL AND POSSIBLE COST ME MY FREEDOM IS JUST WRONG AND THE PEOPLE WHO WORK THERE VARY BUT I COULD HARDLY GET WATER I HAD TOO GET MY OWN THAT'S FINE FOR ME BUT THERE'S PEOPLE WHO CAN'T GET THERE OSN THEIR AND THE PHYSICAL THERAPY WAS BASICALLY FOR MY UPPER BODY THIS PLACE EVEN SEARCHED MY ROOM AND HIDDEN IN MY BAG WAS A NEEDLE IM PREDIABETIC SO THEY SAIC IT WAS PARIFENIALY BUT I HAD IT IN MY BAG WHEN I CAME AND DID THEY CARE NOOD CHRH PUT ME ON THE STREETS IN 109 DEGREES WITH A WALKER WITH ONLY 2 WHEEL'S AND NOW MY PO JUST CALLED ME AMD TOLD ME THAT THEY TOLD HER I BROKE THE WINDOW TOO SELL MY DRUGS TO MY FRIEND I HAVE'NT EVEN HAD MY MEDS IN 4 DAYS. This place is that that of place . SO BUYER BE WHERE THEY LIE DO H TAKE CARE OF YOU AND BASICLLY DON'T CARE . I WOULDN'T STAY THERE IF YOU PAID ME !

The nurses, aids, front desk and case workers have been extremely helpful to me navigating the medical world that is a skilled nursing facility. Bottom line is that she is in good hands and getting great care.

Experiencia negativa: My brother was released today as his insurance wasn't paying and he wasn't on LTC. My brother is paralyzed on his left side and wasn't completely independent. They said he could be released home on home health but didn't set that up for him and he always needed assistance going to the bathroom. They didn't make sure he had a safe place to go home too.

Experiencia negativa: My sister has a blood infection and needs her meds on time they have neglected to provide her with her medicine and will not answer the phone another family had to call the police to check on their family member today. This is horrifying her life depends on the care she receives.

Experiencia positiva: They are understaffed though they do put effort however last night I mentioned to one of the nurses/ staff that I found and killed two bugs/roaches and she just laughed and said ok but anything other than that there not rough or rude they are professional and have proper communication mainly clean

Experiencia negativa: Came here to be an Escort for a client. I was told wait in the Lobby for transport. It's 25 minutes pass out appointment pick up time . Ask Lobby receptionist to call the nursing desk to let them know I been waiting since 8 am and not to think I was late or no show. Once again no communication to me that she call back there and what did they say? I'm looking at her? Wow

Experiencia negativa: Packages seem to disappear once they are delivered to the front desk. The staffs guarantee the patient will receive the packages upon arrival but they don’t keep that promise. When packages are delivered to the front desk staffs leave them on the counter in the open where anyone can walk off with them. That shows lack of support and no consideration for the patients; irresponsibility, & laziness.

Experiencia negativa: First off ANYONE that has to send their loved ones to a rehabilitation center DO NOT send them here . My mom was sent on a saturday afternoon to this horrible place and stayed for one night only. Reason why is because the nurses are horrible they are not on top of their patients when medication is needed. My mom had surgery and needs help getting around and at night she kept calling the nurses to go to the restroom and no one ever showed up to help. However she could she got herself up to go to the restroom. Thank god she didn't slip and fall. Next day sunday before leaving she never got her medication and she was in pain. I asked TANYA (a staff) for it and she said that GEMMA had her pain medication. She needed her pain medication at 3:30 and it was 5 pm and no one had given it to her. When my dad and aunt asked for it they said they did not have it that if she wanted it they had to put in a request so they could send the medication to them. My mom told them that if she was not getting her medication that she was leaving and right when she said that the nurse ran to get her medication and said it was there. These people are LIARS. They are no help at all. PLEASE DO NOT SEND ANYONE TO THIS PLACE. If it is possible take them home. No one needs to be treated bad.

Experiencia fantástica: My husband was a pt there for rehab. Ii was kinda nervous but, WOW, he loved the food and the physical therapy dept was amazing.

