We were very pleased with the care and concern he received from all who took care of him. He felt very cared for. We felt at ease leaving him there knowing he would be well taken care of. He told us the food was great, the place was very clean, everyone he encountered was friendly, caring and professional and if he ever has to go to rehab again that’s where he wants to go. Our family all agrees! Thank you Five Points for taking good care of our family.

Experiencia fantástica: Thank you for the excellent care I received while I was staying at Four Points. My nurses were fantastic along with my therapist. While in therapy they explained why I had to do the rehab and showed me the correct way to achieve my goals. The food was good. I cleaned my plate almost every day and you had a selection to choose from. I can not say enough about Five Points. We played bingo we painted rocks and had Bible study. These were just part of the active and if you didn’t want to participate you were not required to. I’m home now and all I can say is Thank You for letting me return to my house. Experiencia negativa: I would actually give them a negative 10 if that was an option. My mother was moved there from Vibra after hip replacement. It was a horrible experience. Not only was she walking better the day after surgery than she was after she had been there for 20 days they flat out misrepresented her progress in physical therapy. Two days before she was discharged we were told she was walking 60 ft then magically two days later she was walking 150 feet even though she did zero walking the day between. No communication from anyone, never anyone at the nurses station and no one checking on patients. She was a fall risk and most of the time having to get herself to the restroom. She would get there and back before anyone would be there to help her. So about 30 plus minutes she would be waiting. We signed her out for a few hours one day and two hours after we left they called to see if she was with me. Mind you I had signed her out at the nurses station in the “out on pass” book. We left about three in the afternoon and she said she hadn’t seen anyone since they brought her lunch at noon. I could go on and on. Long story short the horror stories you here about rehab facilities and nursing homes are alive and growing at Five Point rehab and nursing.

Experiencia fantástica: My parents were in the skilled nursing department at the same time! Talk about a whirlwind. We really enjoyed the nursing staff, therapists, social worker, and administration. Housekeeping does a great job, too. We are very thankful for this facility.

Experiencia fantástica: I would like to give thanks and kudos to the nursing staff for their excellent performance in making sure every resident is treated equal and taken care of, Also, from the DON, RNs as well as the the whole staff. Everyone is very nice ,very helpful and go out of their way to get the job done. However, that all comes from the speialize and professional training and care of residents that Jaque Robert's their (administrator has implemented Jaque Roberts is an excellent administrator. Hes caring thoughdul professional and very business oriented. Also with the specialize training that he puts down and expects from his whole staff he has constructed a great team. My loved one has been in a couple of other nursing and rehabilitation. I must say this one is the best yet My loved one is happy and taken good care of. I'm satisfied and the rest of our family is extremely satisfied .. Thank you Jaque and Five Point nursing and rehabilitation.

Experiencia fantástica: Mr. Robert’s, the Administrator was very quick to assist with my needs as hospice Social Worker that came to visit one of our patients.

Experiencia fantástica: This is late in coming due to my dad going back in the hospital shortly after being discharged from Five Points. Our family has had the opportunity to use the services at Five Points twice. Almost exactly a year apart. My dad has dementia and following different hospital stays he needed some rehad before going home. After our first experience at Five Points we told the hospital they were our first and ONLY choice this second round. While there you feel like you are in a nice comfortable home. The decor and ambience are warm and tasteful. There is NO smell like some other places we toured the first year. Paul (admissions) was amazing and remembered my dad. Cristian (DON) is very kind and would always give my mom a hug and ask how she was doing. I never ate there, but Daddy ate well and mom said it was tasty. They have a wonderful eating area for the clients. But here is where the awesome sauce really is. The nurses/techs/therapy staff/medical assistants/kitchen staff/ect! I want to give a few some kudos that I know by name! Hailey was a godsend and a gift to anywhere she is helping others! She not only was there for Daddy, but our whole family. We love her Big!! Sonya, Yesenia, Mary, and a few more I didn't see their names, treated Daddy as a person, not another bed to care for. Belinda was his PT several times and was absolutely incredible. Dad wasn't an easy person to work with due to his condition. We were even allowed to bring in our lady who cuts dad's hair to clean him up a bit in the salon room that they have. We only had a couple of issues with two nurse staff, but it was addressed and taken care of immediately. Our family highly recommends Five Points if your love one is looking for a place to rehab. Experiencia negativa: Do not recommend. If you want your love one to be cared for properly do not take here. The nurses are in and out, all they do is give pills all day and most staff members seem stressed and miserable and at times come across frustrated with requests. Seems they are over worked or short staffed. No quality care no detailed care. The food is awful and always cold and everything there is done half fast. The managers reply to any concerns or question as “ I’m not sure”…. The wait time for anything is ridiculous The building is nice, some of the staff members are really nice but that’s about it Do not take your loved ones to this place if you want them to be looked after properly I also see a few employees writing positive reviews, makes me wonder if they are actually real or if they are writing reviews to help the company ratings ? Another note I can say we were impressed with nurse Ashley- fun bubbly girl & Paul - cool guy!

