Experiencia fantástica: While recovering from a bilateral hip replacement I was fortunate enough to be referred to Gresham Post Acute Care and Rehab . For 20 days I received amazing medical attention, and was able to capitalize on rigorous physical therapy. The Physical Therapy Manager named Ty, and his dynamic team provide an experience that is one of a kind! The entire team of therapists are positive, uplifting, compassionate, and well educated. I spent 20 days with this team in May, and again just last month in September. When I arrived to this care and rehab facility I couldn't even walk from my bed to the restroom without support and help from the staff. Twenty days later I was confidently walking outside of the building with a cane in my hand and a proud confident stride, with every step I took. I couldn't be more proud and impressed with the physical therapy team at this establishment. Crystal is another amazing employee who stands out as a thorough team leader whom covers multiple tasks including coordinating with your insurance providers, maximizing your overall experience, handling equipment needs, and ensuring every aspect of your stay is professionally taking care of while in the hands of her staff. She is very active and involved with her clients form the moment they arrive at the center to the moment they are discharged. The Doctors, Nursing Staff, Creative Arts Activity Department, as well as the very hard working Food Preparation staff all provide superior service for the clients at this care center. Honestly I could not be more happy with the staff and the time I spent recovering at this wonderful establishment. If you have experienced debilitating injuries that have been corrected by major surgery, than this facility is the place for you to recover and heal. Trust me, if I did not feel passionate about the quality of stay I had with this facility, I wouldn't have taken the time to write this 5 Star review. Take my word for it folks, Gresham Post Acute Care and Rehab is not just another medical institution, it is a place where people receive the necessary and professional medical attention they need to secure a better future for their lives. Experiencia fantástica: Excellent Resident Care provided by all staff while I experienced recovery here. The staff are extremely friendly and always there to help when I needed it. The meals provided were very excellent. The physical therapy gym has a lot exercise equipment that I was able to use. The therapy team was very prompt addressing my physical needs. The daily activities offered included a wide variety of choices that were better than I ever experienced at any other facility. The social services department, care manager, and receptionist were very good at managing all my transportation, medical supplies, and appointment needs. Housekeeping always made sure my room was well kept. The CNAs were always prompt to help me go to all my appointments and went above and beyond to help me for my daily needs. Because of all the good care and rehab I am able to go home safely. I will miss all the staff and residents here. I plan to return to visit to see all the staff and residents in the future.

Experiencia fantástica: The positive atmosphere was not what I expected after reading reviews. Majority of the staff were helpful and kind. You could tell they enjoyed what they were doing! Of course, a few were there that made me think hmmm. And two CNAs, Ali and Jen made me think Wow! They were always going above and beyond. Unlike previous post care facilities my mom has been placed in, this one offers fun activities and game days to join and have fun. While waiting to play in the Bingo game event I had the joy talking with other patients who shared with me how much they loved it there. It took my mom two weeks of post care to get strong enough to go home. From my experience, I believe there have been definite improvements from the previous reviews. Talking with Jen, there are even more hands on activities that may bring more for the patients to look forward to. How awesome to have found a place that invests in not just the health but the happiness of their patients!

Experiencia negativa: This care center is horrible. Their primary concern is bleeding Medicare and Medicaid for every dime they can get. Their concern for patients is secondary at best. Their License to operate should be revoked. Don't let yourself or your family be placed here. You will regret it!

Experiencia negativa: had 2 stays here. first was last fall just as current administrator came on board. place was very good, esp PT/OT and most of staff. and the residents were a happy family. now it has one star. you can't give less. i went back in spite of that because of my previous experience with staff, PT & resident comradery. But this administrator has taken it into the toilet. and there are dangerous conditions. there is a whole laundry list of issues that make it unsafe & there is lack of care for the quiet wheel patients. the food is lacking in nutrition & served below safe temperatures. diabetic folks are served the same starch & sugar except "diet" kool aid like juice. this fellow has cut resident services to save $. he was given alternatives but ignored them. another really poor thing about him. the other day he was in dining room at lunch & never spoke to a single resident. he does not even meet your eye passing in the hall. the Ombudsman is coming to august resident meeting after extensive reporting of the issues, some dangerous. steer clear of here until he is gone. i won't go back if he is still there. update: the incompetent administrator was fired. Hopefully it will return to being a good place. friends that are still there report improvements already. food is still horrible & any nutrition is cooked out of it. hopefully that will improve too.

Experiencia negativa: Sister is here . If you have a loved do not take them here. They are not giving her meds on time. She is diabetic. They didn't get her pain med order for two days so she had no pain meds. She has been left in a poop depend for over an hour. She is 80 and this will cause skin break down. It took a week for them to start therapy. She had a knee replacement. She went from 9pm till 5am without being checked on.