Experiencia negativa: I did not really have a choice to be placed in here. Between my insurance, other facilities being full and my medical/medications i was limited to here! The pictures are NOT staged! Not a great place to have to be in.... at least I am able to speak up about my care. Most of the patients there are not able to advocate for themselves. Several RN Nurses should not even be taking care of patients. I would hope they treat their own sick family better then they have treated me and some of the other patientst that are stuck there. Especially Nurse Syrah and LPN Shannon. They were my rooms RN (Room 305) just in case there is another Nurse Syrah and LPN Shannon there. All I can say is WOW!! I would try to ask about what what happening with my care, they couldn't be bothered to be stopped, they would just keep walking away down the hallway. When asked for medication it was late and I would have to argue with them about doses. I had a PICC LINE in my arm placed for meds and blood draws. Not once did anyone know how to draw blood from the port and then the nurse left a flushing syringe in my port over night when the IV antibiotic was supposed to be hooked up and running. . Also my roommate smoked Fentanyl and Methamphetamine in our bathroom. Staff was made aware. Nothing was done. Also same roommate would steal from me, the facility and others rooms, nothing was done either. I had regular friends and family visit daily so they saw these incidents happening as well.ON THE OTHER HAND NOT EVERYTHING AND EVERYONE WAS TERRIBLE..... The kitchen staff Ceecee was great! The Wound Dr and wound nurses Tonya any Malee were AWESOME!!!! The Techs (or CNAs) were good Cass, Sunday, Rachel, Sandra and RN Nurses Princess, Jenn, Jess, Holly, Jose, Vonna, Matthew, and Hepsi were phenomenal !!!!! Also last but not least I had a cockroach on my open wound in the middle of the night and bathroom sink constantly overflowing.

Experiencia negativa: Some of these reviews i'd like to know where they are cause they were not at bella Vita when we were there. Horrible place will never ever ever ever recommend it to no one. Our mother was there, she was considered a fall risk so she was not able to get out of bed on her own she laid in her feces for hours before Somone would go and take care of her, kitchen never got her meals right after I talked to the kitchen supervisor and filled out their order for her meals. Night nurses were the worst Made several complaints but nothing changed Now because the insurance forced us to send our brother there he's fighting for his life. Pulling the nurses chain does nothing at Bella Vita. He would tell them when they finally came in to check, he told them he was having trouble breathing, it would take forever before he got breathing treatments. The morning that they called paramedics who knows how long he had to wait. His oxygen level was down to 70% before they found him He was in isolation and one morning he had a roommate and geeze now he has the flu pneumonia and possibly kidney failure I plan on going farther with this. There is no reason why they didn't check on him and treated it HE IS NOW ON LIFESUPPORT BECAUSE OF THEIR NEGLECT Don't take anyone here. Take them home and get a nurse if you have no other options so sorry for sending you there brother. Because of their incompetent we may loose you

Experiencia fantástica: My family is so grateful for the dedication and quality of collaborative care that my mother received while at BellaVita. Knowing that there were circumstances that left her stuck across the country and without family nearby, the staff rallied together to show my mother support, tenderness and love. Everyone we talked to from cna’s like Mary, Pa’Shea the social worker or nurses Atoya and Cindy- all were patient and caring- Kaylyn, the activities director- found ways to send much treasured photos of my mom to us and though we weren’t able to make it to this point, even began planning a birthday party from afar. When it came time for Hospice to be employed, staff rejoiced and welcomed our mom’s return from the hospital and played music to comfort her. They listened to our stories about our Mother and held our mom’s hand for us when they knew how hard it was for us to say goodbye. They also patiently held the phone up to our mom’s ear to let us add “one more” goodbye because one is never enough. All of that was done in addition to careful attention to her skincare and comfort. We can say that BellaVita did love our mom. We are forever grateful

Experiencia negativa: Bella vita means beautiful life, however that is not what you’ll find here. The outside and lobby are presentable, however once you walk through the health and rehab doors that changes. The entire facility reeks of bleach, to the point it is difficult to even breathe. The staff lack organization, skill and compassion . The facility is cramped (2 per room), and not well maintained. The staff is poor on communication. It was nearly impossible for anyone to answer the phones , let alone have a nurse or caregiver return a call. The food was inedible, nor was the coffee hot, if they even served you any. There was absolutely zero PT, MD check or shower provided within a week of “rehab”. This was supposed to be a short term stay post surgery (1-2 weeks), yet target date for reaching any goals were 3 months out. The best decision was springing my father from Bella Vita and taking him home for the care he needed.

Experiencia negativa: Nasty! Roaches crawling all over the place in residents rooms and shower rooms. Old structure. Wonder if there are roaches in the kitchen since they're everywhere else. Over worked staff. People drenched in urine. They don't listen when you tell them a resident has a possible UTI. Just gross.