Couldn’t have been a better experience. The staff was attentive and really helped her get better and make her comfortable!

From the moment we entered the facility, we knew God had led us to the perfect place. The facility is beautiful and clean. But most importantly the care is outstanding. Each individual there is compassionate, loving and caring. The management is incredible and wonderful to work with daily. We are so thankful to be a part of the Five Points family. From the moment Admissions Coordinator Paul picked up my grandmother from the hospital, we knew we were in good hands. Paul's knowledge and patience in answering our many questions were truly appreciated. As we completed the admission paperwork, we were warmly greeted by Administrator Kori and Director of Nursing Cristian. The welcoming atmosphere continued as we entered the room, greeted by balloons and a thoughtful welcome sign. The facility's attention to detail was evident with the welcoming basket and overall ambiance. What truly set Five Points apart was the personal touch from the staff. Department heads, nurses, and CNAs all took the time to introduce themselves, making my grandmother feel valued and cared for. The level of care and dedication we experienced was exceptional. Throughout her stay, the therapy and food provided were top-notch. The administration's responsiveness was remarkable – I lost count of how many times they promptly answered call lights during our visits. Having experienced other rehab facilities in the past, Five Points Nursing & Rehab truly stands out. I am grateful for the peace of mind they provided during this challenging time. I hope there won't be a next time, but if there is, Five Points will be our first choice without hesitation. Highly recommended for anyone seeking quality care and a welcoming environment. Experiencia fantástica: Five Points Rehabilitation was an amazing place for my mom. She is originally from Albuquerque and we had some issues with the SNF in that area. We made a decision to bring her to Amarillo to be closer to my husband and I. During the contact process, Mr Aguirre was easy to communicate with, explained things to us in detail and worked with me in providing the information to the facility prior to having her transferred from Albuquerque so she could be accepted. Once we had her admitted to the facility, the nursing staff immediately got her into a wheelchair for the first time in several weeks and helped her to the toilet. I told the staff and Mr Aguirre that they did more in the first 20 minutes of arrival than the hospital staff in Albuquerque did in several weeks. From there, she progressed very well with help from staff, PT/OT and administrative staff. The administrative staff frequently visited her to ensure she was doing well and make sure things were progressing as expected (this included the Director of Nursing). The facility is extremely clean, well kept and staff ensure things are well maintained throughout. They are quick to respond to the call lights and the food is tasty. As a RN, I am very particular about a facility my mom will be residing in for therapies and support. This facility exceeded our expectations as a medical family and the staff are amazing! Everyone is very friendly, helpful and supportive of their residents, regardless if they are there for SNF or long term care. If our family is ever in need of rehab again, our first choice is Five Points. Take it from a nurse, this place is the best for rehab and staff support!