Experiencia fantástica: We had a very pleasant experience here with my father. I was very concerned in the beginning with the reviews I have seen on Google. I told my wife about it and she quickly called and asked them about the reviews that we saw and our concern. They gave us a reasonable answer so we moved forward and had dad stay there for his wound care and physical therapy. We regularly followed up with staff and also made sure to address any issues in regards to my father as well to make sure we were all on the same page. They were great with follow up and spoke to us regularly. His stay here was made easy by the staff. Never once did I feel reluctant about having my father here. Would definitely recommend this place if your insurance gives you options for your family members care.

Experiencia fantástica: My grandfather Frank was there for 9 days and We absolutely love this facility clean consistent the faculty is pleasant and well kept.. The exercising room was great they get you well to get you home.. Staff was top-notch. Always in a great mood and always so helpful.. i never had to worry about him being there. They would come in several times a day asking him if he needed anything or needed any help.. A+ from my grandfather I've never seen him look so good.. Thank you so much

Experiencia negativa: I am a 68-year-old woman and broke my neck 40 years ago. I am supposed to be turned every 2 hours to prevent pressu rer sores and protect currents wounds I hsve . I have been Waiting since 8:00 p.m. and it is now 12:45 a.m. in the p,20#/morning of February ,3 202.

Experiencia negativa: My 90 year-old mother was a patient for 6-days at Gresham Post Acute Care and Rehab in August 2023. When she initially arrived we were very concerned with what we were seeing with the cleanliness of the facility. Her first night there, she called 90-minutes for help and no one came. Her roommate had to assist in getting her some help. The administration addressed the cleanliness issue, the delay in getting help and things seemed to improve. On her 6th day there, I walk in and they said she was not doing well. She had almost passed out 3x, looked very pale, weak and her head was jerking. No one bothered to call to advise that she was not doing well. As poorly as she was doing, the staff opted to dress her to go to the dining hall for a meal. Something she was yet to do at the facility. Her vitals were not good, I had to tell the staff to get her oxygen and to call 911 as it was apparent she needed a higher level of medical care. She left by ambulance and was in the hospital for a week. The ONLY follow-up from Gresham Post Acute Care and Rehab was a phone call from the social worker whom we had never met. So sad that they have a patient leave by ambulance for the ER and no one in administration followed up with the patient or family. We hope that as an organization they will examine their processes and make the necessary changes so that other patients do not experience what my mother had to endure at 90 years-old.

Experiencia negativa: this place deserves a 0 star rating. my Dad was admitted here after having a horrible diabetic episode. all within the first day of him being there they fed him without checking his blood sugar and it spiked to the point where he was incoherent, AGAIN. and he was left alone for hours. i included a picture of the sugary foods they gave him. the staff didnt care when i questioned them about it, and got defensive as if they didnt know exactly what my dad was in for. they left his medical records out for everyone to see, which included his social security number, etc. and they overdosed him on insulin to the point where his bloodsugar was 32 after he REFUSED IT, knowing what would happen. i have video proof of a nurse attempting to give my dad insulin, after the overdose and after we informed the nursing staff that his insulin pump was up and working again. the nurse stated in the video that his pump was not written in the notes. if we werent there to stop him i dont even want to imagine what wouldve happened. they did not prepare my dad to go home and take care of himself. he just passed away and this rehab facility failed him, places like this dont have a right to toy with peoples lives. im 18, i have a 13 year old brother, and we have to spend the rest of our lives without our father now because a REHAB facility released him when he was still unfit to take care of himself. dont get a job in the medical field if you arent going to help people.

Experiencia negativa: My dad passed away on 5/18, to start the peremdic told me what happened, and left questions about a large gap of time that needed to be answered by the staff. The CNA was so rude and went into a defensive mode when I tried to ask some questions. I needed a death certificate for my dad so I can finish the paperwork to take him overseas for burial, at that time I was told the Dr won't be in until Wednesday 5/23 so I suggested to take the documents to him in Milwaukee on Monday so I can speed up the process. Dr Berhanu kededa ( I have a doubt he is a real doctor) filled the death certificate without including his license number which led the county to reject the paperwork on Tues, then I was told he will be in the Gresham branch to fix the problem on Wednesday, the funeral home rep went to have him fix it to find out he isn't in Gresham he is in Milwaukee, and had to go and find him. I don't know what kind of a facility treats people that lost loved ones in this way. Due to this so called doctor carelessness a process that can be done in a week, it's been almost 2 weeks now I haven't been able to finish the needed paperwork to be able to funeral to bury my dad, because his mistake pushed everything else into a holiday weekend. If you have loved ones don't leave them in a place that doesn't care about them or you as a family.