Experiencia negativa: If I could give negative stars I would. This place is horrible. First off it smells awful. The entire building. The staff just sucks. They say someone will be in to help and no one ever comes. They’re rude, the health care is far far below expectations..They’re not even taking care of these people! My moms been here for almost a week…no shower. I asked on Monday if we could have someone come in and give a shower and I was told no and that they only provide showers on Tuesdays and Thursdays. Tuesday came…no shower. Today I ask why no shower on Tuesday..I was told it wasn’t on the CNA’s list and then the CNA came in with a bucket for her to give herself a bath! If she could give herself a bath she wouldn’t be in this rehab. This is why she is here and she is not being taken care of. This place is the worst. I would not recommend this place to my worst enemy . It’s dirty, it smells, staff and standard of care are the worst or the worst.

Experiencia fantástica: My mom recently stayed at Bella vida and the case management department did a really good job at making sure that all the details of my moms discharge was taking care of. they even called me after my moms discharge to make sure that my mom and I were happy with the resources that were implemented! Thank you Bella vida case management department.

Experiencia negativa: My 92 year old Grandmother was sent to Bella Vita Health and Rehabilitation Center due to COVID in November 2022. It was the only Rehab facility with a bed open with a designated COVID area at the time. I get the fact that she had a contagious virus, so the staff needed to use precautions, but that is no excuse for the terrible treatment that she received. The first night and day, she had no water. No one ever came when she pushed the call button. They took her walker away and told her that she was not allowed to get up and go to the bathroom by herself. Yet, no one ever came to help her when she pushed the call button. She lied there in a bed with no water, and no way to get up and go to the bathroom. At some point, food was brought to her, but still no water. She was there for rehabilitation, yet never really got physical therapy. She states that a physical therapist came in the room to assess her by timing how long it took her to walk across the room. Being a physical therapist myself, it sounds like the Timed up and Go test was done. However, no actual treatment was done during her 4 day stay. I would not recommend Bella Vita Health and Rehabilitation Center to anyone.

Experiencia negativa: i would not recommend this facility for anyone. my grandmother was there for two weeks & they didn’t take care of her at all. every morning we would find her soaked pee despite her pressing her call light over & over just to be ignored by staff. they never showered her, never repositioned her, they never even checked on her regularly. then on top of all that, they claimed she had shingles (with no proof or documentation form a doctor whatsoever) & they didn’t tell us this until 4 days after we had been in with her & cleaning her up. the “shingles” my grandmother had was actually bedsores on her bottom because of negligence from the staff. if my family & i didn’t go in everyday to take care of her i can only imagine the condition she’d be in right now. as a caregiver for the elderly, it breaks my heart to know that this is how they’re treating their patients.

Experiencia negativa: Do not recommend anybody to this place rude people me nurses never took my father-in-law a bath or a shower for a month that he was there they released him on January 2 but no therapy that was done that he needed to go in there for no later than 15 minutes and he got home he passed Away They also sold his clothes brand new clothes we took him to go home and reported it they did not do anything about it call the supervisor supervisor never responded my call back there will be action taken care of paperwork is getting done through a lawyer I am landing up losing 36 pounds there while he was there in their care please do not take your loved ones there

Experiencia positiva: My father was at Bella Vita for two weeks. This is a big busy care facility with a lot of staff and residents. The facility is a little rough but the staff was fantastic. The nurses and CNAs were attentive, caring, responsive and skilled. In particular, we were impressed with nurse Atoya and CNAs Kayla, Rose and Bianca. They took good care of my dad and I am thankful. The PTs were really good as well. Thank you BV staff for taking care of my father!

Experiencia negativa: Not off to a good srart so far. My mother has been at Bella Vita for 2 days and today she had dialisys and was transported to that appointment. Her treatment was over at 12:00pm at that time the nurse at Davita called Bella Vita to have my mother picked up at 12:45pm i was called by my mother that she has not been picked up and is in the lobby at Davita. At 1:00pm i arrive at Davita and i find my mom sitting in s wheel chair with only a hospital gown on she's cold and hungry and thirsty. I called Bella Vita and a lady answered i told her my name and my moms name and mentioned to her that my mom has been waiting over an hour to be transported back to Bella Vita and i asked if i could take her back she tolm me no i am to leave my mom there until transportation arrives and i asked how much longer would that be and i was told i dont know she just going to have to wait and i said Lady how would you like it if you were left in a clinic lobby with only a hospital gown on cold hungry and thirsty and she did t answer me at this point i asked to speak to the owner and i was denied then mom mo.s transportation finally shows up i hung up on y he lady and asked the driver what took so long and he told me that he received the call to pick my mom up at 12:41pm 41 minutes after my moms treatment was done it was now 1:32pm and my mom was finally loaded into a van. Im very dissatisfied.