The building is beautiful & well taken care of. Five Points offers many activities for the residents. The staff are amazing - caring, competent, knowledgeable, loving, compassionate, dedicated, thoughtful & committed to the residents. The Administrator, Kori, is a wonderful leader, teaching by example. Paul has been great & is very knowledgeable. Kathy in the business office has been great to work with & knows the ins & outs regarding paperwork, financials, payment sources, etc. Christian, Director of Nurses, is dedicated to the team. We have been blessed by the nurses, CNA’s, social workers, activities director, dietary, maintenance, custodial, reception and all others in the facility. They also have a wonderful rehab staff. I highly recommend Five Points to anyone needing Rehab or Long Term Nursing Care.

Experiencia negativa: Worst nursing home ever absolutely horrible they got residents lights going off and they wait for up to an hour to be changed or even longer some times just ask your residents who’s been there for awhile food is disgusting school food is better then there food and the supervisor in housekeeping is a lier who said she would go look for our stuff and never did someone else was wearing my sisters cloths we came with a bunch of cloths left with a few the little Filipino lady who says she’s supervisor with Housekeeping was very rude to us

Experiencia fantástica: Five points is a great place to work! Delia is a great boss to work for she's dedicated to help you and understands and listens to anything you need to talk about.

Experiencia negativa: If you or your family member are ever on medications for a brain injury don't waste your time with this facility. A patient was going to be transferred there for rehab and after literally a 1 minute face to face interview with him at the hospital with a nurse as a witness he was asked 2 questions and could not answer the second so they just left. There was no communication with the family that has medical power of attorney. After a call to the facility the staff was snarky and said he was refused because of his meds that he has been taking for 20 years. Come to find out the reason for the refusal was because to have a patient with these meds makes the facilities star ratings go down. So.... they refused the patient services that they needed to make themselves look good. No good patient care by any means.

Experiencia negativa: I don’t recommend this facility. Patient was transported from hospital to this facility mostly for physical therapy and occupational therapy after open heart surgery. Took 3 days before PT was scheduled, the nurse gave him a haircut not one of their stylists they have. The patient was left with half shaved head random long hair in places, and bald spots. The night before being taken by Ambulance from the living facility he had sat in a urine filled bed after alerting staff as well as suffering from chest pain. The nurse that night took his blood pressure gave Nitroglycerin and never returned. The ambulance was only called after the patient called his wife who then drove to facility to find staff on their phones and his nurse outside smoking. His wife trying to get answers as to what was going on, was asked by the nurse what do you want us to do? Medical staff should be able to recognize a medical emergency without family permission. The patient was neglected and in result suffered and is hospitalized for TIA (stroke). When the wife returned to speak with the nursing director she refused to discuss anything and simple was angry at the wife for yelling at her staff that evening and said it doesn’t matter he is in the hospital now. Based on the staff reaction to a medical emergency and the nursing director I’m not surprised to see why they were incompetent of doing their jobs. Oh and she won’t need to call the number offered to corporate which is an automatic response because an investigation has already been filed by his doctors. Stay safe all. Blessings

Experiencia negativa: I don't recommend this place for one its bad enough I heard this was a bad facility but however it's always good to form your own opinion then listen to word of mouth. I had an aunt that was treated here, they tried to put her on her death bad instead of trying to save her and luckily God Stepped in. Sometimes word of mouth can be factual if it's been plenty of people talking about the same issue happening to their loves being mistreated/almost died. It's best this place gets shut down and get the word out now this place is all about money and some of their staff is uneducated, unethical, cruel, absurd the list goes on....

Experiencia negativa: Do NOT bring your loved ones here! They dont compy to diets for diabetics. They brush things under the rug. I doubt its intentional but they are KILLING PEOPLE! Had to visit the grandma and see she needed an ambulance then a ventilator! Now an induced coma Nurses didn't even know she wasnt breathing! Horrible horrible

Experiencia fantástica: I had broken my femur and my family decided on Five Points. This is an excellent rehab facility. From house keeping to the director I was treated with dignity and caring. They have excellent meals with three coices for for lunch and dinner. The meals were served appropriately as what was supposed to be hot was hot and what was cold was served cold.. Everone who works there are polite and friendly. Would make this choice to use their services again for me or other family members.They have a nice dining area and courtyards off of the rooms.housekeeping to the do