Experiencia fantástica: By FAR the BEST care facility for my brother. They have a good backing administratively and it shows - the staff wants to work and they care. Acute care is not easy and we are so grateful that they are caring for my brother.

Experiencia positiva: There are a couple of nurses here that think that they hear you when you talk but don't care what you are saying. some of the CNA'S are nice,friendly. I do have to give props to the Therapy Dept. they have helped me so much they never gave up hope on me we all have came so far, when others said I would not ever had came this far, II just wish that hey would of had a social worker so that fining a facuity In Eugene would have been quicker Since I had to be the one to reach out to them.

Food was bad they didn't give out medicine to help manage their pain but to make sure they're not bothering them.had to call because they left my mom on the toilet too long.

Experiencia negativa: Worst place I've been in. Nurses Irene and Helen are horrible. Terrible bedside manner. Not kind, compassionate or understanding. Strange stuff goes on during the night shift. Denise, Gina, Connie and Todd were great. This is a shady, creepy place. Yolanda was the sweetest nurse there. She and Katie were the only ones that showed a caring side. Irene and Helen should be FIRED.

Experiencia fantástica: I went into Gresham Rehab this week to handle some business regarding a shared client; the front was tidy and the staff who helped me were so friendly and kind amidst the "covid chaos" that is all around us. Thank you to this team for being a nice part of my morning!

Experiencia negativa: I went to see a family member on Christmas the one staff member I delt with was polite however seems to be lots of staff just lingering around or talking instead of cleaning or helping patents. I was not happy to walk into my family members room to see papers and a glove on the ground not clean at all, also this is a fall hazard. The building is absolutely appoling!!! I have been a caregiver before and this really just makes me wonder how this place is even still open with the smell and all the junk in the halls and holes In the ceiling along with staff just lingering around. I also herdas of Dec 30 th there had been a c19 outbreak lord have mercy don't suprize me tho unfortanitly.

Experiencia negativa: Grandma went here and it ended badly. Didn't change my grandmas diapers for over 3 hours despite pressing the call button every 15 minutes. When she was moved out of bed and put in the wheelchair she was left in it for 3+ hours even though she, again, would push the call button every 15 mins. She got bed sores that got infected, they didn't pay attention to her edema and when they did her blood work they didn't pay attention to her protein levels which ultimately led to her being sent back to the hospital and died a week later. They can say they did their best but the fact they didn't acknowledge what was on her blood work is just bull.

Experiencia negativa: The staff treat the patients here worse than prisoners. The pathetic excuse of a sandwich is dinner. The single chicken strip and nine raw tots was lunch. No one comes to help with bathroom visits when one needs help. Managing pain after a major surgery is apparently a luxury not available to some or all. Very unprofessional and scary place. No one should have to endure the abuse here. One star only because zero isn’t an option. Absolutely terrible place. Beds are hand crank so patients would have to get out of bed to adjust for themselves. I hope certain staff members get the opportunity one day to enjoy such treatment at the mercy of people like themselves. I’m disgusted by the facility and staff and actually afraid for the inmates.

Experiencia negativa: Some of the staff care, or seem to when I’m checking on my family member. But the smell is horrific. And I witnessed a lot of neglect. Maybe they are too short staffed, but some employees are just chatting socially when people are really in need of help.

Experiencia negativa: My dad was transferred to this facility after spending 2 weeks in the ICU. He was the most vulnerable he had ever been in his entire adult life. After being in the place for 2 weeks the facility scheduled a meeting between our family and the staff to go over his progress. They told us outdated information, issues that he had the first few days, but that were not going on at the moment. They made us doubt my Dad when he was vehemently denying that the issues they were bringing up were a problem. They basically said that he would never work again, to have his license revoked, that he could not walk, that we would probably have to get an in home nurse if he was to come home like he wanted. My Mom, brother and I were devastated. I could not stop crying and was very scared and upset. Come to find out the next day that he was walking very well with a walker and getting stronger every day. The issues that they brought up had not been a problem since the first few days that he had been there. They were going off old information, instead of talking to the nurses that actually helped him on a daily basis. They had a week to prepare for that meeting. They will never understand the hurt and trauma that their not being prepared for that meeting caused. We doubted my dad when he said he was getting better, and listened to who we thought were professionals that knew what they were talking about. He's now back at work, driving and walking great without a walker, all things they said he would probably never do. The nurses that took care of him on a daily basis were good. The psychical therapist and speech therapist were good, but kept telling us incorrect information on his progress. What was already a very emotional situation was made worse by this facility.