Experiencia negativa: My 86 year old Aunt is there right now. While there for hip replacement rehab, she contracted covid. There are many things that could improve, but at the moment I have no way to talk to her as her phone is out of service. I try to keep her spirits up with daily phone calls as I can't come see her - I am all the way in Pennsylvania. I have made over 36 phone calls to either try to get connected ( it rings then disconnects)or have somebody report the phone. I was told there's only one person to fix it and "we don't know when he'll get to it ." I was told my aunt would be given messages from me but of course, I don't know if that's happening. We all know how busy everyone must be, but communication is important to the patients and should be a priority. I'm very frustrated. I have been trying to reach her since yesterday. ***update- phone repaired on 4th day. Not one promised message given to my Aunt. ***Would love to talk as suggested. I have been unable to reach a human. Thanks anyway.

Experiencia negativa: My husband was in the hospital and transferred to Bella Vita. I was told to regain his strength from Covid. I was home sick with Covid myself so was told I couldn't visit my husband. He had dementia and couldn't answer the phone himself. (Needed assistance). Most of the time they were just to busy to help him talk to me. He passed on 12/14/2022. No one would give me any answers as to why what happened, when he was simply admitted to get stronger. They simply called me asking if I was his wife and I said yes and they said Michael is dead. What a horrible way to be told your LO has passed. I'm still looking for answers. My sweet husband passed and what happened? I wish I knew!

Experiencia negativa: Horrible facility. Filthy dirty, extremely slow on responding to call lights. As you walk around, it looks like somewhere to dump the elderly to die. Depressing atmosphere and located in a bad part of town. I wouldn't put anyone I loved in a bad facility like this.

Experiencia fantástica: Dear people of google (the ppl who review) . I have worked for BV for 9 years coming up this coming up April. I know personally how everything is.You get comfortable at a workplace and fret about leaving and wondering if I am doing the right thing . Well I was a housekeeper and laundry aide for the first seven and half. I learned how to respect this residents individually with humanlity and respect and dignity. They ALL deserve. I put my emotions on at work and my brace smile and keep with grace (most days). I am now a CNA and will continue on in this medical field one way or another. Everyone deserves respect because one day and we have or will need some kind of assistance. Bella Vita has been through a lot of critical criticism but I will be honest. I know we can do better and are doing helluva lot better. They gave me me branches to my tree and nutrition when I was hungry for growth. And the sun still shines here. Yes we need improvement but your concerns are being heard and are working for a better future of better care. We will move forward and stay open to encouragement and discouragement . Experiencia fantástica: I have worked at Bella Vita for 5 years , I love coming to work everyday, I enjoy working with everyone at Bella Vita. I’ve seen Employees all come together during Emergency situations , there are days I’m super busy, but our Residents always come first whether giving them a simple cup of coffee or tying their shoes, seeing a smile on their face that really makes my day.So once again I really enjoy working here at Bella Vita.

Experiencia negativa: i was not impressed with the care there, extrenely sad that i had to leave my brother in their care. He is now gone and im sorry that I chose this place RIP my dear brother

Experiencia negativa: I noticed the aides are more caring and helpful compared to the other staff who socialize between themselves and give attitude when asking questions about services. Gave some constructive criticism at one of the stations and all I got back was attitude and a hateful look. I don't know how hard it is to get up and go to the linen closet, grab a couple blankets and bring them to my mother. If I was able to do this on my own I would have, but with your facilities protocols and procedures it prevents me from doing this. Perhaps it's the lack of training or having to deal with different individuals who have different needs, but at least make the effort to help, even if it's beneath you, it's the effort that shines brighter to those who are asking for help or need the help. I've been coming to visit with my mother, she is observant, has worked with people needing help with medical issues and the lack of care she has not been receiving since her arrival to this facility is very concerning. Young staff not helpful compared to others with experience. Hopefully elderly with hisk risk fall issues and bodily issues in the future will have a better experience and staff will be more willing to help out with keeping up with monitoring their patients, checking in on them and looking out for them. According to their website and listed on the front page: "At Your Service We never forget that you’re our valued guest, and the reason we’re here! Each day you spend with us will be filled with the caliber of service that will make your stay comfortable, safe and therapeutic.", and I haven't seen much of that since I've been coming to check in on my mother.