Experiencia fantástica: If you feel anything like I did when I was reading skilled nursing/long term care reviews, you are feeling scared, helpless, and devastated that you even have to be considering this option for your loved one. It is an absolute terrible feeling to be unable to care for those that we love. If you believe a review that says any long-term facility is perfect, you should get ready for disappointment. Staff at these places have an extremely difficult job. My dad entered Five Points in June 2022. Due to complications, he jumped back and forth from skilled nursing to long-term care to the hospital...My mom visited every morning and I visited every night. I saw this facility at it's worst, and now at it's best. While at it's worst, I toured every other facility in a 50 mile range from our home and still could not find a place that I thought was better. About 6 months ago, this facility got a new Administrator, Kori. Within weeks, everything began to improve. They stopped using agency nurses, and my family finally got to know the people caring for my dad. Some of the staff became like family. They love the residents and take so much pride in their work. Some just do their job, and that is appreciated. Some have no business being in the field at all. You will find this at every facility. CNAs have a low paying job for the difficult work they do, and not everyone is cut out for it. The staffed joked that they could tell if I was marching down to complain by the sound of my steps. Most nights, when I arrived after work, Kori's door was still open, and her sweater was hanging on her chair. I would find her with sleeves rolled up checking on residents, carrying trays, working to demonstrate her expectations to her staff. I admire this. Everytime, I marched to her office to complain, she listened and corrected the problem. Dad's physical therapist was wonderful! She understood that our goal was to get him strong enough to come home, and she never stopped believing in him and encouraging him even though it is rare for this to happen. We spent one year and one day at this facility. Last Thursday my dad came home! He is walking, getting in and out of the car, and continues to get better each day. We will continue to visit our friends, neighbors, roommate, and those staff members that cared so much about my dad at Five Points. No facility is perfect. The food isn't always the best, but sometimes it is really good. The residents enjoy getting their hair done by Ramon, who once came in a blizzard, because he did not want to let his customers down. Bingo, puzzles, visits from the therapy dog, outings all happen on a regular basis. And sometimes showers are missed, the call button is turned off before the problem is solved...Your job as a family member is to advocate for your loved one. Tour facilities in the evenings. Visit on days other than Thanksgiving and Christmas. Go meet the administrators. March down and complain if necessary. Ask how grievances are resolved. Those reading this are in our prayers. I hope we never have to return to Five Points, but if circumstances warranted, it would be my first choice.

Experiencia fantástica: My mom was in 5 Points when she fell and broke both of her legs. From the administration to the staff Mom’s care was excellent. The people are open friendly and the open spaces outside are a wonderful way to get some fresh air.

Experiencia positiva: Our experience with Five Star Nursing and Rehab facility has been positive. There are changes being made that should improve the quality of care even more. The CNA’s are caring and skilled with their treatment of the residents. The Physical Therapy staff is outstanding, they’re all so caring and compassionate and are getting positive results with the treatment they’ve been giving to my husband. We have put our trust in this facility and they have not disappointed us.

Experiencia fantástica: My Mom is 92 and was at 5 Points for rehab after hip surgery. She had a stroke in February prior to her broken hip on Father’s Day. She did well here and every staff member was helpful and friendly. This has been a difficult time in Mom’s life and they made a difficult situation so much better for all of us. Kori Linahan is awesome. There were so many times she would step in and work with her staff. From the bottom to the top of the ladder I give every staff member five stars. People have bad days sometimes and we all deserve a little grace. People that are not understanding of how difficult their job is do not need to send a review!

After a previous and fairly traumatic experience for my mother with another facility (now closed) she was extremely nervous to go to rehab from BSA so I took quite a bit of time in finding a place for her. Five Points has been wonderful! The staff is happy, attentive, and caring. My mom was very happy there, loved the food, and enjoyed everyone she interacted with. They did a fantastic job with her physical therapy and I had great peace of mind with her day-to-day care. I don't know everyone's name that worked with her and I know I'll miss mentioning many of them but a big thank you to Kori, Justin, Melissa, Heidi, Clarissa, Jodi and the many more that made this time so much easier on my mom as well as our family.

Experiencia fantástica: I am Donna. I broke my femur and experienced Five Points for the second time. I was in last spring with a broken ankle. I was happy to see some of the same personnel as last one. I was very well taken care of by all. The food was fine and the rooms were always clean. If, heaven forbid, I need rehab again, I will definitely choose Five Points Rehab and Nursing again. Thanks, everyone!!

The building is beautiful and very well maintained, spacious clean rooms, the staff go over and beyond for their residents and family every day. Experiencia fantástica: My sister and I chose here after viewing 4 other places. I’m so glad we picked this one! The halls are wide and the facility is bright and clean. There is a courtyard that everyone can use. My mom enjoys when we take her out front the building to enjoy the fresh air and to see the rabbits and geese. This facility doesn’t have set visiting hours, which is great for our families to visit her as we all work different schedules. The staff is very friendly and has helped us every time we have needed help in my moms care. The communication with the staff is great also. We have open communication with them, which was important to my sister and I when it came to our moms care. I would 100 percent pick them again! Thank you to the staff for marking a time of hardship easier on us! We know she is in good hands when we are not there.

Experiencia negativa: Do not put your loved ones here especially for skilled care. The nurse practitioner and M.D are terrible. The DON has an excuse for everything. The staff stole from our loved one. My dad was transfered from room to room and contracted covid while in there.

Paul and Chelsea are quick to answer the phone with questions abt the admissions side and is always available to respond about the "day in/day out' care my Mom is receiving. I know there are many other front line workers, taking care of her, but wanted to highlight just a few of your special team members. I sincerely appreciate the diligent effort Five Points gives to family members AND the patient during this time.

They say they are protecting your loved ones from corona virus when they are the ones who are careless and brought it into the facility. Rude staff, horrible food for the residents. Experiencia negativa: Five Points Nursing and Rehab is one of the most unreliable places you could put a family member in, we have been lied to and items stolen from our Dad. They couldn’t keep up with their social media website due to supposedly being so over whelmed with no staff and not enough staff due to Covid. They didn’t post about Covid over taking the hallways. Our father had been in 5 Points over a year and was treated with disrespect, the worst food, I wouldn’t feed my dog the food or slop they feed the residents. Our father was ill from other things but his hallway literally lit up with Covid at Five Points. Dad contracted Covid in 5 Points. He had to be moved in the Covid Unit at this facility We just wonder how he was really treated behind those closed doors when we couldn’t be in there to check on him. He then was moved to a hospital. After several days, Dad passed away in the hands of real medical staff And doctors that cared and loved him in the MICU AT BSA. They even came to his funeral. To this day still no phone call, text, email or a note to say they were sorry for his passing or we loved having him as a resident. Nothing from the Administration! The things your staff told us about this facility is truth and you wonder why you have such a high rate of turnover. You guys are all about quota and not care. Human lives that can’t do or speak for themselves are in your hands. To sum up the 5 Points name and meaning: 1st Point: They are all smile's and everything is cookies and cream when you sign the paperwork. They have to keep quota!! 2nd Point is the dining area where your family member will eat stinks and smells and the will just throw slop in front of your family member. 3rd Point they will steal from your family member and lie to them and family. 4th Point your family member will sit in feces while the alarms have been going off for who knows how long. 5th Point is the facility will reply back to this message with a generic post of I’m sorry you didn’t feel as if you family member was treated right. I have left several messages and made phone calls and still no reply back to me or my siblings. Please don’t put a family member in this facility, I’m sad to know there are others still in there being treated the same as our father. Peace be with all the Residents that have no one in their to watch over them. And many blessing to the staff that really tried and did what they could for our Father.

As well as stealing from them. We have now lost 3 tablets, a phone, and all of his jewelry. It was hard enough having to leave him in that place while covid is happening but it’s even worse knowing the staff is stealing loved ones belongings after they pass away. SHAME ON YOU AND YOUR SO CALLED PLACE OF BUSINESS.
